    Inclusion for a Student with Vision Impairment: “They accept me, like, as in I am there, but they just won’t talk to me.”

    We explore the experiences of Nick, a secondary school student with vision impairment in an Australian mainstream school in this study, and we particularly focus on whether he perceived his education as inclusive. We have used Interpretative Phenomenological Analysis in this single individual case as this approach explores our participant’s understandings which may be revealed by close examination of mindful experiences. The “gem” spoken by Nick (pseudonym), our 16-year old participant, was “They accept me, like as in I am there, but they just won’t talk to me.” This statement summarises his sense of not belonging, of being other, and of being bullied. The inability of his school to provide an inclusive education was apparent to him, and his often-unfulfilled need for access to specialist teachers of vision impairment is explored. Our study includes vivid quotes from Nick to bring his voice to our phenomenological interpretation

    Students with vision impairment who attend mainstream secondary schools in Australia may not experience education as an inclusive and positive experience. This study of one senior secondary student with vision impairment provides a rare opportunity to give voice and provide understandings of the experience from the perspective of the student. The research question that drove this study was: What is the experience of mainstream schooling for a student with a vision impairment? The participant in this Interpretative Phenomenological Analysis study was Edward (pseudonym), a student in his final year of secondary schooling. Edward encountered significant barriers to inclusion, specifically teaching, technology, administrative inflexibility, and restricted social engagement. The participant has become resilient with a strong sense of self and has developed a range of personal strategies to address his challenges. It is evident that Edward was rarely asked about his needs and perceptions, rather decisions were made for and about him by those without a vision impairment. Educators require a clearer understanding of vision impairment and the impact that their often unintentionally exclusionary teaching practices may have on the educational experiences of their students

    Scoping the implementation of recommendations from previous early generation seed studies in sub-Saharan Africa

    The purpose of this report is to document and track progress on the implementation of recommendations from EGS studies that were carried out on selected countries in Eastern and Southern Africa (ESA) and West and Central Africa (WCA). The recommendations were designed to ensure a sustainable supply of EGS based on market-specific archetypes involving private, public and public-private partnerships. For the subsequent years following the conclusion and adoption of the study reports, there have been some significant achievements registered based on the country-specific recommendations from the studies. This report provides a review of the extent of implementation of the recommendations in each of the mentioned countries. The study involved a review of literature which included the country-specific EGS studies and the available literature that comprised of projects reports, recent policy documents and any other relevant available literature. Despite the list of countries in which the studies were conducted, this study only focused on the implementation of recommendations in Uganda, Kenya, Tanzania, Rwanda, Malawi, Ghana and Nigeria. Most countries have to a large extent implemented most of the recommendations from the EGS studies. A lot of emphasis was placed on addressing the creation of enabling environment aspects such as the enactment of seed acts, the plant variety protection bill and policies in Nigeria, Uganda, Ghana as well as harmonization between the national and regional seed laws. There was also emphasis towards capacity building for seed regulation and certification services through training of human resources and enabling private sector participation in seed inspection in addition to accreditation by the international bodies ISTA and OECD. PPPs were established for the root and tuber crops cassava and potatoes; and mainly hybrid and OPV maize for the cereal crops. Fingerprinting of parental lines is the most notably unimplemented for all the countries that it was recommended, but also the national establishements such as Variety Testing Center, demand forecasting frameworks and National Seed Steering committee for the case of Malawi and Tanzania respectively. It is therefore recommended that further investigation of gaps in implementation be carried to understand reasons for non-implementation

    Business models for early generation seed production and marketing

    Sustainable access to early generation seed (EGS) is important for efficient supply of quality certified, standard and/or quality declared seed of improved high-performing crop varieties. Some studies have been conducted from a global perspective on the delivery of EGS with a major focus on the economic analysis and identification of effective pathways for sustainable supply. This study was designed to establish the best practices and critical factors for the delivery of EGS for grain legume and dryland cereal crops in selected countries in sub-Saharan Africa (SSA) and parts of South Asia. A literature review of specific business models applied in SSA and India was conducted. More information was gathered through in-depth interviews with specific key informants drawn from the respective institutions or agencies that were studied. The models include public-sector or research-led models, private -sector models, and public-private partnership models. This working paper presents a synthesis of best practices and approaches through which the specific business models ensure the production and supply of breeder and foundation seeds. The paper identifies practices involving mechanisms for planning, financing, infrastructural facilities required for EGS production, incentives to promote pure line crop EGS production by the private sector, and the methods of production. The study reveals critical constraints for EGS production and supply, including seed market vagaries; the short shelf life of some crop seeds like groundnut; limited maintenance breeding efforts; low quality of breeder seed; lack of infrastructure to produce EGS; breeder seed production does not match the demand for production of foundation seed while foundation seed production does not match the demand for production of certified seed; and high costs of supervising and managing various groups of seed outgrowers. Interventions that build the capacity of seed producers to effectively plan EGS production requirements, guided by a seed roadmap, and product cycle management are needed. The public research institutions need to be strengthened with facilities that enhance their capacity to produce sufficient quantities of breeder seed at the required time. Besides, the proceeds from the sales should “revolve” to enable the unit to make further investments and carry out all the necessary operations in a timely manner. This requires consultation with Governments to set up financial management structures that provide an easier accountability process. Incentive schemes should be developed to encourage investment in the production of pure-line grain legumes and dryland cereals by the private sector as well as enhance their role in popularizing new varieties. Improvement and enforcement of quality assurance and control systems by breeding programmes, including modern technological methods should be considered. Importantly, a handover system that includes lisencing and pre-orders would help to bridge the supply and demand disparity

    Designing self-sustaining early generation seed supply systems: The must-dos

    Shortages in the supply of quality early generation seed (EGS) of new and improved varieties, particularly of dryland cereals and legumes is a major challenge because of the business models.This triggers the following questions: what EGS business models would sustainably avail quality parent seed? To investigate this, we documented 16 EGS interventions. We found that pre-securing seed market ahead of production played important role in the successful seed business. Taking different forms (e.g., pre-orders, demand forecasting, pre-aggregation of demand, joint planning,contractual agreement), it brings confidence to EGS producers to continuously invest in the business and make it profitable. For sustainable EGS production and supply in sub-Saharan Africa, we advocate for market assurance in advance. Two guiding principles are also suggested and four recommendations formulated

    Evaluating telehealth lifestyle therapy versus telehealth psychotherapy for reducing depression in adults with COVID-19 related distress: the curbing anxiety and depression using lifestyle medicine (CALM) randomised non-inferiority trial protocol

    BACKGROUND: There is increasing recognition of the substantial burden of mental health disorders at an individual and population level, including consequent demand on mental health services. Lifestyle-based mental healthcare offers an additional approach to existing services with potential to help alleviate system burden. Despite the latest Royal Australian New Zealand College of Psychiatrists guidelines recommending that lifestyle is a ‘first-line’, ‘non-negotiable’ treatment for mood disorders, few such programs exist within clinical practice. Additionally, there are limited data to determine whether lifestyle approaches are equivalent to established treatments. Using an individually randomised group treatment design, we aim to address this gap by evaluating an integrated lifestyle program (CALM) compared to an established therapy (psychotherapy), both delivered via telehealth. It is hypothesised that the CALM program will not be inferior to psychotherapy with respect to depressive symptoms at 8 weeks. METHODS: The study is being conducted in partnership with Barwon Health’s Mental Health, Drugs & Alcohol Service (Geelong, Victoria), from which 184 participants from its service and surrounding regions are being recruited. Eligible participants with elevated psychological distress are being randomised to CALM or psychotherapy. Each takes a trans-diagnostic approach, and comprises four weekly (weeks 1-4) and two fortnightly (weeks 6 and 8) 90-min, group-based sessions delivered via Zoom (digital video conferencing platform). CALM focuses on enhancing knowledge, behavioural skills and support for improving dietary and physical activity behaviours, delivered by an Accredited Exercise Physiologist and Accredited Practising Dietitian. Psychotherapy uses cognitive behavioural therapy (CBT) delivered by a Psychologist or Clinical Psychologist, and Provisional Psychologist. Data collection occurs at baseline and 8 weeks. The primary outcome is depressive symptoms (assessed via the Patient Health Questionnaire-9) at 8 weeks. Societal and healthcare costs will be estimated to determine the cost-effectiveness of the CALM program. A process evaluation will determine its reach, adoption, implementation and maintenance. DISCUSSION: If the CALM program is non-inferior to psychotherapy, this study will provide the first evidence to support lifestyle-based mental healthcare as an additional care model to support individuals experiencing psychological distress.     Argumentum ad misericordiam - the critical intimacies of victimhood

    This article discusses the widespread use of victim tropes in contemporary Anglo-American culture by using cultural theory to analyse key social media memes circulating on Facebook in 2015. Since the growth of social media, victim stories have been proliferating, and each demands a response. Victim narratives are rhetorical, they are designed to elicit pity and shame the perpetrator. They are deployed to stimulate political debate and activism, as well as to appeal to an all-purpose humanitarianism. Victimology has its origins in Law and Criminology, but this paper opens up the field more broadly to think about the cultural politics of victimhood, to consider how the victim-figure can be appropriated by/for different purposes, particularly racial and gender politics, including in the case of Rachel Dolezal, and racial passing. In formulating an ethical response to the lived experience of victims, we need to think about the different kinds of critical intimacies elicited by such media

    Opportunities for primary and secondary prevention of excess gestational weight gain: General Practitioners' perspectives

    BackgroundThe impact of excess gestational weight gain (GWG) on maternal and child health outcomes is well documented. Understanding how health care providers view and manage GWG may assist with influencing healthy gestational weight outcomes. This study aimed to assess General Practitioner\u27s (GPs) perspectives regarding the management and assessment of GWG and to understand how GPs can be best supported to provide healthy GWG advice to pregnant women.MethodsDescriptive qualitative research methods utilising semi - structured interview questions to assess GPs perspectives and management of GWG. GPs participating in shared antenatal care in Geelong, Victoria and Sydney, New South Wales were invited to participate in semi - structured, individual interviews via telephone or in person. Interviews were digitally recorded and transcribed verbatim. Data was analysed utilising thematic analysis for common emerging themes.ResultsTwenty eight GPs participated, 14 from each state. Common themes emerged relating to awareness of the implications of excess GWG, advice regarding weight gain, regularity of gestational weighing by GPs, options for GPs to seek support to provide healthy lifestyle behaviour advice and barriers to engaging pregnant women about their weight. GPs perspectives concerning excess GWG were varied. They frequently acknowledged maternal and child health complications resulting from excess GWG yet weighing practices and GWG advice appeared to be inconsistent. The preferred support option to promote healthy weight was referral to allied health practitioners yet GPs noted that cost and limited access were barriers to achieving this.ConclusionsGPs were aware of the importance of healthy GWG yet routine weighing was not standard practice for diverse reasons. Management of GWG and perspectives of the issue varied widely. Time efficient and cost effective interventions may assist GPs in ensuring women are supported in achieving healthy GWG to provide optimal maternal and infant health outcomes.<br /

    Epidemiology and awareness of hypertension in a rural Ugandan community: a cross-sectional study

    BACKGROUND: Hypertension is one of the largest causes of preventable morbidity and mortality worldwide. There are few population-based studies on hypertension epidemiology to guide public health strategies in sub-Saharan Africa. Using a community-based strategy that integrated screening for HIV and non-communicable diseases, we determined the prevalence, awareness, treatment rates, and sociodemographic factors associated with hypertension in rural Uganda. METHODS: A household census was performed to enumerate the population in Kakyerere parish in Mbarara district, Uganda. A multi-disease community-based screening campaign for hypertension, diabetes, and HIV was then conducted. During the campaign, all adults received a blood pressure (BP) measurement and completed a survey examining sociodemographic factors. Hypertension was defined as elevated BP (≥140/≥90 mmHg) on the lowest of three BP measurements or current use of antihypertensives. Prevalence was calculated and standardized to age distribution. Sociodemographic factors associated with hypertension were evaluated using a log-link Poisson regression model with robust standard errors. RESULTS: Community participation in the screening campaign was 65%, including 1245 women and 1007 men. The prevalence of hypertension was 14.6%; awareness of diagnosis (38.1%) and current receipt of treatment (20.6%) were both low. Age-standardized to the WHO world standard population, hypertension prevalence was 19.8%, which is comparable to 21.6% in the US and 18.4% in the UK. Sociodemographic factors associated with hypertension included increasing age, male gender, overweight, obesity, diabetes, alcohol consumption, and family history. Prevalence of modifiable factors was high: 28.3% women were overweight/obese and 24.1% men consumed ≥10 alcoholic drinks per month. CONCLUSIONS: We found a substantial burden of hypertension in rural Uganda. Awareness and treatment of hypertension is low in this region. Enhanced community-based education and prevention efforts tailored to addressing modifiable factors are needed

    The field of childhood studies in the US is comprised of cross-disciplinary researchers who theorize and conduct research on both children and youth. US sociologists who study childhood largely draw on the childhood literature published in English. This article focuses on American sociological contributions, but notes relevant contributions from non-American scholars published in English that have shaped and fueled American research.
