A study on adulteration in cereals and bakery products from Poland including a review of definitions
The aim of the study was to critique the food adulteration trends associated with cereals and bakery products from Poland. The methodological approach was to firstly review existing literature to define and outline the challenge of food adulteration and the degree of harmonisation, or not, of definitions, and then to analyse local and European data on the prevalence of food adulteration and mislabelling in the cereals and bakery sector more generally, and specifically in Poland. Analysis of general RASFF notifications of cereal and bakery products linked to Poland (n=177) revealed that most non-compliances were due to mycotoxins, undeclared allergens and undeclared genetically modified materials. Key notable trends included an increase in incorrect allergen labelling with only two incidents directly associated with adulteration firstly with melamine and also suspicion of deliberate contamination of milk powder with rodenticide. Data from IJHARS share similar trends where most reported irregularities associated with cereal and bakery products were related to mislabelling. The definition of adulterated foodstuffs in Poland concentrates on mislabelling particularly regarding product composition rather than being differentiated by the motivation of the perpetrator. This is not in step with other definitions where intent is seen an inherent
aspect of a determination of an instance of food fraud or adulteration. This work demonstrates that even in harmonized regulatory areas such as the EU there are local definitions of adulteration that due to the lack of consistency could influence collective approaches to determining the extent of or addressing the problem of mislabelling, misrepresentation and misbranding as a form of adulteration
Complying with the smoking ban by students before and after introducing legislative intervention
Objectives: More and more countries introduce a total ban on smoking tobacco in public places. The aim of this work was to evaluate the effectiveness of “The Act of 8 April 2010 on amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and act on National Sanitary Inspectorate” and assess the frequency of complying with the smoking bans by the students of the Medical University of Lodz, Poland. Material and Methods: Between 2007–2011, at the Social Medicine Institute of the Medical University of Lodz, a study using random survey was carried out involving students who were starting their studies at the Health Department of the Medical University of Lodz. The analysis of the collected material showed that 1038 people reported being smokers at the time of the study. Among that group, 530 students were included in the study prior to, and 508 after the introduction of the amendment. In order to verify their compliance with the smoking ban, the respondents were asked whether they smoked only in designated areas or wherever they wanted to. Results: The ratio of people claiming they smoked anywhere they wanted to, disregarding the smoking ban, was 60% (N = 318) and after the amendment had been introduced, this ratio was 62.2% (N = 316), it increased by 2.2 percentage points. The observed difference was statistically irrelevant (Chi2 = 0.530, p > 0.05). Conclusions: The Act ”On amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and Act on National Sanitary Inspectorate” in Poland did not result in the expected changes in the frequency of complying with the smoking ban by the 1st year students
Lifestyle of young adults with elevated blood pressure values
Wstęp Ryzyko zgonu z przyczyn naczyniowych
wzrasta wprost proporcjonalnie do wartości ciśnienia
tętniczego, a wzrost wartości ciśnienia tętniczego powyżej
115/75 mm Hg istotnie zwiększa ryzyko chorób
układu krążenia. Dlatego istotne jest pytanie
o znaczenie, już w fazie stanu przednadciśnieniowego,
znanych, modyfikowalnych czynników ryzyka
nadciśnienia tętniczego, związanych z nieprawidłowym
stylem życia.
Celem pracy było porównanie stylu życia młodych
osób dorosłych zakwalifikowanych do trzech grup:
grupy z rozpoznanym nadciśnieniem tętniczym, grupy
ze stanem przednadciśnieniowym i grupy z ciśnieniem
prawidłowym optymalnym.
Materiał i metody Ocenę przeprowadzono na podstawie
wyników przygodnego pomiaru skurczowego
ciśnienia tętniczego (SBP) i rozkurczowego ciśnienia
tętniczego (DBP) oraz badania kwestionariuszowego,
oceniającego w modelu epidemiologicznego
badania przekrojowego podstawowe
uwarunkowania nadciśnienia tętniczego. Badanie
przeprowadzono w grupie 532 studentów w wieku
18-25 lat. Statystyczną analizę danych wykonano
na podstawie procedur zawartych w programie
EpiInfo 6,0. Wyniki Prawidłowe optymalne ciśnienie tętnicze występowało
u 39,3% osób, stan przednadciśnieniowy rozpoznano
u 45,9% badanych, a nadciśnienie tętnicze u
14,8% studentów. Nadwaga i otyłość częściej występowały
u osób z nadciśnieniem tętniczym niż u osób ze
stanem przednadciśnieniowym oraz u osób z prawidłowym
optymalnym ciśnieniem tętni-czym (p < 0,05).
Wykazano, iż osoby z nadciśnieniem tętniczym w porównaniu
z badanymi ze stanem przednadciśnieniowym
i osobami z prawidłowym optymalnym ciśnieniem
tętniczym częściej stosowały dietę bogatotłuszczową
oraz rzadziej piły kawę (p < 0,05). Ponadto w
grupie osób z nadciśnieniem tętniczym zaobserwowano
statystycznie znamiennie częstsze rodzinne występowanie
zaburzeń lipidowych, choroby niedokrwiennej
serca i miażdżycy w porównaniu z osobami z prawidłowymi
wartościami ciśnienia tętniczego (p < 0,05).
Wnioski Wyniki badania wykazały zróżnicowanie
w zakresie stylu życia młodych osób dorosłych z nadciśnieniem
tętniczym, stanem przednadciśnieniowym
oraz z prawidłowym optymalnym ciśnieniem
tętniczym w zakresie nadwagi i niewłaściwej diety.
Z tego względu należy prowadzić prospektywne
badania epidemiologiczne oceniające zagrożenie
związane z nieprawidłowym stylem życia u osób
z podwyższonymi wartościami ciśnienia tętniczego
oraz promować zdrowy styl życia u młodych osób.Background The risk of cardiovascular death is associated
with blood pressure value (BP) and its value above
115/75 mm Hg increases the risk in a statistically significantly
way. According to this, the role of lifestyle and risk
factors of hypertension that can be modified is important
also in the stadium of prehypertension.
The aim of the study was to compare lifestyles of young
adults with hypertension, prehypertension and optimal BP.
Material and methods The cross-sectional study ivolred
a casual BP measurement and the questionnaire estimating basic
risk factors of hypertension in 532 students, aged 18–25. The
statistical analysis was performed with EpiInfo 6.0 procedures.
Results Optimal BP was recognized in 39.3%, prehypertension
in 45.9% and hypertension in 14.8% students.
Students with hypertension were more frequently
overweighted/obese than students with prehypertension
and optimal BP (p < 0.05). Young adults with hypertension
more frequently used diet rich in fat and less frequently
drank coffee than students with prehypertension
and optimal BP (p < 0.05). Hypertensive subjects declared
the family history of dyslipidaemia, ischaemic heart disease
and atheroslerosis more frequently than students with
normal BP (p < 0.05).
Conclusion The results revealed different lifestyles of
young adults with hypertension, prehypertension and optimal
BP, and difference in obesity and improper diet. The
study underlines the necessity of prospective epidemiologic
studies to estimate the cardiovascular risk related to abnormal
lifestyle among people with increased BP level and to promote healthy lifestyle of young adults
Spatial and temporal variability of bronchiectasis cases in Silesian voivodeship in 2006–2010
Objectives: Reports on an increasing number of hospitalizations in other European countries and the lack of epidemiological data on the prevalence of bronchiectasis in Poland constituted motivation for the authors to investigate temporal changes of the registered incidence and hospitalization due to bronchiectasis in Silesian voivodeship, and to evaluate spatial variability of the disease in the study region. Material and Methods: The study is a descriptive epidemiological project. Temporal and spatial variability of coefficients describing numbers of newly diagnosed cases and first time hospitalizations due to bronchiectasis (code J47 according to International Statistical Classification of Diseases and Related Health Problems, 10th revision (ICD-10)) were evaluated based on the registered data available from the National Health Found (2006–2010) and the data from MZ/Szp-11 reports (2000–2011). The data concerned adults aged ≥ 19 years, inhabitants of Silesian voivodeship. Maps of incidence or hospitalization rates due to bronchiectasis were constructed by the use of a geographical information system ArcGIS. Results: The obtained results show a stable trend of reported new diseases, whereas the number of first time hospitalizations is increasing. Values of the standardized incidence were 19.9–25.1/100 000 inhabitants, and values of the standardized first-time hospitalization were 1.2–2.9/100 000 inhabitants. The reported rates of bronchiectasis indicate significant spatial differences in epidemiological situation in the study region. Conclusions: The findings showed territorial variability of the incidence and hospitalization of bronchiectasis recorded in Silesian voivodeship. The observed variability might result from regional differences in the availability of specialized medical services
Risk factors and the prevalence of hypertension in young adults
Wstęp Podwyższone wartości ciśnienia tętniczego
(BP) i wydłużony czas ich trwania sprzyjają wzrostowi
ryzyka powikłań i wskazują na konieczność
wczesnej identyfikacji nadciśnienia tętniczego,
zwłaszcza w grupie osób młodych, w odniesieniu do
których istnieją skuteczne działania profilaktyczne.
Szacuje się, że nadciśnienie dotyczy w Polsce blisko
co trzeciej osoby dorosłej (20-30%), odsetek ten jednak
nie obrazuje skali problemu w populacji młodych
osób dorosłych.
Celem pracy było określenie częstości nadciśnienia
w populacji studentów (osób w wieku 18-25 lat)
i poznanie potencjalnych uwarunkowań występowania
nadciśnienia tętniczego w tej grupie wiekowej.
Materiał i metody Dla realizacji celu badawczego
przeprowadzono epidemiologiczne badanie przekrojowe
w grupie 532 studentów. Pomiary BP przeprowadzono
zgodnie z zaleceniami Polskiego Towarzystwa
Nadciśnienia Tętniczego (PTNT) i Europejskiego
Towarzystwa Nadciśnienia Tętniczego
(ESH). W celu określenia potencjalnych uwarunkowań
nadciśnienia tętniczego przeprowadzono badanie
kwestionariuszowe. Analizę statystyczną danych
prowadzono na podstawie procedur dostępnych
w oprogramowaniu EpiInfo 6.0.
Wyniki Prawidłowe optymalne wartości BP stwierdzono
u 39,3% studentów (częściej u kobiet niż mężczyzn,
p < 0,001). Nadciśnienie tętnicze dotyczyło 14,8% badanych
(częściej mężczyzn niż kobiet, p < 0,001),
w tym znamiennie częściej (p < 0,05) występowało
u mężczyzn izolowane nadciśnienie skurczowe - u 3,7%
badanych. Wśród statystycznie znamiennych uwarunkowań
nadciśnienia znalazły się: otyłość i dieta bogatotłuszczowa,
nie wykazano natomiast wpływu nałogu palenia tytoniu i dodatniego wywiadu rodzinnego
w kierunku nadciśnienia tętniczego.
Wnioski Wyniki badania wykazały dość częste występowanie
nadciśnienia tętniczego u młodych osób
dorosłych (blisko 15%), wśród czynników ryzyka
znalazły się otyłość i niewłaściwa dieta. Uzasadnia to
celowość prowadzenia badań przesiewowych w kierunku
nadciśnienia wśród studentów i promowania
zasad zdrowego żywienia w tej grupie populacyjnej.Background Elevated blood pressure value and its elongated
duration increases the risk of associated complications.
It underlines the necessity of detection of hypertension
at the early stage, especially in young adults, when the
prevention is effective. It was estimated that the prevalence
of hypertension in Poland was 20-30%, but this value is
inadeqate in young adults.
The aim of the study was to assess basic risk factors and the
prevalence of hypertension in medical students, aged 18–25.
Material and methods The cross-sectional study involved
a BP measurement, in accordance with Polish Society of
Hypertension and European Society of Hypertension guidelines,
and the questionnaire estimating basic risk factors of
hypertension in 532 students, aged 18–25. The statistical
analysis was performed with EpiInfo 6.0 procedures.
Results Optimal BP was recognized in 39.3% and hypertension
in 14.8% students (both more frequently in men,
p < 0.001). The prevalence of isolated systolic hypertension
was 3.7% (more frequently in men, p < 0.05). Hypertensive
subjects were more frequently overweight than students
with normal BP (p < 0.05). Young adults with hypertension
more frequently used diet rich in fat than
students with normal BP (p < 0.05). Additionally, smoking
habit and family history of hypertension did not increase
the risk of hypertension.
Conclusions The results revealed frequent incidence of hypertension
in young adults (about 15%). The risk factors
were obesity and improper diet. It justifies the necessity of
cross-sectional studies in young adults to detect hypertension
and to promote healthy lifestyle in young people
Case report of a woman with monoclonal gammapathy and papillary thyroid carcinoma, diagnosed because of detection of CHEK2 (I157T) mutation in genetic examinations
The CHEK2 gene encodes the CHK2 protein, which is kinase involved in DNA repair processes. By activating a lot of cell substrates, it can
regulate the cell cycle, demonstrates suppressive effects, and participates in the senescence and apoptosis processes. Mutations in the
CHEK2 gene are associated with increased risk of numerous cancers. The case described herein is that of a woman with a missense
mutation that results in the substitution of isoleucine for threonine at position 157. This variant of the mutation doubles the risk of papillary
thyroid carcinoma two times and causes up to 9% of these cancer. It is also associated with a two-fold increased risk of cancers of the kidney
(10%), colon (10%), and ovary (10% - G1), a 1.6-fold increased risk of prostate cancer (8% of all of them and 12% of familiar ones), and a 1.5-fold increased risk of breast cancer (7%). The screening procedures were initiated in a carrier who revealed papillary thyroid carcinoma.
Genetic screening of the family diagnosed her daughter as the carrier of this mutation. Until now no active cancer disease has been
recognized in the daughter. On the example of the presented case we discuss indications for screening in cases of positive family history.
The group especially predisposed seem to be patients with at least two coexisting carcinomas. Having diagnosed the mutation, it is
necessary to do genetic screening of family members. Continuous oncological observation of the carriers of CHEK 2 mutation is essential.

bierze udział w regulacji cyklu komórkowego, wykazuje działanie supresyjnie, wpływa również na proces apoptozy i starzenia
się komórek. Mutacje genu CHEK2 są związane z ryzykiem licznych nowotworów. Opisywany przypadek chorej dotyczy mutacji typu
missens, gdzie dochodzi do zamiany izoleucyny na treoninę w pozycji 157. Mutacja ta zwiększa 2-krotnie ryzyko raka brodawkowatego
tarczycy. Predysponuje do występowania 2-krotnie częściej raka nerki (10%), jelita grubego (10%), jajnika (10% - G1), 1,6-krotnie częściej
raka prostaty (8% wszystkich i 12%występujących rodzinnie) oraz 1,5-krotnie częściej raka piersi (7%). Na podstawie diagnostyki
w kierunku predysponowanych nowotworów wykryto u chorej raka brodawkowatego tarczycy. Skrining genetyczny rodziny pozwolił
na wykazanie nosicielstwa tej mutacji u córki pacjentki - dotychczas nie stwierdzono u niej czynnej choroby nowotworowej. Opisywany
przypadek chorej wskazuje na celowość przeprowadzania badań genetycznych w przypadku dodatniego wywiadu rodzinnego w kierunku
chorób nowotworowych. Szczególnie predysponowaną grupą wydają się być chorzy z co najmniej dwoma współistniejącymi
nowotworami. Wykazanie mutacji nakłada obowiązek badań genetycznych u pozostałych członków rodziny, jak również bezterminowy

Regression towards the mean in studies of arterial blood pressure
Regresja w kierunku wartości średniej (RTM, regression
towards the mean) jest zjawiskiem statystycznym,
które występuje w badaniach ciśnienia
tętniczego niezależnie od analizowanego problemu
badawczego, wieku badanych, metody pomiaru
lub czasu prowadzonej obserwacji. Problem
ten pojawia się zawsze, gdy dobór uczestników
do grupy badanej nie jest randomizowany. Regresja
w kierunku wartości średniej jest tym większa,
im słabsza jest korelacja między ocenianymi
zmiennymi. Spostrzeżenie to ma istotne implikacje
w badaniach populacyjnych wykorzystujących
pomiar przygodnego ciśnienia tętniczego,
gdyż w największym stopniu może skutkować
błędną interpretacją otrzymanych rezultatów.
Wartość RTM stanowi problem zwłaszcza w ocenie
skuteczności terapii hipotensyjnej oraz w ocenie
rozpowszechnienia nadciśnienia tętniczego.
Mimo że eliminacja efektu RTM nie jest często
możliwa, warto uwzględnić ten problem w metodologii
i odnieść się do niego w dyskusji uzyskanych
wyników.Regression towards the mean (RTM) is a statistical
phemonenon that occurs in blood pressure studies independently
of the problem or duration of the research, age
of the studied population, or the method being used. RTM
is observed whenever the study group is not randomized.
It is assumed that the less correlated the two variables are
the greater the RTM is. The infuence of RTM should be
considered mostly in casual blood pressure measurements
because of imprecise assessment of the results. Consequently,
RTM may be mistakenly attributed to a treatment
effect and the prevalence of hypertension. Despite the entire
elimination of RTM can be impossible, it is suggested
to remind this problem to the reader either in the section of
„materials and methods” or „discussion”
Białowieża Forest (NE-Poland) and Valday NP (NW-Russia) – biogeographical characteristics of eutrophic deciduous forests
During geobotanical studies in the north-eastern border of hemi-boreal zone, in Valday (NW Russia), rare eutrophic deciduous forests dominated by oak Quercus robur were observed. A comparison of these forests with the model of European deciduous forest in Białowieża National Park (NE-Poland) indicates a great similarity. Therefore, eutrophic deciduous forests in Valday can be classified to the Querco-Fagetea class, the Fagetalia sylvaticae order and to the Capinion betuli alliance, despite the absence of hornbeam Carpinus betulus in the region. Rarity of eutrophic deciduous forests in Valday region results probably from strong anthropogenic pressure in the past
Psychologiczna ocena pacjentów z tachyarytmiami nadkomorowymi leczonych ablacją przeznaczyniową
Wstęp: U chorych z częstoskurczami nadkomorowymi (SVT) po udanej ablacji
przeznaczyniowej przy uzyciu prądu o wysokiej częstotliwości (RFA) wykazywano
istotną poprawę jakości życia.
Cel pracy: Ocena wpływu RFA na obraz samego siebie i poziom lęku pacjentów
z SVT.
Materiał i metody: Do badania włączono 52 pacjentów (w tym 25 kobiet),
w średnim wieku 43 lata (18–75 lat), poddanych ablacji SVT. Wskazaniami do RFA
były: zespół Wolffa-Parkinsona- White’a (WPW) - u 28 chorych, częstoskurcz nawrotny
w obrębie łącza przedsionkowokomorowego (AVNRT) - u 20 pacjentów i migotanie przedsionków
(AF) - u 4 osób. Oceniano wpůyw RFA na obraz samego siebie i poziom lęku u pacjentów
poddanych temu zabiegowi. Badania wykonano przed zabiegiem ablacji oraz 3 miesiące
po RFA. Do badania poziomu lęku użyto inwentarza stanu i cechy lęku (STAI). Do
oceny obrazu samego siebie wykorzystano test przymiotnikowy Gougha i Heilbruna
(ACL), a do badania nastroju - krótką skalę oceny nastroju (SMCS). Wyniki analizowano,
używając testu t-Studenta, wartoąśi p Ł 0,05 uznano za znamienne.
Wyniki: Wstępne wyniki RFA były dobre u wszystkich pacjentów. Po 3 miesiącach
obserwacji po zabiegu zanotowano nawrót SVT u 3 pacjentów. Jedynym istotnym powikłaniem
RFA było wystąpienie bloku przedsionkowo-komorowego III° w 1. dobie po ablacji
AVNRT. U tego chorego wszczepiono układ stymulujŕcy serce typu VDD. W 3. miesiącu
po zabiegu zanotowano istotną redukcj poziomu lęku, mierzonego za pomocą kwestionariusza
STAI (lęk jako: stan, p < 0,002; cecha, p < 0,02). Niektóre aspekty obrazu siebie
("heteroseksualność" i "feminizm") uległy istotnym zmianom (p < 0,03; p < 0,002).
Pogorszenie obrazu samego siebie oraz podwyższenie poziomu lęku obserwowano u
3 pacjentów z nawrotem SVT w 3. miesiącu po RFA.
Wnioski: Mimo krótkiego okresu obserwacji u pacjentów po ablacji SVT zanotowano
istotną redukcję poziomu lęku oraz pozytywne zmiany w obrazie samego siebie
