    Mindfulness Based Self Care Toolkit for Psychiatric Healthcare Staff

    Abstract Background: Healthcare professionals (HCPs) are at high risk of stress, burnout, and compassion fatigue (CF). Nearly half of the doctors and one-third of the nurses experience burnout in the United States. The risk becomes more evident during the coronavirus disease (COVID -19) pandemic as more patients and healthcare providers seek mental health services. Problem: The physical and mental health of HCPs is impacted by burnout and CF, leading to increased practice errors, decreased patient safety, and negative effects on HCPs’ physical and mental health. Mindfulness practice is beneficial to reduce stress and burnout in various populations, but HCPs have poor knowledge about mindfulness and its benefits. Also, Mindfulness-based self-care measures are not promoted in HCPs. Context: The staff at the psychiatric hospital (mental health technicians, nurses, case managers, psychiatrists) who are at high risk of BO, CF, and stress would be educated about mindfulness-based self-care strategies by a toolkit. The needed support from stakeholders at the hospital is acquired by various stakeholder management strategies. Proposed Interventions: A mindfulness-based self-care toolkit and a training session about various mindfulness strategies would be implemented. Other resources like flyers, mindfulness themes in units, and cheer-up sessions for staff would be implemented to encourage them to practice mindfulness. Proposed Outcome Measures: Staff’s knowledge about mindfulness, use of mindfulness as a means of self-care, and ease of use of the toolkit would be measured by a post-intervention questionnaire

Local Problem: HCPs have poor knowledge about mindfulness and its benefits. Mindfulness-based self-care measures are not promoted in HCP. Methods: An extensive database search was done to find evidence about using mindfulness-based strategies to relieve burnout, stress, and compassion fatigue in HCP. Interventions: A critical appraisal of selected articles was done using the Johns Hopkins Evidence-Based Appraisal tool to find the highest quality of evidence. An integrated review and synthesis of the literature was performed to determine if compelling evidence exists to use mindfulness-based strategies in HCP. Results: There is compelling evidence that using various mindfulness-based strategies helps reduce burnout and stress and increases compassion and empathy in HCP. However, there is a lack of high-quality literature measuring the effects of mindfulness practice on compassion fatigue. Conclusions: Mindfulness-based strategies should be promoted in healthcare settings to relieve stress and burnout and increase compassion levels in HCP. Also, more empirical data should be produced showing the effects of mindfulness on compassion fatigue

    PARENTAL AWARENESS ON PRENATAL CONCERNS: AN EDUCATIONAL PERSPECTIVE

    Having a healthy baby is a dream of every parent and a cherished goal of every nation. However, the latest statistics indicate that in US about one in every 33 babies is born with birth defects or disabilities, and that the birth defects are one of the most leading causes of infant deaths, accounting for more than 20% of all infant deaths. Many of these birth defects or disabilities are avoidable if precautions are taken and care is provided during prenatal period. Awareness among parents about such prenatal concerns is therefore, critical. This exploratory pilot study was conducted to find out the level of basic awareness among parents about prenatal concerns and avoidable factors leading to birth defects and disabilities. The subjects (N= 73) were African American parents residing in the city of Dover and surrounding areas in Delaware State. The subjects were interviewed and administered the Prenatal Concerns Survey designed for this research. The results on various aspects revealed unawareness among parent on many critical issues, and the overall results indicate that 19.4% of the parents expressed ignorance about the basic prenatal concerns. A broad based research is recommended for in-depth analysis

    Quantitative characterization of viscoelastic behavior in tissue-mimicking phantoms and ex vivo animal tissues.

    Viscoelasticity of soft tissue is often related to pathology, and therefore, has become an important diagnostic indicator in the clinical assessment of suspect tissue. Surgeons, particularly within head and neck subsites, typically use palpation techniques for intra-operative tumor detection. This detection method, however, is highly subjective and often fails to detect small or deep abnormalities. Vibroacoustography (VA) and similar methods have previously been used to distinguish tissue with high-contrast, but a firm understanding of the main contrast mechanism has yet to be verified. The contributions of tissue mechanical properties in VA images have been difficult to verify given the limited literature on viscoelastic properties of various normal and diseased tissue. This paper aims to investigate viscoelasticity theory and present a detailed description of viscoelastic experimental results obtained in tissue-mimicking phantoms (TMPs) and ex vivo tissues to verify the main contrast mechanism in VA and similar imaging modalities. A spherical-tip micro-indentation technique was employed with the Hertzian model to acquire absolute, quantitative, point measurements of the elastic modulus (E), long term shear modulus (η), and time constant (τ) in homogeneous TMPs and ex vivo tissue in rat liver and porcine liver and gallbladder. Viscoelastic differences observed between porcine liver and gallbladder tissue suggest that imaging modalities which utilize the mechanical properties of tissue as a primary contrast mechanism can potentially be used to quantitatively differentiate between proximate organs in a clinical setting. These results may facilitate more accurate tissue modeling and add information not currently available to the field of systems characterization and biomedical research

    A seven year experience in patients undergoing total thyroidectomy for benign diseases in a single unit of tertiary care centre: a retrospective case study

    Background: Thyroidectomy is a common surgical procedure performed worldwide by surgeons with varied training. It is the experience and the skills by which a surgeon performs, the indication of surgery and the extent of surgery which determines the outcome and the complication rates.Method: This study is a retrospective study performed from January 2012 to December 2018 when a total of 170 patients underwent total thyroidectomy via kocher’s incision for benign diseases with clinically in hypothyroid state at Safdarjung Hospital, New Delhi in a single unit of a tertiary care hospital.Results: The male: female ratio was 1:7.09 with average age of patients being 40years. The overall complication rate was 18.82% (32 patients). Postoperative transient hypocalcaemia being the most common complication. There was no case of permanent hypocalcaemia. There was no injury to any nerve (recurrent or superior laryngeal). Transient recurrent laryngeal paresis was noted in 4(2.35%) which resolved with conservative management. Complications like wound infection, sympathetic chain injury were not noted. The characteristic feature of this study was five cases of giant secondary retrosternal goitre which had average weight of greater than 500 grams which were operated using kocher’s incision. The average weight of other 165 patients was 70grams +/- 12 grams. There were no major post-operative complications except for post-operative recurrent laryngeal nerve paresis and seroma formation which was evacuated after 3 weeks of surgery.Conclusion: Hence, from this it is concluded that it is the surgical experience and skills of the surgeon which make total thyroidectomy a safe procedure providing permanent cure with a low post-operative morbidity risk even with large sized retrosternal thyroid glands which can be operated via neck incision

    Inversion Method for Early Detection of ARES-1 Case Breach Failure

    A document describes research into the problem of detecting a case breach formation at an early stage of a rocket flight. An inversion algorithm for case breach allocation is proposed and analyzed. It is shown how the case breach can be allocated at an early stage of its development by using the rocket sensor data and the output data from the control block of the rocket navigation system. The results are simulated with MATLAB/Simulink software. The efficiency of an inversion algorithm for a case breach location is discussed. The research was devoted to the analysis of the ARES-l flight during the first 120 seconds after the launch and early prediction of case breach failure. During this time, the rocket is propelled by its first-stage Solid Rocket Booster (SRB). If a breach appears in SRB case, the gases escaping through it will produce the (side) thrust directed perpendicular to the rocket axis. The side thrust creates torque influencing the rocket attitude. The ARES-l control system will compensate for the side thrust until it reaches some critical value, after which the flight will be uncontrollable. The objective of this work was to obtain the start time of case breach development and its location using the rocket inertial navigation sensors and GNC data. The algorithm was effective for the detection and location of a breach in an SRB field joint at an early stage of its development

    Peripheral Fluorescein Angiographic Findings in Fellow Eyes of Patients with Branch Retinal Vein Occlusion

    Introduction. Branch retinal vein occlusion (BRVO) is a common retinal vascular condition that results in intraocular inflammatory changes. Ultra wide field fluorescein angiography (UWFFA) is a retinal imaging device that can capture peripheral retinal findings. The purpose of this study was to look for peripheral findings in the fellow eye of patients with BRVO using UWFFA. Methods. Retrospective imaging review of patients diagnosed with BRVO that had both eyes imaged with UWFFA. Images were graded for peripheral findings in other quadrants of the same eye as well as in all quadrants of the fellow eye. Results. Of 81 patients, 14 (17%) patients had late vascular leakage in a quadrant other than the BRVO distribution. Five (6%) findings were in the same eye, 8 (10%) findings were in the fellow eye, and 1 (1%) finding was in both the same eye and the fellow eye. Of these 14 patients, 11 (80%) patients had hypertension. Conclusion. Late peripheral retinal leakage in the fellow eye of patients with BRVO was detected in this cohort of patients with UWFFA. This novel finding may represent underlying systemic inflammation, hypertension, or bilateral BRVOs
