We report a case of a 84-year-old male hospitalized for bacterial pneumonia who, during hospitalization, developed a flexural exanthema in parallel with a positive swab for SARS-CoV-2. Supportive therapy was instituted, the rash disappeared in 7 days and the patient fully recovered. At the same time, two other cases of SARS-CoV-2 infection occurred in the same ward. Histopathology and immunohistochemistry of a skin biopsy showed a scarce predominantly perivascular lymphocytic infiltration in the upper dermis, predominantly by CD4+ T cells, a slight epidermotropism, spongiosis and focal parakeratosis, compatible with a viral exanthema or a maculopapular drug eruption. Patch testing with possible culprit drugs were negative. We seek to add value in understanding all the manifestations of SARS-CoV-2 infection and to draw attention to the importance of early identification of skin manifestations in association with COVID-19.Os autores descrevem o caso de um homem de 84 anos hospitalizado por pneumonia bacteriana que foi posteriormente diagnosticado com COVID-19 e observado com exantema flexural.

Calodium hepaticum: Household Clustering Transmission and the Finding of a Source of Human Spurious Infection in a Community of the Amazon Region

Background: Calodium hepaticum (syn. Capillaria hepatica) is a worldwide helminth parasite of which several aspects of transmission still remain unclear. In the Amazon region, the mechanism of transmission based on the ingestion of eggs present in the liver of wild mammals has been suggested as the cause of the spurious infections described. We performed an epidemiological investigation to determine the incidence, risk of spurious infection and the dynamics of transmission of C. hepaticum in a community of the Brazilian Amazon. Methodology/Principal Findings: Stool samples of 135 individuals, two dog feces and liver tissue from a peccary (captured and eaten by the residents) were analyzed by conventional microscopy. Dog feces were collected from the gardens of households presenting human cases of spurious C. hepaticum infections. Community practices and feeding habits related to the transmission of the parasite were investigated. The individual incidence of spurious infection was 6.7% (95% CI: 2.08-11.24). Cases of spurious infection were observed in 7.5% of the families and the household incidence was from 50% to 83.3%. The risk of spurious infection was 10-fold greater in persons consuming the liver of wild mammals (p = 0.02). The liver tissue of a peccary and one feces sample of a dog presented eggs of C. hepaticum. The consumption of the infected liver was the cause of the spurious infections reported in one household. Conclusions/Significance: This is the first identification of a source of spurious infection by C. hepaticum in humans and we describe a high rate of incidence in household clusters related to game liver alimentary habits. The finding of a dog feces contaminating peridomiciliary ground suggests the risk of new infections. We conclude that the mechanism of transmission based on the ingestion of liver is important for the dynamics of transmission of C. hepaticum in the studied area

Volunteered Geographical Information and Recreational Uses within Metropolitan and Rural Contexts

UIDB/04647/2020 UIDP/04647/2020 DL 57/2016/CP1453/CT0004Data obtained through Volunteered Geographical Information (VGI) have gradually been used to monitor and support planning mainly in urban contexts. Regarding recreational activities in peri-urban green and natural areas, VGI has been used to map, measure use intensity, profile users, and evaluate their preferences and motivations. Given their extensive use, it is now worthwhile to assess the value of VGI data to (1) compare recreational uses, profile users and map recreational activities in different contexts (metropolitan vs. rural areas), and (2) evaluate outdoor and adventure tourist products such as Grand Routes (GR). Data from former GPSies (AllTrails nowadays), one of the most popular web-share services, were used to assess recreational uses in Lisbon Metropolitan Area (LMA) and southwest Portugal (SWPT). A set of 22,031 tracks of “on foot” and “on wheels” activities, submitted by 3297 national and foreign users, covering 12 years, was analysed within a GIS modelling environment. Results indicate that, although there are many more submissions in the LMA, the influence of foreigners in the SWPT is higher (11% vs. 19%). The existing GR in SWPT concentrates the foreign use for hiking (71% of foreign vs. 28% of national users), demonstrating its attractiveness. For the favourite activity in both areas—Mountain biking—results show a higher spatial dispersion, yet part of the activity in SWPT still conforms to the GR (16% of foreign and 20% of national use). This study proves other applications for VGI, showing its usefulness for assessing recreational uses in both metropolitan and rural areas. Spatial knowledge about recreational uses is a valuable tool to evaluate and monitor such activities, and to know what users like to do, and where, and is also useful information when designing recreational products considering their tourist potential, thus adding value to these offers.publishersversionpublishe

    Get PDF
