Background: There is now greater involvement of children in drug trials to ensure that paediatric medicines are supported by sound scientific evidence. The safety of the participating children is of paramount importance. Previous research shows that these children can suffer moderate and severe adverse drug reactions (ADRs) in clinical trials, yet very few of the trials designated a data safety monitoring board (DSMB) to oversee the trial. Methods: Safety data from a systematic review of paediatric drug randomised controlled trials (RCTs) published in 2007 were analysed. All reported adverse events (AEs) were classified and assessed to determine whether an ADR had been experienced. ADRs were then categorised according to severity. Each trial report was examined as to whether an independent DSMB was in place. Results: Of the 582 paediatric drug RCTs analysed, 210 (36%) reported that a serious AE had occurred, and in 15% mortality was reported. ADRs were detected in more than half of the RCTs (305); 66 (11%) were severe, and 79 (14%) were moderate. Severe ADRs involved a wide range of organ systems and were frequently associated with cytotoxic drugs, antiparasitics, anticonvulsants and psychotropic drugs. Two RCTs reported significantly higher mortality rates in the treatment group. Only 69 (12%) of the RCTs stated there was a DSMB. DSMBs terminated five RCTs and changed the protocol in one. Conclusions: Children participating in drug RCTs experience a significant amount and a wide range of ADRs. DSMBs are needed to ensure the safety of paediatric participants in clinical drug trial

Self-determination : its evolution in international law and prescriptions for its application in the post-colonial context.

Since 1945 the focus of the principle of self-determination has been on decolonization. The remarkable success of the application of the principle in the decolonization process has established it as a hximan right, a norm of modern international law and (arguably) part of jus cogens. With decolonization virtually completed, the question is whether self-determination as a right of all peoples should still be legally valid in the post-colonial era. In this research the conclusion is reached that international law neither recognizes nor prohibits selfdetermination as a right in the post-colonial context, and that the law is in this sense \neutral\". The neutrality of international law on the subject does not prejudge the issue as to the desirability of a recognition of the right in the post-colonial context. A fortiori it necessitates a rational analysis of the role of self-determination in the context of decolonization and the potential of the principle in the pos.t-colonial setting with the view to regulating competing claims. To this end the research analyses the function of self-determination in decolonization and arrives at the conclusions that within that context self-determination is a putative remedial right aimed at remedying a specific form of human relations manifested by domination exploitation and (or) a general denial of human rights. In the light of this the research advances the thesis that in the post-colonial context where inter-communal relationships in a sovereign state are characterized by similar trends of domination and denials of human rights the application of self-determination could be a useful remedy. It also indicates that as an inherently democratic principle self-determination could be used to settle certain types of territorial disputes. Using the relationship between human rights and self-determination as the normative basis for a community policy on post-colonial self determination the research recommends a set of substantive and procedural prescriptions on the support for or rejection of specific claims. The first two chapters of the research deal with the emergence of self-determination as a right in international law and its application in the colonial context. Chapter Three discusses the relationship between self-determination and other norms of international law namely territorial integrity domestic jurisdiction and the use of force (within the context of decolonization). In all the first three chapters define the confines of self-determination as lex lata. Chapter Four addresses the issue as to whether self-determination exists as lex lata beyond the context of decolonization and concludes that it does not exist. Chapter Five is devoted to a normative inquiry into the desirability of recognizing a right of self-determination in the post-colonial context. The chapter draws the conclusion that in view of the persistence of competing claims usually accompanied by conflicts of international dimensions and in view of the relationship between human rights and self-determination it is desirable to recognize the right at least in cases that involve gross deprivation of human rights (e.g. genocide). Chapter Six is devoted to a discussion of separatist movements that seek a right of self-determination in the post-colonial context. In Chapter Seven human rights as the normative basis for a community policy on post-colonial self-determination is discussed. On the strength of the discussion recommendations are made as to which claims should be supported and which claims should be rejected. It is also recommended that as a means of expressing popular will the principle of self determination could be useful in the settlement of territorial disputes that involve transfer of populations. Chapters Eight and Nine deal with the substantive and procedural conditions that must precede the support of a claim for self-determination in the post-colonial context.

An investigation into the feasibility of fetal lung maturity prediction using statistical textural features

Fetal lung and liver tissues were examined by ultrasound in 240 subjects during 24 to 38 weeks of gestational age in order to investigate the feasibility of predicting the maturity of the lung from the textural features of sonograms. A region of interest of 64 X 64 pixels is used for extracting textural features. Since the histological properties of the liver are claimed to remain constant with respect to gestational age, features obtained from the lung region are compared with those from liver. Though the mean values of some of the features show a specific trend with respect to gestation age, the variance is too high to guarantee definite prediction of the gestational age. Thus, we restricted our purview to an investigation into the feasibility of fetal lung maturity prediction using statistical textural features. Out of 64 features extracted, those features that are correlated with gestation age and less computationally intensive are selected. The results of our study show that the sonographic features hold some promise in determining whether the fetal lung is mature or immature

Auto-tuned publisher in a pub/sub system: Design and performance evaluation

Pub/sub systems form the underlying framework for many distributed applications including large social networking applications. In this paper, we consider the optimization of the end-to-end latency of a pub/sub system in which the publisher, the broker, and the subscriber are in different administrative domains. While general pub/sub systems provide reliability of message delivery, good end-to-end latency in a multi-domain environment requires that the pub/sub system adapts to workload changes and bottlenecks in the different sub-systems. This study is motivated by two applications. First, a pub/sub based Simple Lookup Service (sLS) that is used in perfSONAR to provide information about network performance in Research and Education (R&E) networks. Second, the pub/sub system that is used to distribute alerts generated in the data pipeline in the Zwicky Transient Factory (ZTF). In this multi-domain pub/sub performance study, we consider a publisher with a multi-threaded architecture that uses batching to coalesce messages over some variable polling period. We propose a control algorithm that auto-tunes the batch-processing parameters namely, the batch size and the polling interval, to the input message load and to broker-side congestion. Using a detailed simulation model, we demonstrate the performance of the control algorithm for different scenarios. We then study the performance using a real-trace obtained from the Simple Lookup Service (sLS). We show that the proposed algorithm quickly adapts to rapid changes in the workload and yields lower mean end-to-end delay performance when compared with delays in the current deployment
