    EXPEDIENCY OF MARKERS FOR EARLY DETECTION OF ACUTE KIDNEY INJURY SEQUELAE TO TYPE 2 DIABETES MELLITUS

    ABSTRACTObjective: Estimation of Cystatin C (Cys C), traditional markers, inflammatory, and endothelial cell activation markers can identify subjects who areat increased risk for future acute kidney injury (AKI) after diabetes.Methods: A total of 210 subjects, having 70 subjects in each group between the age group of 45-75 years were enrolled in our study.Results: Body mass index (BMI), obesity index, waist circumference, and waist–hip ratio higher in Group III and Group II compared to Group I with asignificant p<0.001. All the biochemical parameters were significantly higher in Group III compared to Group I and Group II with a narrow differencebetween Group III and Group II. Serum Cys C was significantly correlated with creatinine and NO. Whereas serum creatinine (SCr) shows strongpositive correlation with BMI, fasting blood sugar, HbA1c, NO, and high-sensitivity C-reactive protein. Estimated glomerular filtration rate (eGFR) andtriglycerides show inverse relation to creatinine with a significant p-value. The serum Cys C showed an area under the curve (AUC) of 0.950 with acutoff value of 1.06, SCr with an AUC of 0.617, and eGFR with AUC of 0.588.Conclusion: Elevated levels of biomarker Cys C, SCr, and albumin-creatinine ratio are predictors of AKI in the setting of diabetes. Baseline inflammatoryand endothelial activation markers may also be useful for predicting future risk of AKI in diabetes mellitus. Hopefully, the advent of new biomarkerswill help defining the kidney at risk rather than relying simply on creatinine. To date, none of the new AKI biomarkers have undergone a similarrigorous assessment, but the current progress will hopefully lead to success and ultimately to improvement in patient outcome.Keywords: Type 2 diabetes mellitus, Acute kidney injury, Biomarkers, Inflammation, Endothelium

    INTERACTION OF ICHNOCARPUS FRUTESCENS WITH BIOGENIC AMINES FOR ITS ANTI-DEPRESSANT ACTIVITY

    The research interest has focused on various herbs that possess antidepressant properties and may be useful adjuncts in helping the management of depression in humans. The present study was therefore designed to investigate the antidepressant-like effects of methanol leaf extract of Ichnocarpus frutescens in animal models of depression. The mice were divided in to different groups and treated with two different doses of methanol extracts (200 & 400 mg/kg), same doses of extracts with different antagonist of 5HT, NA & DA (Ondansetran, Terazosin & Chlorpromazine) and standard groups treated with imipramine 10mg/kg. All groups of animals were separately submitted to forced swim test (FST), and Tail suspension test (TST) tests for the bio-screening of leaf extract with antidepressant profile. Results revealed that the immobility time in the FST & TST was significantly reduced with extracts alone and extracts with Chlorpromazine treated group compared to negative control. There is no significant reduction in immobility time was observed particularly in rats orally administered extracts with Ondansetran & Terazosin when compared with their respective controls. Thus, it is suggested that methanol leaf extract of Ichnocarpus frutescens exhibited significant antidepressant-like effects in animal models of depression and may be served as a potential resource for natural psychotherapeutic agent, against depression and its may act mostly through modifying serotonergic and noradrenergic neurons

    Impacts of organic and conventional crop management on diversity and activity of free-living nitrogen fixing bacteria and total bacteria are subsidiary to temporal effects

    A three year field study (2007-2009) of the diversity and numbers of the total and metabolically active free-living diazotophic bacteria and total bacterial communities in organic and conventionally managed agricultural soil was conducted at the Nafferton Factorial Systems Comparison (NFSC) study, in northeast England. The result demonstrated that there was no consistent effect of either organic or conventional soil management across the three years on the diversity or quantity of either diazotrophic or total bacterial communities. However, ordination analyses carried out on data from each individual year showed that factors associated with the different fertility management measures including availability of nitrogen species, organic carbon and pH, did exert significant effects on the structure of both diazotrophic and total bacterial communities. It appeared that the dominant drivers of qualitative and quantitative changes in both communities were annual and seasonal effects. Moreover, regression analyses showed activity of both communities was significantly affected by soil temperature and climatic conditions. The diazotrophic community showed no significant change in diversity across the three years, however, the total bacterial community significantly increased in diversity year on year. Diversity was always greatest during March for both diazotrophic and total bacterial communities. Quantitative analyses using qPCR of each community indicated that metabolically active diazotrophs were highest in year 1 but the population significantly declined in year 2 before recovering somewhat in the final year. The total bacterial population in contrast increased significantly each year. Seasonal effects were less consistent in this quantitative study

    Comparison of glottic views and intubation times in the supine and 25 degree back-up positions

    Background: We explored whether positioning patients in a 25° back-up sniffing position improved glottic views and ease of intubation. Methods: In the first part of the study, patients were intubated in the standard supine sniffing position. In the second part, the back of the operating table was raised 25° from the horizontal by flexion of the torso at the hips while maintaining the sniffing position. The best view obtained during laryngoscopy was assessed using the Cormack and Lehane classification and Percentage of Glottic Opening (POGO) score. The number of attempts at both laryngoscopy and tracheal intubation, together with the use of ancillary equipment and manoeuvres were recorded. The ease of intubation was indirectly assessed by recording the time interval between beginning of laryngoscopy and insertion of the tracheal tube. Results: Seven hundred eighty one unselected surgical patients scheduled for non-emergency surgery were included. In the back-up position, ancillary laryngeal manoeuvres, which included cricoid pressure, backwards upwards rightward pressure and external laryngeal manipulation, were required less frequently (19.6 % versus 24. 6 %, p = 0.004). The time from beginning of laryngoscopy to insertion of the tracheal tube was 14 % shorter (median time 24 versus 28 s, p = 0.031) in the back-up position. There was no significant difference in glottic views. Conclusions: The 25° back-up position improved the ease of intubation as judged by the need for fewer ancillary manoeuvres and shorter time for intubation.     Dynamics of mechanical waves in periodic grapheme nanoribbon assemblies

    We simulate the natural frequencies and the acoustic wave propagation characteristics of graphene nanoribbons (GNRs) of the type (8,0) and (0,8) using an equivalent atomistic-continuum FE model previously developed by some of the authors, where the C-C bonds thickness and average equilibrium lengths during the dynamic loading are identified through the minimisation of the system Hamiltonian. A molecular mechanics model based on the UFF potential is used to benchmark the hybrid FE models developed. The acoustic wave dispersion characteristics of the GNRs are simulated using a Floquet-based wave technique used to predict the pass-stop bands of periodic mechanical structures. We show that the thickness and equilibrium lengths do depend on the specific vibration and dispersion mode considered, and that they are in general different from the classical constant values used in open literature (0.34 nm for thickness and 0.142 nm for equilibrium length). We also show the dependence of the wave dispersion characteristics versus the aspect ratio and edge configurations of the nanoribbons, with widening band-gaps that depend on the chirality of the configurations. The thickness, average equilibrium length and edge type have to be taken into account when nanoribbons are used to design nano-oscillators and novel types of mass sensors based on periodic arrangements of nanostructures

    Rapid Evaluation in Whole Blood Culture of Regimens for XDR-TB Containing PNU-100480 (Sutezolid), TMC207, PA-824, SQ109, and Pyrazinamide

    There presently is no rapid method to assess the bactericidal activity of new regimens for tuberculosis. This study examined PNU-100480, TMC207, PA-824, SQ109, and pyrazinamide, singly and in various combinations, against intracellular M. tuberculosis, using whole blood culture (WBA). The addition of 1,25-dihydroxy vitamin D facilitated detection of the activity of TMC207 in the 3-day cultures. Pyrazinamide failed to show significant activity against a PZA-resistant strain (M. bovis BCG), and was not further considered. Low, mid, and high therapeutic concentrations of each remaining drug were tested individually and in a paired checkerboard fashion. Observed bactericidal activity was compared to that predicted by the sum of the effects of individual drugs. Combinations of PNU-100480, TMC207, and SQ109 were fully additive, whereas those including PA-824 were less than additive or antagonistic. The cumulative activities of 2, 3, and 4 drug combinations were predicted based on the observed concentration-activity relationship, published pharmacokinetic data, and, for PNU-100480, published WBA data after oral dosing. The most active regimens, including PNU-100480, TMC207, and SQ109, were predicted to have cumulative activity comparable to standard TB therapy. Further testing of regimens including these compounds is warranted. Measurement of whole blood bactericidal activity can accelerate the development of novel TB regimens

    Spatio-temporal Models of Lymphangiogenesis in Wound Healing

    Several studies suggest that one possible cause of impaired wound healing is failed or insufficient lymphangiogenesis, that is the formation of new lymphatic capillaries. Although many mathematical models have been developed to describe the formation of blood capillaries (angiogenesis), very few have been proposed for the regeneration of the lymphatic network. Lymphangiogenesis is a markedly different process from angiogenesis, occurring at different times and in response to different chemical stimuli. Two main hypotheses have been proposed: 1) lymphatic capillaries sprout from existing interrupted ones at the edge of the wound in analogy to the blood angiogenesis case; 2) lymphatic endothelial cells first pool in the wound region following the lymph flow and then, once sufficiently populated, start to form a network. Here we present two PDE models describing lymphangiogenesis according to these two different hypotheses. Further, we include the effect of advection due to interstitial flow and lymph flow coming from open capillaries. The variables represent different cell densities and growth factor concentrations, and where possible the parameters are estimated from biological data.     Different Modes of Retrovirus Restriction by Human APOBEC3A and APOBEC3G In Vivo

    The apolipoprotein B editing complex 3 (A3) cytidine deaminases are among the most highly evolutionarily selected retroviral restriction factors, both in terms of gene copy number and sequence diversity. Primate genomes encode seven A3 genes, and while A3F and 3G are widely recognized as important in the restriction of HIV, the role of the other genes, particularly A3A, is not as clear. Indeed, since human cells can express multiple A3 genes, and because of the lack of an experimentally tractable model, it is difficult to dissect the individual contribution of each gene to virus restriction in vivo. To overcome this problem, we generated human A3A and A3G transgenic mice on a mouse A3 knockout background. Using these mice, we demonstrate that both A3A and A3G restrict infection by murine retroviruses but by different mechanisms: A3G was packaged into virions and caused extensive deamination of the retrovirus genomes while A3A was not packaged and instead restricted infection when expressed in target cells. Additionally, we show that a murine leukemia virus engineered to express HIV Vif overcame the A3G-mediated restriction, thereby creating a novel model for studying the interaction between these proteins. We have thus developed an in vivo system for understanding how human A3 proteins use different modes of restriction, as well as a means for testing therapies that disrupt HIV Vif-A3G interactions.United States.     Types of the cerebral arterial circle (circle of Willis) in a Sri Lankan Population

    <p>Abstract</p> <p>Background</p> <p>The variations of the circle of Willis (CW) are clinically important as patients with effective collateral circulations have a lower risk of transient ischemic attack and stroke than those with ineffective collaterals. The aim of the present cadaveric study was to investigate the anatomical variations of the CW and to compare the frequency of prevalence of the different variations with previous autopsy studies as variations in the anatomy of the CW as a whole have not been studied in the Indian subcontinent.</p> <p>Methods</p> <p>The external diameter of all the arteries forming the CW in 225 normal Sri Lankan adult cadaver brains was measured using a calibrated grid to determine the prevalence in the variation in CW. Chisquared tests and a correspondence analysis were performed to compare the relative frequencies of prevalence of anatomical variations in the CW across 6 studies of diverse ethnic populations.</p> <p>Results</p> <p>We report 15 types of variations of CW out of 22 types previously described and one additional type: hypoplastic precommunicating part of the anterior cerebral arteries (A1) and contralateral posterior communicating arteries (PcoA) 5(2%). Statistically significant differences (p < 0.0001) were found between most of the studies except for the Moroccan study. An especially notable difference was observed in the following 4 configurations: 1) hypoplastic precommunicating part of the posterior cerebral arteries (P1), and contralateral A1, 2) hypoplastic PcoA and contralateral P1, 3) hypoplastic PcoA, anterior communicating artery (AcoA) and contralateral P1, 4) bilateral hypoplastic P1s and AcoA in a Caucasian dominant study by Fisher versus the rest of the studies.</p> <p>Conclusion</p> <p>The present study reveals that there are significant variations in the CW among intra and inter ethnic groups (Caucasian, African and Asian: Iran and Sri Lanka dominant populations), and warrants further studies keeping the methods of measurements, data assessment, and the definitions of hypoplasia the same.</p
