    “Maybe I Made Up the Whole Thing”: Placebos and Patients’ Experiences in a Randomized Controlled Trial

    Patients in the placebo arms of randomized controlled trials (RCT) often experience positive changes from baseline. While multiple theories concerning such “placebo effects” exist, peculiarly, none has been informed by actual interviews of patients undergoing placebo treatment. Here, we report on a qualitative study (n = 27) embedded within a RCT (n = 262) in patients with irritable bowel syndrome. Besides identical placebo acupuncture treatment in the RCT, the qualitative study patients also received an additional set of interviews at the beginning, midpoint, and end of the trial. Interviews of the 12 qualitative subjects who underwent and completed placebo treatment were transcribed. We found that patients (1) were persistently concerned with whether they were receiving placebo or genuine treatment; (2) almost never endorsed “expectation” of improvement but spoke of “hope” instead and frequently reported despair; (3) almost all reported improvement ranging from dramatic psychosocial changes to unambiguous, progressive symptom improvement to tentative impressions of benefit; and (4) often worried whether their improvement was due to normal fluctuations or placebo effects. The placebo treatment was a problematic perturbation that provided an opportunity to reconstruct the experiences of the fluctuations of their illness and how it disrupted their everyday life. Immersion in this RCT was a co-mingling of enactment, embodiment and interpretation involving ritual performance and evocative symbols, shifts in bodily sensations, symptoms, mood, daily life behaviors, and social interactions, all accompanied by self-scrutiny and re-appraisal. The placebo effect involved a spectrum of factors and any single theory of placebo—e.g. expectancy, hope, conditioning, anxiety reduction, report bias, symbolic work, narrative and embodiment—provides an inadequate model to explain its salubrious benefits

    Electrostatic model of atomic ordering in complex perovskite alloys

    We present a simple ionic model which successfully reproduces the various types of compositional long-range order observed in a large class of complex insulating perovskite alloys. The model assumes that the driving mechanism responsible for the ordering is simply the electrostatic interaction between the different ionic species. A possible new explanation for the anomalous long-range order observed in some Pb relaxor alloys, involving the proposed existence of a small amount of Pb^4+ on the B sublattice, is suggested by an analysis of the model.Comment: 4 pages, two-column style with 1 postscript figure embedded.     T-infinity: The Dependency Inversion Principle for Rapid and Sustainable Multidisciplinary Software Development

    The CFD Vision 2030 Study recommends that, NASA should develop and maintain an integrated simulation and software development infrastructure to enable rapid CFD technology maturation.... [S]oftware standards and interfaces must be emphasized and supported whenever possible, and open source models for noncritical technology components should be adopted. The current paper presents an approach to an open source development architecture, named T-infinity, for accelerated research in CFD leveraging the Dependency Inversion Principle to realize plugins that communicate through collections of functions without exposing internal data structures. Steady state flow visualization, mesh adaptation, fluid-structure interaction, and overset domain capabilities are demonstrated through compositions of plugins via standardized abstract interfaces without the need for source code dependencies between disciplines. Plugins interact through abstract interfaces thereby avoiding N 2 direct code-to-code data structure coupling where N is the number of codes. This plugin architecture enhances sustainable development by controlling the interaction between components to limit software complexity growth. The use of T-infinity abstract interfaces enables multidisciplinary application developers to leverage legacy applications alongside newly-developed capabilities.     Sensitivity Analysis for Multidisciplinary Systems (SAMS)

    This report describes the research conducted under an interagency collaboration agreement between the Aerospace Systems Directorate of the Air Force Research Laboratory (AFRL/RQ) and the Computational AeroSciences Branch of NASA Langley (NASA LaRC). Both organizations have a long-term goal of developing a modular computational system for coupling fluids and structures to enable both analysis and optimization of aerospace vehicles. Ultimately, the system should support multiple solvers within the fluid and structure domains to allow the best combination for the task at hand, as well as to allow for institutional preferences of specific software components. Towards this goal, the current research was focused on enhancing the existing modal aeroelastic analysis in the NASA FUN3D (Fully-UNstructured three-dimensional CFD (Computational Fluid Dynamics) code) software (Biedron et al. 2018), as well as developing new aeroelastic analysis and optimization capabilities based on a non-linear finite-element method. The methods and enhancements described in this document pertain to FUN3D Version 13.4

    High Sustained Virologic Response Rates of Glecaprevir/Pibrentasvir in Patients With Dosing Interruption or Suboptimal Adherence

    INTRODUCTION: Pangenotypic, all-oral direct-acting antivirals, such as glecaprevir/pibrentasvir (G/P), are recommended for treatment of hepatitis C virus (HCV) infection. Concerns exist about the impact on efficacy in patients with suboptimal adherence, particularly with shorter treatment durations. These post hoc analyses evaluated adherence (based on pill count) in patients prescribed 8- or 12-week G/P, the impact of nonadherence on sustained virologic response at post-treatment week 12 (SVR12), factors associated with nonadherence, and efficacy in patients interrupting G/P treatment. METHODS: Data were pooled from 10 phase 3 clinical trials of treatment-naive patients with HCV genotype 1-6 without cirrhosis/with compensated cirrhosis (treatment adherence analysis) and 13 phase 3 clinical trials of all patients with HCV (interruption analysis). RESULTS: Among 2,149 patients included, overall mean adherence was 99.4%. Over the treatment duration, adherence decreased (weeks 0-4: 100%; weeks 5-8: 98.3%; and weeks 9-12: 97.1%) and the percentage of patients with ≥80% or ≥90% adherence declined. SVR12 rate in the intention-to-treat (ITT) population was 97.7% (modified ITT SVR12 99.3%) and remained high in nonadherent patients in the modified ITT population (<90%: 94.4%-100%; <80%: 83.3%-100%). Psychiatric disorders were associated with <80% adherence, and shorter treatment duration was associated with ≥80% adherence. Among 2,902 patients in the interruption analysis, 33 (1.1%) had a G/P treatment interruption of ≥1 day, with an SVR12 rate of 93.9% (31/33). No virologic failures occurred. DISCUSSION: These findings support the impact of treatment duration on adherence rates and further reinforce the concept of "treatment forgiveness" with direct-acting antivirals.SCOPUS: ar.jinfo:eu-repo/semantics/publishe

    Absence of extended atmospheres in low-mass star radius-gap planets GJ 9827 b, GJ 9827 d and TOI-1235 b

    \textit{Kepler} showed a paucity of planets with radii of 1.5 - 2 R⊕\mathrm R_{\oplus} around solar mass stars but this radius-gap has not been well studied for low-mass star planets. Energy-driven escape models like photoevaporation and core-powered mass-loss predict opposing transition regimes between rocky and non-rocky planets when compared to models depicting planets forming in gas-poor environments. Here we present transit observations of three super-Earth sized planets in the radius-gap around low-mass stars using high-dispersion InfraRed Doppler (IRD) spectrograph on the Subaru 8.2m telescope. The planets GJ 9827 b and d orbit around a K6V star and TOI-1235 b orbits a M0.5 star. We limit any planet-related absorption in the 1083.3 nm lines of triplet He I by placing an upper-limit on the equivalent width of 14.71 m{\AA}, 18.39 m{\AA}, and 1.44 m{\AA}, for GJ 9827 b (99% confidence), GJ 9827 d (99% confidence) and TOI-1235 b (95% confidence) respectively. Using a Parker wind model, we cap the mass-loss at >>0.25 M⊕\mathrm M_{\oplus} Gyr−1^{-1} and >>0.2 M⊕\mathrm M_{\oplus} Gyr−1^{-1} for GJ 9827 b and d, respectively (99% confidence), and >>0.05 M⊕\mathrm M_{\oplus} Gyr−1^{-1} for TOI-1235 b (95\% confidence) for a representative wind temperature of 5000 K. Our observed results for the three planets are more consistent with the predictions from photoevaporation and/or core-powered mass-loss models than the gas-poor formation models. However, more planets in the radius-gap regime around the low-mass stars are needed to robustly predict the atmospheric evolution in planets around low-mass stars.Comment: Accepted for MNRAS. 12 pages, 15 figure

    You turn me cold: evidence for temperature contagion

    Introduction During social interactions, our own physiological responses influence those of others. Synchronization of physiological (and behavioural) responses can facilitate emotional understanding and group coherence through inter-subjectivity. Here we investigate if observing cues indicating a change in another's body temperature results in a corresponding temperature change in the observer. Methods Thirty-six healthy participants (age; 22.9±3.1 yrs) each observed, then rated, eight purpose-made videos (3 min duration) that depicted actors with either their right or left hand in visibly warm (warm videos) or cold water (cold videos). Four control videos with the actors' hand in front of the water were also shown. Temperature of participant observers' right and left hands was concurrently measured using a thermistor within a Wheatstone bridge with a theoretical temperature sensitivity of <0.0001°C. Temperature data were analysed in a repeated measures ANOVA (temperature × actor's hand × observer's hand). Results Participants rated the videos showing hands immersed in cold water as being significantly cooler than hands immersed in warm water, F(1,34) = 256.67, p0.1). There was however no evidence of left-right mirroring of these temperature effects p>0.1). Sensitivity to temperature contagion was also predicted by inter-individual differences in self-report empathy. Conclusions We illustrate physiological contagion of temperature in healthy individuals, suggesting that empathetic understanding for primary low-level physiological challenges (as well as more complex emotions) are grounded in somatic simulation

    Comparison of polychlorinated biphenyl levels across studies of human neurodevelopment.

    Polychlorinated biphenyls (PCBs) are persistent pollutants that are ubiquitous in the food chain, and detectable amounts are in the blood of almost every person in most populations that have been examined. Extensive evidence from animal studies shows that PCBs are neurotoxins, even at low doses. Interpretation of human data regarding low-level, early-life PCB exposure and subsequent neurodevelopment is problematic because levels of exposure were not similarly quantified across studies. We expressed the exposure levels from 10 studies of PCB and neurodevelopment in a uniform manner using a combination of data from original investigators, laboratory reanalyses, calculations based on published data, and expert opinion. The mainstay of our comparison was the median level of PCB 153 in maternal pregnancy serum. The median concentration of PCB 153 in the 10 studies ranged from 30 to 450 ng/g serum lipid, and the median of the 10 medians was 110 ng/g. We found that (a)) the distribution of PCB 153 exposure in most studies overlapped substantially, (b)) exposure levels in the Faroe Islands study were about 3-4-fold higher than in most other studies, and (c)) the exposure levels in the two recent U.S. studies were about one-third of those in the four earlier U.S. studies or recent Dutch, German, and northern QuĂŠbec studies. Our results will facilitate a direct comparison of the findings on PCBs and neurodevelopment when they are published for all 10 studies
