Postępowanie przygotowawcze i przebieg ciąży u biorczyni nerki przeszczepionej - opis przypadku
Opisano przypadek biorczyni nerki przeszczepionej
w wieku rozrodczym, która ponad 2 lata po
transplantacji nerki zgłosiła chęć posiadania potomstwa.
Przed decyzją o planowej ciąży, opierając
się na dostępnej literaturze, przeprowadzono
analizę dotychczasowego przebiegu choroby i stosowanej
farmakoterapii. Sytuację zdrowotną pacjentki
analizowano pod względem ryzyka pogorszenia
czynności przeszczepu w trakcie ciąży i po
niej. Rozważono i omówiono z chorą możliwość
pojawienia się powikłań, takich jak cukrzyca ciężarnych, nadciśnienie tętnicze związane z ciążą
oraz schorzeń infekcyjnych. Oszacowano ryzyko
powikłań u płodu adekwatnie do czynności przeszczepu,
innych schorzeń i stosowanego schematu
immunosupresji, a następnie podjęto zalecane
modyfikacje w leczeniu. Na podstawie dokumentacji
medycznej przeanalizowano przebieg ciąży
i opiekę w okresie okołoporodowym oraz dalsze
rokowanie

Planowanie ciąży u biorczyni przeszczepionej nerki
umożliwia wybór warunków sprzyjających prawidłowemu
rozwojowi płodu i zmniejsza ryzyko
pogorszenia funkcji lub utraty narządu. Przygotowanie
obejmuje uzyskanie optymalnej i stabilnej
czynności przeszczepu oraz wykluczenie czynnych
infekcji; modyfikacja farmakoterapii polega
na planowym odstawieniu leku o działaniu teratogennym
lub fetotoksycznym lub zamianie na inny. Ze względu na szczególnie wysokie ryzyko powikłań
u matki i noworodka pacjentki powinny pozostawać
pod wnikliwą opieką zarówno transplantologa,
jak i położnika. Według aktualnych poglądów
u kobiet z prawidłową czynnością nerki i prawidłowymi

One of the key research areas in the embodied cognition field is role of metaphors in creating abstract notions. One of such metaphors is vertical dimension (UP-DOWN) used e.g. for conceptualising positive and/or negative emotions. Its importance has been confirmed by many empirical findings, but some of them arouse methodological concerns regarding the stimuli selection and the level to which observed patterns are universal. The main goal of the present study was to replicate findings of one of the most influential experiments in the field. Its results pointed that positive stimuli are processed faster when presented on the top of the screen while negative ones are processed faster when presented on the bottom. The results of our study yielded a slightly different pattern: positive stimuli were indeed processed faster on top of the screen, but we did not replicate faster processing of negative stimuli on the bottom of the screen. Possible explanations of such a pattern of results were discussed, along with ideas for follow up studies

Aspergilioza płuc u pacjenta po przeszczepie nerki : opis przypadku
Background: We present the radiological findings of pulmonary aspergillosis in a patient who had received a renal transplant, this being a case of proven pulmonary aspergillosis in an immunocompromised person. Posteroanterior chest radiography and lateral chest radiography were done several times over two weeks. CT was done before diagnosis was established and followed up after two months of treatment. Case Report: X-rays showed a few coin lesions in the right lung. CT suggested metastases in both lungs. Invasive exploratory methods such as bronchoscopy and thoracotomy were recommended. Open lung biopsy confirmed the diagnosis. Results: We observed atypical radiological and clinical symptoms. The final diagnosis was based on a combination of radiological, clinical, microbiological, and invasive methods
Associations between depressive symptoms and disease progression in older patients with chronic kidney disease: results of the EQUAL study
Background Depressive symptoms are associated with adverse clinical outcomes in patients with end-stage kidney disease; however, few small studies have examined this association in patients with earlier phases of chronic kidney disease (CKD). We studied associations between baseline depressive symptoms and clinical outcomes in older patients with advanced CKD and examined whether these associations differed depending on sex. Methods CKD patients (>= 65 years; estimated glomerular filtration rate <= 20 mL/min/1.73 m(2)) were included from a European multicentre prospective cohort between 2012 and 2019. Depressive symptoms were measured by the five-item Mental Health Inventory (cut-off <= 70; 0-100 scale). Cox proportional hazard analysis was used to study associations between depressive symptoms and time to dialysis initiation, all-cause mortality and these outcomes combined. A joint model was used to study the association between depressive symptoms and kidney function over time. Analyses were adjusted for potential baseline confounders. Results Overall kidney function decline in 1326 patients was -0.12 mL/min/1.73 m(2)/month. A total of 515 patients showed depressive symptoms. No significant association was found between depressive symptoms and kidney function over time (P = 0.08). Unlike women, men with depressive symptoms had an increased mortality rate compared with those without symptoms [adjusted hazard ratio 1.41 (95% confidence interval 1.03-1.93)]. Depressive symptoms were not significantly associated with a higher hazard of dialysis initiation, or with the combined outcome (i.e. dialysis initiation and all-cause mortality). Conclusions There was no significant association between depressive symptoms at baseline and decline in kidney function over time in older patients with advanced CKD. Tyt. z nagłówka.Bibliogr. s. 106-[108].Raport przedstawia wyniki badań dotyczących motywacji decyzji migracyjnych polskich naukowców. W dwuetapowych badaniach (wywiady częściowo ustrukturyzowane, wywiady ankietowe wspomagane komputerowo) zidentyfikowano najważniejsze czynniki motywujące polskich naukowców do wyjazdu z kraju oraz wpływające na ich decyzję o pozostawaniu za granicą lub powrocie do Polski. Analizy przeprowadzono z uwzględnieniem płci, dziedziny naukowej, statusu migracyjnego (czy osoba badana wróciła do Polski, planuje bądź nie planuje powrócić) i etapu kariery naukowej, na którym osoba badana wyjechała. Najważniejsze zidentyfikowane czynniki motywujące do wyjazdu i pozostawania za granicą to obecne w Polsce trudności finansowe i biurokratyczne, a także nadmierne obciążenie obowiązkami pozanaukowymi. Do powrotu motywuje przede wszystkim otrzymanie konkretnej, satysfakcjonującej propozycji pracy w Polsce lub uzyskanie funduszy na badania w Polsce. Omawiane są również zaproponowane przez osoby badane rozwiązania systemowe, które miałyby zachęcać polskich naukowców do powrotu do kraju.The report concerns motivations of Polish scientists to migrate. In the two-stage study (semi-structured interviews, computer-assisted web interview) we identified the most important factors motivating Polish scientists to leave the country, and stay abroad, or to return to Poland. In the analysis we take into account differences in motivations due to gender, scientific discipline, migration status (i.e. whether a participant returned to Poland, is going to return or not) and the stage of the career at which the participant left Poland. We revealed that the most important factors motivating to leave and stay abroad are bureaucratic and financial constraints in Polish academia, as well as overload with non-scientific duties on Polish universities. It appeared that getting a satisfying job offer in Poland or a research grant in Poland would convince Polish scientists the most to come back and continue scientific career in Poland. The psychological aspects of working with a migrant student
The article summarises the most important issues concerning working with students with migration experiences: foreign teenagers and children, young refugees and children of repatriated persons. The article authors underline the psychological aspects and not only refer to changes in school curricula or language differences, but also discuss the cultural differences and issues such as cultural shock (experienced also by people who come back to the country of their origin), adaptive stress or post‑traumatic stress. The authors also highlight the unique abilities of the migrant students and suggest effective ways of using them in school education. The article concludes with a short presentation of the legal acts that define they ways of supporting this group of students and with a list of publications that may become a source of inspiration for anyone interested in the matter
Kontrola hamująca w procesie rozumienia języka przez osoby dwujęzyczne: dowody z zadania polsko-angielskich homografów międzyjęzykowych
The present thesis focuses on nonselective cross-language activation and inhibitory control inlanguage comprehension in bilinguals. The main goal of the thesis was to create a Polish-English adaptation of a semantic relatedness judgment task with interlingual homograph. Thetask is intended to measure both cross-language activation and inhibition of the irrelevantlanguage. The introductory part of the thesis covers theoretical background and a brief reviewof bilingual studies to date that have employed interlingual homographs. Then, the process ofcreating a Polish-English adaptation of the task is presented and three experiments thatemployed the task are reported. Finally, the links between task performance and proficiencyin the second language as well as ideas for improvements and suggestions of future use of thetask are discussed.Niniejsza praca skupia się na zagadnieniach nieselektywnej aktywacji dwóch językówi kontroli hamującej w rozumieniu języka pisanego u osób dwujęzycznych. Głównym celempracy było stworzenie polsko-angielskiej adaptacji zadania oceny związku semantycznegoz wykorzystaniem homografów międzyjęzykowych. Zgodnie z założeniami zadanie tomierzy współaktywację obu języków i hamowanie nieistotnego w danych kontekście języka.We wstępie pracy przedstawione zostało tło teoretyczne oraz przegląd wybranych badań naddwujęzycznością, które dotychczas wykorzystywały homografy międzyjęzykowe. Następnieopisano proces powstawania polskiej adaptacji zadania, trzy eksperymenty przeprowadzonez jego użyciem oraz ich wyniki. W podsumowaniu pracy rozważane są wynikieksperymentów oraz związek poziomu wykonania zadania z biegłością w drugim języku,pomysły na udoskonalenie zadania oraz propozycje zastosowania go w przyszłychbadaniach
