Introduction: A well-developed quality of life (QoL) instrument is valuable in identifying the burden of illness. We were interested in exploring whether existing QoL instruments were suitable for patients in our medical setting and, if not, whether this could be rectified by adapting an existing valid and reliable instrument to meet the specific needs of our patient population. For the purposes of this study, we chose to evaluate the quality of life of patients with breast cancer. Specifically, we were interested in two aspects of QoL in women with breast cancer. The first was whether existing instruments were pertinent to the women in our venue. The second research interest was dependent upon the first. If current instruments were found wanting, could this be rectified through the creation and validation of new domains of relevance to these patients? Method: First, five patients were interviewed to ascertain QoL issues pertinent to women in our medical setting. Second, to determine regional appropriateness of existing breast cancer QoL instruments, a search was conducted to identify and review existing breast cancer specific QoL instruments. Third, an addendum was created (to be used in conjunction with an existing instrument identified through the search) that contained three QoL domains not typically found: Financial, Spirituality and Satisfaction with Medical Care. The addendum was then tested along with an existing instrument (FACT-B). Results: Internal consistency for the new scales, Satisfaction with Medical Care, Spirituality, and Financial had alpha coefficients of 0.81, 0.80, and 0.63 respectively. The total score for FACT-B plus addendum was 0.69. Pearson’s correlation coefficients were 0.49 for Financial, 0.64 for Satisfaction with Medical Care, and 0.70 for Spirituality. Total test/retest was 0.71

Region-Specific Effects of Immunotherapy With Antibodies Targeting α-synuclein in a Transgenic Model of Synucleinopathy

Synucleinopathies represent a group of neurodegenerative disorders which are characterized by intracellular accumulation of aggregated α-synuclein. α-synuclein misfolding and oligomer formation is considered a major pathogenic trigger in these disorders. Therefore, targeting α-synuclein species represents an important candidate therapeutic approach. Our aim was to analyze the biological effects of passive immunization targeting α-synuclein and to identify the possible underlying mechanisms in a transgenic mouse model of oligodendroglial α-synucleinopathy. We used PLP-α-synuclein mice overexpressing human α-synuclein in oligodendrocytes. The animals received either antibodies that recognize α-synuclein or vehicle. Passive immunization mitigated α-synuclein pathology and resulted in reduction of total α-synuclein in the hippocampus, reduction of intracellular accumulation of aggregated α-synuclein, particularly significant in the spinal cord. Lowering of the extracellular oligomeric α-synuclein was associated with reduction of the density of activated iba1-positive microglia profiles. However, a shift toward phagocytic microglia was seen after passive immunization of PLP-α-synuclein mice. Lowering of intracellular α-synuclein was mediated by autophagy degradation triggered after passive immunization in PLP-α-synuclein mice. In summary, the study provides evidence for the biological efficacy of immunotherapy in a transgenic mouse model of oligodendroglial synucleinopathy. The different availability of the therapeutic antibodies and the variable load of α-synuclein pathology in selected brain regions resulted in differential effects of the immunotherapy that allowed us to propose a model of the underlying mechanisms of antibody-aided α-synuclein clearance

Nowadays, a plethora of Web-based environments (Web applications, Web platforms, etc.), publish their functions as RESTful services, i.e., self-contained and self-describing resources that follow the REpresentational State Transfer (REST) architectural style principles. As the Web has become a major medium of communication, integrating objects (e.g., smart devices) into the Web and taking advantage of its open popular standards has created an emerging trend: the Web of Things (WoT). In the WoT, objects expose their functions also as resources respecting the REST principles. Each resource provides well defined functions that meet specific users’ requests. However, there are cases in which a single resource is not sufficient to answer users’ requests, and often, combining two or more resources forming a resource composition, achieves the desired output. Nevertheless, several challenges are to be addressed when composing resources. In this thesis, we address three challenges. The first one consists on verifying the behavior of static resource compositions built manually by the user, as several design errors may occur (e.g., end-loops preventing other resources to run, and datatype mismatch between the Inputs/Outputs of the linked resources). For the other two challenges, the targeted Web environments are hybrid providing: (i) dynamic resources (connected to/removed from the environment at different instances), and (ii) static resources (established to be always available). The challenges focus respectively on the automatic resource discovery, while considering resource location (whenever exposed by objects), and the automatic selection of the appropriate resources to form suitable compositions satisfying users’ requests. To cope with these challenges, we first propose a formal model based on Colored Petri Nets (CPN) that maps resources behavior with their composition to CPN. This allows to use CPN behavioral properties to verify the correctness of static compositions behavior. Then, we propose a formal graph representation linking static resource to dynamic ones, allowing adapted graph algorithms to explore the semantically annotated descriptions of the traversed graph resources, in order to identify automatically the required resources. The resource discovery process uses an original defined indexing schema that allows identifying the resources based on their location (if exposed by objects), and enhancing resource search in large Web environments connecting many resources. As for automatic resource selection, we present a Selection Strategy Adaptor that selects the suitable resources to form several compositions with different implementation alternatives, taking into account Quality of Resource (QoR), Inputs/Outputs matching of related resources, as well as resource availability. Our proposal is generic as it can be applicable in Web environments belonging to different application domains. However, in this thesis, it has been illustrated inthe smart buildings domain, more particularly, in projects for managing buildings’ energy behavior.
