Rydygiera w Bydgoszczy, Uniwersytet Mikołaja Kopernika w Toruniu      StreszczenieNiniejsza praca omawia przypadki 8 chorych hospitalizowanych w Klinice Geriatrii CM UMK z rozpoznanym wg kryteriów Fried zespołem słabości. U badanych chorych zaobserwowano spadek masy ciała w ciągu ostatniego roku, cechy depresji wg skali CES-D,  spowolnienie chodu oraz, osłabienie siły mięśniowej oraz bóle kręgosłupa o nasileniu 9 VAS. W badaniach obrazowych kręgosłupa rozpoznano złamania kompresyjne kręgów, które poddano zabiegowi wertebroplastyki uzyskując poprawę w zakresie bólu, następnie u chorych wdrożono rehabilitację ambulatoryjną oraz dietę wysokobiałkową, po około 3 miesiącach u każdego chorego uzyskano istotną poprawę stanu klinicznego. Chorzy przestali spełniać kryteria Fried, co dowodzi tego że kompleksowe i często wielodyscyplinarne postępowanie korzystnie wpływa na leczenie starszych chorych z zespołem słabości. Słowa kluczowe: zespół słabości, złamanie kompresyjne kręgu, dieta, rehabilitacja, wertebroplastyka   AbstractAuthors present the cases of 8 patients treated in the Department of Geriatrics of CM UMK with frailty syndrome diagnosed. Patients showed a weight loss during the last year, signs of depression in CES-D scale, gait downturn, muscle weakness and severe back pain - 9 VAS. By imaging study the spinal fractures were diagnosed and patients underwent vertebroplasty obtaining good recovery in pain. Then patients have been implemented rehabilitation and high-protein diet. After about 3 months for each patient a significant clinical improvement was achieved. Patients no longer meet the Fried’s criteria for frailty. This proves that this complex and often multi-disciplinary proceeding is beneficial for the treatment of elderly patients with frailty syndrome. Key words: frailty, spine copression fracture, diet, rehabilitaion, vertebroplast

    Sleep quality disorders among the people of old and presenile age

    According to the newest research over the half of geriatric population suffers from sleep disorders. Lowered quality of sleep has got a negative influence on biopsychosocial functioning of an individual. The aim of the study was to define the magnitude of the problem of sleep quality disorders occurrence in the elderly. The research for the study was carried out by means of anonymous survey which consists of two questionnaires: The Pittsburgh Sleep Quality Index (PSQI) and a survey constructed for the sake of the study. 100 people took part in the study and the survey was aimed at people over the age of 50. The research showed that over one fifth of the subjects assess their sleep as rather bad and significant correlations between lengthening the time needed to fall asleep and waking up at night or before dawn and the lack of energy

    Can physical activity influence the quality of sleep among the elderly?

    Introduction: Research shows that over the half of people in the age of more than fifty have got problems with lowered quality of sleep. The cause of lowered quality of sleep is i.a. waking up early, lengthened sleep latency, increased number of waking during the night or shortened time of sleep. The aim of the study was to show how significant are sleep related problems which appear after the age of fifty and how regular physical activity can improve the quality of sleep and as a result functioning of the elderly. Physical activity delays involutional changes and the process of ageing, which lets the elderly stay independent and self-reliant in everyday activities. The aim of the study: The study was to mark the connection between physical activity intensity and the quality of sleep of the elderly. 290 Material and methods: Research for the study was made by means of anonymous survey which consisted of two questionnaires: The Pittsburgh Sleep Quality Index (PSQI) and The International Physical Activity Questionnaire. The survey was targeted at people in the age of over fifty. 100 people took part in the research. Results: Significant statistical correlation between early falling asleep, waking up at night or before dawn with the intensive or moderate physical effort has been obtained. Conclusions: The research has shown that people who performed intense or moderate physical activity woke up less frequently during the night, fell asleep faster and reported better quality of sleep. In addition, it has been noticed that the lowered quality of sleep has got a negative influence on i.a. energy levels and staying alert

    Cognitive disorders and fitness of the elderly in residential homes in Bydgoszcz

    Introduction. Process of ageing and the growing number of people in retirement age is one of the main problems of the modern world. Together with advancing age both fitness and mental capacity deteriorate. Research proves that there is dependence between cognitive functions and fitness of the elderly. Aim. The aim of the study was to compare fitness with cognitive functions among residents of residential home in Bydgoszcz. Material and methods. 59 people (35 women and 24 men) at the age of 60 to 98 participated in the study. Mini-Mental State Examination (MMSE) was used to assess cognitive functions. Fitness was assessed by means of Tinetti Test. The subjects were divided according to a) age: group 60-75 and 75+ and b) gender. Results. Negative correlation was observed between age and Tinetti Test results. In the study there was positive and moderate correlation between MMSE scale and Tinetti Test results. In Tinetti Test results regarding differences due to gender were close to statistical significance. Conclusions. There is a dependence between age and weakening of fitness in the elderly in residential homes in Bydgoszcz. Risk of falls increases with age. There is a dependence between cognitive functions disorders and fitness of the elderly from residential homes in Bydgoszcz

    Application of Nutritional Risk Score-2002 Questionnaire and Other Nutritional Status Parameters Among Hospitalized Elderly

    Background: Proper nutritional status is an important factor of successful aging and wellbeing of elderly patients. Screening and then treatment of malnutrition should be one of the main focuses of public health. The main aim of this study is to evaluate the usefulness of Nutritional Risk Score-2002 (NRS-2002), and the anthropometric and biochemical parameters in the context of nutritional status. Methods: Total of 1100 patient records from September 2012 to December 2014 were analyzed in terms of nutritional status assessment. The following were included for the analysis: NRS questionnaire, Body Mass Index (BMI), arm (AC) and calf circumference (CC), and concentration of albumin. Results: Mean age was 80.5 ± 7.3 years. The most frequent NRS-2002 score was indicating “risk of malnutrition” and its value increased with age. Mean albumin concentration was 3.49 ± 0.53 g/dl. Significant correlations between albumin concentration and arm and calf circumferences were found (p < 0.0001). Conclusions: Percentage of patients at nutritional risk determined with NRS was 18.8%. This result was significantly lower than in other studies. With accordance to results of other researchers, values of the NRS increased with age. Results regarding the relationship between the NRS values and albumin concentration, AC and CC (negative correlations) and albumin concentration with CC and AC (positive correlations) were consistent with the results of other researchers. In the assessment of the nutritional status of hospitalized elderly, the NRS-2002 should be combined with basic anthropometric measurements

    Untargeted Metabolomic Assay of Prefrail Older Adults after Nutritional Intervention

    Frailty is a geriatric syndrome causing a reduction in the body&rsquo;s functional reserves. Proper nutrition may be helpful in delaying transitioning older adults from pre-frail to frailty syndrome. The present study evaluates the nutritional status of pre-frail patients who underwent nutritional intervention and metabolomic changes resulting from this intervention. Sixteen pre-frail patients (68.4 &plusmn; 5.5 years old; 81.3% women) were enrolled for nutritional intervention, and twenty-nine robust elderly people (69.3 &plusmn; 5.3 years old; 82.8% women) were the control group. Pre-frail patients consumed 1.0 g protein/kg BW/day for eight weeks through diet modification and an additional daily intake of a protein powder formula. Taken measurements included: Nutritional anthropometry, assessment of food intake, and blood serum analysis with an untargeted metabolomic assessment. Protein consumption increased by 25.8%; moreover, significant increases in body weight (+1.2 kg; p = 0.023) and muscle mass index (+0.1 kg/m2; p = 0.042) were also observed. The untargeted metabolomic assay showed a significant increase in arachidonic acid (p = 0.038), and valine (p = 0.008) among pre-frail patients. Increased protein consumption is reflected in improved anthropometric and biochemical parameters of pre-frail patients. Moreover, metabolomic assay can be a useful tool in determining compliance with dietary recommendations
