    Possible Role of the Endocannabinoid System in Tourette Syndrome

    Tourette syndrome (TS) is a neuropsychiatric disorder with childhood onset. The core symptoms are motor and vocal tics. The majority of patients also suffer from psychiatric comorbidities. The pathophysiology of TS is not clear, but changes in different neurotransmitter systems—in particular the dopaminergic system—have been confirmed. Since there is increasing evidence that cannabis-based medicine (CBM) is effective in the treatment of TS, an involvement of the endocannabinoid system in the pathophysiology of TS has been suggested. The purpose of this chapter is to present existing evidence suggesting a pathophysiological role of the endocannabinoid system in TS and to summarize available data on beneficial treatment effects of CBM in patients with TS

    Does transversus abdominis function correlate with prone plank and bench bridge holding time in club cricket players?

    Background: Bridge and plank holding times are used to evaluate core stability. Transversus abdominis (TA) muscle function is assessed using ultrasound and also provides input on an individual’s core stability. Objectives: A correlation study comparing TA muscle function with bridge and plank holding time in club cricketers. Methods: Seventeen male, premier league cricketers (age: 22.1 ± 3.3 years) participated in this study. Ultrasound was used to measure bilateral TA, internal oblique (OI) and external oblique (OE) muscle thickness at rest and during abdominal hollowing. Muscle function was measured by means of a Pearson’s correlation as the change in muscle thickness from rest to abdominal hollowing and compared to holding time of the bench bridge and prone plank (seconds). Results: TA muscle thickness was preferentially recruited bilaterally (p=0.00001) during abdominal hollowing. No significant correlations were found between TA muscle function and holding time for the bench bridge (dominant (D): r = 0.03 [95% CI:-0.46-0.50]; non-dominant (ND): r = -0.02 [95% CI:-0.50-0.47]) or prone plank (D: r = -0.16 [95% CI:-0.60-0.34]; ND: r = -0.13 [95% CI:-0.57-0.38]). Conclusion: Prone plank and bench bridge holding times are not correlated with TA muscle function during abdominal hollowing. Core stability cannot rely on a single test to evaluate its effectiveness. In particular, the contribution of the local and global muscle system to ‘core stability’ needs to be evaluated independently. Therefore these tests are not sensitive enough to evaluate the contribution of the local muscle system to the global muscle system in a healthy, pain free, sporting population

    Revisiting the understanding of "transactional sex" in sub-Saharan Africa: A review and synthesis of the literature.

    In sub-Saharan Africa, young women ages 15-24 have more than twice the risk of acquiring HIV as their male counterparts. A growing body of epidemiological evidence suggests that the practice of "transactional sex" may contribute to this disparity. Over the last 15 years, the social sciences have contributed significantly to understanding the meaning of and motivations for this practice. The findings from these studies are rich, but varied, rendering lessons difficult to navigate for intervention and further research. We therefore contribute a historically-grounded, comprehensive literature review on the nature and motivations for women's participation in transactional sex in sub-Saharan Africa. Drawing from over 300 studies (through 2014), we distill three prominent paradigms observed in the literature that we review toward presenting a unified conceptualization of the practice. "Sex for basic needs," the first paradigm, positions women as victims in transactional sexual relationships, with implications for interventions that protect girls from exploitation. In contrast, the "sex for improved social status" paradigm positions women as sexual agents who engage in transactional sex toward attaining a middle-class status and lifestyle. Finally, a third paradigm, "sex and material expressions of love," draws attention to the connections between love and money, and the central role of men as providers in relationships. We find important commonalities in the structural factors that shape the three paradigms of transactional sex including gender inequality and processes of economic change. We suggest that there are three continua stretching across these paradigms: deprivation, agency, and instrumentality. This review proposes a definition of transactional sex and discusses implications for research and interventions aiming to reduce young women's risk of HIV through such relationships. We consider the consequences of drawing from too narrow an understanding of the practice, and highlight the benefits of a broader conceptualization

    Complete Mitogenomes of Two Aragoa Species and Phylogeny of Plantagineae (Plantaginaceae, Lamiales) Using Mitochondrial Genes and the Nuclear Ribosomal RNA Repeat

    Aragoa, comprising 19 high-altitude North Andean species, is one of three genera in the Plantagineae (Plantaginaceae, Lamiales), along with Littorella and Plantago. Based primarily on plastid data and nuclear ITS, Aragoa is sister to a clade of Littorella + Plantago, but Plantagineae relationships have yet to be assessed using multigene datasets from the nuclear and mitochondrial genomes. Here, complete mitogenomes were assembled for two species of Aragoa (A. abietina and A. cleefii). The mitogenomes of both species have a typical suite of genes for 34 proteins, 17 tRNAs, and three rRNAs. The A. abietina mitogenome assembled into a simple circular map, with no large repeats capable of producing alternative isoforms. The A. cleefii mitogenomic map was more complex, involving two circular maps bridged by a substoichiometric linear fragment. Phylogenetics of three mitochondrial genes or the nuclear rRNA repeat placed Aragoa as sister to Littorella + Plantago, consistent with previous studies. However, P. nubicola, the sole representative of subg. Bougueria, was nested within subg. Psyllium based on the mitochondrial and nuclear data, conflicting with plastid-based analyses. Phylogenetics of the nuclear rRNA repeat provided better resolution overall, whereas relationships from mitochondrial data were hindered by extensive substitution rate variation among lineages

    Parental screening measures for language development in Russian

    While a variety of well-established standardised language assessment tools exist in English-speaking countries, only very a few standardised tests with clear norms are available in Russian. The aim of the present study was to contribute to a further development of tests of language ability for monolingual Russian-speaking children. One way to screen young children for a developmental language disorder is by the means of parental questionnaires. However, no such questionnaires are currently standardised for Russian language. We assessed which of the two parent-reported questionnaires, the Russian adaptation of the Children’s Communication Checklist-2 (CCC-2) or the Russian version of the 8-item questionnaire assessing ‘Current Language Skills’ is a more reliable predictor of children’s performance on direct measures of language. The two composite scores available in the CCC-2 (General Communication Composite (GCC), a measure of formal/structural language, and the General Pragmatic Composite (GenPragC), a measure of pragmatic competence) as well as the ‘Current Language Skills’ measure were correlated with the results of a direct assessment of structural and pragmatic language. 19 monolingual typically-developing Russian-speaking children aged between 4;0 and 6;8 years and their parents participated in the study. A strong relationship was found between the parent-reported ‘Current Language Skills’ questionnaire and a direct measure of expressive vocabulary (a Russian version of the Cross Linguistic Lexical Tasks (CLTs), noun production subtest). These results suggest that further investigation is warranted into establishing the validity of a Russian adaptation of the parental questionnaire assessing ‘Current Language Skills’ as a screening tool for a language disorder

    A Critical Examination of the Clinical Diagnosis of Functional Tic‐like Behaviors

    Background: Since the COVID‐19 pandemic, movement disorder clinics have seen an increase in patients with an unusual type of tic‐like symptoms: young adults with abrupt onset complex behaviors. It was quickly suspected that these patients suffered from functional neurological symptoms, later named Functional Tic‐Like Behaviors (FTLB). Subsequent research on the differential diagnosis between FTLB and tics has been substantial and led to the development of diagnostic checklists. Objectives: We conducted a theoretical reappraisal of the FTLB literature to clarify the validity of the concept and its diagnostic implications. Methods: This paper addresses several key aspects of the current FTLB literature: circular reasoning, the complications of the FTLB phenomenology and demographics, the impact of FTLB on tic literature at large, and issues with alignment of the FTLB concept with the diagnostic criteria for functional disorders. Results: The clinical approach to FTLB might involve circular reasoning due to a lack of clinical benchmarks. The FTLB phenomenology and demographics may need more work to ensure a lack of bias and a proper description of this patient group including a clear distinction from tics. The impact of the FTLB discussion on the wider literature needs consideration. The validation of positive signs may help with both these endeavors and pave way to the inclusion of FTLB within psychiatric classification systems. Furthermore, the coexistence of FTLB and tics within the same patient needs to be addressed. Conclusion: More research may be needed to fully establish the diagnosis of FTLB and differentiate it from tics

    European clinical guidelines for Tourette syndrome and other tic disorders:summary statement

    In 2011 a working group of the European Society for the Study of Tourette syndrome (ESSTS) developed the first European Guidelines for Tourette syndrome (TS) published in the ECAP journal. After a decade ESSTS now presents updated guidelines, divided into four sections: Part I: assessment, Part II: psychological interventions, Part III: pharmacological treatment and Part IV: deep brain stimulation (DBS). In this paper, we summarise new developments described in the guidelines with respect to assessment and treatment of tics. Further, summary findings from a recent survey conducted amongst TS experts on these same topics are presented, as well as the first European patient representative statement on research. Finally, an updated decision tree is introduced providing a practical algorithm for the treatment of patients with TS. Interestingly, in the last decade there has been a significant shift in assessment and treatment of tics, with more emphasis on non-pharmacological treatments

    School related factors and 1yr change in physical activity amongst 9-11 year old English schoolchildren.

    BACKGROUND: Activity levels are known to decline with age and there is growing evidence of associations between the school environment and physical activity. In this study we investigated how objectively measured one-year changes in physical activity may be associated with school-related factors in 9- to 10-year-old British children. METHODS: Data were analysed from 839 children attending 89 schools in the SPEEDY (Sport, Physical Activity, and Eating behaviours: Environmental Determinants in Young People) study. Outcomes variables were one year changes in objectively measured sedentary, moderate, and vigorous physical activity, with baseline measures taken when the children were 9-10 years old. School characteristics hypothesised to be associated with change in physical activity were identified from questionnaires, grounds audits, and computer mapping. Associations were examined using simple and multivariable multilevel regression models for both school (9 am - 3 pm) and travel (8-9 am and 3-4 pm) time. RESULTS: Significant associations during school time included the length of the morning break which was found to be supportive of moderate (β coefficient: 0.68 [p: 0.003]) and vigorous (β coefficient: 0.52 [p: 0.002]) activities and helps to prevent adverse changes in sedentary time (β coefficient: -2.52 [p: 0.001]). During travel time, positive associations were found between the presence of safe places to cross roads around the school and changes in moderate (β coefficient: 0.83 [p:0.022]) and vigorous (β coefficient: 0.56 [p:0.001]) activity, as well as sedentary time (β coefficient: -1.61 [p:0.005]). CONCLUSION: This study suggests that having longer morning school breaks and providing road safety features such as cycling infrastructure, a crossing guard, and safe places for children to cross the road may have a role to play in supporting the maintenance of moderate and vigorous activity behaviours, and preventing the development of sedentary behaviours in children.RIGHTS : This article is licensed under the BioMed Central licence at http://www.biomedcentral.com/about/license which is similar to the 'Creative Commons Attribution Licence'.
