    Unusual presentation of ulcerative postauricular swelling as sebaceous cell carcinoma

    Sebaceous glands have high concentration over head and neck region. Despite high concentration, sebaceous cell adenoma and carcinomas are infrequent. Sebaceous cell carcinoma is an uncommon, cutaneous aggressive tumor arising from the sebaceous glands and seen almost exclusively on the eyelids (75%). It accounts for just  0.2–0.7% of all eyelid tumors in the USA and very few cases that have originated in areas other than the eyelids have been reported. A 67-year-old male presented with swelling (3 cm × 4 cm), on the right postauricular region, since about 1‑month. The swelling became ulcerative and associated with progressive tinnitus and hoarseness of voice. The patient was investigated. Fine‑needle aspiration cytology suggested sebaceous cell carcinoma. Then excision biopsy was done, and histopathological examination of excised tissue confirmed the diagnosis. Extraorbital sebaceous cell carcinoma is an aggressive and invasive malignancy. It clinically mimics other  diseases and is difficult to diagnose. Hence, an accurate and prompt diagnosis is crucial because of its fulminant course, serious associations with Muir–Torre syndrome and high potential for regional and distant metastasis.Keywords: Extraocular, sebaceous cell carcinoma, sebaceous glan

    Customized sorting and packaging machine

    India is a country which has a cornerstone of agriculture. And as it comes to fruit/vegetable sorting and packaging in India, human labor has been a vital part. With manual hand picking, it is a very laborious task to classify the quality of fruits/vegetables and simultaneously pack them. One leading-edge technology for the fulfilment of this purpose is ‘Image Processing’ technology which is extremely fast and cost-efficient. Our whole idea revolves around the fact that each fruit will be inspected, sort and simultaneously packed. For the same, a low cost automated mechatronic system has designed consisting of a solitary mechanical arrangement, which is controlled and synchronized through electronic components. Fruits/vegetables are sorted as high-quality and low-quality on the basis of physical appearance and weight. For this, a suitable algorithm is designed using the Open CV library. And the sorting is done using Arduino Uno and Raspberry pi. Hence the aim is to develop a sorting and packaging facility that can be established at the very root level itself which will be economically compact and accurate and will give more justice to farmers

    Relative contribution of susceptibility weighted imaging, compared to conventional MRI, in the detection of common bile-duct calculi

    PURPOSEWe aimed to evaluate the relative contribution of susceptibility weighted imaging (SWI) in the detection of common bile-duct (CBD) stones in comparison to the conventional MRI protocol containing magnetic resonance cholangiopancreatography (MRCP), balanced turbo field echo (BTFE), and T2-weighted spin-echo imaging techniques.METHODSMRI data containing MRCP, BTFE, T2-weighted imaging, and abdominal SWI were independently evaluated by 2 sets of experienced radiologists in 44 patients with confirmed CBD stones. Endoscopic retrograde cholangiopancreatography, and endoscopic ultrasound where available, was used as the reference gold standard. Evaluation was performed for the visualization of CBD stones in each of the MRI techniques. Relative contribution of SWI was classified into one of four categories for each case: (1) no contribution to CBD stone visualization; (2) same as conventional techniques; (3) improved diagnostic confidence; and (4) critical for diagnosis. Stone size was also assessed.RESULTSInter-rater agreement coefficient for CBD stone visualization was found to be “good” in MRCP (0.77), “very good” in SWI (0.94) and BTFE (0.84), and moderate in T2-weighted imaging (0.54). CBD stones were visualized with SWI in 86.4% and 82%, with MRCP in 70.5% and 70.5% cases, with BTFE in 73% and 61.4% cases, with T2-weighted imaging in 45.5% and 52.3% cases by reviewers 1 and 2, respectively. SWI did not contribute to CBD stone visualization in 2.3% (1/44); was the same as conventional techniques in 31.8% (14/44) cases; improved diagnostic confidence in 34.1%; and was critical for diagnosis in 20.5% cases.CONCLUSIONSWI has the potential to serve as a strong adjunct to conventional MRI protocols used for CBD stone evaluation with very small scan-time penalty

    Mechanical and tribological properties of AA7075-T6 metal matrix composite reinforced with ceramic particles and aloevera ash via Friction stir processing

    Mechanical and wear properties of AA7075-T6 reinforced with SiC and Aloevera ash, fabricated using Friction stir processing (FSP) are investigated in this study. Due to less density, easy availability, and cost-effectiveness, aloevera ash is considered as one of the reinforcements. FSP is done using a square tool pin profile, at different tool rotational speeds. To study wear behaviour, pin on disc test is carried out on High Temperature Rotary Tribometer at 20N, 30N and 40N applied load. Wear increases on increasing the applied load and at 20N load Al+SiC/Aloevera ash composite, processed at 600 tool rpm gave the best results due to the formation of oxide tribolayer. At 30N and 40N applied load Al+SiC composite, processed at 900 tool rpm showed the least wear because of proper scattering of ceramic particles due to high tool rotational speed. Coefficient of friction increases on increasing the applied load and all fabricated composite samples showed a lesser coefficient of friction than the base metal. Microhardness, ductility and Ultimate tensile strength increases on the addition of reinforcement and had a direct relation with tool rpm. Wear morphology was analysed using Scanning Electron Microscope (SEM). Energy Dispersive Spectroscopy (EDS) analysis after wear shows the presence of C, Fe, O, Mg, Zn, Si, Al elements and confirms the formation of an oxide layer which is responsible for decreasing wear loss

    Abstract Background It is an established fact that humans and animals are exposed to more than one chemical concurrently from various sources such as food, air and water. In the past, much emphasis was laid on evaluating the toxic effects of a single chemical. Nowadays an increased attention is being paid to the interaction of xenobiotics with one another. Therefore, a study was aimed to evaluate the potentiating effect of imidacloprid (IMI) on arsenic-induced testicular toxicity in rats. Methods Adult male Wistar rats randomly divided into eight groups with six in each were subjected to daily oral administrations for 28 days. Group I served as control, group II received IMI at the dose rate of 16.9 mg/kg body weight, group III, IV and V received arsenic at the dose rate of 50, 100 and 150 ppb in drinking water whereas group VI, VII and VIII received both arsenic and IMI. Results Repeated oral administrations of IMI or arsenic (150 ppb) alone resulted in a significant (P < 0.05) elevation in the levels of malondialdehyde (MDA) and advanced oxidation protein product (AOPP) along with significant (P < 0.05) decline in total thiols and antioxidant enzymatic activities indicating reduced antioxidant defense in testicular tissue of exposed rats. These findings were further corroborated with histological alterations in testes like fluid accumulation in interstitial spaces in IMI administered rats. Similarly, rats provided access exclusively to arsenic-containing drinking water induced degenerative changes in seminiferous tubules in a concentration-dependent manner. Concurrent administration of IMI and arsenic produced more severe antioxidant and histopathological alterations of testes as compared to exposure to either toxicant. Conclusions Reduced antioxidant activities, increased MDA and AOPP levels with severe histopathological alterations in testes of rats on concurrent exposure indicated that IMI potentiated the arsenic-induced testicular toxicity in Wistar rats

    Delayed Repair of Infected Ruptured Patellar Tendon using Suture Anchors

    Patellar tendon rupture are rare injuries that are easily missed in acute phases if careful clinical examination is not carried out. The delayed condition is further difficult to treat and augmentation of end to end repair is generally required. However, literature presents no such case of delayed presentation with presence of infection. We here present one such case of delayed presentation of patellar tendon rupture at three weeks in a 52-year-old male patient. Usual techniques were not sufficient to allow early rehabilitation. Technique of suture anchors was planned for repair after thorough debridement. After this intervention, patient was put on aggressive rehabilitation protocol and he gained excellent range of motion. Patient was followed for one year and he showed no loss of movement or signs of infection. We thus recommend using anchor suture repair of patellar tendon that provides a stable and rigid fixation with possibility of early active rehabilitation even in delayed setting

    Mechanical and wear performance of Al/SiC surface composite prepared through friction stir processing

    In the present research work, AA7075 composite reinforced with silicon carbide particles has been fabricated using Friction stir processing (FSP). The silicon carbide particles having a size of 40 Ό m were placed in grooves of length 160 mm, width 2 mm, depth 3.5 mm, that were generated on the AA7075 plate. The square pin tool is utilized for fabricating the composite at two different rotational speed i.e. 700 and 1000 rpm. Effect of processing, particle addition and tool rotational speed is analyzed on mechanical and wear properties of the material. On friction stir processing the microhardness value and elongation of the material increased. Reinforcement addition contributed to decrease in ductility and tensile strength while on the contrary microhardness and wear resistance of the material improved. Tool rotational speed showed a direct relation with the tested mechanical and wear properties. Adhesive wear was the prominent wear mechanism and Fe layer formation was observed on the worn surface, contributing to increased wear resistance.
