    Secondary pulmonary alveolar proteinosis treated by lung transplant: A case report.

    Pulmonary alveolar proteinosis (PAP) is a pulmonary disease characterized by disruption of surfactant homeostasis resulting in its accumulation in the alveoli. PAP is classically classified into three categories (Table 1): 1/primary (or autoimmune) with antibodies targeting the GM-CSF pathway, 2/secondary to another disease, typically a hematologic malignancy, and 3/genetic. A 30 year-old woman received an allogenic hematopoietic stem cell transplantation (HSCT) after treatment for acute myeloid leukemia (AML). Within the first 6 months post HSCT, she developed an ocular, oral, digestive and hepatic graft-versus-host disease associated with a mixed ventilatory defect with a very severe obstructive syndrome and a severe CO diffusion impairment. High resolution computed tomography showed a classical "crazy paving" pattern. Aspect and differential cell count of BAL were normal. All microbiological samples remained culture negative. Histo-pathological analysis of transbronchial biopsies was unremarkable. Because of the severity of the respiratory insufficiency, open-lung biopsy (OBL) could not be performed. Despite multiple immunosuppressive therapies, lung function deteriorated rapidly; the patient also developed an excavated fungal lesion unresponsive to treatment. She underwent a bilateral lung transplant 48 months after HSCT. Histo-pathological analysis of explanted lungs showed obliterative bronchiolitis (OB), diffuse PAP and invasive cavitary pulmonary aspergillosis. This case illustrates the simultaneous occurrence of OB, PAP and a fungal infection in a 30-year old female patient who underwent HSCT for acute myeloid leukemia (AML). To our knowledge this is the only documented case of PAP associated with OB treated by lung transplantation

    The effects of a stigma awareness intervention on finding and retaining paid employment a cluster randomized controlled trial among unemployed people with mental illness

    Introduction: Stigma is one of the barriers to paid employment for people with mental illness. Deliberate (non-)disclosure decisions may prevent this, but the effects of stigma awareness interventions are mostly unknown. This study aims to examine the effectiveness of a stigma awareness intervention for employment specialists and a decision aid and two infographics about disclosure of mental illness on finding and retaining employment for unemployed people with mental illness, compared to usual guidance. Material and methods: A clustered RCT was conducted. Participants were unemployed people with mental illness who receive social benefits (N=153) and were recruited at eight locations. The control group received guidance as usual and the experimental group received guidance as usual combined with the stigma awareness intervention. Health, wellbeing, job seeking activities and disclosure were measured at baseline and 3, 6 and 12 months. Multilevel analyses were conducted to analyze the effects of the intervention on finding and retaining employment, controlled for other factors. Results: In the experimental group, after six (T2) and twelve months (T3) almost twice as many participants had found paid employment (T2: CG=26.1% vs EG=50.7%, p=0.003; T3: CG=34.4% vs EG=53.8%, p=0.026), and retained paid employment after twelve months (CG=23.4% vs EG=49.2%, p=0.002), compared to the control group. Conclusions: A stigma awareness intervention contributes to more often finding and retaining paid employment for people with mental illness

    Automatic Inference of Determinacy and Mutual Exclusion for Logic Programs Using Mode and Type Analyses

    We propose an analysis for detecting procedures and goals that are deterministic (i.e., that produce at most one solution at most once),or predicates whose clause tests are mutually exclusive (which implies that at most one of their clauses will succeed) even if they are not deterministic. The analysis takes advantage of the pruning operator in order to improve the detection of mutual exclusion and determinacy. It also supports arithmetic equations and disequations, as well as equations and disequations on terms,for which we give a complete satisfiability testing algorithm, w.r.t. available type information. Information about determinacy can be used for program debugging and optimization, resource consumption and granularity control, abstraction carrying code, etc. We have implemented the analysis and integrated it in the CiaoPP system, which also infers automatically the mode and type information that our analysis takes as input. Experiments performed on this implementation show that the analysis is fairly accurate and efficient

    Inference of Well-Typings for Logic Programs with Application to Termination Analysis

    This paper develops a method to infer a polymorphic well-typing for a logic program. One of the main motivations is to contribute to a better automation of termination analysis in logic programs, by deriving types from which norms can automatically be constructed. Previous work on type-based termination analysis used either types declared by the user, or automatically generated monomorphic types describing the success set of predicates. Declared types are typically more precise and result in stronger termination conditions than those obtained with inferred types. Our type inference procedure involves solving set constraints generated from the program and derives a well-typing in contrast to a success-set approximation. Experiments show that our automatically inferred well-typings are close to the declared types and thus result in termination conditions that are as good as those obtained with declared types for all our experiments to date. We describe the method, its implementation and experiments with termination analysis based on the inferred types

    Body mass index is associated with microvascular endothelial dysfunction in patients with treated metabolic risk factors and suspected coronary artery disease

    Background--Obesity is key feature of the metabolic syndrome and is associated with high cardiovascular morbidity and mortality. Obesity is associated with macrovascular endothelial dysfunction, a determinant of outcome in patients with coronary artery disease. Here, we compared the influence of obesity on microvascular endothelial function to that of established cardiovascular risk factors such as diabetes mellitus, hypertension, hypercholesterolemia, and smoking in patients with suspected coronary artery disease. Methods and Results--Endothelial function was assessed during postocclusive reactive hyperemia of the brachial artery and downstream microvascular beds in 108 patients who were scheduled for coronary angiography. In all patients, microvascular vasodilation was assessed using peripheral arterial tonometry; laser Doppler flowmetry and digital thermal monitoring were performed. Body mass index was significantly associated with decreased endothelium-dependent vasodilatation measured with peripheral arterial tonometry (r=0.23, P=0.02), laser Doppler flowmetry (r=0.30, P < 0.01), and digital thermal monitoring (r=0.30, P < 0.01). In contrast, hypertension, hypercholesterolemia, and smoking had no influence on microvascular vasodilatation. Especially in diabetic patients, endothelial function was not significantly reduced (control versus diabetes mellitus, mean±SEM or median [interquartile range], peripheral arterial tonometry: 1.90±0.20 versus 1.67±0.20, P=0.19, laser Doppler flowmetry: 728% [interquartile range, 427-1110] v

    Magnetic field dependence of the exciton energy in a quantum disk

    The groundstate energy and binding energy of an exciton, confined in a^M quantum disk, are calculated as a function of an external magnetic field. The confinement potential is a hard wall of finite height. The diamagnetic shift is investigated for magnetic fields up to 40TT. Our results are applied to InyAl1yAs/AlxGa1xAsIn_{y}Al_{1-y}As/Al_{x}Ga_{1-x}As self-assembled quantum dots and very good agreement with experiments is obtained. Furthermore, we investigated the influence of the dot size on the diamagnetic shift by changing the disk radius. The exciton excited states are found as a function of the magnetic field. The relative angular momentum is not a quantum number and changes with the magnetic field strength.Comment: 10 pages, 17 figure

    Entangled Photons from Small Quantum Dots

    We discuss level schemes of small quantum-dot turnstiles and their applicability in the production of entanglement in two-photon emission. Due to the large energy splitting of the single-electron levels, only one single electron level and one single hole level can be made resonant with the levels in the conduction band and valence band. This results in a model with nine distinct levels, which are split by the Coulomb interactions. We show that the optical selection rules are different for flat and tall cylindrically symmetric dots, and how this affects the quality of the entanglement generated in the decay of the biexciton state.     Linear Responses in Time-dependent Hartree-Fock-Bogoliubov Method with Gogny Interaction

    A numerical method to integrate the time-dependent Hartree-Fock Bogoliubov (TDHFB) equations with Gogny interaction is proposed. The feasibility of the TDHFB code is illustrated by the conservation of the energy, particle numbers, and center-of-mass in the small amplitude vibrations of oxygen 20. The TDHFB code is applied to the isoscalar quadrupole and/or isovector dipole vibrations in the linear (small amplitude) region in oxygen isotopes (masses A = 18,20,22 and 24), titanium isotopes (A = 44,50,52 and 54), neon isotope (A = 26), and magnesium isotopes (A = 24 and 34). The isoscalar quadrupole and isovector dipole strength functions are calculated from the expectation values of the isoscalar quadrupole and isovector dipole moments.Comment: 10 pages, 13 figure

    Spherical and deformed structures in Pb189

    γ-ray spectroscopy of high-spin states of the neutron-deficient nucleus Pb189 has been conducted with the Gd158(Ar36,5n) and Er164(Si29,4n) reactions. With the first of these, detection of evaporation residues and mass gating were used to unambiguously assign a number of prompt γ-ray transitions to Pb189. With the second reaction and a pulsed beam, an isomer with a mean life of 32 μs was found. Although inconclusive, the available evidence favors identification of the isomer with the 332+ state of the ν(i13/2)-3 configuration. The levels observed below the isomer can be identified with states involving three different structures: the neutron (i13/2)-3 multiplet in the spherical well; a prolate-deformed band involving mixed i13/2 neutron orbitals; and a state with the oblate π(2p-2h)0+ν(i13/ 2)-1 configuration. The evidence for structures associated with different shapes is supported by the observation of E0 components in some of the Jπ→Jπ transitions linking them
