    INFLUENCE OF SUPPLYING SOME SAFE NATURAL HONEY BEE PRODUCTS ON FENNEL PLANTS GROWTH, SEEDS YIELD, OIL PRODUCTION AND ITS COMPONENTS

    Honeybee products as propolis (Pp) and royal jelly (RJ) are natural mixtures and powerful source of safe nutrients that could be safely used in agriculture as substitution of poisonous and dangerous chemical fertilizers. The impact of using them either solely or in combination as foliar application on some fennel morphological traits, fruit and oil yield and its essential components and chemical components was studied. RJ and Pp were foliar sprayed at three rates i.e. 0.0, 0.2, 0.4% and 0, 3, 6 g l-1 respectively in sole and combination treatments. All records assured that the application of both materials (RJ, Pp) have a positive useful impact on all traits studied either used individually or in combination. The moderate concentrations of both materials individually or in combination (0.2 % RJ, 3 g l-1 Pp) gave the highest records of growth and yield characters as well as anethole in the oil compared with other treatments with more superiority of RJ results over those of Pp in sole treatments even though RJ concentration is less than that of Pp especially in oil percentage and umbels number. On the other side, the highest concentration of either RJ (0.4%) or moderate one of Pp (3 g l-1) as sole treatment or in combination gave the highest records of most chemical composition. Also 0.4% RJ with 6 g l-1 Pp produced the highest percentage of estragole. Hence, fennel plants can be safely grown and highly produced by these safe natural materials without the help of the chemical fertilizer

    Force of tuberculosis infection among adolescents in a high HIV and TB prevalence community: a cross-sectional observation study

    BACKGROUND: Understanding of the transmission dynamics of tuberculosis (TB) in high TB and HIV prevalent settings is required in order to develop effective intervention strategies for TB control. However, there are little data assessing incidence of TB infection in adolescents in these settings. METHODS: We performed a tuberculin skin test (TST) and HIV survey among secondary school learners in a high HIV and TB prevalence community. TST responses to purified protein derivative RT23 were read after 3 days. HIV-infection was assessed using Orasure(R) collection device and ELISA testing. The results of the HIV-uninfected participants were combined with those from previous surveys among primary school learners in the same community, and force of TB infection was calculated by age. RESULTS: The age of 820 secondary school participants ranged from 13 to 22 years. 159 participants had participated in the primary school surveys. At a 10 mm cut-off, prevalence of TB infection among HIV-uninfected and first time participants, was 54% (n = 334/620). HIV prevalence was 5% (n = 40/816). HIV infection was not significantly associated with TST positivity (p = 0.07). In the combined survey dataset, TB prevalence was 45% (n = 645/1451), and was associated with increasing age and male gender. Force of infection increased with age, from 3% to 7.3% in adolescents [greater than or equal to]20 years of age. CONCLUSIONS: We show a high force of infection among adolescents, positively associated with increasing age. We postulate this is due to increased social contact with infectious TB cases. Control of the TB epidemic in this setting will require reducing the force of infection

    Womens' opinions on antenatal care in developing countries: results of a study in Cuba, Thailand, Saudi Arabia and Argentina

    BACKGROUND: The results of a qualitative study carried out in four developing countries (Cuba, Thailand, Saudi Arabia and Argentina) are presented. The study was conducted in the context of a randomised controlled trial to test the benefits of a new antenatal care protocol that reduced the number of visits to the doctor, rationalised the application of technology, and improved the provision of information to women in relation to the traditional protocol applied in each country. METHODS: Through focus groups discussions we were able to assess the concepts and expectations underlying women's evaluation of concepts and experiences of the care received in antenatal care clinics. 164 women participated in 24 focus groups discussion in all countries. RESULTS: Three areas are particularly addressed in this paper: a) concepts about pregnancy and health care, b) experience with health services and health providers, and c) opinions about the modified Antenatal Care (ANC) programme. In all three topics similarities were identified as well as particular opinions related to country specific social and cultural values. In general women have a positive view of the new ANC protocol, particularly regarding the information they receive. However, controversial issues emerged such as the reduction in the number of visits, particularly in Cuba where women are used to have 18 ANC visits in one pregnancy period. CONCLUSION: Recommendations to improve ANC services performance are being proposed. Any country interested in the application of a new ANC protocol should regard the opinion and acceptability of women towards changes

    Are women and providers satisfied with antenatal care? Views on a standard and a simplified, evidence-based model of care in four developing countries

    BACKGROUND: This study assessed women and providers' satisfaction with a new evidence-based antenatal care (ANC) model within the WHO randomized trial conducted in four developing countries. The WHO study was a randomized controlled trial that compared a new ANC model with the standard type offered in each country. The new model of ANC emphasized actions known to be effective in improving maternal or neonatal health, excluded other interventions that have not proved to be beneficial, and improved the information component, especially alerting pregnant women to potential health problems and instructing them on appropriate responses. These activities were distributed within four antenatal care visits for women that did not need any further assessment. METHODS: Satisfaction was measured through a standardized questionnaire administered to a random sample of 1,600 pregnant women and another to all antenatal care providers. RESULTS: Most women in both arms expressed satisfaction with ANC. More women in the intervention arm were satisfied with information on labor, delivery, family planning, pregnancy complications and emergency procedures. More providers in the experimental clinics were worried about visit spacing, but more satisfied with the time spent and information provided. CONCLUSIONS: Women and providers accepted the new ANC model generally. The safety of fewer visits for women without complications with longer spacing would have to be reinforced, if such a model is to be introduced into routine practice

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Magnetized Water and Proline to Boost the Growth, Productivity and Fruit Quality of ‘Taifi’ Pomegranate Subjected to Deficit Irrigation in Saline Clay Soils of Semi-Arid Egypt

    Water scarcity is becoming a global problem. The shift from traditional irrigation systems to deficit irrigation increased soil salinity, particularly in clay soils. The use of magnetized water (MW) and biostimulants can induce plant resistance to drought and salinity stress. To assess the role of MW and proline (P) on ‘Taifi’ pomegranate shrubs’ growth, productivity, and fruit quality under such conditions, a split-plots experiment was conducted for two seasons using three irrigation levels (100%, 75%, and 50%), based on the crop water requirements (CWR), and four treatments including foliar spray of tap water (control) and P, irrigation with MW, and MW + P. The most pronounced effect was referred to MW + P at 75% CWR that improved shrubs’ chlorophyll content and nutritional status, reduced endogenous proline content, and enhanced vegetative growth with minimum consumptive water use (CWU), optimum water use efficiency (WUE), maximum water productivity (WP), utmost yield and average fruit weight, lowest percentage of fruit cracking, and fairly good total soluble solids (TSS), vitamin C and anthocyanin contents. Overall, MW + P at 75% CWR improved the resistance mechanism of pomegranate shrubs in saline clay soils, plus improving the growers’ net profit. MW generally reduced soil salinity, expressed as reduced pH, EC, Na+, and Ca2+ values

    Integration of Geostatistical and Sentinal-2AMultispectral Satellite Image Analysis for Predicting Soil Fertility Condition in Drylands

    For modelling and predicting soil indicators to be fully operational and facilitate decision-making at any spatial level, there is a requirement for precise spatially referenced soil information to be available as input data. This paper focuses on showing the capacity of Sentinal-2A(S2A) multispectral imaging to predict soil properties and provide geostatistical analysis (ordinary kriging) for mapping dry land soil fertility conditions (SOCs). Conditioned Latin hypercube sampling was used to select the representative sampling sites within the study area. To achieve the objectives of this work, 48 surface soil samples were collected from the western part of Matrouh Governorate, Egypt, and pH, soil organic matter (SOM), available nitrogen (N), phosphorus (P), and potassium (K) levels were analyzed. Multilinear regression (MLR) was used to model the relationship between image reflectance and laboratory analysis (of pH, SOM, N, P, and K in the soil), followed by mapping the predicted outputs using ordinary kriging. Model fitting was achieved by removing variables according to the confidence level (95%).Around 30% of the samples were randomly selected to verify the validity of the results. The randomly selected samples helped express the variety of the soil characteristics from the investigated area. The predicted values of pH, SOM, N, P, and K performed well, with R2 values of 0.6, 0.7, 0.55, 0.6, and 0.92 achieved for pH, SOM, N, P, and K, respectively. The results from the ArcGIS model builder indicated a descending fertility order within the study area of: 70% low fertility, 22% moderate fertility, 3% very low fertility, and 5% reference terms. This work evidence that which can be predicted from S2A images and provides a reference for soil fertility monitoring in drylands. Additionally, this model can be easily applied to environmental conditions similar to those of the studied area
