    Sonography of Affected and Unaffected Shoulders in Hemiplegic Patients: Analysis of the Relationship Between Sonographic Imaging Data and Clinical Variables

    Objective To explore the relationship between a number of clinically relevant variables and sonographic imaging data in respect to the level of impairment experienced in the affected and unaffected shoulders of hemiplegic stroke patients. Method Fifty-one hemiplegic stroke patients (32 males, 19 females; 29 right-sided hemiplegics, 22 left-sided hemiplegics) participated in this study. A musculoskeletal radiologist conducted a sonographic exam on both the affected and unaffected shoulders of all patients and two physicians classified the severity of the injury on a six-point rating scale. Clinical variables including age, sex, duration of injury, spasticity and muscle power of the hemiplegic side, and level of functional activity of the shoulder were assessed. Results The sonographic rating scores of hemiplegic shoulders were positively correlated with age (p<0.01) and negatively correlated with level of muscle spasticity (p<0.05). The sonographic rating scores of unaffected shoulders were positively correlated with duration of injury (p<0.01). Affected shoulders received sonographic rating scores that reflected significantly more impairment than those of unaffected shoulders (p<0.001), and premorbid handedness did not affect the relationship between impairment rating and shoulder injury status. Conclusion Hemiplegic stroke influences not only affected shoulders, but also unaffected sides. Proper management of spasticity, enhancement of motor recovery, and avoidance of unaffected shoulder overuse should be considered to prevent shoulder problems following strokes which result in hemiplegia

    Effects of geogrid reinforcement in low volume flexible pavement

    This paper presents findings of a study on geogrid reinforced flexible pavement in a low volume road through a three-dimensional finite element analysis. A mechanistic model was developed for geogrid reinforced flexible pavement. To analyze the behavior of pavement foundation, stress-dependent resilient modulus models were employed in both base and subgrade layers. The model also incorporated previous research findings to enhance the reliability of the analysis. During the analysis, comparisons were made to contrast the responses of low volume flexible pavement with and without geogrid. The results show geogrid reinforcement reduces critical pavement responses under traffic loading, such as vertical surface deflection, tensile strain in asphalt concrete, and compressive strain in subgrade. The study found up to 18% reduction of vertical strain at the top of subgrade and 68% reduction of tensile strain at the bottom of asphalt concrete. Also, geogrid provides confining stresses in the adjacent aggregate layer, which leads to surrounding layers becoming stiffer. Based on the results of this study, the placement of geogrid reinforcement on top of weak subgrade was found particularly effective compared to that on strong subgrade.     The Nerve/Tunnel Index: A New Diagnostic Standard for Carpal Tunnel Syndrome Using Sonography

    Objectives—To define the relationship between body indices of healthy adults and cross-sectional areas of the carpal tunnel and median nerve and to obtain the nerve/tunnel index, which represents a new standard for diagnosing carpal tunnel syndrome using sonography. Methods—Body indices (height, weight, and body mass index) were analyzed in 60 healthy adults, and electromyography and sonography were also performed. The crosssectional areas of the proximal and distal median nerve and carpal tunnel were obtained by sonography. The proximal and distal nerve/tunnel indices were obtained by calculating the ratio between the proximal and distal cross-sectional areas of the median nerve to those of the carpal tunnel and multiplying the value by 100. Results—Although the proximal cross-sectional areas of the median nerve and body indices showed statistically significant relationships with weak positive correlations, the proximal and distal areas of the carpal tunnel showed relatively stronger correlations with body indices. Between sexes, there were significant differences in the proximal median nerve cross-sectional area (mean ± SD: male, 10.48 ± 3.21 mm2; female, 8.81 ± 3.21 mm2; P < .05) and proximal carpal tunnel area (male, 182.50 ± 21.15 mm2; female, 151.23 ± 21.14 mm2; P < .05). There was no difference in the proximal nerve/tunnel index (male, 5.80% ± 1.72%; female, 5.91% ± 1.63%). There was a statistically significant difference in the distal carpal tunnel cross-sectional area (male, 138.90 ± 20.95 mm2; female, 121.50 ± 18.99 mm2; P < .05) between sexes, but the distal median area (male, 9.99 ± 3.42 mm2; female, 8.46 ± 1.84 mm2) and distal nerve/tunnel index (male, 7.15% ± 2.00%; female, 7.01% ± 1.38%) showed no significant differences. The proximal index was significantly higher than the distal index (proximal, 5.85% ± 1.66%; distal, 7.08% ± 1.71%). Conclusions—The nerve/tunnel index is unaffected by body indices or sex and thus may be a useful and objective standard for diagnosing carpal tunnel syndrome

    The role of Bcl-xL and nuclear factor-κB in the effect of taxol on the viability of dendritic cells

    Taxol has been used effectively in cancer therapies. Our previous study demonstrated that taxol induced altered maturation and improved viability of dendritic cells (DCs). However, the effects of taxol on DC viability have not been fully elucidated. In the present study, flow cytometric analyses revealed that taxol treatment significantly increased the number of viable DCs and the expression levels of a representative anti-apoptotic protein Bcl-xL. Furthermore, mobilization of the p65 subunit of nuclear factor-κB (NF-κB) from the cytosol to the nucleus in DCs was observed by confocal microscopy. An inhibition assay using N-p-tosyl-L-phenylalanine chloromethyl ketone confirmed that NF-κB was intimately involved in the effects of taxol on DC viability. In addition, we investigated the mechanisms of taxol enhancement of DC viability. Since taxol is a popular anticancer agent used in clinic, this study may provide a rationale for the use of taxol in DC immunotherapy to treat cancer patients. Taken together, these results confirm that taxol increases DC viability, and this information may provide new insights for new clinical applications of both taxol and DCs

    Application of Filters to Multiway Joins in MapReduce

    Joining multiple datasets in MapReduce may amplify the disk and network overheads because intermediate join results have to be written to the underlying distributed file system, or map output records have to be replicated multiple times. This paper proposes a method for applying filters based on the processing order of input datasets, which is appropriate for the two types of multiway joins: common attribute joins and distinct attribute joins. The number of redundant records filtered depends on the processing order. In common attribute joins, the input records do not need to be replicated, so a set of filters is created, which are applied in turn. In distinct attribute joins, the input records have to be replicated, so multiple sets of filters need to be created, which depend on the number of join attributes. The experimental results showed that our approach outperformed a cascade of two-way joins and basic multiway joins in cases where small portions of input datasets were joined

    Striatal neuroinflammation promotes parkinsonism in rats

    The specific role of neuroinflammation in the pathogenesis of Parkinson&#x27;s disease remains to be fully elucidated. By infusing lipopolysaccharide (LPS) into the striatum, we investigated the effect of neuroinflammation on the dopamine nigrostriatal pathway. Here, we report that LPS-induced neuroinflammation in the striatum causes progressive degeneration of the dopamine nigrostriatal system, which is accompanied by motor impairments resembling parkinsonism. Our results indicate that neurodegeneration is associated with defects in the mitochondrial respiratory chain related to extensive S-nitrosylation/nitration of mitochondrial proteins. Mitochondrial injury was prevented by treatment of L-N^6^-(l-iminoethyl)-lysine, an inducible nitric oxide synthase (iNOS) inhibitor, suggesting that iNOS-derived NO is responsible for mitochondrial dysfunction. Furthermore, the nigral dopamine neurons exhibited intracytoplasmic [alpha]-synuclein and ubiquitin accumulation. These results demonstrate that degeneration of nigral dopamine neurons by neuroinflammation is associated with mitochondrial malfunction induced by NO-mediated S-nitrosylation/nitration of mitochondrial proteins

    Estimating remineralized phosphate and its remineralization rate in the northern East China Sea during Summer 1997 : a snapshot study before Three-Gorges Dam construction

As the northern East China Sea region is vulnerable to climate forcing and anthropogenic impacts, it is important to investigate how the remineralization rate in the northern East China Sea has changed in response to such external forcing. We used an historical hydrographic dataset from August 1997 to obtain a baseline for future comparison. We estimate the amount of remineralized phosphate by decomposing the physical mixing and biogeochemical process effect using water column measurements (temperature, salinity, and phosphate). The estimated remineralized phosphate column inventory ranged from 0.8 to 42.4 mmol P m-2 (mean value of 15.2 ± 12.0 mmol P m-2). Our results suggest that the Tsushima Warm Current was a strong contributor to primary production during the summer of 1997 in the study area. The estimated summer (June - August) remineralization rate in the region before Three-Gorges Dam construction was 18 ± 14 mmol C m-2 d-1.T.     Microstructure and Mechanical Properties of High-Entropy Alloy Co20Cr26Fe20Mn20Ni14 Processed by High-Pressure Torsion at 77 K and 300 K

    In this work, the mechanical characteristics of high-entropy alloy Co20Cr26Fe20Mn20Ni14 with low-stacking fault energy processed by cryogenic and room temperature high-pressure torsion (HPT) were studied. X-ray diffraction, scanning electron microscopy (SEM), and transmission electron microscopy (TEM) analyses were performed to identify the phase and microstructure variation and the mechanical properties characterized by Vickers hardness measurements and tensile testing. Cryogenic HPT was found to result in a lower mechanical strength of alloy Co20Cr26Fe20Mn20Ni14 than room temperature HPT. Microstructure analysis by SEM and TEM was conducted to shed light on the microstructural changes in the alloy Co20Cr26Fe20Mn20Ni14 caused by HPT processing. Electron microscopy data provided evidence of a deformation-induced phase transformation in the alloy processed by cryogenic HPT. Unusual softening phenomena induced by cryogenic HPT were characterized by analyzing the dislocation density as determined from X-Ray diffraction peak broadening.11Ysciescopu
