    THE EFFECT OF AGRICULTURAL INTENSIFICATION ON ECOSYSTEM SERVICES AROUND IHEMI CLUSTER

    As the world population continue to increase, the demand for food also increases which necessitate the need for agricultural intensification. Agricultural intensification affects large parts of terrestrial area, therefore, assessment of its contribution to the decrease of ecosystem services is critical for successful conservation in the future. A study was conducted in five districts of Iringa and Njombe Regions, part of Ihemi cluster, to assess the effects of agricultural intensification on ecosystem services.  A total of 607 household surveys and 19 Focus Groups discussions were conducted. Descriptive and cross tabulation were used for quantitative analysis while content analysis was used for qualitative data. Findings reveal that there are several benefits communities get from the ecosystem that play a great role on their livelihood. Across all villages, firewood is preferred due to its affordability and availability. Other ecosystems goods such as traditional medicine and mushrooms are hardly available due to clearing of land for agricultural activities, as well as settlement expansion.  Community activities, such as valley bottom farming was mentioned as a practice that jeopardize the long-term sustainability of ecosystem resources within the Cluster. Agricultural intensification by investors was also mentioned as a sources of ecosystem depletion. Sustainable agricultural intensification, if adopted, might be one among the solutions to serve the ecosystem around the cluster

    Influence of Rice Husk Ash Density on the Workability and Strength of Structural Concrete

    Supplementary cementitious materials (SCMs) have been known to improve the properties of fresh and hardened concrete, and at the same time enhance the sustainability of concrete. Rice husk Ash (RHA), is one such material, but has neither been widely studied nor applied in practice. This work investigated the effect of the density of RHA on the workability and compressive strength of fresh and hardened RHA-replaced concrete respectively. Cement was replaced with RHA in concrete by weight (RHA-W) and by volume (RHA-V) at steps of 0%, 5%, 7.5%, 10%, 15%, 20%, 25% and 30%. The 0% replacement was used as the reference point from which performances were measured. Results showed that unlike the characteristic of other established pozzolans, RHA significantly reduced the workability of wet concrete and the rate of compressive strength gain over curing time due to a high water demand that is caused by the increased volume of replaced concrete, which results from its low density. Workability reduced with increased replacement for both RHA-W and RHA-V. Replacements of above 15% were not possible for the RHA-W due to the high water demand. However, replacements of up to 30% were achieved for the RHA-V. RHA-W specimens achieved lower compressive strengths and were observed to gain strength at a lower rate over the 28 to 91-days period of curing compared to RHA-V specimens. This behavior was attributed to the shortage of water that is necessary for the hydration of cement and subsequent pozzolanic reaction, which is the basis of the contribution that is made to the strength and performance of concrete by SCMs. However, the compressive strengths achieved were above the study’s target concrete strength of class C32/40 at 91 days, which is among those classes that are listed as being durable and suitable for structural applications.     Orbito-Maxillofacial Cutaneous Anthrax

    Background: Cutaneous anthrax is a zoonotic disease that spreads to humans primarily through exposure to infected animals, animal products or spore infested soil. Microbiological identification of cutaneous anthrax may be obscured by previous antibiotic use. Thorough history and clinical examination is therefore necessary in making a diagnosis. There is a paucity of reports about these infections in our region.Methods: Five cases are presented with a history of contact with animal or animal products and the typical appearance of skin lesions. Two cases were confirmed through microbiologic gram staining. All cases were treated with antibiotics and resolution of the initial cutaneous lesions was noted within three weeks.Conclusion: Thorough clinical history, examination and a high index of suspicion are paramount in making a diagnosis. The aim is to provide a picture of the challenges and opportunities in transforming the concept of sexual wellness currently being articulated. This is a move from the commonly held view of sexuality as a threat to health, to one promoting sexual wellness or positive view of sexuality.     Ecosystem health and sustainable agricultural development in Ihemi cluster

    Agricultural landscape provide a range of ecosystem services apart from producing crops. However, the inter-linkage of the ecosystem services and agricultural activities is poorly understood. For over 50 years, agriculture has been conducted without considering the natural ecological processes that safeguard agricultural production in the long run. To ensure that agricultural systems are sustainable, we have to make sure that the crucial ecosystem functions in the natural landscape are protected. Agricultural intensification, dramatic land use changes, application of agrochemicals and intensification of resource utilization are among the factors contributing towards biodiversity loss. The process of agricultural intensification is associated with an increase in labour inputs, increase use of natural and artificial fertilizer, use of improved seeds, change in technologies, change in agricultural mechanization & frequency of cultivation, changes to the landscape such as irrigation or soil conservation measures. The agricultural inputs, for instance, have altered the key-hydrological processes of rivers, lakes, floodplains and groundwater-fed wetlands, damaging their ecosystems and services that they provide. Agricultural intensification affects large parts of terrestrial area, therefore, assessment of its contribution to biodiversity loss is critical for successful conservation in the future.     Background: Clinical guidelines when implemented correctly have shown to improve disease outcomes. This study describes utilization of Kenya National guidelines in managing ante partum haemorrhage (APH) in 3rd trimester.Objective: To describe adherence to clinical guideline in management of antepartum haemorrhage at Garissa Provincial General Hospital Design: Crossectional mixed methods studySetting: Garissa Provincial General HospitalSubjects: Medical records of patients managed for APH between 2002 and 2012 and Key Informant Interviews (KIIs) of Health workers.Results: 36.1% of the cases assessed were managed with strict adherence to guidelines. 90% of health care workers had high levels of awareness of the existence of guidelines and sited utilization challenges attributed to resource inadequacies.Conclusion: Clinicians are skilled on APH guidelines, but adherence levels are still low. Therefore, continuous appraisal of clinical practices, availing equipment, facilities and supplies to reinforce adherence is recommended
