Формирование вейвлет-функций в задаче идентификации музыкальных сигналов

    Get PDF

In 2009, we described a child with PFIC-2 who developed PFIC-like symptoms after orthotopic liver transplantation (OLT). BSEP-reactive antibodies were demonstrated to account for disease recurrence. Here, we characterize the nature of this antibody response in 7 more patients with antibody-induced BSEP deficiency (AIBD). Gene sequencing and immunostaining of native liver biopsies indicated absent or strongly reduced BSEP expression in all 7 PFIC-2 patients who suffered from phenotypic disease recurrence post-OLT. Immunofluorescence, western blotting analysis, and transepithelial transport assays demonstrated immunoglobulin (Ig) G-class BSEP-reactive antibodies in these patients. In all cases, the N-terminal half of BSEP was recognized, with reaction against its first extracellular loop (ECL1) in six sera. In five, antibodies reactive against the C-terminal half also were found. Only the sera recognizing ECL1 showed inhibition of transepithelial taurocholate transport. In a vesicle-based functional assay, transport inhibition by anti-BSEP antibodies binding from the cytosolic side was functionally proven as well. Within 2 hours of perfusion with antibodies purified from 1 patient, rat liver showed canalicular IgG staining that was absent after perfusion with control IgG. Conclusions: PFIC-2 patients carrying severe BSEP mutations are at risk of developing BSEP antibodies post-OLT. The antibody response is polyclonal, targeting both extra- and intracellular BSEP domains. ECL1, a unique domain of BSEP, likely is a critical target involved in transport inhibition as demonstrated in several patients with AIBD manifest as cholestasis.publishersversionPeer reviewe

Sustainability trade-offs in Climate Change Geographies in England

This is an open access article distributed under the terms of the Creative Commons Attribution License (CC BY), https://creativecommons.org/licenses/by/4.0/The evidence that climate change is the result of human actions becomes yet stronger, as does the need to take action to limit the worst effects of climate change on the planet. However, politicians continue to equivocate and fail to address the trade-offs which are needed to deliver effective action. In this paper we report on the potential of bottom-up approaches to transport planning to address the trade-offs between the need to reduce car-based travel and the social consequences of poor mobility options in rural areas. Using theories of Sustainable Communities and Communi-ties of Practices, we analyze the implementation of the Robin Demand Responsive Transport service in the West of England, presenting new data relating to the effectiveness of this service in providing low carbon transport alternatives to rural residents. We find that the Robin is indeed effective, and that it has worked better in one location, where engagement with potential new users of the service has been prioritized. We conclude that such bottom-up transport options can be transformative, subject to the support of key stakeholders and integration with top-down systems of governance.Peer reviewe

Abstract

Introduction

Perforation of the gall bladder represents a rare, but life-threatening complication of cholecystitis. Clinical presentation may vary between severe peritonism in acute perforation and absence of symptoms in subacute or chronic progression of perforation. Abdominal imaging like ultrasound or CT-scan are important tools for immediate diagnose of gall bladder perforation.

Case presentation

We report a case of a 30-year old female patient with end-stage kidney disease treated by continuous ambulatory peritoneal dialysis (CAPD) who was admitted to the emergency room with fever and mild abdominal pain. A type II gall bladder perforation by a solitary gall stone with development of a liver abscess was detected by abdominal ultrasound.

Conclusion

Gall bladder perforations are rare but have to be considered in patients with abdominal pain and fever. Abdominal ultrasound is a reliable tool to establish diagnosis.</p

Perceptions of Global and Domestic Agricultural Issues Held by International Agricultural Journalists

The purpose of this study was to provide baseline knowledge of the prevalent global and domestic agricultural issues, ways to educate journalists about these issues, and sources of information used when reporting about agricultural issues, according to international agricultural journalists. The executives of the International Federation of Agricultural Journalists served as this study’s population. The IFAJ is comprised of a membership of 31 countries that practice freedom of the press. A modified Delphi method with three rounds of data collection was utilized for this study. Qualitative feedback was provided by the executives in Round One. In Rounds Two and Three, quantitative feedback was used with the goal of forming consensus on the most important global issues and methods to educate journalists about these issues.A list of important domestic agricultural issues was supplied for 20 countries. The executives also generated a list of important global agricultural issues and ways to educate journalists about these issues. Important global agricultural issues included feeding a growing global population and water quality/quantity. Face-to-face methods of educating journalists were favored by the executives. The executives identified a wide variety of sources they use to report about global and domestic agricultural issues, including government agencies, farmers, universities, online sources, professional organizations, commodity group websites, and academic journals

Zelluläre Abstoßungen herztransplantierter Patienten in Münster
