    How Collections End: Objects, Meaning and Loss in Laboratories and Museums

    Collections are made and maintained for pleasure, status, nation or empire building, cultural capital, as a substrate for knowledge production, and everything in between. In asking how collections end, we shift the focus from acquisition and growth to erosion, loss and decay, and expose the intellectual, material and curatorial labour required to maintain collections. In this introductory essay, we draw together insights from the history of science and from science and technology studies to investigate the dispersal, destruction, absorption, repurposing and repatriation of the diverse scientific collections discussed in the papers that make up this issue of BJHS Themes, and many other collections besides. We develop a distinction first suggested by the bibliographer John Willis Clark between ‘working’ collections of objects valued for the information they hold or produce, and ‘unique’ collections of objects valued for their historical singularity. We show that in many cases, the ‘end’ of an object or collection is merely a shift in the dominant account of its cultural value from ‘working’ to ‘unique’ or vice versa. Moving between the laboratory, the museum, and difficult-to-classify spaces in between, we argue that ‘ending’ is not anathema to ‘collecting’ but is always present as a threat, or as an everyday reality, or even as a necessary part of a collection’s continued existence. A focus on ending draws attention not only to the complex internal dynamics and social contexts of collections, but also to their roles in producing scientific knowledge

    Why is seasonal density stratification in shelf seas expected to increase under future climate change?

    Coastal and shelf seas provide a diverse range of ecosystem services, which are often mediated by seasonal density stratification through its control on biogeochemical cycles. These seas are highly vulnerable to climate change and downscaling studies consistently project an increase in seasonal stratification over the next century, but without a clear explanation. Here we revisit a well-established theory of coastal ocean mixing and demonstrate with a new ensemble of downscaled simulations for the Northwest European continental shelf seas to 2100 that the increase of expansivity with temperature is sufficient to consistently increase the seasonal stratification. Where there is a closed balance between buoyancy input and mixing, small changes in expansivity are amplified to a large relative change in stratification. This simple link between global heating and stratification substantially reduces uncertainty in projections of this key parameter in seas around the world

    Targeted thorium-227 conjugates as treatment options in oncology

    Targeted alpha therapy (TAT) is a promising approach for addressing unmet needs in oncology. Inherent properties make α-emitting radionuclides well suited to cancer therapy, including high linear energy transfer (LET), penetration range of 2–10 cell layers, induction of complex double-stranded DNA breaks, and immune-stimulatory effects. Several alpha radionuclides, including radium-223 (223Ra), actinium-225 (225Ac), and thorium-227 (227Th), have been investigated. Conjugation of tumor targeting modalities, such as antibodies and small molecules, with a chelator moiety and subsequent radiolabeling with α-emitters enables specific delivery of cytotoxic payloads to different tumor types. 223Ra dichloride, approved for the treatment of patients with metastatic castration-resistant prostate cancer (mCRPC) with bone-metastatic disease and no visceral metastasis, is the only approved and commercialized alpha therapy. However, 223Ra dichloride cannot currently be complexed to targeting moieties. In contrast to 223Ra, 227Th may be readily chelated, which allows radiolabeling of tumor targeting moieties to produce targeted thorium conjugates (TTCs), facilitating delivery to a broad range of tumors. TTCs have shown promise in pre-clinical studies across a range of tumor-cell expressing antigens. A clinical study in hematological malignancy targeting CD22 has demonstrated early signs of activity. Furthermore, pre-clinical studies show additive or synergistic effects when TTCs are combined with established anti-cancer therapies, for example androgen receptor inhibitors (ARI), DNA damage response inhibitors such as poly (ADP)-ribose polymerase inhibitors or ataxia telangiectasia and Rad3-related kinase inhibitors, as well as immune checkpoint inhibitors

    Cell salvage and donor blood transfusion during cesarean section: A pragmatic, multicentre randomised controlled trial (SALVO)

    BACKGROUND: Excessive haemorrhage at cesarean section requires donor (allogeneic) blood transfusion. Cell salvage may reduce this requirement. METHODS AND FINDINGS: We conducted a pragmatic randomised controlled trial (at 26 obstetric units; participants recruited from 4 June 2013 to 17 April 2016) of routine cell salvage use (intervention) versus current standard of care without routine salvage use (control) in cesarean section among women at risk of haemorrhage. Randomisation was stratified, using random permuted blocks of variable sizes. In an intention-to-treat analysis, we used multivariable models, adjusting for stratification variables and prognostic factors identified a priori, to compare rates of donor blood transfusion (primary outcome) and fetomaternal haemorrhage ≥2 ml in RhD-negative women with RhD-positive babies (a secondary outcome) between groups. Among 3,028 women randomised (2,990 analysed), 95.6% of 1,498 assigned to intervention had cell salvage deployed (50.8% had salvaged blood returned; mean 259.9 ml) versus 3.9% of 1,492 assigned to control. Donor blood transfusion rate was 3.5% in the control group versus 2.5% in the intervention group (adjusted odds ratio [OR] 0.65, 95% confidence interval [CI] 0.42 to 1.01, p = 0.056; adjusted risk difference -1.03, 95% CI -2.13 to 0.06). In a planned subgroup analysis, the transfusion rate was 4.6% in women assigned to control versus 3.0% in the intervention group among emergency cesareans (adjusted OR 0.58, 95% CI 0.34 to 0.99), whereas it was 2.2% versus 1.8% among elective cesareans (adjusted OR 0.83, 95% CI 0.38 to 1.83) (interaction p = 0.46). No case of amniotic fluid embolism was observed. The rate of fetomaternal haemorrhage was higher with the intervention (10.5% in the control group versus 25.6% in the intervention group, adjusted OR 5.63, 95% CI 1.43 to 22.14, p = 0.013). We are unable to comment on long-term antibody sensitisation effects. CONCLUSIONS: The overall reduction observed in donor blood transfusion associated with the routine use of cell salvage during cesarean section was not statistically significant. While treatment options, including drug, device, and procedural therapies, are available, their use in the setting of CKD is complex and limited.     Reproducible and relocatable regional ocean modelling: Fundamentals and practices

    In response to an increasing demand for bespoke or tailored regional ocean modelling configurations, we outline fundamental principles and practices that can expedite the process to generate new configurations. The paper develops the principle of Reproducibility and advocates adherence by presenting benefits to the community and user. The elements to this principle are reproducible workflows and standardised assessment, with additional effort over existing working practices being balanced against the added value generated. The paper then decomposes the complex build process, for a new regional ocean configuration, into stages and presents guidance, advice and insight on each component. This advice is compiled from across the user community, is presented in the context of NEMOv4, though aims to transcend NEMO version. Detail and region specific worked examples are linked in companion repositories and DOIs. The aim is to broaden the user community skill base, and to accelerate development of new configurations in order to increase available time exploiting the configurations

    Effectiveness of a national quality improvement programme to improve survival after emergency abdominal surgery (EPOCH): a stepped-wedge cluster-randomised trial

    Background: Emergency abdominal surgery is associated with poor patient outcomes. We studied the effectiveness of a national quality improvement (QI) programme to implement a care pathway to improve survival for these patients. Methods: We did a stepped-wedge cluster-randomised trial of patients aged 40 years or older undergoing emergency open major abdominal surgery. Eligible UK National Health Service (NHS) hospitals (those that had an emergency general surgical service, a substantial volume of emergency abdominal surgery cases, and contributed data to the National Emergency Laparotomy Audit) were organised into 15 geographical clusters and commenced the QI programme in a random order, based on a computer-generated random sequence, over an 85-week period with one geographical cluster commencing the intervention every 5 weeks from the second to the 16th time period. Patients were masked to the study group, but it was not possible to mask hospital staff or investigators. The primary outcome measure was mortality within 90 days of surgery. Analyses were done on an intention-to-treat basis. This study is registered with the ISRCTN registry, number ISRCTN80682973. Findings: Treatment took place between March 3, 2014, and Oct 19, 2015. 22 754 patients were assessed for elegibility. Of 15 873 eligible patients from 93 NHS hospitals, primary outcome data were analysed for 8482 patients in the usual care group and 7374 in the QI group. Eight patients in the usual care group and nine patients in the QI group were not included in the analysis because of missing primary outcome data. The primary outcome of 90-day mortality occurred in 1210 (16%) patients in the QI group compared with 1393 (16%) patients in the usual care group (HR 1·11, 0·96–1·28). Interpretation: No survival benefit was observed from this QI programme to implement a care pathway for patients undergoing emergency abdominal surgery. Future QI programmes should ensure that teams have both the time and resources needed to improve patient care.
