    The changing conceptions and focus of health research in East Africa

    Perceptions in health research are a product of the circumstances within the society, where the research activities are situated. In East Africa there has been a change in conceptualization over a period of time from an elitist de-linked status to the present, which has evolved to embrace the local community. Here we trace the changes and highlight some occurrences that exerted the greatest influence in shaping the notions that currently dominate in research. We conclude that the paradigm shift is a positive development and that the present conception is suitable for heath research at this point in time.     Measuring the Hidden Aspects of Solar Magnetism

    2008 marks the 100th anniversary of the discovery of astrophysical magnetic fields, when George Ellery Hale recorded the Zeeman splitting of spectral lines in sunspots. With the introduction of Babcock's photoelectric magnetograph it soon became clear that the Sun's magnetic field outside sunspots is extremely structured. The field strengths that were measured were found to get larger when the spatial resolution was improved. It was therefore necessary to come up with methods to go beyond the spatial resolution limit and diagnose the intrinsic magnetic-field properties without dependence on the quality of the telescope used. The line-ratio technique that was developed in the early 1970s revealed a picture where most flux that we see in magnetograms originates in highly bundled, kG fields with a tiny volume filling factor. This led to interpretations in terms of discrete, strong-field magnetic flux tubes embedded in a rather field-free medium, and a whole industry of flux tube models at increasing levels of sophistication. This magnetic-field paradigm has now been shattered with the advent of high-precision imaging polarimeters that allow us to apply the so-called "Second Solar Spectrum" to diagnose aspects of solar magnetism that have been hidden to Zeeman diagnostics. It is found that the bulk of the photospheric volume is seething with intermediately strong, tangled fields. In the new paradigm the field behaves like a fractal with a high degree of self-similarity, spanning about 8 orders of magnitude in scale size, down to scales of order 10 m.Comment: To appear in "Magnetic Coupling between the Interior and the Atmosphere of the Sun", eds.     Maxillofacial and mandibular phenotypes in the skulls of red Sokoto and Sahel goats in Nigeria: The role of dental abnormalities

    This work examined phenotypic expressions in the anatomy of the mandible and maxillofacial region of the Red Sokoto and Sahel goats in Nigeria. The infraorbital foramen was placed above premolar two (PM2) in Red Sokoto but above premolar one (PM1) in Sahel. The Red Sokoto displayed interdigital septa (ruggae) which entered into mediolateral depressions of the opposing mandible but this was less prominent in the Sahel. Dental abnormalities were more pronounced in the Red Sokoto on the maxilla and mandibular rows. The facial and mandibular tuberosities were more prominent in the Red Sokoto goat and found to be more conspicuous in this breed with dental abnormalities. There was no significant difference between Calcium and Phosphorus in the craniofacial bones of goats with dental abnormalities compared to those without dental abnormalities in both breeds and in conclusion we thus suggest that the harsh environmental conditions of arid North West may be a greater predisposing factor for the negative dental profile of the Red Sokoto breed

    Familial adenomatous patients with desmoid tumours show increased expression of miR-34a in serum and high levels in tumours

    Familial adenomatous polyposis (FAP) is rare affecting 1 in 10,000 people and a subset (10%) are at risk of myofibroblastic desmoid tumours (DTs) after colectomy to prevent cancer. DTs are a major cause of morbidity and mortality. The absence of markers to monitor progression and a lack of treatment options are significant limitations to clinical management. We investigated microRNAs (miRNA) levels in DTs and serum using expression array analysis on two independent cohorts of FAP patients (total, n=24). Each comprised equal numbers of patients who had formed DTs (cases) and those who had not (controls). All controls had absence of DTs confirmed by clinical and radiological assessment over at least three years post- colectomy. Technical qPCR validation was performed using an expanded cohort (29 FAP patients; 16 cases and 13 controls). The most significant elevated serum miRNA marker of DTs was miR-34a-5p and in-situ hybridisation (ISH) showed most DTs analysed (5/6) expressed miRNA-34a-5p. Exome sequencing of tumour and matched germline DNA did not detect mutations within the miR-34a-5p transcript sites or 3'-UTR of target genes that would alter functional miRNA activity. In conclusion, miR-34a-5p is a potential circulatory marker and therapy target. A large prospective world-wide multi-centre study is now warranted

    Neutrophil gelatinase-associated lipocalin in dehydrated patients: a preliminary report

    <p>Abstract</p> <p>Background</p> <p>Acute kidney injury has been recognized as a major contributor to end stage renal disease. Although neutrophil gelatinase-associated lipocalin (Ngal) has been reported as a promising biomarker for early detection of acute kidney injury, no study has yet examined its potential clinical impact in patients with normal renal function. The purpose of current study is to investigate possible difference in serum Ngal levels between dehydrated and control patients.</p> <p>Findings</p> <p>A total of twelve patients presented with symptoms of mild dehydration defined by history of diarrheas or vomiting and orthostatic (postural) hypotension and an age and sex matched group of twelve control patients were included. The two groups of patients did not seem to differ in basic clinical and laboratory parameters. Serum Ngal was higher in dehydrated patients when compared to control group (Ngal = 129.4 ± 25.7 ng/mL vs 60.6 ± 0.4 ng/mL, p = 0.02). Ngal was not correlated with age, hemoglobin, white blood cell count, red blood cell count, urea or creatinine.</p> <p>Conclusions</p> <p>The presence of elevated Ngal levels in dehydrated patients may suggest its role as a very sensitive biomarker in even minimal and "silent" prerenal kidney dysfunction</p

    Inspiratory muscle training reduces blood lactate concentration during volitional hyperpnoea

    Although reduced blood lactate concentrations ([lac−]B) have been observed during whole-body exercise following inspiratory muscle training (IMT), it remains unknown whether the inspiratory muscles are the source of at least part of this reduction. To investigate this, we tested the hypothesis that IMT would attenuate the increase in [lac−]B caused by mimicking, at rest, the breathing pattern observed during high-intensity exercise. Twenty-two physically active males were matched for 85% maximal exercise minute ventilation (V˙Emax) and divided equally into an IMT or a control group. Prior to and following a 6 week intervention, participants performed 10 min of volitional hyperpnoea at the breathing pattern commensurate with 85% V˙Emax

    Emotional persistence in online chatting communities

    How do users behave in online chatrooms, where they instantaneously read and write posts? We analyzed about 2.5 million posts covering various topics in Internet relay channels, and found that user activity patterns follow known power-law and stretched exponential distributions, indicating that online chat activity is not different from other forms of communication. Analysing the emotional expressions (positive, negative, neutral) of users, we revealed a remarkable persistence both for individual users and channels. I.e. despite their anonymity, users tend to follow social norms in repeated interactions in online chats, which results in a specific emotional "tone" of the channels. We provide an agent-based model of emotional interaction, which recovers qualitatively both the activity patterns in chatrooms and the emotional persistence of users and channels. While our assumptions about agent's emotional expressions are rooted in psychology, the model allows to test different hypothesis regarding their emotional impact in online communication.Comment: 34 pages, 4 main and 12 supplementary figure
