Introduction: Individuals with a family history of systemic arterial hypertension (SAH) or prehypertension have autonomic and vascular abnormalities, both at rest and in response to stressful situations. Considering that these changes are related to the development of SAH and that in individuals with a family history of SAH there is a high incidence of prehypertension, the understanding of these disorders in prehypertensive individuals with a history of SAH is of fundamental importance. Objective: To compare the autonomic and vascular functions in rest and the hemodynamic response during mental stress and isometric handgrip exercise the prehypertensive and normotensive patients with family history of SAH. Methods: Twenty-five young volunteers with family history of SAH (30±5 years, 24±4 kg/m²), 14 normotensive (SBP: 116 [105-119], DBP: 67[60-71] mmHg) and 11 prehypertensive subjects (SBP: 128[124-132], DBP: 75[7175] mmHg) were submitted to vascular function evaluation (reactive hyperemia, Hokanson®), and cardiac and peripheral autonomic modulation, quantified, respectively, by spectral analysis of heart rate (ECG) and systolic blood pressure (SBP, FinometerPRO®). The transfer function analysis was used to measure the gain and response time of baroreflex. Additionally, were recorded blood pressure, heart rate (Dixtal®) and forearm blood flow and calculated forearm vascular conductance (Hokanson®) for 3 minutes during rest and 1º, 2º and 3º minutes of the protocols of mental stress (Stroop Color Word Conflict Test) and isometric exercise at 30% of maximal voluntary contraction (Saehan®). Data were presented as mean ± standard deviation of the mean or as median [1º quartile – 3º quartile]. The statistical significance adopted was 5%. Results: Pre-hypertensive individuals, in relation to normotensive individuals, have higher forearm vascular conductance both at rest (3.48 ± 1.26 vs. 2.67 ± 0.72 units, p = 0.05, effect size (ES): 1.13) and peak reactive hyperemia (25, 02 ± 8.18 vs. 18.66 ± 6.07 units, p = 0.04, ES: 1.05). The indices of cardiac autonomic modulation were similar between the groups. However, in the peripheral autonomic modulation, greater variability was observed in prehypertensive patients compared to normotensive individuals (9.4 [4.9-12.7] vs. 18.3 [14.8-26.7] mmHg²; p < 0.01) and higher spectral components of very low (6.9 [2.0-11.1] vs. 13.5 [10.7-22.4] mmHg², p = 0.01) and low frequencies (1.7 [1.0-3.0] vs. 3.0 [2.0-4.0] mmHg², p = 0.04) of SBP. Additionally, we observed a lower gain of baroreflex control in prehypertensive patients compared to normotensive patients (12.16 ± 4.18 vs. 18.23 ± 7.11 ms/mmHg, p = 0.03, ES: 1.1), but similar delay time (-1.55 ± 0.66 vs. -1.58 ± 0.72 s, p = 0.90). During the
protocols of mental stress normotensive and prehypertensive groups increased (deltas) significantly and similarly systolic blood pressure (time effect: p<0.01, interaction effect: p=0.70, group effect: p=0.58), diastolic (time effect:p<0.01, interaction effect: p=0.35, group effect: p=0.78) and mean (time effect: p<0.01, interaction effect: p=0.82, group effect: p=0.60), heart rate (time effect: p<0.01, interaction effect: p=0.68, group effect: p=0.66), forearm muscle blood flow (time effect: p<0.01, interaction effect: p=0.42, group effect: p=0.91) and forearm vascular conductance (time effect: p=0.01, interaction effect: p=0.47, group effect: p=0.84) in relation to the baseline. No differences were observed between group responses in any of the variables. Similarly, throughout the isometric exercise protocol, both groups increased (deltas) significantly and similarly systolic blood pressure (time effect: p<0.01, interaction effect: p=0.35, group effect: p=0.99), diastolic (time effect:p<0.01, interaction effect: p=0.93, group effect: p=0.79) and mean (time effect: p<0.01, interaction effect: p=0.89, group effect: p=0.89) and heart rate (time effect: p<0.01, interaction effect: p=0.65, group effect: p=0.79) in relation to the baseline. However, the forearm muscle blood flow increased from baseline only in the third minute time effect: p<0.01, interaction effect: p=0.62, group effect: p=0.98) and forearm vascular conductance (time effect: p=0.06, interaction effect: p=0.66, group effect: p=0.92) remained similar to baseline in both groups in all moments. No differences were observed between groups in any of the variables. Conclusion: Young prehypertensive patients with family history of SAH have autonomic dysfunction and increased forearm vascular conductance when compared to normotensive patients with the same risk factor. During mental stress and physical exercise, the groups studied had a similar hemodynamic response. Additionally, both groups present vascular dysfunction during exercise, characterized by the absence of vasodilation during this maneuver. Adicionalmente, observamos menor ganho do controle barorreflexo
nos pré-hipertensos em relação aos normotensos (12,16±4,18 vs. 18,23±7,11 ms/mmHg; p=0,03, TE:1,1), porém com tempo de retardo semelhante (-1,55±0,66 vs. -1,58±0.72 s; p=0,90). Durante todo o protocolo de estresse mental os grupos normotenso e pré-hipertenso aumentaram (deltas) significativa e similarmente a pressão arterial sistólica (efeito tempo: p<0,01, efeito interação: p=0,70, efeito grupo: p=0,58), diastólica (efeito tempo: p<0,01, efeito interação: p=0,35, efeito grupo: p=0,78) e média (efeito tempo: p<0,01, efeito interação: p=0,82, efeito grupo: p=0,60), a frequência cardÃaca (efeito tempo: p<0,01, efeito interação: p=0,68, efeito grupo:p=0,66), o fluxo sanguÃneo muscular (efeito tempo: p<0,01, efeito interação: p=0,42, efeito grupo: p=0,91) e a condutância vascular do antebraço (efeito tempo: p=0,01, efeito interação: p=0,47, efeito grupo: p=84) em relação ao basal.
Aging is a multifactorial process that causes changes in the quantity and quality of
skeletal muscle. Muscle atrophy, along with functional impairment, is associated with
an impaired state of health with high personal losses. Studies have shown that the
ability to generate force quickly may be more functionally relevant than the capacity
of maximum force, since many of the activities of daily life are characterized by
limited time of force development (50-200ms), which is considerably less than that
required to achieve maximum force (≥ 300ms). However, the age-related changes do
not occur evenly among muscle groups. Based on the lack of studies which analyze
the neuromuscular and biodynamic aspects in the superior and lower limbs in related
to age women, which are characterized by having the highest rates of disability and
longevity, this study aimed to investigate in women of distinct age group: whether the
strength, the fat-free mass (FFM) and the power of the electromyography signal of
upper and lower limbs are affected in the same magnitude by aging (Study 1) and
age-related changes in the temporal development of force of handgrip and knee
extensors (study 2). In the first study, we analyzed the maximal isometric strength of
handgrip and knee extension, the power of the electromyography signal of the flexor
muscles of the hand and the vastus lateralis through the root mean square variable
normalized by the FFM of the dominant limb (RMSn) and the FFM by Dual-energy Xray
absorptiometry in 63 women, subdivided in three groups (JO. n=33, 24.7±3.5
years; MI. n=15, 58.6±4.2 years; ID. n=15, 72.0±4.2 years). The handgrip strength
was similar among groups (p=0,523), the FFM of the upper limb was lower in group
ID in compared JO (p=0.108) and the RMSn values of the hand flexors was similar
among groups (p=0.754). However, the strength of the knee extensors, the FFM of
the lower limb and the RMSn values of the vastus lateralis were lower in group MI
(p=0.014, p=0.006 e p=0.013, respectively) and ID (p=0.000, p=0.000 e p<0.000,
respectively) in compared JO. In Study 2, we assessed the FFM, maximal isometric
strength of handgrip and knee extension and the temporal development of force
during those tests in 52 women, subdivided in three groups (JO. n=24, 25.2±3.7
years; MI. n=13, 58±4.2 years; ID. n=15, 71.3±3.9 years). Values were similar in
strength (p=0.831) and FFM (p=0.207) of the upper limb. The rate of force
development (RFD) was lower in the initial periods of muscle contraction (0-50 and 0-
100ms) in the MI group (p=0.005 and p=0.000 for 50ms and 100ms, respectively)
and ID (p=0.003 p=0.000 for 50ms and 100ms, respectively). The FFM and lower
limb strength was lower in the MI group (p=0.006, p =0.014, respectively) and ID
(p<0.000, p=0.000, respectively) compared to the JO. Already the RFD of the knee
extensors was similar in the MI group (p>0.05), but lower in the ID group (p<0.05)
compared to the JO in all analyzed intervals. According to the results of Studies 1
and 2, the lower limbs are most affected by the aging process. Thus, it is necessary
to develop a strategy of adequate training for elderly and middle-aged women,
especially for the lower limbs, in order to maintain function and quality of life for this
Todavia, as
extensores do joelho (Estudo 2).
respectivamente) e ID (p=0,000, p=0,000 e p<0,000, respectivamente). No Estudo 2,
anos; MI. n=13, 58±4,2 anos; ID. n=15, 71,3±3,9 anos). Foram observados entre os
grupos valores semelhantes de força (p=0,831) e MLG (p=0,207) do membro
superior.
Dessa forma, a
The aim of the present study was to investigate the motivational differences between recreational and elite road runners. Road runners (97 athletes being 64 men) were recruited, divided into two groups, elite (E) (time in official competition of 10km equal to or less than 32 minutes for male volunteers and equal to or less than 37 minutes for female volunteers) and recreational (R) (trained at least three times a week and competed at least in regional events). The Sport Motivation Scale (SMS) was used to investigate the theory of Self-Determination. Elite runners are more motivated, both intrinsically (E: 17.98 ± 2.40; R: 16.86 ± 2.58; p=0.04) and extrinsically (E: 16.18 ± 2.81; R: 14.53 ± 3.35; p=0.01), however, in both groups the intrinsic motivation is greater than the extrinsic (p=0.01). And yet the intrinsic motivation to know (p=0.01), the identified extrinsic motivation (p=0.05) and the external regulated extrinsic motivation (p=0.01) were higher in elite athletes. We conclude that intrinsic motivations are high in both groups, which may possibly favor the maintenance of these athletes in the training and competition process, however, probably for reasons of high competitiveness, extrinsic motivation was high in elite athletes.O objetivo do presente estudo foi investigar as diferenças motivacionais entre corredores de rua recreacionais e de elite.
Autonomic and Vascular Control in Prehypertensive Subjects with a Family History of Arterial Hypertension
<div><p>Abstract Background: Individuals with a family history of systemic arterial hypertension (FHSAH) and / or prehypertension have a higher risk of developing this pathology. Objective: To evaluate the autonomic and vascular functions of prehypertensive patients with FHSAH. Methods: Twenty-five young volunteers with FHSAH, 14 normotensive and 11 prehypertensive subjects were submitted to vascular function evaluation by forearm vascular conductance(VC) during resting and reactive hyperemia (Hokanson®) and cardiac and peripheral autonomic modulation, quantified, respectively, by spectral analysis of heart rate (ECG) and systolic blood pressure (SBP) (FinometerPRO®). The transfer function analysis was used to measure the gain and response time of baroreflex. The statistical significance adopted was p ≤ 0.05. Results: Pre-hypertensive individuals, in relation to normotensive individuals, have higher VC both at rest (3.48 ± 1.26 vs. 2.67 ± 0.72 units, p = 0.05) and peak reactive hyperemia (25, 02 ± 8.18 vs. 18.66 ± 6.07 units, p = 0.04). The indices of cardiac autonomic modulation were similar between the groups. However, in the peripheral autonomic modulation, greater variability was observed in prehypertensive patients compared to normotensive individuals (9.4 [4.9-12.7] vs. 18.3 [14.8-26.7] mmHg2; p < 0.01) and higher spectral components of very low (6.9 [2.0-11.1] vs. 13.5 [10.7-22.4] mmHg2, p = 0.01) and low frequencies (1.7 [1.0-3.0] vs. 3.0 [2.0-4.0] mmHg2, p = 0.04) of SBP. Additionally, we observed a lower gain of baroreflex control in prehypertensive patients compared to normotensive patients (12.16 ± 4.18 vs. 18.23 ± 7.11 ms/mmHg, p = 0.03), but similar delay time (-1.55 ± 0.66 vs. -1.58 ± 0.72 s, p = 0.90). Conclusion: Prehypertensive patients with FHSAH have autonomic dysfunction and increased vascular conductance when compared to normotensive patients with the same risk factor.</p></div
