    Infective endocarditis due to Bartonella bacilliformis associated with systemic vasculitis: a case report

    Infective endocarditis due to Bartonella bacilliformis is rare. A 64-year-old woman, without previous heart disease, presented with 6 weeks of fever, myalgias, and arthralgias. A systolic murmur was heard on the tricuspid area upon examination, and an echocardiogram showed endocardial lesions in the right atrium. Bartonella bacilliformis was isolated in blood cultures, defining the diagnosis of infective endocarditis using Duke’s criteria. Subsequently, the patient developed clinical and laboratory features compatible with antineutrophil cytoplasmic antibody-associated vasculitis. This case presents an uncommon complication of B. bacilliformis infection associated with the development of systemic vasculitis.Revisión por pare

    Determinantes sociales asociados con la realización de mamografías bienales en mujeres de 50 a 59 años en Perú, 2015

    Introduction: Breast cancer is a disease of great incidence and mortality in the world. Early identification, through mammography, could reduce its mortality. The objective of this study is to identify the social determinants associated with mammography in the last two years in women aged 50 to 59 years in Peru. Material and methods: Cross-sectional analytical study of the Demographic and Family Health Survey (ENDES) 2015 of Peru. The mammography was measured with the self-report of the survey. The survey asked about mammography in the last two years (yes vs no). We calculated prevalence ratios as a measure of association (with a confidence interval of 95%), using the generalized linear models of the Poisson family. All analyzes considered the complex nature of the sample. Results: We evaluated a final sample of 1808 women. The average age was 54 years (d.e: 2.5 years). We found a prevalence of 21.9% [95% CI: 18.9 to 25.1] of women who reported having undergone mammography less than two years ago. The higher the socioeconomic position, the higher the prevalence of screening (3.2% vs 41.4% in extreme quintiles of the socioeconomic level, p <0.001). In the adjusted models, we found that a higher socioeconomic level (PR: 5.81, 95% CI: 2.28 to 14.79), the level of higher education (PR: 2.03, IC95%: 1.30 a 3,15), found to be insured by MINSA (RP: 2.21, IC95%: 1.28 to 3.82), EsSalud (RP: 4.37, IC95%: 2.67 to 7.15), were associated with the realization of this screening. Conclusion: The realization of mammography is associated with various sociocultural factors such as the level of education, socioeconomic level and being insured. It is urgent to initiate social and health policies to eliminate inequalities.TesisIntroducción: El cáncer de mama es una patología de gran incidencia y mortalidad en el mundo. Su identificación temprana, a través de la mamografía, podría reducir su mortalidad. El presente estudio tiene como objetivo identificar los determinantes sociales asociados con la realización de la mamografía en los últimos dos años en mujeres de 50 a 59 años en Perú. Material y métodos: Estudio analítico transversal de la Encuesta Demográfica y de Salud Familiar (ENDES) 2015 de Perú. La realización de mamografía se midió con el autoreporte de la encuesta. Se preguntó sobre la realización de mamografía en los últimos dos años (sí vs no). Calculamos razones de prevalencias como medida de asociación (con un intervalo de confianza del 95%), utilizando los modelos lineales generalizados de la familia Poisson. Todos los análisis consideraron la naturaleza compleja de la muestra. Resultados: Evaluamos una muestra final de 1808 mujeres. La media de edad fue 54 años (d.e: 2,5 años). Encontramos una prevalencia del 21,9 % [IC95%: 18,9 a 25,1] de mujeres que reportaron haberse sometido a una mamografía hace menos de dos años. Mientras más alta la posición socioeconómica, mayor fue la prevalencia del screening (3,2% vs 41,4% en quintiles extremos del nivel socioeconómico; p<0,001). En los modelos ajustados, encontramos que un mayor nivel socioeconómico (RP: 5,81; IC 95%: 2,28 a 14,79), el nivel de instrucción superior (RP: 2,03; IC95%: 1,30 a 3,15), encontrarse asegurada por MINSA (RP: 2,21; IC95%: 1,28 a 3,82), EsSalud (RP: 4,37; IC95%: 2,67 a 7,15), se encontraban asociados a la realización de este screening. Conclusión: La realización de mamografía está asociado a diversos factores socioculturales como el grado de instrucción, nivel socioeconómico y el estar asegurado. Es urgente iniciar políticas de abordaje social y sanitario para eliminar estar desigualdades

Our objective is to identify the social determinants associated with mammography screening (MS) in women aged 50 to 59 in Peru. In this cross-sectional analysis of the Peruvian Demographic Health Survey, 2015, MS within the past two years was evaluated through self-report. Prevalence for MS was 21.9% [95% CI: 18.9 to 25.1]. The average age was 54 years (s.d.: 2.5). The higher the socioeconomic status, the higher the prevalence of screening (3.2% vs 41.4% in extreme quintiles, p <.001). In the adjusted models, higher socioeconomic status (PR: 5.81, 95% CI: 2.28 to 14.79), higher education level (PR: 2.03, 95% CI: 1.30 a 3,15) and having health insurance from the Ministry of Health (PR: 2.21, 95% CI: 1.28 to 3.82) and EsSalud (PR: 4.37, 95% CI: 2.67 to 7.15), were positively associated with MS. Social inequalities in screening access exist and might translate into inequalities in cancer morbidity and mortality. The Peruvian government urgently needs to improve screening rates in these vulnerable populations.Revisión por pare
