The importance of the ligation of the inferior thyroid artery in parathyroid function after subtotal thyroidectomy
We prospectively studied the effects of the ligation of the inferior thyroid artery (ITA) on postoperative hypoparathyroidism in 48 patients who underwent functional subtotal thyroidectomy. Patients were randomized into two groups: A, with bilateral ligation of the ITA and B, without ligation of the ITA. Parathyroid function was checked preoperatively and after surgery by clinical examination and measurement of total calcium, intact PTH, urinary calcium, and AMPc. RESULTS: A significant incidence of postoperative hypocalcemia occurred: 17% in group A and 13% in B on the 4th postoperative day. Six months later, the incidence was 5% in Group A and 0% in Group B. These differences were not statistically significant between the two groups, and neither were any of the other clinical and laboratory observations. CONCLUSION: The ligation of the ITA was not an important causal factor for the occurrence of postoperative hypocalcemia after subtotal thyroidectomy.Com o objetivo de observar os efeitos da ligadura da artéria tireoidiana inferior (ITA) no hipoparatireoidismo, 48 pacientes submetidos à tireoidectomia subtotal funcional foram estudados de forma prospectiva. Dois grupos foram randomizados, A: com a ligadura troncular bilateral da ITA e B: sem a ligadura troncular da ITA. A função paratireoidiana foi verificada no período pré-operatório e após a cirurgia através de exame clínico e dosagens de cálcio total, PTH, cálcio urinário e cAMP. Os resultados demostraram incidência importante e significativa de hipocalcemia pós-operatória, de 17% no grupo A e 13% no grupo B no 4º PO. Seis meses após, a incidência foi de 5% no grupo A e 0% no grupo B. Estas diferenças não foram estatisticamente significativas entre os dois grupos, nem nenhum dos outros dados clínicos ou laboratoriais estudados. Concluímos que a ligadura da ITA não é um fator causal importante de hipocalcemia pós-operatória após tireoidectomia subtotal
Modelagem do crescimento e da produção de povoamento de Pinus taeda sob manejo intensivo
The objective of this work was to develop and validate a prognosis system for volume yield and basal area of intensively managed loblolly pine (Pinus taeda) stands, using stand and diameter class models compatible in basal area estimates. The data used in the study were obtained from plantations located in northern Uruguay. For model validation without data loss, a three‑phase validation scheme was applied: first, the equations were fitted without the validation database; then, model validation was carried out; and, finally, the database was regrouped to recalibrate the parameter values. After the validation and final parameterization of the models, a simulation of the first commercial thinning was carried out. The developed prognosis system was precise and accurate in estimating basal area production per hectare or per diameter classes. There was compatibility in basal area estimates between diameter class and whole stand models, with a mean difference of ‑0.01 m2 ha-1. The validation scheme applied is logic and consistent, since information on the accuracy and precision of the models is obtained without the loss of any information in the estimation of the models’ parameters.O objetivo deste trabalho foi desenvolver e validar um sistema de prognose para produção em volume e área basal de povoamento de Pinus taeda manejado intensamente, com uso de modelos de povoamento e de classes diamétricas compatíveis em estimativas de área basal. Os dados utilizados no estudo foram obtidos de plantios localizados no norte do Uruguai. Para a validação dos modelos sem perda de informação, foi aplicado um esquema de validação em três fases: primeiro, as equações foram ajustadas sem a base de dados de validação; depois, a validação do modelo foi realizada; e, por fim, a base de dados foi reagrupada para recalibrar os valores dos parâmetros. Após a validação e a parametrização final dos modelos, foi realizada uma simulação do primeiro desbaste comercial. O sistema de prognose desenvolvido foi preciso e acurado na estimativa de produção em área basal por hectare ou por classes diamétricas. Houve compatibilidade nas estimativas de área basal entre os modelos de classes diamétricas e do talhão como um todo, com diferença média de ‑0,01 m2 ha-1. O esquema de validação aplicado é lógico e consistente, uma vez que informações sobre a acurácia e a precisão dos modelos são obtidas sem perda de informação nas estimativas dos parâmetros dos modelos
Thinning regimes and initial spacing for Eucalyptus plantations in Brazil
ABSTRACT This study focuses on the effects of different thinning regimes on clonal Eucalyptus plantations growth. Four different trials, planted in 1999 and located in Bahia and Espírito Santo States, were used. Aside from thinning, initial planting density, and post thinning fertilization application were also evaluated. Before canopy closure, and therefore before excessive competition between trees took place, it was found that stands planted under low densities (667 trees per hectare) presented a lower mortality proportion when compared to stand planted under higher densities (1111 trees per hectare). However, diameter growth prior to thinning operations was not statistically different between these two densities, presenting an overall mean of 4.9 cm/year. After canopy closure and the application of the thinning treatments, it was found that thinning regimes beginning early in the life of the stand and leaving a low number of residual trees presented the highest diameter and height growth. Unthinned treatments and thinning regimes late in the life of the stand (after 5.5 years), leaving a large number of residual trees presented the highest values of basal area production. The choice of the best thinning regime for Eucalyptus clonal material will vary according to the plantation objective
Características clínicas associadas a esteatose hepática à ultrassonografia em pacientes com alanina aminotransferase elevada
CONTEXT AND OBJECTIVE: The main causes of hepatic steatosis (HS) are alcoholic liver disease and nonalcoholic fatty liver disease (NAFLD). Although liver biopsy is the gold standard for NAFLD diagnosis, the finding of abnormal aminotransferases in abstinent individuals, without known liver disease, suggests the diagnosis of NAFLD in 80-90% of the cases. Identification of clinical factors associated with HS on abdominal ultrasound may enable diagnoses of fatty liver non-invasively and cost-effectively. The aim here was to identify clinical variables associated with HS in individuals with elevated alanine aminotransferase (ALT) levels. DESIGN AND SETTING: Cross-sectional study in a single tertiary care center. METHODS: Individuals with elevated ALT, serologically negative for hepatitis B and C, were evaluated by reviewing medical files. Patients who did not undergo abdominal ultrasonography were excluded. RESULTS: Among 94 individuals included, 40% presented HS on ultrasonography. Compared with individuals without HS, those with fatty liver were older (P = 0.043), with higher body mass index (BMI) (P = 0.003), diabetes prevalence (P = 0.024), fasting glucose levels (P = 0.001) and triglycerides (P = 0.003). Multivariate analysis showed that BMI (odds ratio, OR = 1.186; 95% confidence interval, CI: 1.049-1.341; P = 0.006) and diabetes mellitus (OR = 12.721; 95% CI: 1.380-117.247; P = 0.025) were independently associated with HS. CONCLUSIONS: Simple clinical findings such as history of diabetes and high BMI may predict the presence of HS on ultrasonography in individuals with elevated ALT and negative serological tests for hepatitis.CONTEXTO E OBJETIVO: Doença hepática alcoólica e doença hepática esteatótica não alcoólica (DHENA) são as principais causas de esteatose hepática (EH). Apesar de a biópsia hepática ser o método de escolha para diagnóstico DHENA, o achado de aminotransferases elevadas em indivíduos abstêmios, sem doença hepática conhecida, sugere o diagnóstico de DHENA em 80-90% dos casos. A identificação de variáveis clínicas associadas à EH na ultrassonografia abdominal pode permitir o diagnóstico de DHENA de forma não invasiva e custo-efetiva. O objetivo foi identificar variáveis clínicas associadas à EH em indivíduos com níveis elevados de alanina aminotransferase (ALT). TIPO DE ESTUDO E LOCAL: Estudo transversal em um único centro de atendimento terciário. MÉTODOS: Indivíduos com ALT elevada e sorologias negativas para os vírus de hepatite B e C foram avaliados por meio de revisão de prontuários. Os pacientes não submetidos à ultrassonografia foram excluídos. RESULTADOS: Foram incluídos 94 indivíduos, 40% deles com EH à ultrassonografia. Quando comparados aos indivíduos sem EH, aqueles com EH apresentaram maior prevalência de diabetes (P = 0,024), maiores idade (P = 0,043) e índice de massa corpórea (IMC) (P = 0,003), glicemia de jejum mais elevada (P = 0,001) e triglicerídeos mais elevados (P = 0,003). A análise multivariada evidenciou que o IMC (odds ratio, OR = 1,186, 95% intervalo de confiança, IC 1,049-1,341, P = 0,006) e o diabetes mellitus (OR = 12,721, 95% IC 1,380-117,247, P = 0,025) foram associadas independentemente à EH. Do differences exist between chronic hepatitis C genotypes 2 and 3?
Introduction Six genotypes of the hepatitis C virus (HCV) have been identified thus far, and their distribution is well defined. Genotype 1, which is the most prevalent worldwide, is always compared to genotypes 2 and 3, particularly in terms of treatment response. However, little is known about the differences between genotypes 2 and 3 because these genotypes are analyzed together in most studies. Therefore, the aim of this study was to evaluate differences in the clinical, epidemiological, laboratory, and histological parameters between HCV-2 and HCV-3. Methods Patients with chronic hepatitis C infected with genotypes 2 and 3 were studied retrospectively and compared according to clinical, laboratory, and histological aspects. Hepatitis C virus-ribonucleic acid (HCV-RNA) was analyzed quantitatively by TaqMan® real-time PCR, and the HCV genotype was determined by sequencing the 5′-untranslated region. Results A total of 306 patients with chronic HCV-2 (n=50) and HCV-3 (n = 256) were studied. Subtype 2b (n=17/50) and subtype 3a (n=244/256) were the most prevalent among patients infected with HCV-2 and HCV-3, respectively. The mean age was 47 ± 10 years, and there was a predominance of men in the group studied (61%). Comparative analysis between HCV-2 and HCV-3 showed a younger age (p=0.002), less prevalence of arterial hypertension (p=0.03), higher serum albumin levels (p=0.01), more advanced stage of liver fibrosis (p=0.03), and higher frequency of steatosis in patients with HCV-3 (p=0.001). After multivariate regression analysis, all the variables, except serum albumin, remained as variables associated with HCV-3 in the final model. Pediatric Multiple Sclerosis—A Challenging Demyelinating Disease: Case Report and Brief Review of the Literature
Multiple sclerosis (MS) is an inflammatory, demyelinating, neurodegenerative disorder of the central nervous system (CNS) of unknown etiology. The peak onset is between age 20 and 40 years and usually affects more women than men. Although much knowledge has been achieved on the diagnosis and treatment of adult patients with MS, it remains a matter of debate and controversy in childhood. We present a case of MS in 9-year-old girl, review the current state of the knowledge on pediatric MS, and discuss the available tools for the diagnosis and treatment
High incidence of tuberculosis in patients treated for hepatitis C chronic infection
AbstractBrazil is one of the 22 countries that concentrates 80% of global tuberculosis cases concomitantly to a large number of hepatitis C carriers and some epidemiological risk scenarios are coincident for both diseases. We analyzed tuberculosis cases that occurred during α-interferon-based therapy for hepatitis C in reference centers in Brazil between 2001 and 2012 and reviewed their medical records. Eighteen tuberculosis cases were observed in patients submitted to hepatitis C α-interferon-based therapy. All patients were human immunodeficiency virus-negative. Nine patients (50%) had extra-pulmonary tuberculosis; 15 (83%) showed significant liver fibrosis. Hepatitis C treatment was discontinued in 12 patients (67%) due to tuberculosis reactivation and six (33%) had sustained virological response. The majority of patients had a favorable outcome but one died. Considering the evidences of α-IFN interference over the containment of Mycobacterium tuberculosis, the immune impairment of cirrhotic patients, the increase of tuberculosis case reports during hepatitis C treatment with atypical and severe presentations and the negative impact on sustained virological response, we think these are strong arguments for latent tuberculosis infection screening before starting α-interferon-based therapy for any indication and even to consider IFN-free regimens against hepatitis C when a patient tests positive for latent tuberculosis infection
