    Pneumothorax following subclavian venipuncture is a well-known risk. Less well recognized is the potential for life-threatening bilateral pneumothoraces occurring at the time of subclavian vein catheterization in patients who have previously undergone median sternotomy. Inadvertent bilateral pleural entry at the time of sternotomy may result in a common pleural space which subsequently places the patient in special jeopardy when the complication of pneumothorax occurs. This report documents the successful management of this important sequence of complications associated with now widely applied therapeutic interventions

    Direct and indirect orthotic management of medial compartment osteoarthritis of the knee

    Osteoarthritis (OA) is a painful condition and affects approximately 80% of individuals by the age of 55 [1], with knee OA occurring two times more frequently than OA of the hand or hip [2].The condition is more prevalent in the medial compartment and restricts the daily lives of individuals due to pain and a lack of functional independence. Patients with medial compartment osteoarthritis often have a varus alignment, with the mechanical axis and load bearing passing through this compartment with a greater adduction moment leading to greater pain and progression of osteoarthritis [3]. Surgery for the condition is possible although in some cases, particularly younger patients or those not yet requiring surgery, clinical management remains a challenge. Before surgery is considered, however, conservative management is advocated, though no one treatment has been shown to be most effective, and there are few quality biomechanical or clinical studies. Of the conservative approaches the principal orthotic treatments are valgus knee braces and laterally wedged foot inlays. Studies of knee valgus bracing have consistently demonstrated an associated decreased pain and improved function [4], and greater confidence [5]. A laterally wedged foot inlay has a thicker lateral border and applies a valgus moment to the heel. It is theorised that by changing the position of the ankle and subtalar joints during weight-bearing [6] the lateral wedges may apply a valgus moment across the knee as well as the rearfoot, with the assumed reduction on load in the medial knee compartment [7]. However, there has been no study to directly compare these orthotic treatments in the same study. Maximum values were obtained between 45-49 days after flowering, when seed moisture was 29-38%. Fatty acid composition of canola seed oil changed during seed development. The proportion of oleic (C18:1) and linolenic (C18:3) acids increased, while that of most other fatty acids decreased, as seeds developed, while Eicosenoic (C20:1) and erucic (C22:1) acids did not show a clear trend. Fatty-acids tended to stabilize by the 49th day after flowering. Early seeding resulted in higher seed yield, larger seeds, lower seed protein content and higher seed oil content, when compared to late seeding

    Avrami exponent under transient and heterogeneous nucleation transformation conditions

    The Kolmogorov-Johnson-Mehl-Avrami model for isothermal transformation kinetics is universal under specific assumptions. However, the experimental Avrami exponent deviates from the universal value. In this context, we study the effect of transient heterogeneous nucleation on the Avrami exponent for bulk materials and also for transformations leading to nanostructured materials. All transformations are assumed to be polymorphic. A discrete version of the KJMA model is modified for this purpose.     Spin Precession and Oscillations in Mesoscopic Systems

    We compare and contrast magneto-transport oscillations in the fully quantum (single-electron coherent) and classical limits for a simple but illustrative model. In particular, we study the induced magnetization and spin current in a two-terminal double-barrier structure with an applied Zeeman field between the barriers and spin disequilibrium in the contacts. Classically, the spin current shows strong tunneling resonances due to spin precession in the region between the two barriers. However, these oscillations are distinguishable from those in the fully coherent case, for which a proper treatment of the electron phase is required. We explain the differences in terms of the presence or absence of coherent multiple wave reflections.Comment: 9 pages, 5 figure
