    Sexual Violence Victimization, Mental Health, and University-Based Health Service Use Among College Females

    Females between the ages of 18 and 24 consistently experience higher rates of sexual violence compared to females in any other age group (Sinozich & Langton, 2014). Approximately one in five college women experience sexual violence victimization (SVV; Krebs, Lindquist, Berzofsky, Shook-Sa & Peterson, 2016) during the academic year. Among undergraduate female students, 23.1% are victims of sexual violence due to physical force, violence or incapacitation (Cantor et al., 2015). Sexual victimization has been associated with several short- and long-term psychological health consequences, accounting for enhanced symptoms of depression, anxiety, PTSD, and suicide risk (Basile, Smith, Breiding, Black & Mahendra, 2014). Compared with the general population, college women are consistently less likely to disclose an experience of sexual assault (Fisher, Cullen & Turner, 2000). Despite the high rates of sexual violence against college females, only 11% of rapes are reported to college authorities, making sexual victimization the most underreported violent crime among this population (Kilpatrick, Resnick, Ruggiero, Conoscenti & McCauley, 2007). College students are 13% less likely to report an incident of sexual assault to police versus nonstudents in the same age group (Sinozich & Langton, 2014) and one-third of female students never tell anyone about the victimization (Fisher et al., 2000) The overarching purpose of this dissertation was to examine sexual violence victimization, mental health and health service utilization among college females using an emotion science framework. The first purpose of this dissertation was to perform a systematic review to examine the frequency of sexual victimization and the moderating characteristics of utilization of college-based health resources (Stoner & Cramer, 2017; Article I). The second purpose was to test a coping-mental health framework for the prevention of suicide among sexual minority and heterosexual victims of assault sexual assault victims (Article II). The third purpose was to examine rates of sexual victimization and health service utilization in a sample population, to examine how mental health symptoms impact health service use, and examine the rates of actual and willingness to use health services (Article III). The systematic review (Article I) revealed a gross mismatch between the prevalence rates of sexual victimization and the utilization rates of health services post victimization. Findings from Article II suggested that an association exists between sexual assault and suicidality and is significantly stronger among individuals who self-identify as a sexual minority. Further, the findings supported a coping-mental health framework for the prevention of suicide among sexual assault victims. Article III uncovered higher rates of victimization among the sample population compared to rates found in other national studies. Further, this research supported prior literature on health service use, suggesting a low frequency of usage of on-campus health resources

    Methods to Improve the Reliability of the Functional Reach Test in Children and Adolescents With Typical Development

    Purpose: Test-retest reliability of the Functional Reach Test was examined in children with typical development by comparing standard and alternate methods. Methods: Eighty subjects ages seven to 16 years were tested and 69 retested for four methods of Functional Reach Test (ie, one-arm finger-to-finger, two-arm finger-to-finger, one-arm toe-to-finger, and two-arm toe-to-finger). Intraclass correlation coefficients and limits of agreement were calculated. Results: Intraclass correlation coefficients were high in toe-to-finger measurement methods (0.97– 0.98) for the entire group and specific age groups (00.83–0.93). Toe-to-finger methods were more reliable than finger-to-finger methods. The two-arm toe-to-finger method had the best limis of agreement with approximately ±5 cm indicated by the 95% confidence interval. Conclusions: Test-retest reliability using a toe-to-finger method of measuring is stronger than previously reported when using traditional methods. Limits of agreement analyses imply a change of 5 cm or more is likely to represent a true clinical difference when using the two-arm toe-to-finger method

    Conflict, coexistence, or both? Cougar habitat selection, prey composition, and mortality in a multiple-use landscape

    Western North America is experiencing remarkable human population growth and land-use change. Irrigation and associated cultivation have led to colonization of urban-wildland interface (UWI) environments by mule deer (Odocoileus hemionus), and consequently, cougars (Puma concolor). In the wake of these changes, human-wildlife conflicts have increased in tandem with questions about long-term species conservation. To address these concerns, we fit 79 cougars with radio-telemetry collars in the Oquirrh Mountains near Salt Lake City, Utah (2002–2010). Our goal was to evaluate variation in cougar habitat selection, diet, and cause-specific mortality in a landscape dominated by urban, military, and industrial activities. We used radio-telemetry data in concert with Re-source Selection Functions to address three hypotheses: (1) that cougars would select wildland over UWI land-uses; (2) prey composition would reflect differences in land-use; and (3) mortality would be predominantly human-caused. Cougars largely selected wildland habitats associated with seasonal mule deer presence, but contrary to expectation, they also selected habitats closer to urban and mined areas. Prey composition in the UWI did not differ from wildland habitats. Domestic ungulates represented only 2% of 540 recovered prey items and were found primarily in wildlands. Native ungulates comprised \u3e 90% of the total kill, irrespective of season or land-use, suggesting that use of UWI habitats was linked to mule deer presence. Cougar mortality was disproportionately due to natural causes in wildlands, but individuals that died of human causes in UWI habitats were more likely to be inexperienced hunters, supporting young kittens, or compromised by physical handicaps. In general, presence of mule deer was the key predictor of cougar habitat use, even in this highly disturbed, anthropogenically altered landscape. As such, management designed to reduce conflict and ensure conservation will need to focus on urban deer, land-use planning, and targeted education campaigns to reduce food subsidies

    Factors Affecting Functional Reach Scores in Youth with Typical Development

    Purpose: Functional Reach Test scores were examined for the effects of traditional and alternate methods and subject characteristics. Methods: Eighty subjects aged 7 to 16 years were tested. Effects of measurement method (from finger-to-finger or from toe-to-finger) and style of reach (1 or 2 arms) were investigated. Five subject variables were analyzed for interactions among the methods and groups defined by subject characteristics. Results: Measurement method and style of reach showed a significant interaction. Interaction of method with subject characteristics was significant for age, height, and base of support only. Height groups by quartile were significantly different and scores increased with height, especially in toes-to-finger methods. Conclusions: Functional Reach Test scores were affected by method of reach and method of measurement. Height categories may be more useful when using the test for discriminative purposes, especially with toes-to-finger methods

    Urinary phthalate metabolite concentrations and blood glucose levels during pregnancy

    Purpose: To examine associations between phthalate metabolite urinary concentrations during early pregnancy and blood glucose levels obtained at the time of screening for gestational diabetes mellitus (GDM). Methods: Upon initiation of prenatal care, women with a mean gestational age of 12.8 weeks were recruited for a study of environmental chemical exposures (n = 110) and provided a spot urinary specimen. Blood glucose concentrations (mg/dl) were obtained from the electronic medical record for those patients who did not experience a pregnancy loss and did not transfer care to another facility prior to glucose screening (n = 72). Urinary concentrations of nine phthalate metabolites and creatinine were measured at the US Centers for Disease Control and Prevention. Associations between tertiles of phthalate metabolites concentrations and blood glucose levels were estimated using linear regression. Results: Compared to pregnant women in the lowest concentration tertile, women with the highest urinary concentrations (≥ 3 rd tertile) of mono-iso-butyl phthalate (tertile: ≥ 15.3 μg/l, β = -18.3, 95% CI: -35.4, -1.2) and monobenzyl phthalate (tertile: ≥ 30.3 μg/l, β = -17.3, 95% CI: -34.1, -0.4) had lower blood glucose levels at the time of GDM screening after adjustment for urinary creatinine and demographic covariates. Conclusion: Because maternal glucose levels increase during pregnancy to provide adequate nutrition for fetal growth and development, these findings may have implications for fetal health. However, given the limitations of our study, findings should be interpreted cautiously

    Is bisphenol-A exposure during pregnancy associated with blood glucose levels or diagnosis of gestational diabetes?

    Recent epidemiological studies indicate bisphenol A (BPA), an estrogenic chemical used in production of epoxy, polycarbonate, and plastic may increase risk of insulin resistance and type 2 diabetes. Exposure to BPA during pregnancy may contribute to development of gestational diabetes mellitus (GDM), a precursor to type 2 diabetes in women. This pilot study examined the association between BPA exposure, fasting blood glucose levels (FBG), and GDM diagnosis during pregnancy. Banked urine samples from 22 cases of GDM and 72 controls were analyzed for total (free BPA + conjugates) urinary BPA concentrations (μg/L). FBG levels (mg/dl) were obtained from 1-h 50-g glucose tolerance tests (GTT) that women underwent for routine GDM screening (mean gestational age = 26.6 weeks, SD = 3.8). Those with an initial screening value ≥ 135 mg/dl underwent 3-h 100 g oral GTT. GDM diagnoses were made when the initial screening value was ≥ 200 mg/dl or when values at ≥ 2 time points exceeded 3-h oral GTT thresholds. Among controls, median FBG levels (mg/dL) did not differ across exposure tertiles, defined according to the distribution of total specific-gravity-adjusted urinary BPA concentrations. Logistic regression models controlling for race/ethnicity did not provide evidence of association between BPA exposure and case status across increasing tertiles of BPA exposure (number of GDM cases/controls in tertile1: 13/24; in tertile 2: 6/24; in tertile 3: 3/24). Findings do not support a relationship between total urinary BPA concentrations and altered glucose metabolism during pregnancy. However, due to study limitations, findings need to be interpreted with caution
