    Drug policy history, design and practice: introduction

    The history, policies and practice of drug control in Asia have been historically multifaceted, particularly concerning substances like opium, cannabis, and various indigenous psychoactive plants. The opium trade, notably in the 19th and early 20th centuries, significantly impacted Asian societies, triggering conflicts, influencing international relations, and altering the socio-economic fabric. Yet, the history of drug control across different parts of Asia reflects a complex interplay of factors and a stark regional diversity. This special issue serves as a platform for interdisciplinary studies that link Asian drug trafficking with collaborative legal responses across the region. The primary objective is to compile an overview of the history, current practices, and policies addressing drug production, trafficking, and usage in a continent that houses 60% of the global population. This is done by sampling articles that encompass the large geographical scope of Asia, from Northeast Asia to the Middle East. The special issue focuses on three key dimensions of drug control that affect Asian countries: Historical landmarks, including milestones of drug control policy developments at the national level, which shaped the international regime over the last two centuries; public health and history of local responses with the analysis of the burden of infectious diseases, and the state of access to controlled essential medicines; and, criminal justice and historical landmarks of its development through legal responses and punishments

    Cost-effectiveness comparison between palpation- and ultrasound-guided thyroid fine-needle aspiration biopsies

    <p>Abstract</p> <p>Background</p> <p>The aim of this study is to perform a cost-effectiveness comparison between palpation-guided thyroid fine-needle aspiration biopsies (P-FNA) and ultrasound-guided thyroid FNA biopsies (USG-FNA).</p> <p>Methods</p> <p>Each nodule was considered as a case. Diagnostic steps were history and physical examination, TSH measurement, Tc<sup>99m </sup>thyroid scintigraphy for nodules with a low TSH level, initial P-FNA versus initial USG-FNA, repeat USG-FNA for nodules with initial inadequate P-FNA or USG-FNA, hemithyroidectomy for inadequate repeat USG-FNA. American Thyroid Association thyroid nodule management guidelines were simulated in estimating the cost of P-FNA strategy. American Association of Clinical Endocrinologists guidelines were simulated for USG-FNA strategy. Total costs were estimated by adding the cost of each diagnostic step to reach a diagnosis for 100 nodules. Strategy cost was found by dividing the total cost to 100. Incremental cost-effectiveness ratio (ICER) was calculated by dividing the difference between strategy cost of USG-FNA and P-FNA to the difference between accuracy of USG-FNA and P-FNA. A positive ICER indicates more and a negative ICER indicates less expense to achieve one more additional accurate diagnosis of thyroid cancer for USG-FNA.</p> <p>Results</p> <p>Seventy-eight P-FNAs and 190 USG-FNAs were performed between April 2003 and May 2008. There were no differences in age, gender, thyroid function, frequency of multinodular goiter, nodule location and diameter (median nodule diameter: 18.4 mm in P-FNA and 17.0 mm in USG-FNA) between groups. Cytology results in P-FNA versus USG-FNA groups were as follows: benign 49% versus 62% (p = 0.04), inadequate 42% versus 29% (p = 0.03), malignant 3% (p = 1.00) and indeterminate 6% (p = 0.78) for both. Eleven nodules from P-FNA and 18 from USG-FNA group underwent surgery. The accuracy of P-FNA was 0.64 and USG-FNA 0.72. Unit cost of P-FNA was 148 Euros and USG-FNA 226 Euros. The cost of P-FNA strategy was 534 Euros and USG-FNA strategy 523 Euros. Strategy cost includes the expense of repeat USG-FNA for initial inadequate FNAs and surgery for repeat inadequate USG-FNAs. ICER was -138 Euros.</p> <p>Conclusion</p> <p>Universal application of USG-FNA for all thyroid nodules is cost-effective and saves 138 Euros per additional accurate diagnosis of benign versus malignant thyroid nodular disease.</p> <p>Trial registration</p> <p>ClinicalTrials.gov, NCT00571090</p

    Electron beam crosslinked natural rubber/multiwalled carbon nanotube nanocomposite

    The physical properties of the rubber blends are influenced by vulcanization and filler distribution. Normally, rubbers are vulcanized by systems based on sulfur or peroxide with the most common filler carbon black. Radiation can also produce crosslink densities like those obtained by sulphur curing, but the net effects, are similar, though not identical. The type of crosslink formed in this method (–C–C–) give rise to better mechanical properties at higher temperature. This work reports on the investigations carried out on natural rubber (SMR) filled with the multiwall carbon nanotubes (MWCNTs). This system of SMR/MWCNTs was subjected to different radiation dosages and compared with nonradiated samples in order to determine the improvement in mechanical properties of the rubber system in the presence of MWCNTs and irradiation dosages. The amount of MWCNTs in this study was varied from 1 to 7 Phr and the irradiation doses were varied from 50 to 200 KGy. Mechanical properties, especially, tensile strength (TS), elongation at break had been studied as a function of irradiation dose and degree of loading with MWCNTs. Gel fraction indicated an increase in the degree of crosslink with the increase in the MWCT and radiation dosage. XRD was carried out to check the increase in the crytallinty of the nanocomposite system. The overall results obtained indicate significant improvement in the mechanical and thermal properties by radiation crosslinking in presence of MWCNTs.     Use of an amplatzer vascular plug in embolization of a pulmonary artery aneurysm in a case of hughes-stovin syndrome: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Hughes-Stovin syndrome is a rare condition characterized by peripheral deep venous thrombosis accompanied by single or multiple pulmonary arterial aneurysms. The limited number of cases has precluded controlled studies of the management of pulmonary artery aneurysms, which usually cause massive hemoptysis leading to death. This is the first report of a new endovascular treatment of a single large pulmonary arterial aneurysm.</p> <p>Case presentation</p> <p>An 18-year-old Caucasian man was referred to our department with recurrent severe hemoptysis. His medical history included Hughes-Stovin syndrome diagnosed during a recent hospital admission. The patient was initially treated with corticosteroids. Because of his recurrent hemoptysis, we decided to embolize a 3.5 cm pulmonary arterial aneurysm using an Amplatzer Vascular Plug. The procedure was not complicated, and the patient's post-intervention course was uneventful. The patient has remained free from any complications of the embolization 36 months after the procedure.</p> <p>Conclusion</p> <p>Percutaneous embolization of a single large pulmonary artery aneurysm with an Amplatzer Vascular Plug in a patient with Hughes-Stovin syndrome is a less invasive procedure that represents the best multidisciplinary approach in treating these patients.</p

    Brokering Gender Empowerment in Energy Access in the Global South

    How do energy professionals in the Global South facilitate the brokerage of gender equity and empowerment in energy access? Energy sector professionals, including planners and members of non-governmental organisations (NGOs), are crucial development actors in off-grid contexts. They operate at the intersection between grassroots-level energy access in off-grid household and community buildings and overarching policy frameworks. However, despite their central role, the relationship between their professional practices and gender empowerment in energy access has received little attention. This paper investigates ‘energy brokers’ across Ghana, India, Nigeria and Pakistan based on interviews (n = 86). Subsequent thematic analysis explores these energy brokers’ overarching understandings of gender equity and empowerment, their agency, and brokering practices for energy access (including in relation to emerging energy technologies). Analysis shows ‘differentiated brokerage’ in that energy professionals from the NGOs and the delivery sectors are often better positioned to broker gender equity and women’s empowerment in energy access. However, linkages between equitable access and empowerment need to be better understood, especially at the ‘top’ and go beyond women’s economic productivity. Women’s participation across supply chains of emerging energy-access technology, in energy governance and as energy brokers needs strengthening. Practice relevance Energy professionals occupy an important ‘middle’ position and can help to create changes to overcome gender bias in access to energy. They facilitate the brokerage (understandings, agency and practices) of gender equity and empowerment in energy access in off-grid contexts, including household and community buildings. The evidence from this study shows the performance of energy professionals is critical in facilitating women’s empowerment in energy access. Key recommendations are: (1) energy professionals at the top need to recognise differentiated brokerage across the grassroots–policy spectrum to better identify and equip key actors; (2) energy brokers need to move beyond gender neutrality and economic participation acting on the breadth of women’s empowerment, including psychological dimensions; and (3) women’s participation across energy system transitions needs to be strengthened, with regard to energy supply chains, energy governance and as energy brokers

    Spontaneous Eosinophilic Nasal Inflammation in a Genetically-Mutant Mouse: Comparative Study with an Allergic Inflammation Model

    Background: Eosinophilic inflammation is a hallmark of chronic rhinosinusitis with nasal polyps. To model this disease process experimentally, nasal sensitization of mice with ovalbumin or aspergillus has been described. Here, we describe a genetically mutant mouse that develops robust spontaneous nasal eosinophilic inflammation. These mice lack the enzyme SHP-1 that down-regulates the IL-4Ra/stat6 signaling pathway. We compared nasal inflammation and inflammatory mediators in SHP-1 deficient mice (mev) and an ovalbumin-induced nasal allergy model. Methods: A novel technique of trans-pharyngeal nasal lavage was developed to obtain samples of inflammatory cells from the nasal passages of allergic and mev mice. Total and differential cell counts were performed on cytospin preparations. Expression of tissue mRNA for IL-4, IL-13, and mouse beta-defensin-1 (MBD-1) was determined by quantitative PCR. Eotaxin in the lavage fluid was assessed by ELISA. Results: Allergic and mev mice had increased total cells and eosinophils compared with controls. Expression of IL-4 was similarly increased in both allergic and mev mice, but expression of IL-13 and eotaxin was significantly greater in the allergic mice than mev mice. Eotaxin was significantly up-regulated in both allergic rhinitis and mev mice. In both models of eosinophilic inflammation, down-regulation of the innate immune marker MBD-1 was observed.     miR-141 mediates recovery from acute kidney injury

    Acute kidney injury (AKI) is a global clinical problem characterised by a sudden decline in renal function and mortality as high as 60%. Current AKI biomarkers have limited ability to classify disease progression and identify underlying pathological mechanisms. Here we hypothesised that alterations in urinary microRNA profiles could predict AKI recovery/nonrecovery after 90 days, and that injury-specific changes would signify microRNA mediators of AKI pathology. Comparison of urinary microRNA profiles from AKI patients with controls detected significant injury-specific increases in miR-21, miR-126 and miR-141 (p < 0.05) and decreases in miR-192 (p < 0.001) and miR-204 (p < 0.05). Expression of miR-141 increased in renal proximal tubular epithelial cells (PTECs) under oxidative stress in vitro and unilateral ischaemic reperfusion injury in vivo. Forced miR-141 expression in the presence of H2O2 increased PTEC death and decreased cell viability. Of nine messenger RNA targets with two or more miR-141 3’-untranslated region binding sites, we confirmed protein tyrosine phosphatase receptor type G (PTPRG) as a direct miR-141 target in PTECs. PTPRG-specific siRNA knockdown under oxidative stress increased PTEC death and decreased cell viability. In conclusion, we detected significant alterations in five urinary microRNAs following AKI, and identified proximal tubular cell PTPRG as a putative novel therapeutic target
