    O cuidado com a saúde em âmbito hospitalar exige o trabalho em equipe multidisciplinar, fato que demandou a introdução da Odontologia nesse ambiente de trabalho. A odontologia Hospitalar tem como objetivo, dá assistência e cuidados a pacientes que se encontram sob internação hospitalar, visto que o cirurgião-dentista é um profissional capacitado para, detectar, prevenir e cuidar das alterações bucais. A inserção do CD em ambiente hospitalar tem sido instituída mediante a resoluções e leis. Sendo assim, o objetivo deste trabalho é apresentar trajetória da Odontologia Hospitalar no Brasil, verificando a situação atual e mostrar sobre a importância desse profissional na equipe multidisciplinar dos hospitais. As buscam foram feitas nas principais bases de dados na área da saúde: PubMed, Scopus e BVS (Biblioteca Virtual em Saúde). Não sendo estipulado limite para o período de busca dos artigos, bem como não houve restrição de idioma

    Em busca da identidade dos instrumentos musicais no Brasil: um estudo exploratório da literatura de cordel

    Based on a collection of 2340 poems, the present article aims to explore the identity of musical instruments considered most popular by the printed Literatura de Cordel (Cordel Literature) in the countryside of Northeastern and Northern regions of Brazil, from the end of the 19th Century to present days. The Cordel Literature is known for representing the views of the social group from which it is originated rather than the creative work of its poets/writers. In search of musical instruments mentioned in the text, some of them were selected due to frequency and relevance of the context found: e.g. the Brazilian viola (a five course guitar), the violão (the six string guitar), the violin, the mandolin, the rabeca (Brazilian fiddle), the electric guitar and the piano. The violão and the Brazilian viola, which are similar in shape, are seen by that population in quite different ways. The fiddle is a popular instrument, but had only a few mentions. Other instruments like the violin, the mandolin, the piano and the electric guitar are described as urban instruments, thus less known in that context.O presente trabalho procura, em consulta a uma coleção de 2340 obras da Literatura de Cordel, explorar identidades culturais presentes nos instrumentos musicais aparentemente mais populares junto à população cultora de tal forma literária - sertão nordestino e parte da Região Norte do Brasil - desde o final do século XIX até o presente. Há no trabalho o pressuposto de que a Literatura de Cordel representa, muito mais que o trabalho criativo dos seus autores, as práticas vigentes no grupo social que a origina. Assim, à procura de menções a instrumentos musicais, este estudo detém-se em alguns deles, pela frequência e relevância da sua caracterização: viola caipira, violão, violino, bandolim, rabeca, guitarra elétrica e piano. Instrumentos aparentemente próximos como a viola e o violão são vistos de formas muito distintas pela população considerada. A rabeca, instrumento popular, poucas vezes é citada. Outros instrumentos, como violino, bandolim, piano e guitarra elétrica, são retratados como instrumentos urbanos e menos conhecidos

Infection can lead to two main symptomatologies: adult T-cell lymphoma/leukemia (ATLL) and HTLV-1 associated myelopathy/tropical spastic paraparesis (HAM/TSP). Each manifestation is associated with distinct characteristics, as ATLL presents as a leukemia-like disease, while HAM/TSP presents as severe inflammation in the central nervous system, leading to paraparesis. Previous studies have identified molecules associated with disease development, e.g., the downregulation of Foxp3 in Treg cells was associated with increased risk of HAM/TSP. In addition, elevated levels of CXCL10, CXCL9, and Neopterin in cerebrospinal fluid also present increased risk. However, these molecules were only associated with specific patient groups or viral strains. Furthermore, the majority of studies did not jointly compare all clinical manifestations, and robust analysis entails the inclusion of both ATLL and HAM/TSP. The low numbers of samples also pose difficulties in conducting gene expression analysis to identify specific molecular relationships. To address these limitations and increase the power of manifestation-specific gene associations, meta-analysis was performed using publicly available gene expression data. The application of supervised learning techniques identified alterations in two genes observed to act in tandem as potential biomarkers: GBP2 was associated with HAM/TSP, and CD40LG with ATLL. Together, both molecules demonstrated high sample-classification accuracy (AUC values: 0.88 and 1.0, respectively). Next, other genes with expression correlated to these genes were identified, and we attempted to relate the enriched pathways identified with the characteristic of each clinical manifestation. The present findings contribute to knowledge surrounding viral progression and suggest a potentially powerful new tool for the molecular classification of HTLV-associated diseases

    Evaluation of a quality improvement intervention to reduce anastomotic leak following right colectomy (EAGLE): pragmatic, batched stepped-wedge, cluster-randomized trial in 64 countries

    Background Anastomotic leak affects 8 per cent of patients after right colectomy with a 10-fold increased risk of postoperative death. The EAGLE study aimed to develop and test whether an international, standardized quality improvement intervention could reduce anastomotic leaks. Methods The internationally intended protocol, iteratively co-developed by a multistage Delphi process, comprised an online educational module introducing risk stratification, an intraoperative checklist, and harmonized surgical techniques. Clusters (hospital teams) were randomized to one of three arms with varied sequences of intervention/data collection by a derived stepped-wedge batch design (at least 18 hospital teams per batch). Patients were blinded to the study allocation. Low- and middle-income country enrolment was encouraged. The primary outcome (assessed by intention to treat) was anastomotic leak rate, and subgroup analyses by module completion (at least 80 per cent of surgeons, high engagement; less than 50 per cent, low engagement) were preplanned. Results A total 355 hospital teams registered, with 332 from 64 countries (39.2 per cent low and middle income) included in the final analysis. The online modules were completed by half of the surgeons (2143 of 4411). The primary analysis included 3039 of the 3268 patients recruited (206 patients had no anastomosis and 23 were lost to follow-up), with anastomotic leaks arising before and after the intervention in 10.1 and 9.6 per cent respectively (adjusted OR 0.87, 95 per cent c.i. 0.59 to 1.30; P = 0.498). The proportion of surgeons completing the educational modules was an influence: the leak rate decreased from 12.2 per cent (61 of 500) before intervention to 5.1 per cent (24 of 473) after intervention in high-engagement centres (adjusted OR 0.36, 0.20 to 0.64; P < 0.001), but this was not observed in low-engagement hospitals (8.3 per cent (59 of 714) and 13.8 per cent (61 of 443) respectively; adjusted OR 2.09, 1.31 to 3.31). Conclusion Completion of globally available digital training by engaged teams can alter anastomotic leak rates. Registration number: NCT04270721 (http://www.clinicaltrials.gov)
