    One-Parameter Squeezed Gaussian States of Time-Dependent Harmonic Oscillator and Selection Rule for Vacuum States

    By using the invariant method we find one-parameter squeezed Gaussian states for both time-independent and time-dependent oscillators. The squeezing parameter is expressed in terms of energy expectation value for time-independent case and represents the degree of mixing positive and negative frequency solutions for time-dependent case. A {\it minimum uncertainty proposal} is advanced to select uniquely vacuum states at each moment of time. We show that the Gaussian states with minimum uncertainty coincide with the true vacuum state for time-independent oscillator and the Bunch-Davies vacuum for a massive scalar field in a de Sitter spacetime.Comment: 13 Pages, ReVTeX, no figure

    Medicinal leeches and the microsurgeon: A four-year study, clinical series and risk benefit review

    Background: There are case reports and small series in the literature relating to the use of medicinal leeches by plastic surgeons; however, larger series from individual units are rare. The aim of this article is to present a comprehensive 4-year case series of the use of medicinal leeches, discuss the current evidence regarding indications, risks, and benefits and highlight the recent updates regarding leech speciation. Methods: Patients prescribed leeches in a 4-year period (July 2004–2008) were collated from hospital pharmacy records (N = 35). The number of leeches used, demographic, clinical, and microbiological details were retrospectively analyzed. Results: Thirty-five patients were treated with leeches. The age range was 2 to 98 years (mean = 49.3). Leeches were most commonly used for venous congestion in pedicled flaps and replantations. Blood transfusions were necessary in 12 cases (34%) [mean = 2.8 units, range 2–5 units]. Our infection rate was 20% (7/35) including five infections with Aeromonas spp. (14.2%). The proportion of patients becoming infected after leech therapy was significantly greater in the group of patients that did not receive prophylactic antibiotic treatment (Fisher's Exact test P = 0.0005). In total, 14 cases (40%) were salvaged in entirety, in 7 cases 80% or more, in 2 cases 50 to 79%, and in 1 case less than 50% of the tissues were salvaged. In 11 cases (31%), the tissues were totally lost. Conclusion: Our study highlights both the benefits and the risks to patients in selected clinical situations and also the potential risks. The routine use of antibiotic prophylaxis is supported. In view of the emerging evidence that Hirudo verbana are now used as standard leech therapy, and the primary pathogen is Aeromonas veronii, until a large prospective multicenter study is published, large series of patients treated with leeches should be reported. © 2011 Wiley-Liss, Inc.     Social representations of HIV/AIDS in five Central European and Eastern European countries: A multidimensional analysis

    Cognitive processing models of risky sexual behaviour have proliferated in the two decades since the first reporting of HIV/AIDS, but far less attention has been paid to individual and group representations of the epidemic and the relationship between these representations and reported sexual behaviours. In this study, 494 business people and medics from Estonia, Georgia, Hungary, Poland and Russia sorted free associations around HIV/AIDS in a matrix completion task. Exploratory factor and multidimensional scaling analyses revealed two main dimensions (labelled ‘Sex’ and ‘Deadly disease’), with significant cultural and gender variations along both dimension scores. Possible explanations for these results are discussed in the light of growing concerns over the spread of the epidemic in this region

    Tissue engineering and regenerative medicine (TERM) has caused a revolution in present and future trends of medicine and surgery. In different tissues, advanced TERM approaches bring new therapeutic possibilities in either general population as in young patients and high-level athletes, improving restoration of biological functions and rehabilitation. The mainstream components required to obtain a functional regeneration of tissues may include biodegradable scaffolds, drugs or growth factors and different cell types (either autologous or heterologous) that can be cultured in bioreactor systems (in vitro) prior implantation into the patient. Particularly in the ankle, that is subject of many different injuries (e.g. acute, chronic, traumatic, degenerative), there is still no definitive and feasible answer to “conventional” methods. This review aims to provide current concepts of TERM applications to ankle injuries under pre-clinical and/or clinical research applied to skin, tendon, bone and cartilage problems.     Don't Stop Thinking About Leptoquarks: Constructing New Models

    We discuss the general framework for the construction of new models containing a single, fermion number zero scalar leptoquark of mass 200220\simeq 200-220 GeV which can both satisfy the D0/CDF search constraints as well as low energy data, and can lead to both neutral and charged current-like final states at HERA. The class of models of this kind necessarily contain new vector-like fermions with masses at the TeV scale which mix with those of the Standard Model after symmetry breaking. In this paper we classify all models of this type and examine their phenomenological implications as well as their potential embedding into SUSY and non-SUSY GUT scenarios. The general coupling parameter space allowed by low energy as well as collider data for these models is described and requires no fine-tuning of the parameters.Comment: Modified text, added table, and updated reference

    State business: gender, sex and marriage in Tajikistan

    This article examines the relation of the state to masculinity and sexuality by way of an exploration of the sexual problems of a young man and his wife in Tajikistan at the end of the Soviet era. It suggests that the regime’s inattention to this kind of issue was bound up with the importance to the state of projecting appropriate versions of masculinity. It further posits the idea that the continued refusal of the independent Tajik state to offer appropriate treatments for sexual dysfunction is consistent with the image of modernity President Rahmon wishes to present to the world. The article shows that as masculinity discursively occupies the superior gender position, with men expected to dominate, the state is itself impotent to respond when they are, in fact, unable to do so in sexual practice. However, the myth of male dominance persists to the point that it may prevent women from seeing beyond their subordination and finding mutually beneficial solutions in their familial and sexual relationships

    Much Ado About Leptoquarks: A Comprehensive Analysis

    We examine the phenomenological implications of a 200 GeV leptoquark in light of the recent excess of events at HERA. Given the relative predictions of events rates in e^+p versus e^-p, we demonstrate that classes of leptoquarks may be excluded, including those contained in E_6 GUT models. It is shown that future studies with polarized beams at HERA could reveal the chirality of the leptoquark fermionic coupling and that given sufficient luminosity in each e^\pm_{L,R} channel the leptoquark quantum numbers could be determined. The implications of 200-220 GeV leptoquarks at the Tevatron are examined. While present Tevatron data most likely excludes vector leptoquarks and leptogluons in this mass region, it does allow for scalar leptoquarks. We find that while leptoquarks have little influence on Drell-Yan production, further studies at the Main Injector are possible in the single production channel. We investigate precision electroweak measurements as well as the process e^+e^-\to q\bar q at LEP II and find they provide no further restrictions on these leptoquark models. We then ascertain that cross section and polarization asymmetry measurements at the NLC provide the only direct mechanism to determine the leptoquark's electroweak quantum numbers. The single production of leptoquarks in \gamma e collisions by both the backscattered laser and Weisacker-Williams techniques at the NLC is also discussed. Finally, we demonstrate that we can obtain successful coupling constant unification in models with leptoquarks, both with or without supersymmetry.     Fluoroquinolones and isoniazid-resistant tuberculosis: implications for the 2018 WHO guidance.

    INTRODUCTION: 2018 World Health Organization (WHO) guidelines for the treatment of isoniazid (H)-resistant (Hr) tuberculosis recommend a four-drug regimen: rifampicin (R), ethambutol (E), pyrazinamide (Z) and levofloxacin (Lfx), with or without H ([H]RZE-Lfx). This is used once Hr is known, such that patients complete 6 months of Lfx (≥6[H]RZE-6Lfx). This cohort study assessed the impact of fluoroquinolones (Fq) on treatment effectiveness, accounting for Hr mutations and degree of phenotypic resistance. METHODS: This was a retrospective cohort study of 626 Hr tuberculosis patients notified in London, 2009-2013. Regimens were described and logistic regression undertaken of the association between regimen and negative regimen-specific outcomes (broadly, death due to tuberculosis, treatment failure or disease recurrence). RESULTS: Of 594 individuals with regimen information, 330 (55.6%) were treated with (H)RfZE (Rf=rifamycins) and 211 (35.5%) with (H)RfZE-Fq. The median overall treatment period was 11.9 months and median Z duration 2.1 months. In a univariable logistic regression model comparing (H)RfZE with and without Fqs, there was no difference in the odds of a negative regimen-specific outcome (baseline (H)RfZE, cluster-specific odds ratio 1.05 (95% CI 0.60-1.82), p=0.87; cluster NHS trust). Results varied minimally in a multivariable model. This odds ratio dropped (0.57, 95% CI 0.14-2.28) when Hr genotype was included, but this analysis lacked power (p=0.42). CONCLUSIONS: In a high-income setting, we found a 12-month (H)RfZE regimen with a short Z duration to be similarly effective for Hr tuberculosis with or without a Fq. This regimen may result in fewer adverse events than the WHO recommendations

    The impact of migration on the sexual health, behaviours and attitudes of Central and East European gay/bisexual men in London

    Extensive social psychological research emphasises the importance of groups in shaping individuals' thoughts and actions. Within the child sexual abuse (CSA) literature criminal organisation has been largely overlooked, with some key exceptions. This research was a novel collaboration between academia and the UK's Child Exploitation and Online Protection Centre (CEOP). Starting from the premise that the group is, in itself, a form of social situation affecting abuse, it offers the first systematic situational analysis of CSA groups. In-depth behavioural data from a small sample of convicted CSA group-offenders (n =3) were analysed qualitatively to identify factors and processes underpinning CSA groups' activities and associations: group formation, evolution, identity and resources. The results emphasise CSA groups' variability, fluidity and dynamism. The foundations of a general framework are proposed for researching and assessing CSA groups and designing effective interventions. It is hoped that this work will stimulate discussion and development in this long-neglected area of CSA, helping to build a coherent knowledge-base
