Pulsations and Long-Term Light Variability of Three Candidates to Protoplanetary Nebulae
We present new photometric data and analysis of the long-duration UBV
photoelectric observations for three candidates to protoplanetary objects -
F-supergiants with IR-excesses located at large galactic latitudes, IRAS
18095+2704, IRAS 19386+0155, and IRAS 19500-1709. All three stars have revealed
quasiperiodic low-amplitude variabilities caused by pulsations observed against
the long-term trends of brightnesses. For IRAS 18095+2704=V887 Her we have
found a pulsation period of 109 days and a linear trend of brightness under the
constant colours if being averaged over the year timescale. The light curve of
IRAS 19386+0155=V1648 Aql over 2000-2008 can be approximated by a wave with a
main period of 102 days which is modulated by close frequency, with a period of
98 days, that results in brightness oscillations with a variable amplitude.
V1648 Aql has also shown synchronous reddening together with a persistent rise
of brightness in the V-band. IRAS 19500-1709=V5112 Sgr experiences irregular
pulsations with the periods of 39 and 47 days. The long-term component of the
variability of V5112 Sgr may be related to the binary character of this star.Comment: 11 pages, 6 figures, accepted for publication in Pis'ma Astron. Z

Iron metabolism in tuberculosis and iron-containing chemotherapeutic drugs for its treatment
This review included the Russian and international articles on the iron metabolism in tuberculosis and the use of iron-containing drugs in the treatment of tuberculosis over the past 20 years. The main topics covered by the researchers include the features of iron metabolism in mycobacteria, the varieties and pathogenesis of anemia that can develop in tuberculosis: iron deficiency (absolute iron deficiency), associated with a chronic disease (relative iron deficiency) or drug-induced anemia (siderohrestic, hemolytic, aplastic). The possible correction of treatment regimens for tuberculosis is analyzed with the introduction of a complex compound of iron with isoniazid in order to reduce undesirable adverse reactions to isoniazid .

Epidemiology, practice of ventilation and outcome for patients at increased risk of postoperative pulmonary complications
BACKGROUND Limited information exists about the epidemiology and outcome of surgical patients at increased risk of postoperative pulmonary complications (PPCs), and how intraoperative ventilation was managed in these patients.
OBJECTIVES To determine the incidence of surgical patients at increased risk of PPCs, and to compare the intraoperative ventilation management and postoperative outcomes with patients at low risk of PPCs.
DESIGN This was a prospective international 1-week observational study using the ‘Assess Respiratory Risk in Surgical Patients in Catalonia risk score’ (ARISCAT score) for PPC for risk stratification.
PATIENTS AND SETTING Adult patients requiring intraoperative ventilation during general anaesthesia for surgery in 146 hospitals across 29 countries.
MAIN OUTCOME MEASURES The primary outcome was the incidence of patients at increased risk of PPCs based on the ARISCAT score. Secondary outcomes included intraoperative ventilatory management and clinical outcomes.
RESULTS A total of 9864 patients fulfilled the inclusion criteria. The incidence of patients at increased risk was 28.4%. The most frequently chosen tidal volume (VT) size was 500 ml, or 7 to 9 ml kg1 predicted body weight, slightly lower in patients at increased risk of PPCs. Levels of positive end-expiratory pressure (PEEP) were slightly higher in patients at increased risk of PPCs, with 14.3% receiving more than 5 cmH2O PEEP compared with 7.6% in patients at low risk of PPCs (P < 0.001). Patients with a predicted preoperative increased risk of PPCs developed PPCs more frequently: 19 versus 7%, relative risk (RR) 3.16 (95% confidence interval 2.76 to 3.61), P < 0.001) and had longer hospital stays. The only ventilatory factor associated with the occurrence of PPCs was the peak pressure.
CONCLUSION The incidence of patients with a predicted increased risk of PPCs is high. A large proportion of patients receive high VT and low PEEP levels. PPCs occur frequently in patients at increased risk, with worse clinical outcome

Only through trials, it becomes possible to obtain reliable data for the inclusion of new drugs in practice, which are designed to save millions of lives. The question of the ethics of conducting research in the context of the COVID-19 pandemic is very relevant at the moment, when it is necessary to make quick decisions in the interests of all mankind, assessing all possible risks that they may entail
PROGNOSTIC SIGNIFICANCE OF TISSUE DOPPLER IMAGING IN PATIENTS WITH HYPERTROPHIC CARDIOMYOPATHY
Objective: to assess the value of tissue Doppler (TD) imaging in predicting the clinical course of patients with hypertrophic cardiomyopathy (HCM). Materials and Methods: clinical and demographic data were analyzed in 212 subjects with HCM (141 males and 107 females, mean-age 45 yo). Median follow-up was 3,2 years (0,7-12,5 years). Patients were stratified into two groups: with present or absent adverse outcomes. Results: over the follow-up period, 7 fatal cases were registered due to SCD, and 4 successfully resuscitated subjects who experienced SCD received implantable pacemaker, 6 patients developed a stroke, cardiovascular death (1 patient) or hospitalization due to worsening of heart failure symptoms developed 7 patients. Rearrangement of the RNA polymerase subunit H and the lower jaw in archaeal elongation complexes
The lower jaws of archaeal RNA polymerase and eukaryotic RNA polymerase II include orthologous subunits H and Rpb5, respectively. The tertiary structure of H is very similar to the structure of the C-terminal domain of Rpb5, and both subunits are proximal to downstream DNA in pre-initiation complexes. Analyses of reconstituted euryarchaeal polymerase lacking subunit H revealed that H is important for open complex formation and initial transcription. Eukaryotic Rpb5 rescues activity of the ΔH enzyme indicating a strong conservation of function for this subunit from archaea to eukaryotes. Photochemical cross-linking in elongation complexes revealed a striking structural rearrangement of RNA polymerase, bringing subunit H near the transcribed DNA strand one helical turn downstream of the active center, in contrast to the positioning observed in preinitiation complexes. The rearrangement of subunits H and A′′ suggest a major conformational change in the archaeal RNAP lower jaw upon formation of the elongation complex
