    Anestesisjuksköterskans uppfattning av WHOŽs checklista för sÀkerhet vid operationer

    VÀrdshÀlsoorganisationen (WHO), utarbetade 2007 en kirurgisk checklista för att stÀrka och öka patientsÀkerheten vid kirurgiska ingrepp. I Region SkÄne infördes checklistan 2009 för anvÀndande i skÄnsk sjukvÄrd. Syfte: Att undersöka anestesisjuksköterskors uppfattning om hur checklistan anvÀnds samt att beskriva eventuella skillnader i anvÀndandet pÄ fyra sjukhus i SkÄne. Metod: En tvÀrsnittsstudie med enkÀter som besvarades av 68 respondenter genomfördes pÄ fyra sjukhus i SkÄne mars 2013. Resultat: sammantaget pÄ de fyra sjukhusen i SkÄne upplevs att patientsÀkerheten ökat efter checklistans införande. TeamkÀnslan upplevs ha ökat men med variation bÄde inom sjukhuset och mellan sjukhusen. Det upplevs Àven att checklistan alltid anvÀnds vid planerade ingrepp men med en lÀgre frekvens och anvÀndandegrad vid akuta ingrepp. Konklusion: Anestesisjuksköterskor pÄ fyra sjukhus i SkÄne upplever generellt att checklistan anvÀnds tillfredstÀllande och att den bidragit till ökad patientsÀkerhet och teamkÀnsla. Det upplevs dock fortfarande vissa brister i teamkÀnsla och checklistans anvÀndandefrekvens och i dess anvÀndandegrad

    Correlational analysis for identifying genes whose regulation contributes to chronic neuropathic pain

    <p>Abstract</p> <p>Background</p> <p>Nerve injury-triggered hyperexcitability in primary sensory neurons is considered a major source of chronic neuropathic pain. The hyperexcitability, in turn, is thought to be related to transcriptional switching in afferent cell somata. Analysis using expression microarrays has revealed that many genes are regulated in the dorsal root ganglion (DRG) following axotomy. But which contribute to pain phenotype versus other nerve injury-evoked processes such as nerve regeneration? Using the L5 spinal nerve ligation model of neuropathy we examined <b><it>differential </it></b>changes in gene expression in the L5 (and L4) DRGs in five mouse strains with contrasting susceptibility to neuropathic pain. We sought genes for which the degree of regulation correlates with strain-specific pain phenotype.</p> <p>Results</p> <p>In an initial experiment six candidate genes previously identified as important in pain physiology were selected for in situ hybridization to DRG sections. Among these, regulation of the Na<sup>+ </sup>channel α subunit <it>Scn11a </it>correlated with levels of spontaneous pain behavior, and regulation of the cool receptor <it>Trpm8 </it>correlated with heat hypersensibility. In a larger scale experiment, mRNA extracted from individual mouse DRGs was processed on Affymetrix whole-genome expression microarrays. Overall, 2552 ± 477 transcripts were significantly regulated in the axotomized L5DRG 3 days postoperatively. However, in only a small fraction of these was the degree of regulation correlated with pain behavior across strains. Very few genes in the "uninjured" L4DRG showed altered expression (24 ± 28).</p> <p>Conclusion</p> <p>Correlational analysis based on in situ hybridization provided evidence that differential regulation of <it>Scn11a </it>and <it>Trpm8 </it>contributes to across-strain variability in pain phenotype. This does not, of course, constitute evidence that the others are unrelated to pain. Correlational analysis based on microarray data yielded a larger "look-up table" of genes whose regulation likely contributes to pain variability. While this list is enriched in genes of potential importance for pain physiology, and is relatively free of the bias inherent in the candidate gene approach, additional steps are required to clarify which transcripts on the list are in fact of functional importance.</p

    A lake as a microcosm: reflections on developments in aquatic ecology

    In the present study, we aim at relating Forbes' remarkable paper on "The lake as a microcosm", published 125 years ago, to the present status of knowledge in our own research group. Hence, we relate the observations Forbes made to our own microcosm, Lake Krankesjon in southern Sweden, that has been intensively studied by several research groups for more than three decades. Specifically, we focus on the question: Have we made any significant progress or did Forbes and colleagues blaze the trail through the unknown wilderness and we are mainly paving that intellectual road? We conclude that lakes are more isolated than many other biomes, but have, indeed, many extensions, for example, input from the catchment, fishing and fish migration. We also conclude that irrespective of whether lakes should be viewed as microcosms or not, the paper by Forbes has been exceptionally influential and still is, especially since it touches upon almost all aspects of the lake ecosystem, from individual behaviour to food web interactions and environmental issues. Therefore, there is no doubt that even if 125 years have passed, Forbes' paper still is a source of inspiration and deserves to be read. Hence, although aquatic ecology has made considerable progress over the latest century, Forbes might be viewed as one of the major pioneers and visionary scientists of limnology

    Identification of Novel α-Synuclein Isoforms in Human Brain Tissue by using an Online NanoLC-ESI-FTICR-MS Method

    Parkinson’s disease (PD) and Dementia with Lewy bodies (DLB) are neurodegenerative diseases that are characterized by intra-neuronal inclusions of Lewy bodies in distinct brain regions. These inclusions consist mainly of aggregated α-synuclein (α-syn) protein. The present study used immunoprecipitation combined with nanoflow liquid chromatography (LC) coupled to high resolution electrospray ionization Fourier transform ion cyclotron resonance tandem mass spectrometry (ESI-FTICR-MS/MS) to determine known and novel isoforms of α-syn in brain tissue homogenates. N-terminally acetylated full-length α-syn (Ac-α-syn1–140) and two N-terminally acetylated C-terminally truncated forms of α-syn (Ac-α-syn1–139 and Ac-α-syn1–103) were found. The different forms of α-syn were further studied by Western blotting in brain tissue homogenates from the temporal cortex Brodmann area 36 (BA36) and the dorsolateral prefrontal cortex BA9 derived from controls, patients with DLB and PD with dementia (PDD). Quantification of α-syn in each brain tissue fraction was performed using a novel enzyme-linked immunosorbent assay (ELISA)
