    Bilateral stenting methods for hilar biliary obstructions

    OBJECTIVE: There is no consensus regarding the most appropriate methods (i.e., the side-by-side versus the stent-in-stent technique) for placing bilateral stents for malignant hilar biliary obstructions. We aimed to perform a quantitative review of the published data regarding the clinical efficacy of the side-by-side and stentinstent bilateral drainage techniques for hilar biliary obstructions. METHODS: A comprehensive search of several databases was conducted and a fixed-effects or random-effects model was used to pool the data from all of the study end-points. RESULTS: Four clinical trials were identified. A comparison of the side-by-side and stent-in-stent groups revealed no significant differences with respect to the rates of successful placement, successful drainage, early complications, late complications and stent occlusions. There were also no significant inter-group differences in stent patency and patient survival and no publication bias was observed. CONCLUSIONS: The performance of the side-by-side technique appears to be similar to that of the stent-instent technique for bilateral drainage in patients with malignant hilar biliary obstructions

    Molecular Characterization and Antimicrobial Susceptibility of Nasal Staphylococcus aureus Isolates from a Chinese Medical College Campus

    Staphylococcus aureus colonization and infection occur more commonly among persons living or working in crowded conditions, but characterization of S. aureus colonization within medical communities in China is lacking. A total of 144 (15.4%, 144/935) S. aureus isolates, including 28 (3.0%, 28/935) MRSA isolates, were recovered from the nares of 935 healthy human volunteers residing on a Chinese medical college campus. All S. aureus isolates were susceptible to vancomycin, quinupristin/dalfopristin and linezolid but the majority were resistant to penicillin (96.5%), ampicillin/sulbactam (83.3%) and trimethoprim/sulfamethoxazole (93.1%). 82%, (23/28) of the MRSA isolates and 66% (77/116) of the MSSA isolates were resistant to multiple antibiotics, and 3 MRSA isolates were resistant to mupirocin—an agent commonly used for nasal decolonization. 16 different sequence types (STs), as well as SCCmec genes II, III, IVd, and V, were represented among MRSA isolates. We also identified, for the first time, two novel STs (ST1778 and ST1779) and 5 novel spa types for MRSA. MRSA isolates were distributed in different sporadic clones, and ST59-MRSA-VId- t437 was found within 3 MRSA isolates. Moreover, one isolate with multidrug resistance belonging to ST398-MRSA-V- t571 associated with animal infections was identified, and 3 isolates distributed in three different clones harbored PVL genes. Collectively, these data indicate a high prevalence of nasal MRSA carriage and molecular heterogeneity of S. aureus isolates among persons residing on a Chinese medical college campus. Identification of epidemic MRSA clones associated with community infection supports the need for more effective infection control measures to reduce nasal carriage and prevent dissemination of MRSA to hospitalized patients and health care workers in this community

    High Prevalence of Extended-Spectrum Beta Lactamases among Salmonella enterica Typhimurium Isolates from Pediatric Patients with Diarrhea in China

    We investigated the extended-spectrum beta lactamases among 62 Salmonella enterica Typhimurium isolates recovered from children with diarrhea in a Chinese pediatric hospital. A large proportion of S. enterica Typhimurium isolates were resistant to multiple antimicrobial agents, including ampicillin (90.3%), tetracycline (80.6%), trimethoprim/sulfamethoxazole (74.2%), chloramphenicol (66.1%), cefotaxime (27.4%). Forty-nine (79.0%) of S. enterica Typhimurium isolates were positive for blaTEM-1b and resistant to ampicillin. Thirteen S. enterica Typhimurium isolates (21.0%) were positive for blaCTX-M-1-group and blaCTX-M-9-group, and all isolates harboring blaCTX-M genes were positive for ISEcp1. Two main clones (PFGE type A and D) accounted for nearly 70% of S. enterica Typhimurium isolates, and 7 CTX-M-producing isolates belonged to PFGE type D. Collectively, our data reveal multi-drug resistance and a high prevalence of extended spectrum beta lactamases among S. enterica Typhimurium isolates from children in China. In addition, we report the first identification of blaCTX-M-55 within Salmonella spp. Our data also suggest that clonal spread is responsible for the dissemination of S. enterica Typhimurium isolates

    Prioritizing multiple therapeutic targets in parallel using automated DNA-encoded library screening

    AbstractThe identification and prioritization of chemically tractable therapeutic targets is a significant challenge in the discovery of new medicines. We have developed a novel method that rapidly screens multiple proteins in parallel using DNA-encoded library technology (ELT). Initial efforts were focused on the efficient discovery of antibacterial leads against 119 targets from Acinetobacter baumannii and Staphylococcus aureus. The success of this effort led to the hypothesis that the relative number of ELT binders alone could be used to assess the ligandability of large sets of proteins. This concept was further explored by screening 42 targets from Mycobacterium tuberculosis. Active chemical series for six targets from our initial effort as well as three chemotypes for DHFR from M. tuberculosis are reported. The findings demonstrate that parallel ELT selections can be used to assess ligandability and highlight opportunities for successful lead and tool discovery.</jats:p

    Seismic AVOA Inversion for Weak Anisotropy Parameters and Fracture Density in a Monoclinic Medium

    In shale gas development, fracture density is an important lithologic parameter to properly characterize reservoir reconstruction, establish a fracturing scheme, and calculate porosity and permeability. The traditional methods usually assume that the fracture reservoir is one set of aligned vertical fractures, embedded in an isotropic background, and estimate some alternative parameters associated with fracture density. Thus, the low accuracy caused by this simplified model, and the intrinsic errors caused by the indirect substitution, affect the estimation of fracture density. In this paper, the fractured rock of monoclinic symmetry assumes two non-orthogonal vertical fracture sets, embedded in a transversely isotropic background. Firstly, assuming that the fracture radius, width, and orientation are known, a new form of P-wave reflection coefficient, in terms of weak anisotropy (WA) parameters and fracture density, was obtained by substituting the stiffness coefficients of vertical transverse isotropic (VTI) background, normal, and tangential fracture compliances. Then, a linear amplitude versus offset and azimuth (AVOA) inversion method, of WA parameters and fracture density, was constructed by using Bayesian theory. Tests on synthetic data showed that WA parameters, and fracture density, are stably estimated in the case of seismic data containing a moderate noise, which can provide a reliable tool in fracture prediction

    Users’ Behaviour under the Uneven Gender Ratio of Social Media Platforms

    Up to now, in order to satisfy the increasingly abundant needs of users, there have been many kinds of social media applications, some of which are targeted at specific gender groups. Xiaohongshu and Hupu are two social media platforms in China, which are dominated by female and male users respectively over the past decade. Apart from the difference in function and form, the most important difference between the two platforms is the gender difference of the target audience, which is obvious. This article will take these two social platforms as examples and adopts quantitative research method to ask questions about users’ behaviours through questionnaire survey, so as to understand users’ behaviours when gender ratio is imbalanced. Research shows that users have a universal understanding of gender differences on the platform, and most of them are close to the dominant gender group in user behaviour. In the background of new media, if the community structure is not changed, social platforms with large gender differences among users will regularly deepen gender stereotypes, and users’ sense of belonging will also fluctuate

    Assessment of Remote Sensing Ecological Quality by Introducing Water and Air Quality Indicators: A Case Study of Wuhan, China

    In the context of ecological protection and urban expansion, the quality of the ecological environment and ecological security are gravely at risk. A simple, effective, and comprehensive assessment method for regional environmental quality monitoring is urgently required at this time. This study proposes a comprehensive approach for evaluating regional ecological quality. Based on Landsat TM+OLI/TIRS images, indicators representing the ecological quality of land and water were constructed. Land ecological quality consists of land surface moisture (WET), normalized difference vegetation index (NDVI), normalized building bare soil index (NDBSI), and land surface temperature (LST), which represent humidity, greenness, dryness, and temperature, respectively. At the same time, the remote sensing indices of chlorophyll_a (chl_a) and suspended solids (SS) were constructed to characterize the water quality. Air quality was characterized based on aerosol optical depth (AOD) in MCD19A2. By introducing water and air quality indicators and utilizing principal component analysis, a remote sensing ecological index that improves water area assessment (WIRSEI) was established and applied to Wuhan from 2000 to 2020. The driving force of WIRSEI change was analyzed using the geographically weighted regression (GWR) model. The results revealed that (1) air quality AOD and humidity WET greatly impacted the ecological quality (WIRSEI). WIRSEIs in seven central urban areas were significantly lower than that in six remote urban regions, and the ecological quality of lakes was higher than that of rivers. (2) From 2000 to 2020, Wuhan’s overall WIRSEI showed a “rising–falling–rising–stable” trend. In most regions, the degree of ecological quality change was relatively small; most grades were “no change”, “slightly better”, and “slightly worse”, representing 88–93% of the total area. (3) The change in WIRSEI from 2000 to 2020 was closely related to urban expansion, population change, and economic development. The effects of land use and socioeconomic changes on WIRSEI were significantly different in spatial distribution. Compared to the driving factors, land use dynamics (LUCD) significantly impacted WIRSEI changes, while the effects of gross domestic product (GDP) and population (POP) were very small. This study uses WIRSEI to evaluate the regional ecological quality, providing a vital reference and basis for enhancing regional ecological quality assessment methods, promoting ecological environmental protection and restoration, regional coordination, and sustainable development. The research results show that the proposed approach is simple and effective, strongly supporting regional ecological quality and protection monitoring
