    Secondary implementation of interactive engagement teaching techniques: Choices and challenges in a Gulf Arab context

    We report on a "Collaborative Workshop Physics" instructional strategy to deliver the first IE calculus-based physics course at Khalifa University, UAE. To these authors' knowledge, this is the first such course on the Arabian Peninsula using PER-based instruction. A brief history of general university and STEM teaching in the UAE is given. We present this secondary implementation (SI) as a case study of a novel context and use it to determine if PER-based instruction can be successfully implemented far from the cultural context of the primary developer and, if so, how might such SIs differ from SIs within the US. With these questions in view, a pre-reform baseline of MPEX, FCI, course exam and English language proficiency data are used to design a hybrid implementation of Cooperative Group Problem Solving. We find that for students with high English proficiency, normalized gain on FCI improves from = 0.16+/-0.10 pre- to = 0.47+/-0.08 post-reform, indicating successful SI. We also find that is strongly modulated by language proficiency and discuss likely causes. Regardless of language skill, problem-solving skill is also improved and course DFW rates drop from 50% to 24%. In particular, we find evidence in post-reform student interviews that prior classroom experiences, and not broader cultural expectations about education, are the more significant cause of expectations at odds with the classroom norms of well-functioning PER-based instruction. This result is evidence that PER-based innovations can be implemented across great changes in cultural context, provided that the method is thoughtfully adapted in anticipation of context and culture-specific student expectations.     • Acute Kidney Injury (AKI) is defined using widely accepted international criteria that are based on changes in serum creatinine concentration and degree of oliguria. • AKI, when defined in this way, has a strong association with poor patient outcomes, including high mortality rates and longer hospital admissions with increased resource utilisation and subsequent chronic kidney disease. • The detection of AKI using current criteria can assist with AKI diagnosis and stratification of individual patient risk. • The diagnosis of AKI requires clinical judgement to integrate the definition of AKI with the clinical situation, to determine underlying cause of AKI, and to take account of factors that may affect performance of current definitions

    Risk perception of blood transfusions – a comparison of patients and allied healthcare professionals

    Background: Due to an increasing demand in health care services plans to substitute selective physician-conducted medical activities have become attractive. Because administration of a blood transfusion is a highly standardized procedure, it might be evaluated if obtaining a patient’s consent for a blood transfusion can be delegated to allied healthcare professionals. Physicians and patients perceive risks of transfusions differently. However, it is unknown how allied healthcare professionals perceive risks of transfusion-associated adverse events. Methods: Patients (n = 506) and allied healthcare professionals (n = 185) of an academic teaching hospital were asked to quantify their concerns about transfusions including five predefined transfusion-associated risks and their incidences. Results: Blood transfusions were considered to be generally harmful by 10.9% of patients and 14.6% of caregivers (P = 0.180). Among all surveyed patients, 36.8% were worried about infection-transmissions (caregivers: 27.6%; P = 0.024). Compared to 5.4% of caregivers, 13.6% of patients believed infection-transmission was a frequent complication (P = 0.003). Caregivers ranked the risks of receiving an AB0-mismatch transfusion (caregivers: 29.7% vs. patients: 19.2%, P = 0.003) or a transfusion-associated allergic reaction (caregivers: 17.3% vs. patients: 11.1%, P = 0.030) significantly higher than patients and were aware of the high incidence of transfusion-associated fever (caregivers: 17.8% vs. patients: 8.3%, P < 0.001). Conclusion: A significant part of interviewees perceived transfusions as a general health hazard. Patients perceived infection-transmissions as the most frequent and greatest transfusion-associated threat while caregivers focused on fatal AB0-mismatch transfusions and allergic reactions. Understanding the patients’ main concerns about blood transfusions and considering that these concerns might differ from the view of healthcare professionals might improve the process of shared decision making.     Laser cooling with intermediate state of spin–orbit coupling of LuF molecule

    Abstract This work presents a theoretical study of the laser cooling feasibility of the molecule LuF, in the fine structure level of approximation. An ab-initio complete active space self-consistent field (CASSCF)/MRCI with Davidson correction calculation has been done in the Λ(±) and Ω(±) representations. The corresponding adiabatic potential energy curves and spectroscopic parameters have been investigated for the low-lying electronic states. The calculated values of the internuclear distances of the X3Σ0+ and (1)3Π0+ states show the candidacy of the molecule LuF for direct laser cooling. Since the existence of the intermediate (1)3Δ1 state cannot be ignored, the investigation has been done by taking into consideration the two transitions (1)3Π0+−(1)3Δ1 and (1)3Π0+ −X3Σ0+. The calculation of the Franck–Condon factors, the radiative lifetimes, the total branching ratio, the slowing distance, and the laser cooling scheme study prove that the molecule LuF is a good candidate for Doppler laser cooling
