Based on the cumulative data of scientific research reference values for the central blood pressure and its amplification were proposed in patients with different cardiovascular risk factors. Still, the possibility of its practical implementation in patients of different age groups has not been fully studied. There were examined 132 untreated patients (23 females and 109 males) with uncomplicated essential hypertension, which were divided into 3 groups depending on age (WHO, 2012): I group included 47 patients of the young age, II group - 50 middle-aged patients  and III group – 35 elderly patients. Measurements of central systolic blood pressure (cSBP), SBP amplification (SBP Amp), pulse wave velocity (PWV) were carried out by Arteriograph Tensioclinic (Tensiomed, Hungary). Target-organ damage included LVH which was determined by echocardiographic left ventricular mass index (LVMI), adjusted for body surface (g/m2) or height (LVMI2,7), aortic PWV>10 m/s. It was established that the use of a personalized approach in the examination of patients with arterial hypertension makes it possible to increase the efficiency of diagnosis of hypertensive-mediated subclinical organ damage. So in order to identify impairment of the vascular wall in patients of a young age with hypertension, with overweight and obesity, the reference values of PWV and cSBP in accordance with the age norm should be taken into account. It was particularly in this cohort of patients that the greatest adverse effect of vascular stiffness and body mass index (BMI) on the formation of left ventricular hypertrophy was revealed, while in elderly people it is necessary to use the reference values of cSBP, depending on the degree of hypertension

The paper discusses the problem of e-learning accessibility for persons with disabilities. Development of e-learning content accessible to all students is gaining special relevance in the context of global trends in higher education related to the massive transition of learning to distance and hybrid formats. Based on the analysis of the official websites of 25 world’s top universities (according to the QS World University Rankings 2021), data on university digital accessibility services were systematized. The data search was carried out by total browsing of university sites’ structure and headings and by keywords. Data on 14 checklist categories were registered. Signs of administrative and project activities in the field of digital accessibility were found for 20 universities (80%), among which 10 (40%) have specialized structural subdivisions for digital accessibility. The content of digital accessibility web services contains data on the following topics: training in digital accessibility for educational content for authors and developers (14 university sites – 56%); a guide for authors (15–60%) and web developers (12–48%); consulting and assistance for authors and web developers (17–68%); accessibility examination (5–20%); legal norms (11–44%) and accessibility standards (16–64%); accessibility testing guide (16–64%); procurement requirements for accessible web products and services (8–32%); frequently asked questions (6–24%), etc. Based on the results of the analysis, the model of university digital accessibility services was developed and corresponding web site was published. It seems important to create similar web resources and corresponding structural subdivisions in universities of the Russian Federation, as well as to approve the intra-university local acts regulating the mandatory fulfillment of digital accessibility requirements in the development of educational content and the procurement of digital products and services. GOST R 52872-2019, based on WCAG 2.1, should be used as the basis for local legal regulations

The article provides a practical example of the positive experience of the students of the medical academy in the international social project on combating arterial hypertension in order to improve the level of general competence and effective intellectual and professional socialization in getting higher medical education. The phenomenon of professional socialization should be considered along with the concepts of professional orientation, professional development, professional adaptation and professional becoming. Professional socialization is the ultimate link in the professional way of the individual. The algorithm of professional socialization of students in higher education consists of the following stages: professional orientation → professional development → professional adaptation → professional becoming → professional socialization. It should be noted that a personality who successfully socializes, has a well-defined social orientation, actively strives to self-determination and self-realization in society and is able of productive adaptation to the surrounding society on the basis of knowledge about social reality. In this case, the success of socialization is connected not only with the presence of specified qualities in the person, but, first of all, with the real actions in which these qualities are realized. By mastering the roles system, students acquire socially significant qualities, develop their worldview, their goals, motives, interests, feelings, personal and socially important needs, develop different types of competencies, including social competence, which will allow them to become what they want to become. Professional development of young talented individuals is possible only in open communication and interaction systems, it is in the international social projects of medical societies that constantly exchange energy and information with the environment, and at the same time, being a source of development and prospects for preserving the life of nation. Such a model of education supposes changing the role of teacher: the transition to joint actions with the student in new situations in an open, changing, modern world

Background. The chances of fatal outcome in out-of-hospital cardiac arrest are exceeding 90%. However, the early initiation of bystander cardiopulmonary resuscitation (CPR) substantially improves the probability of survival. The study was aimed to determine the extent of community CPR training, level of CPR knowledge, willingness and motivation to learn CPR among the population of the Crimea.Materials and methods. The representative sample of adult residents of the Crimean Peninsula (n=384) has been surveyed by means of individual structured interview from November 2017 to January 2018. The results were analyzed with social statistics.Results. According to the survey, 53% of respondents were previously trained in CPR. The training was performed mainly (82%) at work, school, college/technical school or university, or when acquiring a driver's license. The majority of females, people over 60, unemployed and retired, widowed and those with monthly income lower than 20,000 roubles are not trained. Of previously trained, 44% respondents attended a single CPR course, 72% were trained more than one year ago, 47% of participants had no previous training in CPR, mostly never thinking about the need to go for training. Being dependent from previous CPR training, the knowledge of CPR is generally poor: the proportions of correct answering as of the proper location and rate of chest compressions were 46% and 4%, respectively. Among the respondents, 56% expressed their willingness to attend CPR training. The main motivating factors to attend CPR training were awareness of importance of CPR training, potential health problems in relatives/friends and free-of-charge training.Conclusion. The Crimean population is insufficiently and non-uniformly trained in CPR, has limited knowledge of CPR and low motivation to learn. In order to increase the commitment of the community to provide first aid in out-of-hospital cardiac arrest, mass CPR training programs should be implemented with active involvement of the least trained and motivated social strata.

We conducted the meta-analysis of morbidity by infections associated with giving medical care in hospitals of Russia. A total of 23 publications were included in the review, and random effects model by the maximum likelihood method was applied to summarize their results. High heterogeneity of sample estimates was revealed, which is apparently associated with different approaches to the identification and synthesis of cases. The highest morbidity rates were recorded among patients of emergency department and intensive care (353 ‰, 95 % CI = 268–439) and patients undergoing surgery on the abdominal organs (194, 163–224); infections of the bloodstream (36, 28–43 ‰), postoperative pneumonia (26, 22–31 ‰), PSI (purulent septic infections) of newborns (30, 27–32 ‰) and new mothers (23, 20–27 ‰) were less commonПроведен мета-анализ заболеваемости инфекциями, связанными с оказанием медицинской помощи, в стационарах России.
