Outbreak of gastroenteritis caused by Yersinia pestis in Afghanistan.
Plague, which is most often caused by the bite of Yersinia pestis-infected fleas, is a rapidly progressing, serious disease that can be fatal without prompt antibiotic treatment. In late December 2007, an outbreak of acute gastroenteritis occurred in Nimroz Province of southern Afghanistan. Of the 83 probable cases of illness, 17 died (case fatality 20·5%). Being a case was associated with consumption or handling of camel meat (adjusted odds ratio 4·4, 95% confidence interval 2·2-8·8, P<0·001). Molecular testing of patient clinical samples and of tissue from the camel using PCR/electrospray ionization-mass spectrometry revealed DNA signatures consistent with Yersinia pestis. Confirmatory testing using real-time PCR and immunological seroconversion of one of the patients confirmed that the outbreak was caused by plague, with a rare gastrointestinal presentation. The study highlights the challenges of identifying infectious agents in low-resource settings; it is the first reported occurrence of plague in Afghanistan
PREVALENCE OF LICE SPECIES ON COWS AND BUFFALOES OF QUETTA, PAKISTAN
The prevalence of lice with species identification was studied during April to November 2006 in cows and buffaloes. During this period, various farm houses of Quetta city were randomly visited for the collection of lice. A total 909 cows and 671 buffaloes were examined for the presence of lice. Out of these, 38.3% cows and 41.2% buffaloes gave positive results for different lice species. Only one lice species i. e Bovicola bovis (38.3%) was recorded from cows and two species belonging to genus Haematopinus i. e. H. quadrpertusis (23.5%) and H. eurysternus (17.7%) were encountered in buffaloes. Lice seem to be host specific with B. bovis occur in cows, while both H. quadripertusus and H. eurysternus usually infest buffaloes
Review of Physical Activity Benefits and Potential Considerations for Individuals with Surgical Fusion of Spine for Scoliosis
International Journal of Exercise Science 10(2): 166-177, 2017 Evidence-based recommendations for physical activity following spinal fusion surgeries for idiopathic scoliosis are limited, specifically in the adolescent population. Individuals with scoliosis treated operatively or non-operatively have been reported to participate in less than 1-3 days/week of even mildly strenuous physical exercises. Over 40% of individuals with scoliosis returned to sports at a level lower than pre-operative participation levels or did not return at all post spinal fusion. It is particularly important for human movement specialists, such as physical therapists, occupational therapists, athletic trainers and kinesiologists to assist these individuals effectively transition to and maintain engagement in physical activity. This review provides a snapshot of common considerations and potential factors influencing individuals with spinal-fusion for scoliosis to participate in safe physical activity
The Devastating 2022 M6.2 Afghanistan Earthquake: Challenges, Processes, and Implications
On June 21st, a Mw6.2 earthquake struck the Afghan-Pakistan-border-region, situated within the India-Asia collision. Thousand thirty-nine deaths were reported, making the earthquake the deadliest of 2022. We investigate the event\u27s rupture processes by combining seismological and geodetic observations, aiming to understand what made it that fatal. Our Interferometric Synthetic Aperture Radar-constrained slip-model and regional moment-tensor inversion, confirmed through field observations, reveal a sinistral rupture with maximum slip of 1.8 m at 5 km depth on a N20°E striking, sub-vertical fault. We suggest that not only external factors (event-time, building stock) but fault-specific factors made the event excessively destructive. Surface rupture was favored by the rock foliation, coinciding with the fault strike. The distribution of Peak-Ground-Velocity was governed by the sub-vertical fault. Maximum slip was large compared to other events globally and might have resulted in peak-frequencies coinciding with resonance-frequencies of the local buildings and demonstrates the devastating impact of moderate-size earthquakes
Modification of asphalt mixtures for cold regions using microencapsulated phase change materials
Phase change materials (PCMs) may be used to regulate the temperature of road surfaces to avoid low-temperature damages when asphalt materials become brittle and prone to cracking. With this in mind, different asphalt mixtures were modified with microencapsulated phase change materials (i.e. tetradecane) to assess their thermal benefits during the phase change process. Likewise, the effect on the mechanical performance of PCMs as a replacement of mineral filler was assessed. Special attention was paid to dry and wet modification processes for incorporating the PCMs into the mixtures. The results showed that PCM modifications are indeed able to slow down cooling and affect temperatures below zero. Approximately, a maximum of 2.5 °C offset was achieved under the tested cooling conditions compared to the unmodified reference specimens. Regarding the mechanical response at 0 °C and 10 °C, the results indicated that the PCM modification significantly reduces the stiffness of the material in comparison with the values obtained for the reference mixture

Molecular testing for the clinical diagnosis of fibrolamellar carcinoma.
Fibrolamellar carcinoma has a distinctive morphology and immunophenotype, including cytokeratin 7 and CD68 co-expression. Despite the distinct findings, accurate diagnosis of fibrolamellar carcinoma continues to be a challenge. Recently, fibrolamellar carcinomas were found to harbor a characteristic somatic gene fusion, DNAJB1-PRKACA. A break-apart fluorescence in situ hybridization (FISH) assay was designed to detect this fusion event and to examine its diagnostic performance in a large, multicenter, multinational study. Cases initially classified as fibrolamellar carcinoma based on histological features were reviewed from 124 patients. Upon central review, 104 of the 124 cases were classified histologically as typical of fibrolamellar carcinoma, 12 cases as 'possible fibrolamellar carcinoma' and 8 cases as 'unlikely to be fibrolamellar carcinoma'. PRKACA FISH was positive for rearrangement in 102 of 103 (99%) typical fibrolamellar carcinomas, 9 of 12 'possible fibrolamellar carcinomas' and 0 of 8 cases 'unlikely to be fibrolamellar carcinomas'. Within the morphologically typical group of fibrolamellar carcinomas, two tumors with unusual FISH patterns were also identified. Both cases had the fusion gene DNAJB1-PRKACA, but one also had amplification of the fusion gene and one had heterozygous deletion of the normal PRKACA locus. In addition, 88 conventional hepatocellular carcinomas were evaluated with PRKACA FISH and all were negative. These findings demonstrate that FISH for the PRKACA rearrangement is a clinically useful tool to confirm the diagnosis of fibrolamellar carcinoma, with high sensitivity and specificity. A diagnosis of fibrolamellar carcinoma is more accurate when based on morphology plus confirmatory testing than when based on morphology alone

Molecular study of pathogenic and saprophytic fungal species on infected parts of Malus pumila L. of district Qilla Abdullah, Balochistan, Pakistan
Apple (Malus pumila L) of the family Rosaceae, most cultivated fruit in temperate regions of the world and is used fresh or processed. The apple production is affected by several pathogens including fungi. The present study was designed to identify disease-causing agents that reduce fruit production in the district Qilla Abdullah of Balochistan, Pakistan, which is the main apple production area of the province. Three varieties of apple: ‘Tur-Kulu’ (‘Red Delicious’), ‘Shin-Kulu’ (‘Golden Delicious’), and ‘Kaja’ were selected. Infected leaf samples were collected from eight different sites of tehsil Gulistan, district Qilla Abdullah. The cultures of fungal micro-flora were grown on two media, potato dextrose agar (PDA), and malt extract agar (MEA) followed by incubation for one week. The resulting colonies were observed under a microscope and identified based on morphological characters. Predominant fungal species was identified through ITS marker and PCR amplification. The isolated pathogens belonged to Zygomycota and Ascomycota divisions. The pathogens found were Aspergillus niger, A. oryzae, A. terrus, Colletotrichum gloeosporioides, Fusarium oxysporum, Mucor spp., Penicillium expansum, and one species of Absidia as well as Rhizopus. Colletotrichum gloeosporioides were predominantly found in all varieties. Morphological and phylogenetic analysis confirmed the identity of Colletotrichum gloeosporioides. As a result of this study, the predominant pathogen species Colletotrichum gloeosporioides is one of the causes of leaf infection in apple varieties
HER2 Testing and Clinical Decision Making in Gastroesophageal Adenocarcinoma
CONTEXT: ERBB2 (erb-b2 receptor tyrosine kinase 2 or HER2) is currently the only biomarker established for selection of a specific therapy for patients with advanced gastroesophageal adenocarcinoma (GEA). However, there are no comprehensive guidelines for the assessment of HER2 in patients with GEA.
OBJECTIVES: To establish an evidence-based guideline for HER2 testing in patients with GEA, to formalize the algorithms for methods to improve the accuracy of HER2 testing while addressing which patients and tumor specimens are appropriate, and to provide guidance on clinical decision making.
DESIGN: The College of American Pathologists, American Society for Clinical Pathology, and American Society of Clinical Oncology convened an expert panel to conduct a systematic review of the literature to develop an evidence-based guideline with recommendations for optimal HER2 testing in patients with GEA.
RESULTS: The panel is proposing 11 recommendations with strong agreement from the open-comment participants.
RECOMMENDATIONS: The panel recommends that tumor specimen(s) from all patients with advanced GEA, who are candidates for HER2-targeted therapy, should be assessed for HER2 status before the initiation of HER2-targeted therapy. Clinicians should offer combination chemotherapy and a HER2-targeted agent as initial therapy for all patients with HER2-positive advanced GEA. For pathologists, guidance is provided for morphologic selection of neoplastic tissue, testing algorithms, scoring methods, interpretation and reporting of results, and laboratory quality assurance.
CONCLUSIONS: This guideline provides specific recommendations for assessment of HER2 in patients with advanced GEA while addressing pertinent technical issues and clinical implications of the results
