    Retrospective Analysis of Esophageal Heat Transfer for Active Temperature Management in Post-cardiac Arrest, Refractory Fever, and Burn Patients

    Core temperature management is an important aspect of critical care; preventing unintentional hypothermia, reducing fever, and inducing therapeutic hypothermia when appropriate are each tied to positive health outcomes. The purpose of this study is to evaluate the performance of a new temperature management device that uses the esophageal environment to conduct heat transfer. De-identified patient data were aggregated from three clinical sites where an esophageal heat transfer device (EHTD) was used to provide temperature management. The device was evaluated against temperature management guidelines and best practice recommendations, including performance during induction, maintenance, and cessation of therapy. Across all active cooling protocols, the average time-to-target was 2.37 h and the average maintenance phase was 22.4 h. Patients spent 94.9% of the maintenance phase within ±1.0°C and 67.2% within ±0.5°C (574 and 407 measurements, respectively, out of 605 total). For warming protocols, all of the patient temperature readings remained above 36°C throughout the surgical procedure (average 4.66 h). The esophageal heat transfer device met performance expectations across a range of temperature management applications in intensive care and burn units. Patients met and maintained temperature goals without any reported adverse events

    Analysis of grain-boundary structure in Al–Cu interconnects

    The role of crystallographic texture in electromigration resistance of interconnect lines is well documented. The presence of a strong (111) fiber texture results in a more reliable interconnect structure. It is also generally accepted that grain-boundary diffusion is the primary mechanism by which electromigration failures occur. It has been difficult to this point, however, to obtain statistically reliable information of grain-boundary structure in these materials as transmission electron microscopy investigations are limited by tedious specimen preparation and small, nonrepresentative, imaging regions. The present work focuses upon characterization of texture and grain-boundary structure of interconnect lines using orientation imaging microscopy, and particularly, upon the linewidth dependence of these measures. Conventionally processed Al–1%Cu lines were investigated to determine the affects of a postpatterning anneal on boundary structure as a function of linewidth. It was observed that texture tended to strengthen slightly with decreasing linewidth subsequent to the anneal procedure. Grain morphology changed substantially as the narrow lines became near bamboo in character and the crystallographic character of the boundary plane changed as a function of linewidth. These results are contrasted with those obtained from Al–1%Cu lines, which were fabricated using the damascene process. The damascene lines show a marked weakening in texture as the linewidth decreases, accompanied by a more random misorientation distribution.     Xenon depresses aEEG background voltage activity whilst maintaining cardiovascular stability in sedated healthy newborn pigs

    Changes in electroencephalography (EEG) voltage range are used to monitor the depth of anaesthesia, as well as predict outcome after hypoxia-ischaemia in neonates. Xenon is being investigated as a potential neuroprotectant after hypoxic-ischaemic brain injury, but the effect of Xenon on EEG parameters in children or neonates is not known. This study aimed to examine the effect of 50% inhaled Xenon on background amplitude-integrated EEG (aEEG) activity in sedated healthy newborn pigs.Five healthy newborn pigs, receiving intravenous fentanyl sedation, were ventilated for 24 h with 50%Xenon, 30%O2 and 20%N2 at normothermia. The upper and lower voltage-range of the aEEG was continuously monitored together with cardiovascular parameters throughout a 1 h baseline period with fentanyl sedation only, followed by 24 h of Xenon administration.The median (IQR) upper and lower aEEG voltage during 1 h baseline was 48.0 μV (46.0-50.0) and 25.0 μV (23.0-26.0), respectively. The median (IQR) aEEG upper and lower voltage ranges were significantly depressed to 21.5 μV (20.0-26.5) and 12.0 μV (12.0-16.5) from 10 min after the onset of 50% Xenon administration (p=0.002). After the initial Xenon induced depression in background aEEG voltage, no further aEEG changes were seen over the following 24h of ventilation with 50% xenon under fentanyl sedation. Mean arterial blood pressure and heart rate remained stable.Mean arterial blood pressure and heart rate were not significantly influenced by 24h Xenon ventilation. 50% Xenon rapidly depresses background aEEG voltage to a steady ~50% lower level in sedated healthy newborn pigs. Therefore, care must be taken when interpreting the background voltage in neonates also receiving Xenon

    Self-Selected Versus Standardised Warm-Ups; Physiological Response on 500 m Sprint Kayak Performance

    This study investigated the effectiveness of a self-selected (SS) warm-up on 500 m sprint kayak performance (K500) compared to continuous (CON) and intermittent high intensity (INT)-type warm-ups. Twelve nationally ranked sprint kayakers (age 17.7 ± 2.3 years, mass 69.2 ± 10.8 kg) performed CON (15 min at the power at 2 m·mol−1), INT (10 min at 2 m·mol−1, followed by 5 × 10 s sprints at 200% power at VO2max with 50 s recovery at 55% power at VO2max), and SS (athlete’s normal competition warm-up) warm-ups in a randomised order. After a five-minute passive recovery, K500 performance was determined on a kayak ergometer. Heart rate and blood lactate (BLa) were recorded before and immediately after each warm-up and K500 performance. Ratings of perceived exertion (RPE) were recorded at the end of the warm-up and K500. BLa, heart rate, and RPE were generally higher after the INT than CON and SS warm-ups (p 0.05). RPE and changes in BLa and heart rate after the K500 were comparable. There were no differences in K500 performance after the CON, SS, or INT warm-ups. Applied practitioners can, therefore, attain similar performance independent of warm-up type

    Real-Time Measurement of Xenon Concentration in a Binary Gas Mixture Using a Modified Ultrasonic Time-of-Flight Anesthesia Gas Flowmeter

    Background: Xenon (Xe) is an anesthetic gas which may have potential as a neuroprotectant. To measure each gas in a xenon/oxygen mixture one would typically use a thermal conductivity meter and a fuel cell respectively. The speed of sound in a binary gas mixture like this is related to fractional concentration, temperature, pressure, and masses of the gases present. We assessed the feasibility of developing a novel single sterilizable device which used the time of flight of ultrasound through the gas to measure both real-time gas flow and also the concentration of Xe in O2.Method: We adapted an ultrasonic time-of-flight flow meter from a conventional anesthetic machine to also measure the real time fractional concentration of Xe in O2. A total of 5 095 readings of were taken in the range 5 % to 95 % xenon, and compared with simultaneous measurements from a commercially available thermal conductivity xenon analyser.Results: Ultrasonic measurements of Xe (%) showed agreement with thermal conductivity meter measurements but there was marked discontinuity in the central region of the measurement range. Bland-Altman analysis (95% CI): Mean Difference (Bias) 3.1 (2.9 % to 3.2 %); lower 95 % Limit of Agreement -4.6 % (-4.8 % to -4.4 %); upper 95 % Limit of Agreement 10.8 % (10.5 % to 11.0 %).Conclusions: The modified ultrasonic flow meter estimated the Xe (%) however at present the accuracy is not sufficient for clinical use. With further work it may be possible to improve the accuracy sufficiently for potential clinical use

    Fentanyl induces cerebellar internal granular cell layer apoptosis in healthy newborn pigs

    Background: Opioids like fentanyl are regularly used in neonates for analgesia and sedation. So far, they have been reported to be safe and eligible to use. The cerebellum has become a focus of neurodevelopmental research within the last years, as it is known to play an important role in long-lasting motor, cognitive, and other behavioral changes. The cerebellar cortex is of major importance in the coordinative role of the cerebellum and highly vulnerable to injury and impaired growth. Objective: This study was performed to evaluate the apoptotic effect of intravenous fentanyl infusion on the cerebellum in healthy newborn pigs. Methods: Thirteen healthy pigs ( < median 12 h old) were randomized into (1) 24 h of intravenous fentanyl at normothermia (NTFe, n = 6) or (2) non-ventilated controls at normothermia (NTCTR, n = 7). Cerebellar sections were morphologically assessed after staining with hematoxylin–eosin. In addition, paired sections were immuno-stained for cell death [Cleaved caspase-3 and terminal deoxynucleotidyl transferase-mediated deoxyuridine-triphosphate nick-end labeling (TUNEL)], and positive cells were counted in defined areas of the internal granular cell layer. In total, cells in three cerebellar gyri were counted. Results: We found that there was an increase in cells with apoptotic morphology in the internal granular cell layer in the NTFe group. For quantification, we found a significant increase in cell death in group (1) [median (range) number of caspase-3-positive cell group (1) 8 (1–22) vs. group (2) 1 (1–6) and TUNEL-positive cells (1) 6 (1–10) vs. (2) 1 (0–4)]. In both groups, there was no difference in the number of Purkinje cells. Both groups had comparable and stable physiological parameters throughout the 24 h period. Conclusion: Twenty-four hours of continuous intravenous fentanyl infusion increased apoptosis in the internal granular cell layer in the cerebellum of healthy newborn pigs

    Group versus Individualised Minimum Velocity Thresholds in the Prediction of Maximal Strength in Trained Female Athletes

    This study examined the accuracy of di erent velocity-based methods in the prediction of bench press and squat one-repetition maximum (1RM) in female athletes. Seventeen trained females (age 17.8 1.3 years) performed an incremental loading test to 1RM on bench press and squat with the mean velocity being recorded. The 1RM was estimated from the load–velocity relationship using the multiple- (8 loads) and two-point (2 loads) methods and group and individual minimum velocity thresholds (MVT). No significant e ect of method, MVT or interaction was observed for the two exercises (p > 0.05). For bench press and squat, all prediction methods demonstrated very large to nearly perfect correlations with respect to the actual 1RM (r range = 0.76 to 0.97). The absolute error (range = 2.1 to 3.8 kg) for bench press demonstrated low errors that were independent of the method and MVT used. For squat, the favorable group MVT errors for the multiple- and two-point methods (absolute error = 7.8 and 9.7 kg, respectively) were greater than the individual MVT errors (absolute error = 4.9 and 6.3 kg, respectively). The 1RM can be accurately predicted from the load–velocity relationship in trained females, with the two-point method o ering a quick and less fatiguing alternative to the multiple-point method
