    Analysis of 5 patients with gallbladder tuberculosis who had open cholecystectomy and review of literature have shown that, although still rare it presents as a part of systemic miliary tuberculosis, abdominal tuberculosis, isolated gallbladder tuberculosis and as acalculus cholecystitis in anergic patients. There are no pathognomonic signs, the diagnosis depends on suspicion of tuberculosis, peroperative findings and histological examination

    Background: The Portulaca oleracea belonging to portulacaceae family. It is a herbaceous plant widely distributed throughout the world and used in traditional medicine for many ailments. The present study was to evaluate the antinociceptive action of petroleum ether extract of P. oleracea in vincristine induced peripheral neuropathic pain and the possible mechanisms involved.Methods: Peripheral neuropathy was induced in rats by administration of vincristine sulfate (50 μg/kg i.p.) for 10 consecutive days. The cold tail hyperalgesia was assessed by cold water tail immersion test. To identify the possible mechanisms involved in the antinociceptive action of petroleum ether extract of P. oleracea, acetic acid writhing method was employed. Mice were pretreated with naloxone, glibenclamide before petroleum ether extract treatment to identify the involvement of opioid and potassium channels, respectively.Results: The administration of petroleum ether extract of P. oleracea (100 and 200 mg/kg p.o.) for 10 days significantly attenuated vincristine-induced cold hyperalgesia. Pre-treatment with glibenclamide reversed the antinociceptive effect of P. oleracea, but the naloxone pre-treatment did not reverse the antinociceptive activity of P. oleracea.Conclusion: The results of the present study reveal the antinociceptive effect of P. oleracea in vincristine induced peripheral neuropathy and involving ATP-sensitive potassium channels pathway

    Kikuchi-Fujimoto disease presenting as pyrexia of unknown origin

    Background: Kikuchi-Fujimoto disease, a benign self-limited lymphadenopathy is an uncommon cause of pyrexia of unknown origin (PUO). Methods: We retrospectively studied the case-records of 13 patients presenting with PUO who were diagnosed to have Kikuchi-Fujimoto disease on peripheral lymph node excision biopsy and report the salient clinical manifestations and histopathological findings in them. All of them received symptomatic treatment. Results: Their median age was 28 [interquartile range (IQR) 18.5-38.0] years. Women (11/13, 84.6%) were more frequently affected. All of them were human immunodeficiency virus (HIV) seronegative. Prior to presenting to us, two were being treated for lymph node tuberculosis with DOTS. Cervical lymph nodes were predominantly involved, the distribution being: right cervical (n=10, 76.9%); left cervical (n=4); and bilateral cervical (n=2). Axillary and generalized lymphadenopathy were rare being seen in 2 and 1 patient respectively. The median (IQR) erythrocyte sedimentation rate (n=11) was 53 (35-89) mm at the end of first hour. Salient histopathological features were paracortical patchy zones of eosinophilic fibrinoid necrosis with karyorrhectic debris, large numbers of histiocytes, including histiocytes with peripherally placed “crescentic” nuclei. Spontaneous regression of fever and lymphadenopathy was observed over a median (IQR) period of 8 (6.75-10.25) months in all of them. Conclusions: Kikuchi-Fujimoto disease is a rare but important cause of PUO presenting with peripheral lymphadenopathy. Women are most often affected and cervical lymph nodes are the most frequently involved site. Clinical suspicion and thoughtful collaboration between clinicians and pathologists are essential for accurate diagnosis, and to minimize unnecessary investigations and inappropriate aggressive treatment

    Plant origin natural compounds believed to be safe for human have received much attention all over the world in the recent years. In ayurveda and unani medicine, different parts of the renowned herbal medicinal plant Boerhavia diffusa have been used for treating several human disorders. But very little is known about the antiproliferative activity of B. diffusa root extract. Hence investigation was carried out on the growth inhibitory property of B. diffusa on HeLa cell line. Results showed maximum of 28.95 % of hemolysis at 320 µg/ml test concentration and good cytotoxiic effect with an IC50 value of 54.06 μg/ml inhibition on HeLa cell line. The present research work using the ethanolic extract of B. diffusa root has demonstrated the antiproliferative property on HeLa cell line

    Antimicrobial susceptibility patterns of uropathogenic Escherichia coli and their prevalence among people in and around Dhanbad, Jharkhand

    Background: Urinary tract infection (UTI) is one of the most common infections, which causes high morbidity and mortality among human population. The purpose of this study was to evaluate the prevalence and their antibiogram profile of uropathogenic Escherichia coli (UPEC) in and around Dhanbad. Methods: A total of 641 urine samples were collected from the suspected patients of UTI. The samples were cultured on MacConkey agar for isolation and identification. Antibiotic susceptibility test was done by disc diffusion method. Both male and female patients of different age groups were included for this study. Results: 45.70% urinary isolates were identified as E. coli. 43.56% UPEC isolates were sensitive to nitrofurantoin and piperacillin/tazobactum. 22.77% isolates were susceptible to levofloxacin and amikacin followed by cefotaxime (21.78%). These isolates were mostly resistant to ampicillin and trimethoprim/sulfamethoxazole, their susceptibility pattern was found to be 11.88% and 5.94% respectively. Conclusion: Prevalence of E. coli among urinary isolates was high in our study. Antibiogram profile of these isolates varies to different antibiotics in terms of their susceptibility pattern. Continuous surveillance of antibiogram of UPEC isolate is mandatory because it vary significantly in different geographical area. Thus empirical selection of antimicrobials should be based on the knowledge of local prevalence and individual sensitivity rather than on universal guideline

    Studies in biogas technology. Part I. Performance of a conventional biogas plant

    This paper gives an account of a conventional 5.66 m3/day (200 cubic ft/day) biogas plant which has been instrumented, operated and monitored for 2 ½ years. The observations regarding input to the plant, sludge and biogas outputs, and conditions inside the digester, have been described. Three salient features stand out. First, the observed average daily gas yield is much less than the rated capacity of the plant. Secondly, the plants show ease of operation and a very slow response to reductions and cessations of dung supply. Thirdly, the unexpectedly marked uniformity of density and temperature inside the digester indicates the almost complete absence of the stratification which is widely believed to take place; hence, biogas plants may be treated as isothermal, 'uniform' density, most probably imperfectly mixed, fed-batch reactors operating at the mean ambient temperature and the density of water

    Primary total knee replacement using dished polyethylene with resected posterior cruciate ligament

    Background: The choice between preserving, sacrificing or substituting the posterior cruciate ligament (PCL) is always a controversial topic in total knee replacement (TKR). Dished polyethylene insert with PCL resection enables correction of the commonly present fixed flexion and varus deformities. Additionally, the risk of premature wear of polyethylene is less because of the confirming articular geometry between the femoral and tibial component.Methods: This is a retrospective study in which we studied 120 knees in 95 consecutive patients undergoing primary TKR by the senior author at our institute. We used TKR system with dished metal backed polyethylene tibial component. PCL resection was performed in all cases. Pre-operative and post-operative functional assessment were done using knee society clinical scores and Western Ontario and McMaster universities osteoarthritis index (WOMAC). All radiographs were assessed using the knee society Roentgenographic scoring system (KSRES). Statistical analysis was performed using paired student t tests. Survivorship was determined using Kaplan-Meier survivorship curves. Results: Mean follow-up was 8 years. Range of motion increased from 75 degrees to 110 degrees.  The knee society pain score increased from 30 to 94. The knee society function score increased from 35 to 75. WOMAC score increased in terms of pain, stiffness and physical function.Conclusions: We conclude that deep dish bearing is a viable option in presence of deficient PCL and provides adequate stability and functional outcome. We need a larger sample size, multicentre trial and longer follow-up to see for complication rate, revision rate and survival

    Development of bioconjugated nano-molecules against targeted microbial pathogens for enhanced bactericidal activity

    The study reports development of bio-conjugated nano-molecules (BCM's) for enhanced antibacterial activity against targeted pathogens of global concern. The conjugation was achieved with biomolecular interaction of silver clusters with 3-Dichloro-5, 6-dicyano-1, 4-benzoquinone (DDQ) which was purified and characterized with TLC which displayed prominent band at R-f 0.5 and HPLC analysis displayed chromatographic peak eluting at (t(R)) 6-8 min. The LC-MS analysis displayed single major peak at t(R) = 4.610 corresponding to molecular ion peak at m/z = 227.19. The developed BCM molecule was determined with UV-Visible spectroscopy which displayed the absorbance peaks of conjugated molecules with shoulder peak observed and morphological characteristics were well defined with TEM analysis which showed cluster formation. The size ranged from 20 to 80 nm in size with majority of the BCM displaying spherical in size. The process of bio-conjugation was further studied with FTIR analysis which corresponded with different vibrational stretches owing to the presence of amide, carbonyl alkynes, nitriles and carboxylic acid groups. The presence of functional moieties was also studied using H-1 and C-13 NMR spectra. The crystalline characteristics was confirmed with XRD analysis which displayed Bragg's intensities along with additional peaks occurring at 2 theta angle corresponds to (111), (200), (220), (311) face centric cube of silver planes. The antibacterial activity of BCM's was profound against all the test pathogen which was validated and compared with standard antibiotics. Among the test pathogens, highest activity was conferred against Staphylococcus epidermidis (MTCC 435) with 29 mm. The minimal inhibition concentration of BCM's was in the range of 0.97-3.12 mu g/ml. The results of MIC were in accordance with well diffusion assay indicating the Gram + ve test pathogen Staphylococcus epidermidis to be the most sensitive. In addition, the broth dilution assay resulted in decrease in the optical density measured at 600 nm against the increase in the concentration of BCM's. The outcome of the present investigation revealed the role of bio-conjugation chemistry to increase fold activity against pathogens which can act as alternative tool to combat drug resistant menace across the globe
