    In Silico Domain Structural Model Analysis of Coronavirus ORF1ab Polyprotein

    The world today is battling with a coronavirus infection that is considered a global pandemic. Coronavirus infection is mainly attribute to the varying technique of the replication and release of different genomic components of the virus. The present study aims to establish the physical and chemical features, as well as the basic structural and functional properties of Coronavirus ORF1ab domain. A molecular approach was adopt in this study using the Swiss Model and Phyre2 server whereas the prediction of the active ligand binding sites was done using Phyre2. The analysis of the structure of the protein showed that it has good structural and heat stability, as well as better hydrophilic features and acidic in nature. Based on the Homology modeling, only two binding active sites were noted with catalytic function being mediated by Zn2+ as the metallic heterogeneous ligand for binding sites prediction. The proteins mostly exhibited helical secondary configurations. This study can help in predicting and understanding the role of this domain protein in active coronavirus infection

    Status of oxidants and antioxidants in rheumatoid arthritis patients of north Karnataka

    Background: Oxidant stress as a result of increased production of reactive oxygen species (ROS) or a reduction in the body’s endogenous antioxidant defense system is a hallmark of chronic inflammatory diseases including rheumatoid arthritis (RA). The objective of the study was to establish the status of oxidative/nitrosative stress in RA patients of north Karnataka.Methods: Thirty RA patients and equal number of healthy individuals (controls) were included in the study. The morning stiffness (MS) and disease activity score 28 (DAS28) of RA patients were examined. Erythrocyte sedimentation rate (ESR) and hemoglobin (Hb) levels were estimated. Biochemical parameters uric acid, lipid peroxide (LPO), nitric oxide (NOx), vitamin C & E, erythrocyte superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) were measured.Results: Significant increased ESR (11.20 mm/hr vs. 40.97 mm/hr, 265.80%), uric acid (3.85 mg/dL vs. 6.73 mg/dL, 74.81%), LPO (1.12 μmol/L vs. 2.35 μmol/L, 109.82%) and NOx (16.19 μmol/L vs. 45.43 μmol/L, 180.61%) levels with significant decreased of the levels of Hb (12.59 g/dL vs. 10.16 g/dL, 19.30%), vitamin C & E (11.20.mg/L vs. 3.87.mg/dL, 65.45% and 13.01 mg/L vs. 7.05 mg/dL, 45.81%), SOD (3757.90 U/g Hb vs. 2201.20 U/g Hb, 41.42%) and GSH-Px (1.81 U/g Hb vs. 0.92 U/g Hb, 49.17%) were observed in RA. There was direct correlation found between MS, DAS-28 and ESR with serum LPO and NOx in RA patients. A significant negative correlation was detected between activity parameters with biochemical parameters in RA patients.Conclusions: Increased oxidants level and decreased non-enzymatic/enzymatic antioxidants level in RA patients of north Karnataka confirm an association between oxidative/nitrosative stress and rheumatoid arthritis.

    Cancer is a cellular change caused by uncontrolled cell growth and division. This disease develops from the growth of abnormal cells that have the potential to invade or spread to other parts of the body. The aim of this study was to investigate the relationship between blood parameters (e.g., Mean corpuscular volume (MCV), Mean corpuscular hemoglobin (MCH), Mean Corpuscular Hemoglobin Concentration (MCHC), Haemoglobin (HGB), White Blood Cell (WBC), and Red Blood Cell (RBC)) and different types of cancer. Breast cancer, thyroid gland cancer, ovarian cancer, testicular cancer, brain tumors as well as other types of cancer are based on the cell of the tissue found on or in the body. Any of these types of cancer are associated with a variety of health issues that put the patient's life in danger. Performing the complete blood count (CBC) test prior to or after a cancer diagnosis is necessary as abnormalities in the body can cause blood component rates to either increase or decrease, depending on the type of cancer, the patient's physiological mechanism, and the structural component. Since the CBC test belongs to hematology, drawing a blood sample and putting it into the anticoagulant tube for testing were preferred. In this study, the blood components of almost all patients were normal except for a few of them which may be due to other medical and biological factors. There is a significant relationship between blood parameters and cancer types.&nbsp

    Anemia is a disease that caused due to inflammation, autoimmune disease, or chronic disease as cancer, kidney failure, heart failure, diabetes, but the main reason of anemia is iron deficiency. Breathlessness, weakness, and exhaustion are all possible effects of anemia. Anemia comes in a variety of types. Everyone has a unique reason. Anemia may be chronic or transient. It could be minor or really serious. Anemia may indicate a more serious medical condition.  In this research we will explain the anemia due to chronic disease especially kidney failure. Anemia occurs when decreasing the number of red blood cells that carry oxygen to the body. According to world health organization (WHO), the person has anemia when hemoglobin (which is present in red blood cells, transports oxygen from the lungs to every other organ in the body), (Hb) levels <12.0 g/dl in women and <13.0 g/dl in men. We can treat anemia by iron supplement, medications, blood transfusion, vitB12, blood and bone transplant but it occur in hospital and by healthy diet. If anemia remained untreated it will be a risk of irregular heartbeat, heart failure, infection, and in children it may cause developmental delay. We can diagnosis anemia by blood tests which are used by medical practitioners to look for indications of inflammation-related anemia, other anemias, or other health issues. You will give blood to a medical professional who will then submit the sample to a lab for analysis. The National institutes of health (NIH) approved that we can examine a variety of components and characteristics of your blood, such as how many red blood cells ,the dimensions of red blood cells ,how much hemoglobin is present in your blood and red blood cells ,the quantity of reticulocytes, or growing red blood cells, in your blood. Blood tests are another tool that a medical expert may use to measure how much iron is stored in blood, transferrin, and ferritin. If the results of a blood test indicate that you have anemia low blood iron levels will appear, determining the amount of iron in low and normal range.     Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     Multiple novel prostate cancer susceptibility signals identified by fine-mapping of known risk loci among Europeans

    Genome-wide association studies (GWAS) have identified numerous common prostate cancer (PrCa) susceptibility loci. We have fine-mapped 64 GWAS regions known at the conclusion of the iCOGS study using large-scale genotyping and imputation in 25 723 PrCa cases and 26 274 controls of European ancestry. We detected evidence for multiple independent signals at 16 regions, 12 of which contained additional newly identified significant associations. A single signal comprising a spectrum of correlated variation was observed at 39 regions; 35 of which are now described by a novel more significantly associated lead SNP, while the originally reported variant remained as the lead SNP only in 4 regions. We also confirmed two association signals in Europeans that had been previously reported only in East-Asian GWAS. Based on statistical evidence and linkage disequilibrium (LD) structure, we have curated and narrowed down the list of the most likely candidate causal variants for each region. Functional annotation using data from ENCODE filtered for PrCa cell lines and eQTL analysis demonstrated significant enrichment for overlap with bio-features within this set. By incorporating the novel risk variants identified here alongside the refined data for existing association signals, we estimate that these loci now explain ∼38.9% of the familial relative risk of PrCa, an 8.9% improvement over the previously reported GWAS tag SNPs. This suggests that a significant fraction of the heritability of PrCa may have been hidden during the discovery phase of GWAS, in particular due to the presence of multiple independent signals within the same regio

    Endovascular suction thrombectomy for severe cerebral venous sinus thrombosis: A report of two cases

    Cerebral venous sinus thrombosis (CVST) is a severe clinical condition that manifests with diverse symptoms that can be mild, taking the form of a headache, or can be life-threatening, with raised intracranial pressure and herniation. Despite systemic anticoagulation treatment, CVST still carries a mortality risk of approximately 10% because of the failure of venous recanalization by anticoagulants alone. This paper describes two cases with extensive CVST who presented with progressive neurological deterioration despite adequate systemic anticoagulation treatment. Both patients were successfully treated with endovascular clot removal using suction thrombectomy techniques with immediate recanalization of the sinuses and rapid radiographic improvement. The described therapy appears effective and safe; however, further studies are needed to validate the effectiveness of this treatment

    Microstructural and Mechanical Characterization of Ledeburitic AISI D2 Cold-Work Tool Steel in Semisolid Zones via Direct Partial Remelting Process

    The success of the thixoforming process largely depends on the created microstructure, which must be globular in the liquid phase. The solid–liquid structural changes that occur on as-annealed D2 tool steel when it is subjected to the so-called DPRM are described in this work (direct partial remelting method). The paper discusses phase changes and how carbide dissolution affects grain boundary liquation and grain spheroidization. Equiaxed grains with undissolved carbide particles have been found in the microstructural analysis at 1250 °C; however, the carbides gradually disappear as the temperature rises. Additionally, the equiaxed grains were transformed to a globular structure, which improves the shape factor and grain size for the thixoforming process. For AISI D2 thixoforming, which produced grains with a diameter of 50 μm and a shape factor of 1.18, temperatures of 1300 °C and a holding period of 5 min were the optimum parameters. The outcomes also showed that the mechanical properties of the AISI D2 were greatly enhanced after using direct partial remelting, where hardness was increased from 220 Hv to 350 Hv and tensile strength from 791 MPa to 961 MPa
