    Evaluation of the traditional and revised world health organization classifications of dengue cases in Brazil

    OBJECTIVE: Dengue is a worldwide public health problem with approximately 50 million cases reported annually. The World Health Organization proposed a revised classification system in 2008 to more effectively identify the patients who are at increased risk of complications from dengue. Few studies have validated this new classification system in clinical practice. We conducted a cross-sectional study of patients hospitalized for dengue in Dourados, Mato Grosso do Sul, Brazil, to evaluate the capacity of the two classification systems for detecting severe cases of dengue. MATERIALS AND METHODS: We conducted a cross-sectional study of survey data from the medical records of patients admitted to the University Hospital of the Federal University of Grande Dourados under clinical suspicion of dengue during an epidemic from September 2009 to April 2010. RESULTS: The distribution of patients according to the traditional classification system was as follows: dengue fever, 150/181 (82.9%); dengue hemorrhagic fever, 27/181 (14.9%); and dengue hemorrhagic shock, 4/181 (2.2%). Using the revised classification system, the distribution was as follows: dengue without warning signs, 45/181 (24.3%); dengue with warning signs, 107/181 (59.1%); and severe dengue, 29/181 (15.6%). Of the 150 patients classified as having dengue fever, 105 (70%) were reclassified as having dengue with warning signs or severe dengue. CONCLUSION: These data demonstrate that the revised classification system has greater discriminatory power for detecting patients at risk of progression to severe disease and those needing hospitalization

    Impact of the anatomical location, alcoholism and smoking on the prevalence of advanced oral cancer in Brazil

    To evaluate the prevalence of oral cancer in Brazil according to the clinical stage, anatomical location, alcoholism and smoking. Data referring to 31,217 cases of oral cancer, from 2000 to 2010, were obtained from the Integrator Module of the Hospital Registry of Cancer. Inconsistent data (?non-classified? cases) was eliminated and 21,160 cases were analyzed. The frequency distribution according to clinical stage, anatomical location, alcoholism and smoking was analyzed descriptively and through a binary logistic regression model (?<0.05). The clinical stage (dependent variable) was dichotomized in early stage (I and II) or advanced stage (III and IV). The year of diagnosis, anatomical location and deleterious habits (alcoholism and smoking) were considered independent variables. The most frequent characteristics were: oropharynx location (n=3856, 18.41%), clinical stage IV (n=11924, 56.09%) and combined use of alcohol and tobacco (n=19226; 61.59%). The year 2009 (p<0.01, PR = 1.162, CI-95%=1.053-1.283) and location at the base of tongue (p<0.01, PR = 2.485, CI-95% = 2.182-2.807) presented a higher prevalence ratio for advanced stage oral cancer. The combined use of alcohol and tobacco showed a higher prevalence rate for the advanced clinical stage of cancer (p<0.01, PR =1.449, CI-95%=1.382-1.520) if compared to individuals without habits, or just alcoholics. Higher prevalence of advanced stage of oral cancer is related to the localization at the base of the tongue and to the concomitant use of alcohol and tobacco. Therefore, it can be suggested that all these characteristics lead to a worse prognosis of oral cancer

    Uso de análogos de GLP-1 no manejo da Síndrome dos Ovários Policísticos (SOP): uma revisão integrativa

    A síndrome dos ovários policísticos (SOP) é ​​considerada a patologia endócrina mais comum em mulheres em idade reprodutiva em todo o mundo, com uma prevalência descrita entre 5% e 15%, variando conforme os critérios diagnósticos aplicados. Uma nova abordagem da SOP diz respeito aos análogos do receptor do peptídeo 1 semelhante ao glucagon (GLP-1) para a redução de peso em pacientes acometidos pela doença. O presente estudo de revisão buscou avaliar novos estudos acerca da eficácia dos análogos de GLP-1 em pacientes com síndrome dos ovários policísticos, a partir de ensaios clínicos publicados na literatura médica atual. Trata-se de uma pesquisa de revisão integrativa realizada por meio da base de dados PubMed, que levou em consideração os seguintes critérios de inclusão: ensaios clínicos randomizados; artigos publicados nos últimos 05 anos (2019-2024); que possuíam texto completo disponível e que abordassem acerca do uso de análogos de GLP-1 em pacientes com síndrome dos ovários policísticos. Ficou destacado o potencial promissor da combinação de terapias, com metformina e análogos do GLP-1 como dulaglutida, liraglutida e exenatida, além de dieta de restrição calórica, no tratamento de mulheres com síndrome dos ovários policísticos (SOP) e obesidade. Os benefícios observados em todos os estudos recentes incluem uma redução significativa no peso corporal, melhorias nos níveis de glicose e sensibilidade à insulina

    Effects of enoxaparin on the healing of abdominal wall in rats: study on breaking strength and histopatology Efeitos da enoxaparina sobre a cicatrização da parede abdominal de ratos: estudo da força de ruptura e histopatológico

    PURPOSE: To evaluate the influence of enoxaparin on the abdominal wall healing. METHODS: Sixty Wistar male rats were randomly distributed into two experimental groups. Group E: consisted of 30 rats, submitted to abdominal suture and treated with enoxaparin in a dose of 1 mg/kg of body weight, subcutaneously administered from immediate postoperative until the 7th day; Group C: consisted of 30 rats submitted to abdominal suture and treated with the equivalent dose per volume of NaCl 0.9%. The animals were submitted to median laparotomy and were operated again at the 3rd, 7th or 14th postoperative day for the resection of the anterior abdominal wall. The tissue's stretching resistance was tested and a histopathological evaluation was performed. Through Mann-Whitney's U test, a statistical analysis was conducted considering significant values for p <0.05. RESULTS: The tensile breaking strength presented a median of 0.64N for group C and 0.41N for group E (p=0.60) at the 3rd postoperative day; of 2.27N for group C and of 4.32N for group E (p=0.01) at the 7th POD; of 5.72N for group C and of 6.62N for group E (p=0.97) at the 14th POD. In relation to the histopathological study, the results did not show significant statistical differences, except for the amount of collagen at the 7th POD, which medians were 2 on group C and 1 on group E (p=0.007). CONCLUSION: Considering the conditions in which this study was conducted, the administration of enoxaparin led to an strengthening of the tissue's breaking resistance at the 7th POD; on the histopathological exam, a significant difference was observed at the 7th POD, with a better collagen disposition on group C.<br>OBJETIVO: Avaliar a influência da enoxaparina, sobre a cicatrização da parede abdominal. MÉTODOS: Foram utilizados 60 ratos Wistar, distribuídos aleatoriamente em grupo estudo (E) e controle (C). O grupo E recebeu 1 mg/kg de enoxaparina, via subcutânea, do pós-operatório imediato até o 7º dia pós-operatório (DPO); o grupo C doses-volume equivalentes de NaCl 0,9%. Os animais foram submetidos à laparotomia mediana e reoperados no 3º, 7º ou 14º DPO, para ressecção da parede abdominal anterior. A resistência tênsil foi testada e feita avaliação histopatológica. Análise estatística realizada com o teste U de Mann-Whitney, considerando significantes valores de p<0,05. RESULTADOS: A força tênsil de ruptura apresentou mediana de 0,64N para grupo C e de 0,41N para grupo E (p=0,60) no 3º DPO; de 2,27N para grupo C e de 4,32N para grupo E (p=0,01) no 7º DPO; de 5,72N para grupo C e de 6,62N para grupo E (p=0,97) no 14º DPO. Quanto ao estudo histopatológico, os resultados não obtiveram diferenças estatísticas significantes, exceto pela disposição do colágeno no 7º DPO, cujas medianas foram 2 no grupo C e 1 no grupo E (p= 0,007). CONCLUSÃO: Nas condições em que foi realizado o presente estudo, a enoxaparina provocou aumento da força tênsil de ruptura e pior disposição do colágeno no 7º DPO
