Inter- and Intra-observer Reliability of MRI for Lumbar Lateral Disc Herniation
Background: The authors analyzed inter- and intra-observer agreement with respect to interpretation of simple magnetic resonance T1- and T2-weighted axial and sagittal images for the diagnosis of lumbar lateral disc herniation, including foraminal and extraforaminal disc herniations.Methods: Forty-two patients in whom lumbar lateral disc herniation was suspected or confirmed by simple magnetic resonance imaging at one institute between May 2003 and December 2004 were included. The magnetic resonance images consisting of T1- and T2-weighted axial and sagittal images, and these were reviewed blindly and independently by three orthopaedic spine surgeons in a random manner. The images were interpreted as positive or negative for lateral disc herniation on 2 different occasions 3 months apart. Results were analyzed using Cohens kappa statistic, and strengths of agreements were determined using the Landis and Koch criteria.Results: The kappa values for inter-observer agreement averaged 0.234 (0.282, 0.111, and 0.308 respectively) on the first occasion, and 0.166 (0.249, 0.111, and 0.137 respectively) on the second occasion, with an overall mean value of 0.200. Thus, the strength of agreement was only slight-to-fair according to the Landis and Koch criteria. Kappa values for intra-observer agreement averaged 0.479 (0.488, 0.491, and 0.459 respectively), indicating moderate agreement.Conclusions: The present study indicates that simple magnetic resonance imaging is not a reliable imaging modality for diagnosing lumbar lateral disc herniation. Quantifying gas emissions from the "Millennium Eruption" of Paektu volcano, Democratic Peoples Republic of Korea/China
Paektu volcano (Changbaishan) is a rhyolitic caldera that straddles the border between the Democratic People’s Republic of Korea and China. Its most recent large eruption was the Millennium Eruption (ME; 23 km dense rock equivalent) circa 946 CE, which resulted in the release of copious magmatic volatiles (HO, CO, sulfur, and halogens). Accurate quantification of volatile yield and composition is critical in assessing volcanogenic climate impacts but is challenging, particularly for events before the satellite era. We use a geochemical technique to quantify volatile composition and upper bounds to yields for the ME by examining trends in incompatible trace and volatile element concentrations in crystal-hosted melt inclusions. We estimate that the ME could have emitted as much as 45 Tg of S to the atmosphere. This is greater than the quantity of S released by the 1815 eruption of Tambora, which contributed to the “year without a summer.” Our maximum gas yield estimates place the ME among the strongest emitters of climate-forcing gases in the Common Era. However, ice cores from Greenland record only a relatively weak sulfate signal attributed to the ME. We suggest that other factors came into play in minimizing the glaciochemical signature. Effect of nonstationarities on detrended fluctuation analysis
Detrended fluctuation analysis (DFA) is a scaling analysis method used to
quantify long-range power-law correlations in signals. Many physical and
biological signals are ``noisy'', heterogeneous and exhibit different types of
nonstationarities, which can affect the correlation properties of these
signals. We systematically study the effects of three types of
nonstationarities often encountered in real data. Specifically, we consider
nonstationary sequences formed in three ways: (i) stitching together segments
of data obtained from discontinuous experimental recordings, or removing some
noisy and unreliable parts from continuous recordings and stitching together
the remaining parts -- a ``cutting'' procedure commonly used in preparing data
prior to signal analysis; (ii) adding to a signal with known correlations a
tunable concentration of random outliers or spikes with different amplitude,
and (iii) generating a signal comprised of segments with different properties
-- e.g. different standard deviations or different correlation exponents. We
compare the difference between the scaling results obtained for stationary
correlated signals and correlated signals with these three types of

Spontaneous Interlayer Coherence in Double-Layer Quantum Hall Systems: Symmetry Breaking Interactions, In-Plane Fields and Phase Solitons
At strong magnetic fields double-layer two-dimensional-electron-gas systems
can form an unusual broken symmetry state with spontaneous inter-layer phase
coherence. The system can be mapped to an equivalent system of pseudospin
particles with pseudospin-dependent interactions and easy-plane magnetic order.
In this paper we discuss how the presence of a weak interlayer tunneling term
alters the properties of double-layer systems when the broken symmetry is
present. We use the energy functional and equations of motion derived earlier
to evaluate the zero-temperature response functions of the double-layer system
and use our results to discuss analogies between this system and
Josephson-coupled superconducting films. We also present a qualitative picture
of the low-energy charged excitations of this system. We show that parallel
fields induce a highly collective phase transition to an incommensurate state

Spontaneous Inter-layer Coherence in Double-Layer Quantum-Hall Systems I: Charged Vortices and Kosterlitz-Thouless Phase Transitions
At strong magnetic fields double-layer two-dimensional-electron-gas systems
can form an unusual broken symmetry state with spontaneous inter-layer phase
coherence. In this paper we explore the rich variety of quantum and
finite-temperature phase transitions associated with this broken symmetry. We
describe the system using a pseudospin language in which the layer
degree-of-freedom is mapped to a fictional spin 1/2 degree-of-freedom. With
this mapping the spontaneous symmetry breaking is equivalent to that of a spin
1/2 easy-plane ferromagnet. In this language spin-textures can carry a charge.
In particular, vortices carry e/2 electrical charge and vortex-antivortex pairs
can be neutral or carry charge e. We derive an effective low-energy action and
use it to discuss the charged and collective neutral excitations of the system.
We have obtained the parameters of the Landau-Ginzburg functional from
first-principles estimates and from finite-size exact diagonalization studies.
We use these results to estimate the dependence of the critical temperature for

A wide-angle outflow with the simultaneous presence of a high-velocity jet in the high-mass Cepheus A HW2 system
We present five epochs of VLBI water maser observations around the massive
protostar Cepheus A HW2 with 0.4 mas (0.3 AU) resolution. The main goal of
these observations was to follow the evolution of the remarkable water maser
linear/arcuate structures found in earlier VLBI observations. Comparing the
data of our new epochs of observation with those observed five years before, we
find that at "large" scales of > 1" (700 AU) the main regions of maser emission
persist, implying that both the surrounding medium and the exciting sources of
the masers have been relatively stable during that time span. However, at
smaller scales of < 0.1" (70 AU) we see large changes in the maser structures,
particularly in the expanding arcuate structures R4 and R5. R4 traces a nearly
elliptical patchy ring of ~ 70 mas size (50 AU) with expanding motions of ~ 5
mas/yr (15 km/s). This structure is probably driven by the wind of a still
unidentified YSO located at the centre of the ring (~ 0.18" south of HW2). On
the other hand, the R5 expanding bubble structure (driven by the wind of a
previously identified YSO located ~ 0.6" south of HW2) is currently dissipating
in the circumstellar medium and losing its previous degree of symmetry,
indicating a very short-lived event. In addition, our results reveal, at scales
of ~ 1" (700 AU), the simultaneous presence of a relatively slow (~ 10-70 km/s)
wide-angle outflow (opening angle of ~ 102 deg, traced by the masers, and the
fast (~ 500~km/s) highly collimated radio jet associated with HW2 (opening
angle of ~ 18 deg, previously observed with the VLA. This simultaneous presence
of a wide-angle outflow and a highly collimated jet associated with a massive
protostar is similar to what is found in some low-mass YSOs. The implications
of these results in the study of the formation of high-mass stars are

Speaking and Listening with the Eyes: Gaze Signaling during Dyadic Interactions
Cognitive scientists have long been interested in the role that eye gaze plays in social interactions. Previous research suggests that gaze acts as a signaling mechanism and can be used to control turn-taking behaviour. However, early research on this topic employed methods of analysis that aggregated gaze information across an entire trial (or trials), which masks any temporal dynamics that may exist in social interactions. More recently, attempts have been made to understand the temporal characteristics of social gaze but little research has been conducted in a natural setting with two interacting participants. The present study combines a temporally sensitive analysis technique with modern eye tracking technology to 1) validate the overall results from earlier aggregated analyses and 2) provide insight into the specific moment-to-moment temporal characteristics of turn-taking behaviour in a natural setting. Dyads played two social guessing games (20 Questions and Heads Up) while their eyes were tracked. Our general results are in line with past aggregated data, and using cross-correlational analysis on the specific gaze and speech signals of both participants we found that 1) speakers end their turn with direct gaze at the listener and 2) the listener in turn begins to speak with averted gaze. Convergent with theoretical models of social interaction, our data suggest that eye gaze can be used to signal both the end and the beginning of a speaking turn during a social interaction. The present study offers insight into the temporal dynamics of live dyadic interactions and also provides a new method of analysis for eye gaze data when temporal relationships are of interest
Pulmonary Tuberculosis and Delay in Anti-Tuberculous Treatment Are Important Risk Factors for Chronic Obstructive Pulmonary Disease
OBJECTIVE: Tuberculosis (TB) remains the leading cause of death among infectious diseases worldwide. It has been suggested as an important risk factor of chronic obstructive pulmonary disease (COPD), which is also a major cause of morbidity and mortality. This study investigated the impact of pulmonary TB and anti-TB treatment on the risk of developing COPD. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used the National Health Insurance Database of Taiwan, particularly the Longitudinal Health Insurance Database 2005 to obtain 3,176 pulmonary TB cases and 15,880 control subjects matched in age, sex, and timing of entering the database. MAIN OUTCOME MEASURES: Hazard ratios of potential risk factors of COPD, especially pulmonary TB and anti-TB treatment. RESULTS: The mean age of pulmonary TB cases was 51.9±19.2. The interval between the initial study date and commencement of anti-TB treatment (delay in anti-TB treatment) was 75.8±65.4 days. Independent risk factors for developing COPD were age, male, low income, and history of pulmonary TB (hazard ratio 2.054 [1.768-2.387]), while diabetes mellitus was protective. The impact of TB persisted for six years after TB diagnosis and was significant in women and subjects aged >70 years. Among TB patients, delay in anti-TB treatment had a dose-response relationship with the risk of developing COPD. CONCLUSIONS: Some cases of COPD may be preventable by controlling the TB epidemic, early TB diagnosis, and prompt initiation of appropriate anti-TB treatment. Follow-up care and early intervention for COPD may be necessary for treated TB patients
Somatic coding mutations in human induced pluripotent stem cells
Defined transcription factors can induce epigenetic reprogramming of adult mammalian cells into induced pluripotent stem cells. Although DNA factors are integrated during some reprogramming methods, it is unknown whether the genome remains unchanged at the single nucleotide level. Here we show that 22 human induced pluripotent stem (hiPS) cell lines reprogrammed using five different methods each contained an average of five protein-coding point mutations in the regions sampled (an estimated six protein-coding point mutations per exome). The majority of these mutations were non-synonymous, nonsense or splice variants, and were enriched in genes mutated or having causative effects in cancers. At least half of these reprogramming-associated mutations pre-existed in fibroblast progenitors at low frequencies, whereas the rest occurred during or after reprogramming. Thus, hiPS cells acquire genetic modifications in addition to epigenetic modifications. Extensive genetic screening should become a standard procedure to ensure hiPS cell safety before clinical use
