    Constraint-based Query Distribution Framework for an Integrated Global Schema

    Distributed heterogeneous data sources need to be queried uniformly using global schema. Query on global schema is reformulated so that it can be executed on local data sources. Constraints in global schema and mappings are used for source selection, query optimization,and querying partitioned and replicated data sources. The provided system is all XML-based which poses query in XML form, transforms, and integrates local results in an XML document.     Prevalence of Vitamin B12 deficiency in patients of type 2 diabetes mellitus on metformin: A case control study from Pakistan

    Introduction: Diabetes Mellitus is the most common endocrine disorder and metformin is the most commonly prescribed oral hypoglycemicagent. Metformin is well known to cause viamin B12 deficiency due to effect on calcium-dependent membrane action in the terminal ileum leading to malabsorption of vitamin B12. The purpose of this study is to determine prevalence and associations of Vitamin B12 deficiency in patients of type 2 diabetes mellitus treated with metformin. Methods: This case control study was carried out in department of medicine, Combined Military Hospital, Kharian from 1st Jan 2012 to 30 december 2012.We enrolled 114 outdoor patients of type 2 diabetes mellitus currently on metformin for atleast 12 months, by consecutive sampling, and 105 age and sex matched patients taken as control. Patients with vitamin B12 levels of less than 150 pg/ml were said to be B12 deficient. The results were analyzed on SPSS version 16.Results: Serum B12 levels were low in 35 patients (31%) on metformin as compared to only 9 patients (8.6%) among controls,(p value 0.002). Mean B12 levels were significantly low in metformin group 311 pg/ml (±194.4), p value 0.03. Dose of metformin had inverse correlation with B12 levels and the difference was statistically significant with pvalue < 0.001.Conclusion: Our study demonstrated significantly high prevalence of  vitamin B12 deficiency in patients treated with metformin with significant effect of dose and duration of metformin use on B12 levels. Physicians must recognize this important fact and screen diabetics on metformin therapy for underlying B12 deficiency

    Terrace Soil Suitability for Highway Construction: Case Study in Lesser Himalaya (CPEC Project E-35), North Pakistan

    In this study, terrace soil investigation was carried out in project E-35 (phase-I) China-Pak Economic Corridor, Lesser Himalayas, North Pakistan. The methodology in current research is based on tests that include sieve analysis, plastic index, proctor, California Bearing Ratio, Los Angeles, sand equivalent and specific gravity. The results of these tests for different layers were compared with AASHTO and NHA specifications. The results show that the embankment, subgrade and subbase layers were composed of silt, sand and gravel, respectively while the aggregate base coarse was composed of sand, aggregate and less amount of fine clay material. The sieve analysis test shows that soil and aggregate base coarse has less clay with high silt, sandy material and index plastic to low plastic, which is appropriate for the construction. The California Bearing Ratio shows that the soil and aggregate base coarse have high load-bearing capacity. The Los Angeles abrasion reveal that the sub base and aggregate base coarse are resistive. The sand equivalent shows that aggregate base coarse has high sand material. The specific gravity illustrates that aggregate base coarse material is denser. The current study shows that terrace soil is suitable for the construction of the road in project E-35 (phase-I) China-Pak Economic Corridor

    Salvage of infected non-union of the tibia with an Ilizarov ring fixator.

    Abstract PURPOSE: To review outcomes of 24 patients who underwent Ilizarov ring fixation for infected nonunion of the tibia. METHODS: Medical records of 21 men and 3 women aged 13 to 74 (mean, 38) years who underwent Ilizarov ring fixation for infectednon-union of the tibia were reviewed. The mean bone defect was 3.3 (range, 2-5) cm. The mean time from injury to presentation was 11.9 (range, 1-36) months. The mean number of previous surgeries was 2 (range, 0-14). A local flap was used in 2 patients and a free flap was used in one patient. Nine of the patients underwent Ilizarov ring fixation without soft tissue and bony resection, as inadequate stability was the reason for non-union. Patients were assessed using the Association for the Study and Application of the Method of Ilizarov criteria. RESULTS: Patients were followed up for a mean of 11 (range, 8-46) months. Functional outcome was excellent in 8 patients, good in 12, fair in 2, and failure in one, whereas bone union outcome was excellent in 6 patients, good in 14, fair in one, and poor in 2. The mean time to union was 8 (range, 3-31) months. The mean external fixation index was 4.2 (range, 1.5-15.7) cm/month. Complications encountered were pin tract infection (n=5), re-fracture (n=2), soft tissue impingement by Ilizarov rings (n=2), recurrence of wound infection (n=1), mal-union (n=1), and mortality (n=1). CONCLUSION: Ilizarov ring fixation is a viable option for infected non-union of the tibia. Adequate assessment of bone union is crucial before removal of fixator to prevent re-fracture

    Comparison of Outcome of two different Methods for the treatment of Intra-articular fracture of Distal Radius

    Objective: It is to compare the radiological and functional outcome of two different fixation methods for displaced intra articular distal radius fracture (DRF): open reduction and internal fixation with anatomical plate and closed reduction with percutaneous K-wires. Methodology: A randomized controlled trial was conducted in Orthopaedics department, Shaikh Zayed Hospital, Lahore for one year; from April 2017 to March, 2018. Total 90 patients were selected in the study by ‘non-probability, consecutive sampling technique’ with 45 patients in each group of the study; Group-A (fixation method: Open reduction with internal fixation with volar plate) and Group-B (Fixation method: Kirschner-wire known as K-wire). Results: The mean age in the group A and group B was 53.7 ± 11.8 years and 55.2 ± 12.3 years respectively. There were 58 male and 32 females in the study. The differences in volar tilt, radial inclination, radial length and modified Mayo score were significantly better in group A than group B (p-value < 0.05). The differences were not significantly different for articular step off in the two study groups (p-value > 0.05). Conclusion: Open reduction with internal fixation using volar locking plates yielded significantly better radiological and functional outcomes than percutaneous fixation using K-wire in the 6 months’ postoperative period

    Big Data in the construction industry: A review of present status, opportunities, and future trends

    © 2016 Elsevier Ltd The ability to process large amounts of data and to extract useful insights from data has revolutionised society. This phenomenon—dubbed as Big Data—has applications for a wide assortment of industries, including the construction industry. The construction industry already deals with large volumes of heterogeneous data; which is expected to increase exponentially as technologies such as sensor networks and the Internet of Things are commoditised. In this paper, we present a detailed survey of the literature, investigating the application of Big Data techniques in the construction industry. We reviewed related works published in the databases of American Association of Civil Engineers (ASCE), Institute of Electrical and Electronics Engineers (IEEE), Association of Computing Machinery (ACM), and Elsevier Science Direct Digital Library. While the application of data analytics in the construction industry is not new, the adoption of Big Data technologies in this industry remains at a nascent stage and lags the broad uptake of these technologies in other fields. To the best of our knowledge, there is currently no comprehensive survey of Big Data techniques in the context of the construction industry. This paper fills the void and presents a wide-ranging interdisciplinary review of literature of fields such as statistics, data mining and warehousing, machine learning, and Big Data Analytics in the context of the construction industry. We discuss the current state of adoption of Big Data in the construction industry and discuss the future potential of such technologies across the multiple domain-specific sub-areas of the construction industry. We also propose open issues and directions for future work along with potential pitfalls associated with Big Data adoption in the industry

    Antihyperlipidemic and Hepatoprotective Activity of Dodonaea viscosa Leaves Extracts in Alloxan-Induced Diabetic Rabbits (Oryctolagus cuniculus)

    Hyperlipidemia is associated with diabetes and hepatotoxicity has emerged as a common clinical complication by antidiabetics. The present study was conducted to determine the antihyperlipidemic and hepatoprotective activity of Dodonaea viscosa leaves extracts in the alloxan-induced diabetic rabbits. The rabbits (n = 70) were divided into seven groups including normal and diabetic control, the remaining were aqueous, aqueous:methanol (70:30), aqueous:methanol (50:50), aqueous: methanol (30:70), and methanol extracts given for 30 days. Serum levels of triglycerides, total cholesterol, LDL-cholesterol, HDL-cholesterol, ALT and AST were estimated by using commercially available kits. The oral administration of aqueous:methanol (70:30) extract of the Dodonaea viscosa leaves significantly (P<0.01) decreased the raised parameters (triglyceride, total cholesterol and LDL-cholesterol) to normal values. But the extract has significantly increased HDL-cholesterol, ALT and AST levels. For the aqueous:methanol (70:30) extract given animals, the average serum level of total cholesterol was 60.00±1.30 mg/dL, LDL-cholesterol was 92.80±2.29 mg/dL, HDL-cholesterol was 31.80±1.0 mg/dL and triglyceride was 15.40±0.75 mg/dL while the average serum levels of ALT and AST were 45.60±3.08 and 27.20±1.36 IU/dL, respectively. It is concluded from the study that aqueous:methanolic (70:30) extract of Dodonaea viscosa leaves exerts antihy- perlipidemic and hepatoprotective effects in the alloxan-induced diabetic rabbits

    Global, regional, and national burden of diabetes from 1990 to 2021, with projections of prevalence to 2050: a systematic analysis for the Global Burden of Disease Study 2021

The corrected version first appeared at thelancet.com on September 28, 2023BACKGROUND : Diabetes is one of the leading causes of death and disability worldwide, and affects people regardless of country, age group, or sex. Using the most recent evidentiary and analytical framework from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD), we produced location-specific, age-specific, and sex-specific estimates of diabetes prevalence and burden from 1990 to 2021, the proportion of type 1 and type 2 diabetes in 2021, the proportion of the type 2 diabetes burden attributable to selected risk factors, and projections of diabetes prevalence through 2050. METHODS : Estimates of diabetes prevalence and burden were computed in 204 countries and territories, across 25 age groups, for males and females separately and combined; these estimates comprised lost years of healthy life, measured in disability-adjusted life-years (DALYs; defined as the sum of years of life lost [YLLs] and years lived with disability [YLDs]). We used the Cause of Death Ensemble model (CODEm) approach to estimate deaths due to diabetes, incorporating 25 666 location-years of data from vital registration and verbal autopsy reports in separate total (including both type 1 and type 2 diabetes) and type-specific models. Other forms of diabetes, including gestational and monogenic diabetes, were not explicitly modelled. Total and type 1 diabetes prevalence was estimated by use of a Bayesian meta-regression modelling tool, DisMod-MR 2.1, to analyse 1527 location-years of data from the scientific literature, survey microdata, and insurance claims; type 2 diabetes estimates were computed by subtracting type 1 diabetes from total estimates. Mortality and prevalence estimates, along with standard life expectancy and disability weights, were used to calculate YLLs, YLDs, and DALYs. When appropriate, we extrapolated estimates to a hypothetical population with a standardised age structure to allow comparison in populations with different age structures. We used the comparative risk assessment framework to estimate the risk-attributable type 2 diabetes burden for 16 risk factors falling under risk categories including environmental and occupational factors, tobacco use, high alcohol use, high body-mass index (BMI), dietary factors, and low physical activity. Using a regression framework, we forecast type 1 and type 2 diabetes prevalence through 2050 with Socio-demographic Index (SDI) and high BMI as predictors, respectively. FINDINGS : In 2021, there were 529 million (95% uncertainty interval [UI] 500–564) people living with diabetes worldwide, and the global age-standardised total diabetes prevalence was 6·1% (5·8–6·5). At the super-region level, the highest age-standardised rates were observed in north Africa and the Middle East (9·3% [8·7–9·9]) and, at the regional level, in Oceania (12·3% [11·5–13·0]). Nationally, Qatar had the world’s highest age-specific prevalence of diabetes, at 76·1% (73·1–79·5) in individuals aged 75–79 years. Total diabetes prevalence—especially among older adults—primarily reflects type 2 diabetes, which in 2021 accounted for 96·0% (95·1–96·8) of diabetes cases and 95·4% (94·9–95·9) of diabetes DALYs worldwide. In 2021, 52·2% (25·5–71·8) of global type 2 diabetes DALYs were attributable to high BMI. The contribution of high BMI to type 2 diabetes DALYs rose by 24·3% (18·5–30·4) worldwide between 1990 and 2021. By 2050, more than 1·31 billion (1·22–1·39) people are projected to have diabetes, with expected age-standardised total diabetes prevalence rates greater than 10% in two super-regions: 16·8% (16·1–17·6) in north Africa and the Middle East and 11·3% (10·8–11·9) in Latin America and Caribbean. By 2050, 89 (43·6%) of 204 countries and territories will have an age-standardised rate greater than 10%. INTERPRETATION : Diabetes remains a substantial public health issue. Type 2 diabetes, which makes up the bulk of diabetes cases, is largely preventable and, in some cases, potentially reversible if identified and managed early in the disease course. However, all evidence indicates that diabetes prevalence is increasing worldwide, primarily due to a rise in obesity caused by multiple factors. Preventing and controlling type 2 diabetes remains an ongoing challenge.
