    Individualized assessment predictive models for risk and overall survival in elderly patients of primary kidney cancer with bone metastases: A large population-based study

    BackgroundElderly people are at high risk of metastatic kidney cancer (KC), and, the bone is one of the most common metastatic sites for metastatic KC. However, studies on diagnostic and prognostic prediction models for bone metastases (BM) in elderly KC patients are still vacant. Therefore, it is necessary to establish new diagnostic and prognostic nomograms.MethodsWe downloaded the data of all KC patients aged more than 65 years during 2010–2015 from the Surveillance, Epidemiology, and End Results (SEER) database. Univariate and multivariate logistic regression analyses were used to study independent risk factors of BM in elderly KC patients. Univariate and multivariate Cox regression analysis for the study of independent prognostic factors in elderly KCBM patients. Survival differences were studied using Kaplan–Meier (K–M) survival analysis. The predictive efficacy and clinical utility of nomograms were assessed by receiver operating characteristic (ROC) curve, the area under curve (AUC), calibration curve, and decision curve analysis (DCA).ResultsA final total of 17,404 elderly KC patients (training set: n = 12,184, validation set: n = 5,220) were included to study the risk of BM. 394 elderly KCBM patients (training set: n = 278, validation set: n = 116) were included to study the overall survival (OS). Age, histological type, tumor size, grade, T/N stage and brain/liver/lung metastasis were identified as independent risk factors for developing BM in elderly KC patients. Surgery, lung/liver metastasis and T stage were identified as independent prognostic factors in elderly KCBM patients. The diagnostic nomogram had AUCs of 0.859 and 0.850 in the training and validation sets, respectively. The AUCs of the prognostic nomogram in predicting OS at 12, 24 and 36 months were: training set (0.742, 0.775, 0.787), and validation set (0.721, 0.827, 0.799), respectively. The calibration curve and DCA also showed excellent clinical utility of the two nomograms.ConclusionTwo new nomograms were constructed and validated to predict the risk of developing BM in elderly KC patients and 12-, 24-, and 36-months OS in elderly KCBM patients. These models can help surgeons provide more comprehensive and personalized clinical management programs for this population

    Heat transport capacity of an axial-rotating single- loop oscillating heat pipe for abrasive-milling tools

    In order to enhance heat transfer in the abrasive-milling processes to reduce thermal damage, the concept of employing oscillating heat pipes (OHPs) in an abrasive-milling tool is proposed. A single-loop OHP (SLOHP) is positioned on the plane parallel to the rotational axis of the tool. In this case, centrifugal accelerations do not segregate the fluid between the evaporator and condenser. The experimental investigation is conducted to study the effects of centrifugal acceleration (0–738 m/s2), heat flux (9100–31,850 W/m2) and working fluids (methanol, acetone and water) on the thermal performance. Results show that the centrifugal acceleration has a positive influence on the thermal performance of the axial-rotating SLOHP when filled with acetone or methanol. As for water, with the increase of centrifugal acceleration, the heat transfer performance first increases and then decreases. The thermal performance enhances for higher heat flux rises for all the fluids. The flow inside the axial-rotating SLOHP is analyzed by a slow-motion visualization supported by the theoretical analysis. Based on the theoretical analysis, the rotation will increase the resistance for the vapor to penetrate through the liquid slugs to form an annular flow, which is verified by the visualization

    Inherent Safer Design for Chemical Process of 1,4-dioldiacetate-2-butene Oxidized by Ozone

    PresentationOxidation reaction is the typical thermal runaway reaction. The reaction of 1,4-dioldiacetate-2- Butene oxidized by ozone was chosen to study the thermal hazards during the chemical process and the inherent safer designs (ISD) were proposed after analysis. The Qualitative Assessment for Inherently Safer Design (QAISD) was used to identify the risk during the chemical process. Meanwhile, the Reaction Calorimeter (RC1e) was used to analyze the thermal hazards of the chemical process. Two Inherent safer designs were proposed to increase the safe level of the process. ISD I is the improved reaction condition of reaction temperature at -5°C and ventilation rate of 200L•h-1, as well, ISDII is using a tubular reactor. The results indicate that the classification of the reaction hazard was lower with improvements of two ISDs, and the severity was reduced by 43%. Moreover, the inherent safety level of the reaction was increased by ISD I &IIof 63% and 43.4% respectively, which both have positive effects on inherent safety theories of "minimize", "substitute" and "moderate"

    Protective effect of midazolam against convulsion in neonatal rats via down-regulation of LC3 and Beclin-1 expression

    Purpose: To investigate the effect of midazolam on growth of neurocytes in vitro and in neonatal rats. Methods: Neurocyte proliferation and activity of lactate dehydrogenase were assessed by MTT and lactate dehydrogenase assays, respectively. Western blotting was used to determine the effect of midazolam on LC3, Bax, p62 and Beclin-1 protein expressions. Results: The suppression of neurocyte proliferation byconvulsion was alleviated significantly (p < 0.05) by midazolum treatment. Exposure of convulsion model of neurocytes to midazolum suppressed LC3, Bax, p62 and Beclin-1 protein expression. Midazolum exposure of convulsion model of neurocytes suppressed LDH, caspase-3, caspase-8 and caspase-9 activities. The 3-MA (autophagy inhibitor) treatment also significantly (p < 0.05) promoted neurocyte viability after convulsion induction. In convulsion-induced neurocytes, 3-MA exposure suppressed expression of caspase-3/8/9, LC3, Bax, Beclin-1 and p62, while application of midazolum treatment to the rats with convulsion markedly decreased brain water content and neurocyte apoptosis (p < 0.05). Treatment with midazolum inhibited LC3, p62 and Beclin-1 expression in the rat model of convulsion. Conclusion: Midazolum promotes neurocyte proliferation and inhibits edema development via downregulation of autophagy. Therefore, midazolum can potentially be used for the treatment of convulsion, but further studies need to be carried out first.     Active Contour Driven by Local Region Statistics and Maximum A Posteriori Probability for Medical Image Segmentation

    This paper presents a novel active contour model in a variational level set formulation for simultaneous segmentation and bias field estimation of medical images. An energy function is formulated based on improved Kullback-Leibler distance (KLD) with likelihood ratio. According to the additive model of images with intensity inhomogeneity, we characterize the statistics of image intensities belonging to each different object in local regions as Gaussian distributions with different means and variances. Then, we use the Gaussian distribution with bias field as a local region descriptor in level set formulation for segmentation and bias field correction of the images with inhomogeneous intensities. Therefore, image segmentation and bias field estimation are simultaneously achieved by minimizing the level set formulation.
