    Evaluation of the apical infiltration after root canal disruption and obturation

    The aim of this study was to evaluate two root canal filling techniques used in teeth that had their apical foramen disrupted and compare the apical infiltration with an ideal clinical situation. Twenty-seven freshly extracted single-rooted teeth were selected and radiographed to confirm the existence of a single and straight root canal. The crowns were removed at a mean distance of 11 mm from the apex. The teeth had the root canals instrumented and were randomly assigned to 3 groups (n=9): ND group - root canals were filled using the lateral compaction technique and no disruption was performed; DRF group - the apical constriction was disrupted by advancing a #40 K-file 1 mm beyond the original working length, the canals were reinstrumented to create an apical ledge at 1 mm from the apical foramen and were obturated with a master gutta-percha cone with same size as the last file used for reinstrumentation; DF group - the teeth had the apical constriction disrupted and the canals were obturated with a master gutta-percha cone that fit at 1 mm from the apex. The teeth were submitted to dye leakage test with Rhodamine B for 7 days, using vaccum on the initial 5 min. The teeth were sectioned longitudinally and the leakage was measured in a linear fashion from apex to crown. There was no statistically significant difference (p>;0.05) between the groups that had the apical foramen disrupted (DF, DRF), but significant difference was found between the disrupted groups and the non-disrupted one (

    Factors affecting the periapical healing process of endodontically treated teeth

    Tissue repair is an essential process that reestablishes tissue integrity and regular function. Nevertheless, different therapeutic factors and clinical conditions may interfere in this process of periapical healing. This review aims to discuss the important therapeutic factors associated with the clinical protocol used during root canal treatment and to highlight the systemic conditions associated with the periapical healing process of endodontically treated teeth. The antibacterial strategies indicated in the conventional treatment of an inflamed and infected pulp and the modulation of the host's immune response may assist in tissue repair, if wound healing has been hindered by infection. Systemic conditions, such as diabetes mellitus and hypertension, can also inhibit wound healing. The success of root canal treatment is affected by the correct choice of clinical protocol. These factors are dependent on the sanitization process (instrumentation, irrigant solution, irrigating strategies, and intracanal dressing), the apical limit of the root canal preparation and obturation, and the quality of the sealer. The challenges affecting the healing process of endodontically treated teeth include control of the inflammation of pulp or infectious processes and simultaneous neutralization of unpredictable provocations to the periapical tissue. Along with these factors, one must understand the local and general clinical conditions (systemic health of the patient) that affect the outcome of root canal treatment prediction

    Comparison of the biocompatibility of different root canal irrigants

    The purpose of this study was to compare the reaction of rat subcutaneous connective tissue to 0.9% sterile saline, 2.5% sodium hypochlorite (NaOCl), 5.25% NaOCl and 2% chlorhexidine gluconate solution or gel. Six circles were demarcated on the dorsal skin of 24 male Wistar rats, leaving 2 cm between each circle. Using a syringe, 0.1 mL of each root canal irrigant was injected subcutaneously into 5 circles. In the 6th circle, the needle of an empty syringe was introduced into the skin, but no irrigant was injected (control group). Evaluations were undertaken at 2 h, 48 h, 14 days and 30 days post-procedure. Tissue samples were excised, embedded in paraffin blocks and 3-µm-thick sections were obtained and stained with hematoxylin and eosin. The areas of inflammatory reaction were evaluated and analyzed statistically by ANOVA and Tukey's test. The control group showed few or no inflammatory reaction areas in the subcutaneous tissue. 0.9% saline solution, 2.0% chlorhexidine solution and 2.5% NaOCl showed a good biocompatibility, as very mild inflammatory reaction was detected at 14 days and tissue repair occurred at 30 days. 5.25% NaOCl was the most toxic irrigant, as the number of inflammatory cells remained elevated at 14 and 30 days. The group treated with 2.0% chlorhexidine gluconate gel presented a moderate inflammatory response at 14 days, which decreased at 30 days, being considered similar to that of the control group, 0.9% saline solution, 2.0% chlorhexidine solution and 2.5% NaOCl at this experimental period

    Potent hepatoprotective effect in CCl4-induced hepatic injury in mice of phloroacetophenone from Myrcia multiflora

    Background: This study investigated the hepatoprotective effect and antioxidant properties of phloroacetophenone (2′,4′,6′-trihydroxyacetophenone – THA), an acetophenone derived from the plant Myrcia multiflora.Material & Method: The free radical scavenging activity in vitro and induction of oxidative hepatic damage by carbon tetrachloride (CCl4) (0.5 ml/kg, i.p.) were tested in male Swiss mice (25±5 g). Results: This compound exhibited in vitro antioxidant effects on FeCl2–ascorbate-induced lipid peroxidation (LPO) in mouse liver homogenate, scavenging hydroxyl and superoxide radicals, and 2,2-diphenyl-1-picrylhydrazyl. The in vivo assays showed that THA significantly (p<0.01) prevented the increases of hepatic LPO as measured by the levels of thiobarbituric acid-reactive substances, mitochondrial swelling. It also protected hepatocytes against protein carbonylation and oxidative DNA damage. Consistent with these observations, THA pre-treatment normalized the activities of antioxidant enzymes, such as catalase, glutathione peroxidase, and superoxide dismutase, and increased the levels of reduced glutathione (GSH) in CCl4-treated mice. In addition, THA treatment significantly prevented the elevation of serum enzymatic activities of alanine amino transferase, aspartate amino transferase, and lactate dehydrogenase, as well as histological alterations induced by CCl4. Silymarin (SIL) (24 mg/kg), a known hepatoprotective drug used for comparison, led to a significant decrease (p<0.01) in activities of theses enzymes in way very similar to that observed in pre-treatment with THA. Conclusion: These results suggest that the protective effects are due to reduction of oxidative damage induced by CCl4 resulting from the antioxidant properties of THA

    Evaluation of subcutaneous and alveolar implantation surgical sites in the study of the biological properties of root-end filling endodontic materials

    OBJECTIVE: The aim of this study was to compare two methodologies used in the evaluation of tissue response to root-end filling materials in rats. MATERIAL AND METHODS: Forty rats were divided into 4 groups: in Groups I and II (control groups), empty polyethylene tubes were implanted in the extraction site and in the subcutaneous tissue, respectively; in Groups III and IV, polyethylene tubes filled with ProRoot MTA were implanted in the extraction site and in the subcutaneous tissue, respectively. The animals were killed 7 and 30 days after tube implantation, and the hemi-maxillas and the capsular subcutaneous tissue, both with the tubes, were removed.     Glycerol Acetylation with Propionic Acid Using Iron and Cobalt Oxides in Al-MCM-41 Catalysts

    In this work, Al-MCM-41 molecular sieves were synthesized, containing iron and/or cobalt oxides, impregnated by incipient wetness method, characterized and applied as catalysts in the acetylation reaction of glycerol with propionic acid to produce green glyceryl propionate molecules of high commercial value. According to this, X-ray Diffraction (XRD), X-ray Fluorescence (XRF), Fourier Transform Infra Red (FT-IR), adsorption/desorption N2 isotherms, textural analysis, and Scanning Electron Microscope (SEM) analysis were recorded to evaluate the main characteristics of materials. The presence of Lewis and Brønsted acidic sites and catalysts surface area were observed as important key points to functionalize acetylation reaction. Thus, time reaction, temperature, and glycerol / propionic acid ratio varied to improve the most suitable reaction conditions and behaviors. As a result, glycerol conversion was above 96%, followed by 68% of selectivity to glyceryl monopropionate as well as the formation of glyceryl di- and tri- propionate and a small amount of ethylene glycol dipropionate as an undesired product.     Dental materials in general are tested in different animal models prior to the clinical use in humans, except for bleaching agents. Objectives To evaluate an experimental rat model for comparative studies of bleaching agents, by investigating the influence of different concentrations and application times of H2O2 gel in the pulp tissue during in-office bleaching of rats’ vital teeth. Material and Methods The right and left maxillary molars of 50 Wistar rats were bleached with 20% and 35% H2O2 gels, respectively, for 5, 10, 15, 30, or 45 min (n=10 rats/group). Ten animals were untreated (control). The rats were killed after 2 or 30 days, and the maxillae were examined by light microscopy. Inflammation was evaluated through histomorphometric analysis with inflammatory cell count in the coronal and radicular thirds of the pulp. Fibroblasts were also counted. Scores were attributed to odontoblastic layer and vascular changes. Tertiary dentin area and pulp chamber central area were measured histomorphometrically.     Study of anatomical variations in premolars by cone beam computerized tomography in a radiologic clinic in Piauí

    Introduction: Root canal cleaning is the main objective of endodontic treatment and requires knowledge of the internal anatomy. The premolars are evidenced in the literature with great anatomical variations. In view of this, studies indicate that the use of Cone Beam Computed Tomography helps in the visualization of highly complex anatomy. Objective: to describe the anatomical variations in maxillary and mandibular premolars using cone beam computed tomography in a radiologic clinic in Piaui. Methods: 54 cone beam computed tomography scans with 160 premolars were used, produced using the Orthopantomograph OP300 equipment and analyzed by multiplanar reconstructions: axial, coronal and sagittal. Data regarding sex, number of roots and canals were recorded to compare and classify according to Vertucci. Results: the maxillary first pre-molars had 63.5% two roots,83.7% with one root and the mandibular pre-molars mostly with one root. Regarding the number of channels, 92.3% of the first premolars had two channels, most of them maxillary second premolars and mandibular premolars only one channel. Vertucci variations of types I, II, III and IV were verified in single-rooted elements, observing a great variation in superior elements. As for the prevalence of sex, only the first superiors showed greater variation in males. Conclusions: the upper first premolars prevailed with a great anatomical variation in relation to the other premolars with prevalence of Vertucci Type I and in males.Introducción: La limpieza del conducto radicular es el principal objetivo del tratamiento endodóntico y requiere del conocimiento de la anatomía interna. Los premolares se evidencian en la literatura con grandes variaciones anatómicas. Por lo tanto, los estudios indican que el uso de la tomografía computarizada de haz cónico ayuda en la visualización de anatomías altamente complejas. Metodología: Se utilizaron 4 equipos computarizados 60 premolares con 1 equipo, equipo sin equipo Orthopanto y ahora por reconstrucciones, coronal y sargital. Se registraron datos referentes al sexo, número de raíces y conductos para su comparación y clasificación según Vers. Resultados: Los primeros molares maxilares tenían el 63,5% de dos raíces, el 83,7% de los segundos premolares superiores tenían una raíz y la mayoría de los premolares mandibulares tenían una raíz. En cuanto al número de canales, el 92,3% de los primeros premolares tenían dos canales, siendo la mayoría segundos premolares maxilares y premolares inferiores de un canal. Se verificaron variaciones de Vertucci de los tipos I, II, III y IV en los elementos monoraíces, observándose la gran observación en los elementos superiores. En cuanto a la mayoría de machos, los primeros presentan el mayor número de machos. Conclusión: Los primeros premolares superiores prevalecerán con los demás premolares en relación a la prevalencia de Vertucci y en el sexo masculino.Introduction : Le nettoyage canalaire est l'objectif principal du traitement endodontique et nécessite une connaissance de l'anatomie interne. Les prémolaires sont mises en évidence dans la littérature avec de grandes variations anatomiques. Par conséquent, des études indiquent que l'utilisation de la tomodensitométrie à faisceau conique aide à la visualisation d'anatomies très complexes. Méthodologie : 4 équipements informatisés ont été utilisés 60 prémolaires avec 1 équipement, équipement sans équipement Orthopanto et maintenant par reconstructions, coronales et sarcitales. Les données relatives au sexe, au nombre de racines et de canaux ont été enregistrées pour comparaison et classification selon Vers. Résultats : Les premières molaires maxillaires avaient 63,5 % de deux racines, 83,7 % des deuxièmes prémolaires maxillaires avaient une racine et la plupart des prémolaires mandibulaires avaient une racine. En ce qui concerne le nombre de canaux, 92,3% des premières prémolaires avaient deux canaux, la majorité n'étant que des deuxièmes prémolaires maxillaires et des prémolaires inférieures à un seul canal. Les variations de Vertucci des types I, II, III et IV ont été vérifiées dans les éléments à racine unique, en observant la grande observation dans les éléments supérieurs. Quant à la majorité des mâles, les premiers à présenter le plus grand nombre de mâles. Conclusion : Les premières prémolaires supérieures prédominent avec les autres prémolaires en relation avec la prévalence de Vertucci et chez les hommes.Introdução: a limpeza do canal radicular é o principal objetivo do tratamento endodôntico e requer conhecimento da anatomia interna. Os pré-molares são evidenciados na literatura com grandes variações anatômicas. Diante disso, estudos indicam que o uso da Tomografia Computadorizada Cone Beam auxilia na visualização de anatomias de alta complexidade. Metodologia: foram utilizadas 54 tomografias computadorizadas de feixe cônico com 160 pré-molares, produzidas no equipamento Orthopantomograph OP300 e analisadas por reconstruções multiplanares: axial, coronal e sagital. Os dados referentes ao sexo, número de raízes e canais foram registrados para comparação e classificação segundo Vertucci. Resultados: os primeiros pré-molares superiores apresentavam 63,5% de duas raízes, 83,7% dos segundos pré-molares superiores tinham uma raiz e a maioria dos pré-molares inferiores tinha uma raiz. Em relação ao número de canais, 92,3% dos primeiros pré-molares possuíam dois canais, sendo a maioria segundos pré-molares superiores e pré-molares inferiores apenas um canal. Vertucci variações dos tipos I, II, III e IV foram verificadas nos elementos uniradiculares, observando-se a grande variação nos elementos superiores. Quanto à prevalência do sexo, apenas os primeiros superiores apresentaram maior variação no sexo masculino.
