    Single Versus Multi‑visit Endodontic Treatment of Teeth with Apical Periodontitis: An In vivo Study with 1‑year Evaluation

    Background: Apical periodontitis (AP) is one of the most prevalent diseases of the teeth. Treatment of AP is based on the removal of the cause, i.e., bacteria from the root canals. Achievement of adequate bacterial eradication in one appointment treatment remains a controversy.Aim: This prospective study was conducted with the objective to compare the periapical healing of teeth with AP treated in (a) single visit versus (b) two visits, either with or without Vitapex as an intracanal medicament.Subjects and Methods: Patients were selected randomly from the Department of Conservative Dentistry and Endodontics. Forty‑three patients (81 teeth) met the inclusion criteria, i.e., AP (both symptomatic and asymptomatic) visible radiographically size ≥2 mm × 2 mm, not suffering from any immune‑compromising disease, age between 16 and 65 years and tooth not accessed previously. Patients were randomly divided into three groups, i.e., single‑visit group (Group 1), multi‑visit group without any intracanal medicament (Group 2), and multi‑visit group with Vitapex as interim intracanal medicament (Group 3). Comparison was done radiographically using periapical index (PAI). The primary outcome measure was the change in periapical radiolucency after 1 year assessed by PAI scores. The Mann–Whitney U‑test was used to evaluate differences between groups at baseline (immediate postoperative) and at the 12‑month follow‑up evaluation. Change in PAI score for each group from baseline to 12‑month follow‑up evaluation was tested with the Wilcoxon signed rank test. The secondary outcome measures, proportion of teeth in each group that could be considered improved (decreased PAI score) or healed (PAI <2), were assessed with the Chi‑square test.Results: No statistically significant difference in periapical healing was found between three groups.Conclusion: After 1‑year evaluation, no difference in periapical healing was found between single‑visit treatment and multi‑visit treatment groups with the given sample size.Keywords: 1‑year evaluation, Apical periodontitis, Multi‑visit treatment, Single visit, Vitape

    Pattern of regional metastasis in papillary thyroid cancer: our experience of 86 cases

    Background: Papillary thyroid cancer (PTC) have a high propensity for regional metastasis which ranges from 30- 80%. The objective of the study is to assess the pattern of lymph node metastasis and to plan the extent of neck dissection accordingly. Though central neck dissection (CND) is routinely done in PTC but the indication of extent of  neck dissection is still controversial.Methods: The medical records of   86 patients with PTC  who underwent total thyroidectomy (TT) and neck dissection at Dr. B. Borooah Cancer Institute(BBCI) from January 2010 to  December 2014 were retrospectively reviewed.Results: Out of 86 patients 22 were males and 64 were females. The median age of presentation was 40.0 years. 43 out of 86 patients (50%) had cervical lymph node metastasis. Ipsilateral nodal metastasis was found in 37 patients (43.0%) and contralateral metastasis was found in only 6 patients (7.0%).Tumors with size more than 3cm had ipsilateral nodal metastasis in 21(56.7%) patients which is statistically significant (p 0.03).A strong association was found between level VI and the ipsilateral group of lymph nodes involving level II,III,IV and V.Conclusions: Majority of patients present with multiple level nodal metastasis, with the central compartment commonly involved. In view of the high incidence of metastatic lymph nodes in levels II, III, IV and level VI ,our study  supports the recommendation  for posterolateral  and anterior  neck dissection in patients with clinically positive neck  nodes and tumor with aggressive criteria

    Clockwise rotation of the Brahmaputra Valley relative to India: Tectonic convergence in the eastern Himalaya, Naga Hills, and Shillong Plateau

    GPS data reveal that the Brahmaputra Valley has broken from the Indian Plate and rotates clockwise relative to India about a point a few hundred kilometers west of the Shillong Plateau. The GPS velocity vectors define two distinct blocks separated by the Kopili fault upon which 2–3 mm/yr of dextral slip is observed: the Shillong block between longitudes 89 and 93°E rotating clockwise at 1.15°/Myr and the Assam block from 93.5°E to 97°E rotating at ≈1.13°/Myr. These two blocks are more than 120 km wide in a north‐south sense, but they extend locally a similar distance beneath the Himalaya and Tibet. A result of these rotations is that convergence across the Himalaya east of Sikkim decreases in velocity eastward from 18 to ≈12 mm/yr and convergence between the Shillong Plateau and Bangladesh across the Dauki fault increases from 3 mm/yr in the west to \u3e8 mm/yr in the east. This fast convergence rate is inconsistent with inferred geological uplift rates on the plateau (if a 45°N dip is assumed for the Dauki fault) unless clockwise rotation of the Shillong block has increased substantially in the past 4–8 Myr. Such acceleration is consistent with the reported recent slowing in the convergence rate across the Bhutan Himalaya. The current slip potential near Bhutan, based on present‐day convergence rates and assuming no great earthquake since 1713 A.D., is now ~5.4 m, similar to the slip reported from alluvial terraces that offsets across the Main Himalayan Thrust and sufficient to sustain a Mw ≥ 8.0 earthquake in this area

    Reconstructing dynamics of the Baltic Ice Stream Complex during deglaciation of the Last Scandinavian Ice Sheet

    Landforms left behind by the last Scandinavian Ice Sheet (SIS) offer an opportunity to investigate controls governing ice sheet dynamics. Terrestrial sectors of the ice sheet have received considerable attention from landform and stratigraphic investigations. In contrast, despite its geographical importance, the Baltic Sea remains poorly constrained due to limitations in bathymetric data. Both ice-sheet-scale investigations and regional studies at the southern periphery of the SIS have considered the Baltic depression to be a preferential route for ice flux towards the southern ice margin throughout the last glaciation. During the deglaciation the Baltic depression hosted the extensive Baltic Ice Lake, which likely exerted a considerable control on ice dynamics. Here we investigate the Baltic depression using newly available bathymetric data and peripheral topographic data. These data reveal an extensive landform suite stretching from Denmark in the west to Estonia in the east and from the southern European coast to the Åland Sea, comprising an area of 0.3 million km2. We use these landforms to reconstruct aspects of the ice dynamic history of the Baltic sector of the ice sheet. Landform evidence indicates a complex retreat pattern that changes from lobate ice margins with splaying lineations to parallel mega-scale glacial lineations (MSGLs) in the deeper depressions of the Baltic Basin. Ice margin still-stands on underlying geological structures indicate the likely importance of pinning points during deglaciation, resulting in a stepped retreat signal. Over the span of the study area we identify broad changes in the ice flow direction, ranging from SE–NW to N–S and then to NW–SE. MSGLs reveal distinct corridors of fast ice flow (ice streams) with widths of 30 km and up to 95 km in places, rather than the often-interpreted Baltic-wide (300 km) accelerated ice flow zone. These smaller ice streams are interpreted as having operated close behind the ice margin during late stages of deglaciation. Where previous ice-sheet-scale investigations inferred a single ice source, our mapping identifies flow and ice margin geometries from both Swedish and northern Bothnian sources. We anticipate that our landform mapping and interpretations may be used as a framework for more detailed empirical studies by identifying targets to acquire high-resolution bathymetry and sediment cores and also for comparison with numerical ice sheet modelling.</p

    AN ETHNOBOTANICAL SURVEY OF ANTI-MALARIAL PLANTS IN SOME HIGHLY MALARIA AFFECTED DISTRICTS OF ASSAM

    Objective: An ethnobotanical survey of potent anti-malarial plants used by different tribes and communities of highly malaria affected seven districts of Assam, India.Methods: Personal discussions and group interviews were held with the traditional practitioners. Samples were collected and authentically identified. Detailed ethno knowledge on the plant parts used and mode of administration of the plant extracts was documented. Literature survey was carried out on identified plant species and analyzed for their medicinal importance.Results: A total of 22 plant species belonging to 20 botanical families were collected and identified from different locations of seven districts of Assam. Different plant parts such as leaves, root, flower, bark etc. were found to process through different basic methods (infusion, decoction, direct mechanical crushing and maceration) for extracting the anti-malarial crude phytochemicals by the traditional healers. Extensive literature on the broad spectrum medicinal values other than anti-malarial activity of the plant species suggested a strong scientific background of selecting those particular plants species by the traditional practitioners.Conclusion: Indigenous plant species with strong claim of the natural source of anti-malarial moieties were identified in the present ethno-botanical survey. All the 22 plant species investigated could be a potential source of new antimalarial therapies.Â

    Evidence for Cold-stream to Hot-accretion Transition as Traced by Ly alpha Emission from Groups and Clusters at 2 < z < 3.3

    We present Keck Cosmic Web Imager observations of giant Ly alpha halos surrounding nine galaxy groups and clusters at 2 < z < 3.3, including five new detections and one upper limit. We find observational evidence for the cold-stream to hot-accretion transition predicted by theory by measuring a decrease in the ratio between the spatially extended Ly alpha luminosity and the expected baryonic accretion rate (BAR), with increasing elongation above the transition mass (M-stream). This implies a modulation of the share of BAR that remains cold, diminishing quasi-linearly (logarithmic slope of 0.97 +/- 0.19, 5 sigma significance) with the halo to M-stream mass ratio. The integrated star formation rates (SFRs) and active galactic nucleus (AGN) bolometric luminosities display a potentially consistent decrease, albeit significant only at 2.6 sigma and 1.3 sigma, respectively. The higher scatter in these tracers suggests the Ly alpha emission might be mostly a direct product of cold accretion in these structures rather than indirect, mediated by outflows and photoionization from SFR and AGNs; this is also supported by energetics considerations. Below M-stream (cold-stream regime), we measure L (Ly alpha) /BAR = 10(40.51 +/- 0.16) erg s(-1) M-circle dot(-1) yr, consistent with predictions, and SFR/BAR = 10(-0.54 +/- 0.23): on average, 30(-10)(+20) M-stream (hot-accretion regime), L-Ly alpha is set by M-stream (within 0.2 dex scatter in our sample), independent of the halo mass but rising 10-fold from z = 2 to 3.Peer reviewe

    First Measurement of the Atomic Electric Dipole Moment of \u3csup\u3e225\u3c/sup\u3eRa

    The radioactive radium-225 (225Ra) atom is a favorable case to search for a permanent electric dipole moment. Because of its strong nuclear octupole deformation and large atomic mass, 225Ra is particularly sensitive to interactions in the nuclear medium that violate both time-reversal symmetry and parity. We have developed a cold-atom technique to study the spin precession of 225Ra atoms held in an optical dipole trap, and demonstrated the principle of this method by completing the first measurement of its atomic electric dipole moment, reaching an upper limit of |d(225Ra)|\u3c5.0×10−22  e cm (95% confidence)
