    MRT-Leberdiagnostik mit dem leberspezifischen Kontrastmittel Gd-EOB-DTPA

    In a Phase III clinical trial the liver-specific MRI contrast agent Gd-EOB-DTPA was administered to 40 Patients. The substance showed very good tolerance. After intravenous bolus injection a non-specific vascular phase can be performed. After that liver-specific imaging is possible. Sensitivity and specificity in unenhanced MRI were 83% and 100% and could be increased to 91% and 98% by Gd-EOB-DTPA administration. Sensitivity and specificity in contrast-enhanced spiral-CT were 73% and 100%. Characterization rate could be increased from 92% in the unenhanced scans to 99% after Gd-EOB-DTPA administration. Biphasic spiral-CT showed a correct characterization rate of 93%. Quantitative assessment showed an increase of information content in the post-contrast scans due to a siginificant increase of liver-lesion contrast. Increased contrast-to-noise values are helpful to perform correct characterization. Gd-EOB-DTPA enhanced MRI of the liver is superior in detection and characterization of focal liver lesions to both unenhanced MRI and contrast-enhanced spiral-CT. Vascular and liver-specific imaging can be performed within one examination. This allows better treatment planning and higher efficiency in scanner utilization.Im Rahmen der Phase III der klinischen Prüfung des leberspezifischen MRT-Kontrastmittels Gd-EOB-DTPA wurde die Substanz 40 Patienten verabreicht. Die Substanz zeigte eine sehr gute Verträglichkeit. Nach der intravenösen Gabe als Bolus lässt sich zunächst eine unspezifische vaskuläre Phase beobachten. Anschliessend kann eine leberspezifische Bildgebung erfolgen. Die Sensitivität und Spezifität in der nativen MRT-Bildgebung betrug 83% und 100% und konnte durch GD-EOB-DTPA auf 91% und 98% gesteigert werden. Die kontrastmittelverstärkte Spiral-CT zeigte eine Sensitivität von 73% und eine Spezifität von 100%. Die Rate an richtigen Charakterisierungen konnte von 92% in den nativen Sequenzen auf 99% nach Gd-EOB-DTPA-Gabe gesteigert werden. Die Rate an richtigen Charakterisierungen in der kontrastverstärkten biphasischen Spiral-CT betrug 93%. Die quantitative Auswertung zeigte eine Erhöhung des Bildinformationsgehaltes durch einen signifikanten Anstieg des Leber-zu-Läsions-Kontrastes. Aus den Kontrast-zu-Rausch Werten in der dynamischen und leberspezifschen Phase lassen sich wichtige Hinweise auf die Charakterisierung fokaler Läsionen ableiten. Die Gd-EOB-DTPA-verstärkte MRT der Leber ist der nativen MRT sowie der kontrastmittelverstärkten Spiral-CT der Leber bei der Detektion und Charakterisierung fokaler Leberläsionen signifikant überlegen. Die Durchführung der vaskulären als auch der leberspezifischen Bildgebung ist in einem einzigen Untersuchungsdurchgang möglich. Durch die Möglichkeit einer besseren Geräteauslastung und der Fähigkeit, innerhalb eines einzigen Untersuchungsdurchganges Informationen zu gewinnen, für die sonst mehrere Einzeluntersuchungen im Sinne einer Stufendiagnostik notwenig wären, leistet Gd-EOB-DTPA einen wichtigen Beitrag zu einer effizienten und kostengünstigen Diagnosefindung

    Emergency Left Colon Resection for Acute Perforation. Primary Anastomosis or Hartmann's Procedure? A Case-matched Control Study

    Background: The optimal treatment remains controversial for acute left-sided colon perforation. Therefore, the effectiveness and safety of primary anastomosis versus Hartmann's operation (HP) was compared in a case-matched control study. Methods: Thirty consecutive patients with primary anastomosis and protective ileostomy (PAS) were matched to 30 HP patients, controlling for age, gender, American Society of Anesthesiologists (ASA) score, body mass index (BMI), and peritonitis severity (Hinchey). In a second analysis, PAS patients with purulent peritonitis (Hinchey 3) were matched to patients with primary anastomosis without ileostomy (PA). Results: Hospital mortality was similar between HP (17%) and PAS (10%). Complication frequency and severity (requiring re-intervention or admission to the Intensive Care Unit [ICU]) were comparable for the first operation (60% versus 56% and 30% versus 32%). The stoma reversal rate was higher in PAS than in HP (96% versus 60%, p=0.001), with significantly fewer complications (23% versus 66%, p=0.02), and lower severity (7% versus 33%, p=0.02). Additional analysis of PAS versus PA showed similar morbidity (52% versus 41%, p=0.45) and complication severity (18% versus 24%, p=0.51), whereas overall operation time and hospital stay were significantly shorter in PA (169 versus 320 min, p=0.003, 17 versus 28 days, p<0.001). Conclusions: Primary anastomosis and protective ileostomy is a superior treatment to HP in acute left-sided colon perforation. In the absence of feculent peritonitis an ileostomy appears unnecessar

    Functional recovery after implantation of artificial nerve grafts in the rat- a systematic review

    <p>Abstract</p> <p>Purpose</p> <p>The aim of this study was to compare functional data of different nerve-gap bridging materials evaluated in rat experiments by means of a systematic review.</p> <p>Materials and methods</p> <p>A systematic review was conducted, searching MEDLINE, HTS and CENTRAL to identify all trials evaluating functional recovery of artificial nerve conduits in the rat model.</p> <p>Results</p> <p>There was a trend towards a favourable outcome of conduits coated with Schwann-cells compared to the plain synthetics. Histomorphometry, electrophysiology and muscle-weight correlated poorly with functional outcome.</p> <p>Conclusion</p> <p>Schwann-cell coated conduits showed promising results concerning functional recovery. Further standardization in outcome reporting is encouraged.</p

    The Fight against Cancer by Microgravity: The Multicellular Spheroid as a Metastasis Model

    Cancer is a disease exhibiting uncontrollable cell growth and spreading to other parts of the organism. It is a heavy, worldwide burden for mankind with high morbidity and mortality. Therefore, groundbreaking research and innovations are necessary. Research in space under microgravity (µg) conditions is a novel approach with the potential to fight cancer and develop future cancer therapies. Space travel is accompanied by adverse effects on our health, and there is a need to counteract these health problems. On the cellular level, studies have shown that real (r-) and simulated (s-) µg impact survival, apoptosis, proliferation, migration, and adhesion as well as the cytoskeleton, the extracellular matrix, focal adhesion, and growth factors in cancer cells. Moreover, the µg-environment induces in vitro 3D tumor models (multicellular spheroids and organoids) with a high potential for preclinical drug targeting, cancer drug development, and studying the processes of cancer progression and metastasis on a molecular level. This review focuses on the effects of r- and s-µg on different types of cells deriving from thyroid, breast, lung, skin, and prostate cancer, as well as tumors of the gastrointestinal tract. In addition, we summarize the current knowledge of the impact of µg on cancerous stem cells. The information demonstrates that µg has become an important new technology for increasing current knowledge of cancer biology

    DNA methylation analysis on purified neurons and glia dissects age and Alzheimer's disease-specific changes in the human cortex

    Background: Epigenome-wide association studies (EWAS) based on human brain samples allow a deep and direct understanding of epigenetic dysregulation in Alzheimer's disease (AD).However, strong variation of cell-type proportions across brain tissue samples represents a significant source of data noise.Here, we report the first EWAS based on sorted neuronal and non-neuronal (mostly glia) nuclei from postmortem human brain tissues. Results: We show that cell sorting strongly enhances the robust detection of disease-related DNA methylation changes even in a relatively small cohort.We identify numerous genes with eell-type-specific methylation signatures and document differential methylation dynamics associated with aging specifically in neurons such as CLU, SYNJ2 and NCOR2 or in glia RAI1,CXXC5 and INPP5A.Further, we found neuron or glia-specific associations with AD Braak stage progression at genes such as MCF2L,ANK1, MAP2, LRRC8B, STK32C and S100B.A comparison of our study with previous tissue-based EWAS validates multiple AD-associated DNA methylation signals and additionally specifies their origin to neuron, e.g., HOXA3 or glia (ANK1). In a meta-analysis, we reveal two novel previously unrecognized methylation changes at the key AD risk genes APP and ADAM17. Conclusions: Our data highlight the complex interplay between disease, age and cell-type-specific methylation changes in AD risk genes thus offering new perspectives for the validation and interpretation of large EWAS results

    Evolutionarily Conserved Herpesviral Protein Interaction Networks

    Herpesviruses constitute a family of large DNA viruses widely spread in vertebrates and causing a variety of different diseases. They possess dsDNA genomes ranging from 120 to 240 kbp encoding between 70 to 170 open reading frames. We previously reported the protein interaction networks of two herpesviruses, varicella-zoster virus (VZV) and Kaposi's sarcoma-associated herpesvirus (KSHV). In this study, we systematically tested three additional herpesvirus species, herpes simplex virus 1 (HSV-1), murine cytomegalovirus and Epstein-Barr virus, for protein interactions in order to be able to perform a comparative analysis of all three herpesvirus subfamilies. We identified 735 interactions by genome-wide yeast-two-hybrid screens (Y2H), and, together with the interactomes of VZV and KSHV, included a total of 1,007 intraviral protein interactions in the analysis. Whereas a large number of interactions have not been reported previously, we were able to identify a core set of highly conserved protein interactions, like the interaction between HSV-1 UL33 with the nuclear egress proteins UL31/UL34. Interactions were conserved between orthologous proteins despite generally low sequence similarity, suggesting that function may be more conserved than sequence. By combining interactomes of different species we were able to systematically address the low coverage of the Y2H system and to extract biologically relevant interactions which were not evident from single species
