    Fibromatose desmoĂŻde Ă  localisation nasale

    Introduction : Les fibromatoses desmoïdes sont des lésions histologiquement bénignes infiltrant les muscles et les aponévroses. Ces tumeurs siègent le plus souvent au niveau de la paroi abdominale et thoracique. La localisation cervicofaciale est rare. Observation : Nous rapportons le cas d’un nourrisson âgé de 1 an 9 mois qui a présenté une fibromatose desmoïde à localisation nasale. Le patient a été opéré par voie endonasale sous guidage endoscopique avec exérèse tumorale en monobloc. L’évolution a été favorable sans récidive pour un recul de 24 mois. Conclusion : Les fibromatoses desmoïdes correspondent à des proliférations fibreuses infiltrantes mais non métastasiantes. La chirurgie est le traitement de choix, toutefois ces tumeurs sont caractérisées par un risque de récidive imposant un suivi à long terme. Mots clés : Fibromatose desmoïde, cavité nasale, chirurgie

    Fluid Injection Optimization Using Modified Global Dynamic Harmony Search

    Abstract One of the mostly used enhanced oil recovery methods is the injection of water or gas under pressure to maintain or reverse the declining pressure in a reservoir. Several parameters should be optimized in a fluid injection process. The usual optimizing methods evaluate several scenarios to find the best solution. Since it is required to run the reservoir simulator hundreds of times, the process is very time consuming and cumbersome. In this study a new intelligent method of optimization, called "global dynamic harmony search" is used with some modifications in combination with a commercial reservoir simulator (ECLIPSE ® ) to determine the optimum solution for fluid injection problem unknowns. Net present value (NPV) is used as objective function to be maximized. First a simple homogeneous reservoir model is used for validating the developed method and then the new optimization method is applied to a real model of one of the Iran oil reservoirs. Three strategies, including gas injection, water injection, and well placement are considered. Comparing the values of NPV and field oil efficiency (FOE) of gas injection and water injection strategies, it is concluded that water injection strategy surpasses its rival. Considering water injection to be the base case, a well placement optimization is also done and best locations for water injection wells are proposed. The results show the satisfying performance of the algorithm regarding its low iterations

    AbstractBackgroundSurgical treatment of early-onset scoliosis has greatly developed in recent years. Early-onset scoliosis covers a variety of etiologies (idiopathic, neurologic, dystrophic, malformative, etc.) with onset before the age of 5 years. Progression and severity threaten respiratory development and may result in respiratory failure in adulthood. Many surgical techniques have been developed in recent years, aiming to protect spinal and thoracic development.Material and methodsPresent techniques are based on one of two main principles. The first consists in posterior distraction of the spine in its concavity (single growing rod, or vertical expandable prosthetic titanium rib [VEPTR]), or on either side (dual rod); this requires iterative surgery, for lengthening, unless motorized using energy provided by a magnetic system. The second option is to use spinal growth force to lengthen the assembly; these techniques (Luque Trolley, Shilla), using a sliding assembly, are known as growth guidance.ResultsThese techniques are effective in controlling early scoliotic deformity, and to some extent restore spinal growth. However, they show a high rate of complications: infection, rod breakage, spinal fixation pull out and, above all, progressive spinal stiffness, reducing long-term efficacy. Respiratory gain is harder to assess, as thoracic expansion does not systematically improve respiratory function, particularly due to impaired compliance of the thoracic cage

    L’hématome organisé du sinus maxillaire est une entité rare. Son étiopathogénie reste incertaine. Nous rapportons le cas d’une patiente âgée de 16 ans présentant une épistaxis unilatérale droite de grande abondance avec une endoscopie nasale strictement normale. L’imagerie était en faveur d’une tumeur vasculaire du sinus maxillaire. Une exérèse chirurgicale complète a été réalisée par voie combinée avec des suites simples. L’examen anatomopathologique a conclu à un hématome organisé du sinus maxillaire. Nous discutons, dans ce travail, l’étiopathogénie, les caractéristiques cliniques, radiologiques et les volets thérapeutiques de cette entité.Mots clés : Hématome organisé, sinus maxillaire, tomodensitométrie, chirurgie

    Genome-wide footprints in the carob tree (Ceratonia siliqua) unveil a new domestication pattern of a fruit tree in the Mediterranean

    Intense research efforts over the last two decades have renewed our understanding of plant phylogeography and domestication in the Mediterranean basin. Here we aim to investigate the evolutionary history and the origin of domestication of the carob tree (Ceratonia siliqua), which has been cultivated for millennia for food and fodder. We used >1000 microsatellite genotypes to delimit seven carob evolutionary units (CEUs). We investigated genome-wide diversity and evolutionary patterns of the CEUs with 3557 single nucleotide polymorphisms generated by restriction-site associated DNA sequencing (RADseq). To address the complex wild vs. cultivated status of sampled trees, we classified 56 sampled populations across the Mediterranean basin as wild, seminatural or cultivated. Nuclear and cytoplasmic loci were identified from RADseq data and separated for analyses. Phylogenetic analyses of these genomic-wide data allowed us to resolve west-to-east expansions from a single long-term refugium probably located in the foothills of the High Atlas Mountains near the Atlantic coast. Our findings support multiple origins of domestication with a low impact on the genetic diversity at range-wide level. The carob was mostly domesticated from locally selected wild genotypes and scattered long-distance westward dispersals of domesticated varieties by humans, concomitant with major historical migrations by Romans, Greeks and Arabs. Ex situ efforts to preserve carob genetic resources should prioritize accessions from both western and eastern populations, with emphasis on the most differentiated CEUs situated in southwest Morocco, south Spain and eastern Mediterranean. Our study highlights the relevance of wild and seminatural habitats in the conservation of genetic resources for cultivated trees

    Hormonal crises following receptor radionuclide therapy with the radiolabeled somatostatin analogue [177Lu-DOTA0,Tyr3]octreotate

    textabstractIntroduction: Receptor radionuclide therapy is a promising treatment modality for patients with neuroendocrine tumors for whom alternative treatments are limited. The aim of this study was to investigate the incidence of hormonal crises after therapy with the radiolabeled somatostatin analogue [177Lu-DOTA0,Tyr3]octreotate (177Lu-octreotate). Materials and methods: All177Lu- octreotate treatments between January 2000 and January 2007 were investigated. Four hundred seventy-six patients with gastroenteropancreatic neuroendocrine tumors and three patients with metastatic pheochromocytoma were included for analysis. Results: Four hundred seventy-nine patients received a total of 1,693 administrations of177Lu-octreotate. Six of 479 patients (1%) developed severe symptoms because of massive release of bioactive substances after the first cycle of177Lu-octreotate. One patient had a metastatic hormone-producing small intestinal carcinoid; two patients had metastatic, hormone-producing bronchial carcinoids; two patients had vasoactive intestinal polypeptide-producing pancreatic endocrine tumors (VIPomas); and one patient had a metastatic pheochromocytoma. With adequate treatment, all patients eventually recovered. Conclusion: Hormonal crises after177Lu- octreotate therapy occur in 1% of patients. Generally,177Lu- octreotate therapy is well tolerated

    Tolllike receptor 4 (TLR4) polymorphisms in Tunisian patients with Crohn's disease: genotype-phenotype correlation

    <p>Abstract</p> <p>Background</p> <p>The immune responses to bacterial products through the pattern recognition receptor (PRR) play a pivotal role in pathogenesis of Crohn's disease. A recent study described an association between CD and some gene coding for bacterial receptor like NOD2/CARD15 gene and TLR4. In this study, we sought to determine whether TLR4 gene was associated with Crohn's disease (CD) among the Tunisian population and its correlation with clinical manifestation of the disease.</p> <p>Methods</p> <p>90 patients with CD and 80 healthy individuals are genotyped for the <it>Asp299Gly </it>and <it>Thr399Ile </it>polymorphisms by restriction fragment length polymorphism analysis.</p> <p>Results</p> <p>The allele and genotype frequency of the TLR4 polymorphisms did not differ between patients and controls. The genotype-phenotype correlation permitted to show that the <it>Thr399Ile </it>polymorphism was associated with early onset disease.</p> <p>Conclusion</p> <p>this study reported the absence of association between CD and TLR4 gene in the Tunisian population, but this gene could play a role in clinical expression of the disease.</p

Association of nucleolin with F. tularensis during infection continues intracellularly after endocytosis of the bacteria. The present work therefore unravels for the first time the presence of nucleolin in the phagosomal compartment of macrophages
