Dietary habits and nutrient intake in adolescent girls living in Northern Iran
Rapid changes in lifestyle and industrialization of communities have an important effect on food intake pattern of society. Regarding the lack of enough data about dietary habits and nutrient intake of adolescents in our society, this study was performed in a group of adolescent girls in Lahijan, North of Iran. In this cross-sectional study, 400 high school girls aged 14-17 years selected by random stratified sampling. Nutritional data were collected by 24-hour dietary recall, food habits and food frequency questionnaires for all samples. The mean energy intake was 2338±611 kcal/d. The contributions of carbohydrate, protein and fat to the total energy intake were 59.3, 11.9 and 28.8%, respectively. The daily intake of energy obtained from breakfast, lunch, dinner and snacks were 16.3, 23.5, 25.9 and 34.3%, respectively. The mean intakes of vitamin A, vitamin D, calcium, phosphorous and zinc were below the Recommended Daily Allowances. The consumption of fresh vegetables and fruit was generally low. Twelve percent in total did not drink milk at all. Almost all the subjects had a prepared meal, most often in the evening, at least four times a week. Regarding the undesirable food pattern and proportions of nutrient intakes, it is necessary to development means of motivating adolescents to eat nutritionally rich foods, good for health and well-being
Strength and weakness of the National Nutrition Improvement Program in Rural and Nomadic Women: findings from a policy triangle framework
IntroductionNutrition-sensitive agriculture (NSA) is a comprehensive, inter-sectoral approach to improve food security and nutrition. In Iran, âNational Nutrition Improvement Program in Rural and Nomadic Womenâ has been developed and implemented as a NSA program. The main purpose of this study was analysis of this program using the Policy Triangle Framework.MethodsThis was a qualitative policy analysis study, which was conducted retrospectively. The study population included policymakers and executors at macro (Tehran Province) and micro levels (County and village) from the two involved ministries, as well as rural women in Tehran province. The collected data included the program document (N =â210), in-depth semi-structured interviews (N =â40), as well as focus group discussions (N =â8). Data was analyzed using MAXQDA 2010 software with a deductive approach.ResultsThe findings of this study indicated that the underlying factors including social, economic and cultural status, health, structural-environmental and political are associated to the development of the program. The current program is a multi-faceted, in accordance with the existing needs, which provides opportunities to improve nutrition and community health, empower women, strengthen socio-economic status at the micro and macro levels and pave the way for other projects, by connecting the nutrition sector to agriculture. The analysis has also shown that the content of this program requires more consideration in budget and motivational measures. Although developing an NSA program based on inter-sectoral collaboration is a valuable step, it needs to be improved in the areas of sustainability, inter-sectoral collaboration, resources and facilities, monitoring and evaluation, as well as needs assessment.ConclusionThe findings of the present study can be used as evidence by policymakers and planners in redesigning and implementing the program, or developing other NSA programs
Relationship between thyroid stimulating hormone and metabolic syndrome in overweight/obese children
Background: Background: Obesity, especially central obesity is related to many endocrine abnormalities, such as thyroid dysfunctions. Elevated levels of thyroid stimulating hormone (TSH) is common in obese children, however, it is not clear if such condition is associated with increased cardiovascular risk factors. The study aimed to determine the association between levels of TSH in overweight and obese children with components of metabolic syndrome (Mets).
Methods: The study sample included 197 overweight/obese 6-7 year old children in Tehran, Iran. Anthropometric (weight, waist circumference and height), metabolic (high-density lipoprotein cholesterol, triglycerides and fasting blood glucose) and hormonal (TSH) variables, as well as blood pressure were measured. Mets was defined according to Cook definition.
Results: Totally, 20.3% and 79.7% of children were overweight and obese, respectively. Elevated levels of TSH were diagnosed in 10 subjects (5.1%), while Mets was seen in 35.4%. The most frequent component of Mets was abdominal obesity (72.5%). A weak positive correlation between BMI for age, Z scores and TSH level (r =0.11, P value= 0.123) was observed only in girls (r=0.2, P value= 0.034). TSH was not associated with components of Mets.
Conclusion: Elevated TSH levels may be found in obese children; however, the association between TSH elevation and cardiovascular disease risk factors, including components of metabolic syndrome needs further investigation
Efficacy of a Comprehensive Weight Reduction Intervention in Male Adolescents With Different FTO Genotypes
Background: The FTO gene polymorphisms may influence the effects of lifestyle interventions on obesity. The present study aimed to assess the influence of the rs9930506 FTO gene polymorphism on the success of a comprehensive weight loss intervention in male adolescents with overweight and obesity. Methods: This study was carried out on 96 adolescent boys with overweight and obesity who were randomly assigned to the intervention (n = 53) and control (n = 43) groups. The blood samples of the participants were collected, and the FTO gene was genotyped for the rs9930506 polymorphism. A comprehensive lifestyle intervention including changes in diet and physical activity was performed for 8 weeks in the intervention group. Results: Following the lifestyle intervention, BMI and fat mass decreased significantly in the intervention group compared with the control group (both p < 0.05), while no change was found in weight, height or body muscle percentage between the groups. The participants in the intervention group with the AA/AG genotype and not in carriers of the GG genotype had a significantly higher reduction in BMI (â1.21 vs. 1.87 kg/m2, F = 4.07, p < 0.05) compared with the control group. Conclusion: The intervention in individuals with the AA/AG genotype has been significantly effective in weight loss compared with the control group. The intervention had no association effect on anthropometric indices in adolescents with the GG genotype of the FTO rs9930506 polymorphism. Prevalence of malnutrition among Iranâs under five-year-old children and the related factors: A systematic review and meta-analysis
Context: Childrenâs health is one of the priorities in most societies. Nevertheless, the highest prevalence rate of malnutrition occurs among under five-year-old children worldwide. Objectives: The aim of this study was to estimate the rate of malnutrition in Iranian children through a systematic review and meta-analysis. Also we tried to identify the most prevalent factors causing malnutrition. Data Sources: The required data were collected by searching the keywords of nutrition, malnutrition, under-nutrition, prevalence, epidemiology, etiology, occurrence and Iran in PubMed, Google Scholar, Scopus, Index Copernicus, DOAJ, EBSCO-CINAHL, SID, Magi-ran and Iranmedex. Results: Twenty studies were selected for the final analysis stage, dating from 1999 to 2014. Number of children investigated for wasting, stunting and underweight was 53612, 54312 and 55012 respectively. The results showed that the prevalence of each type of malnutrition, in terms of wasting, stunting and underweight were 7.8% (95% CI: 4.8%-12.6%), 12.4% (95% CI: 8.3%-18.5%) and 10.5% (95% CI: 7.1%-15.4%), respectively. Parentsâ educational level, in particular mothersâ, gender, birth weight, the place of residence, and motherâs occupation were mentioned as important factors causing malnutrition. Conclusion: The rate of malnutrition in Iranâs under five-year-old children is lower than the average of that in world and the Eastern Mediterranean region. Breastfeeding, the proper use of complementary feeding, and the principled spacing between births and improving the quality of maternal care have important role in prevention of malnutrition
Factors associated with malnutrition among under five-year-old children in Iran:A systematic review
Background: Children are one of the most important population groups in the world and malnutrition is considered as one of death causes among them, especially those under 5 years old. The aim of our study was to conduct a systematic review of malnutrition and its associated factors among under five-year-old children of Iran. Materials and Methods: Data were collected through searching electronic databases and searching motors of PubMed, Science Direct, Scopus, Google Scholar, Scientific Information Database, Magiran and IranMedex using key words of 'malnutrition,' 'under-nutrition,' 'stunting,' 'underweight,' 'wasting,' 'factor,' 'children,' 'under 5 years,' 'Iran,' and their Persian synonyms. Some of the relevant journals and websites were hand searched. Results: Of 608 preliminarily retrieved articles, 36 were selected for the final phase of the study. The most important factors related to underweight were mother's education level, father's education level, and birth weight. In regard to wasting, gender was one of the main factors and the next four were living location, birth weight, mother's, and father's education levels. Mother education level, father education, gender, birth weight, and age group were mentioned as the most important factors resulting in stunting as well. Conclusions: In some regions with low socioeconomic status, especially rural areas, the focus should be on the improvement of households living conditions such as mothers' and children's nutrition. Improvement in parents' education, particularly mothers' education, can have a significant impact on children's health. Under 5 years old girls need more attention than boys regarding to their poorer indicators
Prevention of malnutrition among children under 5 years old in Iran: A policy analysis.
BackgroundMalnutrition is one of the main causes of death in children under 5 years of age and one of the most common factors threatening children's life and health. Nutrition policy analysis and solving existing problems in children can reduce the effects of malnutrition. This study aimed to analyze the current policies of malnutrition prevention in children under five years of age in Iran.MethodThis study was conducted in 2017 to analyze policies using the "policy triangle framework". In order to examine the policy-making process, the Kingdon's multiple streams model was used. A combination of two sampling methods, including purposeful and snowball sampling, was applied to select the interviewees. In relation to the implemented documents and policies, the country's most important policies were selected based on the suggestions of policy makers as well as searching scientific databases and electronic portals. A data collection form was used to identify the current policies and documents and a semi-structured interview guide form was used for the interviews. The framework analysis and MaxQDA software were applied to analyze the data obtained from the interviews.ResultsThe key factors affecting policies in Iran included the status of indicators as well as economic, social, structural-legal, policy and international factors. Among the most important policies and implemented programs, the following can be mentioned: growth monitoring, oral rehydration, breastfeeding, immunization, female education, family spacing, food supplementation, nutrition for children under five years of age, and control of nutritional deficiencies. Currently there is a need for a nationwide program and comprehensive document in the field of the nutrition in children under 5 years of age, which requires strengthening of the political process. Participants and stakeholders in nutrition-related policies for children under the age of five were divided into four categories of governmental, semi-governmental, non-governmental, and international organizations.ConclusionMore attention should be paid to the shortage of some micronutrients, accurate implementation of breastfeeding programs, supplementary nutrition, fortification and supplementation programs for children and mothers, utilization of the advantages of each region and its resources, and better coordination between organizations and their policies, and finally strong incentives are needed to promote macro nutritional goals for children under five years of age
