    Plasma-cell type Castleman’s disease of the neck and lymphocyte-depletion Hodgkin lymphoma associated with intestinal intussusception in an AIDS patient

    A 36-year-old man was diagnosed with plasma-cell type Castleman’s disease with the presentation of recurrent lymphadenpathy of the neck. HIV infection was not suspected or confirmed until esophageal candidiasis developed one year later. Meanwhile, surgery was performed for intestinal intussusception and obstruction caused by lymphocyte-depletion Hodgkin lymphoma. However, he died of rapidly progressive pneumonia and disseminated intravascular coagulation associated with intracerebral hemorrhage, which occurred 6 months later during the course of chemotherapy. This case suggests that HIV infection should be considered in patients who present with plasma-cell type Castleman’s disease or lymphocyte-depletion Hodgkin lymphoma with extra-nodal involvement in order to conduct appropriate diagnosis and initiate treatment for HIV infection

    Antioksidativna i citotoksična aktivnost ekstrakata mlijeka proizvedenog iz fermentirane crne soje

    In this study, ethanol extracts from 2-day fermented black soybean milk (FBE) by immobilized Rhizopus oligosporus NTU5 have been evaluated for both antioxidant and cytotoxic activities. The results reveal that a 2-day FBE had strong 2,2-diphenyl-1-picrylhydrazyl (DPPH) scavenging effect (76 %). The extracts were further fractionated by silica gel column chromatography and an unknown compound, FBE5-A, was obtained, which exhibited strong antioxidant activity. IC50 of the DPPH scavenging effect of FBE5-A was 7.5 μg/mL, which is stronger than a commonly used antioxidant, vitamin E (α-tocopherol; 17.4 μg/mL), and similar to vitamin C (ascorbic acid; 7.6 μg/mL). The cytotoxic test demonstrated that extracts of 2-day fermented broth exhibited selective cytotoxic activity towards human carcinoma cells, Hep 3B (IC50=150.2 μg/mL), and did not affect normal human lung fibroblasts, MRC-5 (p<0.05). The results indicate the potential applications of fermented black soybean milk as functional food, pharmaceutical or cancer therapy formula.U radu je ispitana antioksidativna i citotoksična aktivnost etanolnih esktrakata mlijeka dobivenog nakon 2 dana fermentacije crne soje pomoću imobiliziranoga soja Rhizopus oligosporus NTU5. Rezultati su pokazali da takvo mlijeko ima izraženu sposobnost uklanjanja slobodnih DPPH radikala (76 %). Ekstrakti su mlijeka zatim frakcionirani kromatografijom na silikagelu i izdvojen je nepoznati sastojak, nazvan FBE5-A, izraženog antioksidativnog svojstva. Sposobnost toga sastojka da uklanja slobodne DPPH radikale kudikamo je veća (IC50=7,5 μg/mL) nego vitamina E, koji se obično koristi kao antioksidans (α-tokoferol, IC50=17,4 μg/mL) i približno jednaka vitaminu C (askorbinska kiselina, IC50=7,6 μg/mL). Ispitivanje citotoksičnog učinka FBE5-A pokazalo je da selektivno djeluje na ljudske stanice raka Hep 3B (IC50=150,2 μg/mL), a pritom ne utječe na normalne fibroblaste iz pluća MRC-5 (p<0,05).     Aging-Induced Dynamics for Statically Indeterminate System

    Statically indeterminate systems are experimentally demonstrated to be in fact dynamical at the microscopic scale. Take the classic ladder-wall problem, for instance. Depending on the Young's modulus of the wall, it may take up to twenty minutes before its weight saturates. This finding is shown to be shared by other statically indeterminate systems, such as a granule silo and a beam with three support points. We believe that the aging effect is responsible for this surprising phenomenon because it can be correlated with the evolution of microscopic contact area with the wall and floor. Finally, a heuristic and simple method is introduced that can uniquely determine and analytically solve the saturated weight without invoking detailed material properties.Comment: 5 pages, 5 figure

    Factors Related to Significant Improvement of Estimated Glomerular Filtration Rates in Chronic Hepatitis B Patients Receiving Telbivudine Therapy

    Background and Aim. The improvement of estimated glomerular filtration rates (eGFRs) in chronic hepatitis B (CHB) patients receiving telbivudine therapy is well known. The aim of this study was to clarify the kinetics of eGFRs and to identify the significant factors related to the improvement of eGFRs in telbivudine-treated CHB patients in a real-world setting. Methods. Serial eGFRs were calculated every 3 months using the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation. The patients were classified as CKD-1, -2, or -3 according to a baseline eGFR of ≥90, 60–89, or <60 mL/min/1.73 m2, respectively. A significant improvement of eGFR was defined as a more than 10% increase from the baseline. Results. A total of 129 patients were enrolled, of whom 36% had significantly improved eGFRs. According to a multivariate analysis, diabetes mellitus (DM) (p=0.028) and CKD-3 (p=0.043) were both significantly related to such improvement. The rates of significant improvement of eGFR were about 73% and 77% in patients with DM and CKD-3, respectively. Conclusions. Telbivudine is an alternative drug of choice for the treatment of hepatitis B patients for whom renal safety is a concern, especially patients with DM and CKD-3

    Waste to energy: air pollutant emissions from the steam boilers using recycled waste wood

    In Taiwan, combustible wood mostly comes from waste pallets and scrap packaging materials discarded by factories, which produced a total of 278,067 tons of waste wood in 2019. In this study, the heat value of waste wood was 18.3 ± 1.07 MJ kg–1. The measured volatile fraction was 76.5 ± 7.34%, the fixed carbon was 15.7 ± 3.19%, the ash content was 2.96 ± 2.45%, and the moisture content was 21.6 ± 10.2%. The proportions of the elemental constituents in the waste wood were 45.3 ± 4.95%, 46.9 ± 3.94%, 5.9 ± 0.44%, 0.21 ± 0.17%, 0.29 ± 0.26%, and 0.02 ± 0.02% for carbon, oxygen, hydrogen, sulfur, nitrogen, and chlorine, respectively. The average boiler capacity was 11.5 ± 6.84 ton hr–1, the average fuel consumption of the boilers was 1.47 ± 1.81 ton hr–1, the average operating temperature of the boilers was 853 ± 228°C, the average steam generation of the boilers was 7.63 ± 5.97 ton hr–1, and the average exhaust flow rate was 246.6 ± 200.9 m3 min–1. The main air pollution control systems used in the waste wood combustion boilers were systems combining a cyclone, a baghouse and a scrubber (37.8%), a cyclone and a baghouse (28.4%), a cyclone and a scrubber (10.2%), and systems using a baghouse only (9.8%). Based on our fuel consumption data, the air pollutant emission factors were 0.71 ± 1.44 kg per ton of wood for PM, 0.86 ± 1.47 kg per ton of wood for SOx, and 5.24 ± 9.56 kg per ton of wood for NOx. In July 2022, new emission standards for boilers will be implemented, and emission reductions of at least 30% for PM, 35% for NOx and 7% for SO2 will be required

    The relationship between gallbladder status and recurrent biliary complications in patients with choledocholithiasis following endoscopic treatment

    AbstractBackgroundEndoscopic methods are currently the treatment of choice for patients with common bile duct (CBD) stones, but subsequent management of the intact gallbladder for patients following endoscopic treatment is still controversial. The primary aim of this study was to discover the association between gallbladder status and recurrent biliary complications for patients with CBD stones after endoscopic treatment. Additionally, we also sought to determine risk factors for recurrent biliary complications in these patients.MethodsThe records of 1625 patients with CBD stones following endoscopic treatment were reviewed. A total of 681 patients were enrolled and subsequently categorized into four groups: Group 1 (n = 201), calculous gallbladder; Group 2 (n = 140), acalculous gallbladder; Group 3 (n = 175), elective cholecystectomy after endoscopic treatment; and Group 4 (n = 165), prior cholecystectomy. The basic demographics and recurrent biliary complications during follow-up among these four groups were analyzed by Chi-square test, ANOVA, Kaplan-Meier analysis, and log-rank test.ResultsDuring the median follow-up period of 34 months, 133 patients (20%) with recurrent biliary complications were identified. The recurrence rates of Groups 1, 2, 3, and 4 were 29%, 11%, 15%, and 19%, respectively. Kaplan-Meier analysis showed that patients with calculous gallbladder had a significantly higher rate of recurrent biliary complication. In multivariate analysis, patients with a history of cirrhosis, juxta-papillary diverticulum, calculous gallbladder, CBD size ≥1.5 cm, and endoscopic management with endoscopic sphincterotomy were at a higher risk for developing biliary complications (p = 0.029, p = 0.039, p < 0.001, p = 0.002, p = 0.021, respectively.)ConclusionPatients with cholecystolithiasis and CBD stones had a higher incidence of recurrent biliary complications. For some of these patients, elective cholecystectomy following endoscopic treatment may be considered. However, routine elective cholecystectomy in patients with normal gallbladder is not appropriate because of the low recurrence of biliary complications. Whether gallbladder function affects the biliary clearance and biliary complications requires further research

    Identifying Chinese Herbal Medicine Network for Endometriosis: Implications from a Population-Based Database in Taiwan

    Background. Endometriosis is a common but bothersome gynecological disease, and Chinese herbal medicine (CHM) is used for treating endometriosis. The aim of this study is to explore CHM network and core treatments for endometriosis by analyzing nationwide CHM prescription database. Methods. From 1998 to 2013, the CHM prescriptions made primarily for endometriosis among women diagnosed with endometriosis (ICD-9-CM code: 671) by gynecologists during their reproductive age were collected. CHM network analysis was then carried out by using association rule mining and social network analysis. Results. A total of 12,986 CHM prescriptions made for endometriosis were analyzed. There were 556 kinds of CHM ever used, and, in average, each prescription was composed of 6.2 CHMs. Gui-Zhi-Fu-Ling-Wan (GZFLW) was used most frequently, followed by Cyperus rotundus (28.1% and 18.8% of all prescriptions, resp.). Additionally, the combination of Cyperus rotundus with GZFLW (8.0%) was the most frequently used combination of two CHMs. CHM network showed that GZFLW was the core CHM for endometriosis and graphically demonstrated the extensive coverage of TCM syndromes and pathogenesis of endometriosis. Conclusions. CHM network provides graphical demonstration and summary of commonly used CHMs for endometriosis, and further studies are warranted based on these findings

    A QoS-Guaranteed Coverage Precedence Routing Algorithm for Wireless Sensor Networks

    For mission-critical applications of wireless sensor networks (WSNs) involving extensive battlefield surveillance, medical healthcare, etc., it is crucial to have low-power, new protocols, methodologies and structures for transferring data and information in a network with full sensing coverage capability for an extended working period. The upmost mission is to ensure that the network is fully functional providing reliable transmission of the sensed data without the risk of data loss. WSNs have been applied to various types of mission-critical applications. Coverage preservation is one of the most essential functions to guarantee quality of service (QoS) in WSNs. However, a tradeoff exists between sensing coverage and network lifetime due to the limited energy supplies of sensor nodes. In this study, we propose a routing protocol to accommodate both energy-balance and coverage-preservation for sensor nodes in WSNs. The energy consumption for radio transmissions and the residual energy over the network are taken into account when the proposed protocol determines an energy-efficient route for a packet. The simulation results demonstrate that the proposed protocol is able to increase the duration of the on-duty network and provide up to 98.3% and 85.7% of extra service time with 100% sensing coverage ratio comparing with LEACH and the LEACH-Coverage-U protocols, respectively

    Fixel-Based Analysis Effectively Identifies White Matter Tract Degeneration in Huntington’s Disease

    Microstructure damage in white matter might be linked to regional and global atrophy in Huntington’s Disease (HD). We hypothesize that degeneration of subcortical regions, including the basal ganglia, is associated with damage of white matter tracts linking these affected regions. We aim to use fixel-based analysis to identify microstructural changes in the white matter tracts. To further assess the associated gray matter damage, diffusion tensor-derived indices were measured from regions of interest located in the basal ganglia. Diffusion weighted images were acquired from 12 patients with HD and 12 healthy unrelated controls using a 3 Tesla scanner. Reductions in fixel-derived metrics occurs in major white matter tracts, noticeably in corpus callosum, internal capsule, and the corticospinal tract, which were closely co-localized with the regions of increased diffusivity in basal ganglia. These changes in diffusion can be attributed to potential axonal degeneration. Fixel-based analysis is effective in studying white matter tractography and fiber changes in HD
