    Understanding the Experiences of Provisionally Admitted Black Male College Students in the State of Georgia

    This article is the result of research conducted with provisionally admitted Black make students enrolled at four different public institutions in Georgia. The researcher sought to answer the following research question: What are the common perceptions of provisionally admitted Black male students that influence their academic success at state colleges and state universities with the University System of Georgia? The researcher utilized a phenomenological approach and multiple methods of data collection. This resulted in rich data that has been categorized into themes. The article concludes with implications for student affairs administrators and faculty, as well as for future research on provisionally admitted Black male college students

    Predicting Falls in Community-Dwelling Older Adults Using the Repetitive Step Test

    Background/Purpose: The Repetitive Step Test (RST) is a newly developed dynamic balance test that requires stepping in multiple directions. It has previously demonstrated excellent test-retest and inter-rater reliability, along with ability to retrospectively classify individuals who had fallen more than once in 12 months though its ability to predict falls has not been evaluated. This study aimed to determine the capability of the RST to identify individuals likely to fall prospectively over 12 months. Methods: 63 community-dwelling adults \u3e65 years old were tested using the following balance and mobility measures: RST, 10 Meter Walk Test (10MWT), Activities-specific Balance Confidence scale (ABC), Four Square Step Test (FSST), and Timed Up and Go (TUG). For the RST, participants completed as many steps as possible in 15 seconds at 50% and 75% of their right leg length in the anterolateral and posterolateral directions bilaterally. Participants completed a fall log that was reported on a monthly basis for 12 months. Results: Based on 12 months of data, receiver operating characteristic (ROC) curve analysis was used to compare adults who fell \u3e 2 times to adults (n=17) who did not fall (n=29). Area under the curve (AUC) results with 95% confidence interval are as follows: RST at 50% 0.40 (0.22-0.58), RST at 75% 0.41 (0.24-0.57), FSST 0.45 (0.28-0.62), TUG 0.49 (0.32-0.66), ABC 0.61 (0.44-0.78) and 10MWT 0.67 (0.51-0.83). Conclusion: At 12 months, the RST and other balance and mobility measures either failed to or poorly predicted the number of falls. Research is needed to determine how to best predict falls in this population, and whether any single balance test can effectively identify individuals who are most likely to fall. Clinical Relevance: Currently, there is no simple measure of balance and mobility that can accurately predict falls in community-dwelling older adults. The RST was ineffective in predicting falls at 12 months. Given the unique characteristics of the RST, it may still be a useful tool for assessing and monitoring bilateral lower extremity performance and stepping ability, especially in individuals who present with asymmetrical impairments (e.g., stroke, multiple sclerosis, Parkinson’s disease).https://ecommons.udayton.edu/dpt_symposium/1005/thumbnail.jp

    Each year one in every three adults over the age of 65 experience a fall resulting in serious injury and in some instances death. In this population, fall injuries are the leading cause of death and are associated with the greatest number of nonfatal injuries and trauma hospital admissions. Since balance and muscle performance decreases as one ages, it is vitally important to assess these factors as part of a comprehensive strategy to monitor and predict fall risk.     Multivariate and network meta-analysis of multiple outcomes and multiple treatments: rationale, concepts, and examples

    Organisations such as the National Institute for Health and Care Excellence require the synthesis of evidence from existing studies to inform their decisions—for example, about the best available treatments with respect to multiple efficacy and safety outcomes. However, relevant studies may not provide direct evidence about all the treatments or outcomes of interest. Multivariate and network meta-analysis methods provide a framework to address this, using correlated or indirect evidence from such studies alongside any direct evidence. In this article, the authors describe the key concepts and assumptions of these methods, outline how correlated and indirect evidence arises, and illustrate the contribution of such evidence in real clinical examples involving multiple outcomes and multiple treatment

    Food security status and cardiometabolic health among pregnant women in the United States

    IntroductionPregnant women and their offspring are particularly vulnerable to food insecurity and its adverse effects during critical periods of fetal development. Racially/ethnically minoritized women in the United States (US) who are pregnant are additionally burdened by food insecurity, which may exacerbate cardiovascular health (CVH) disparities. Despite heightened social vulnerability, few studies have employed an intersectional framework, including race and gender, to assess the food insecurity and CVH relationship.MethodsWe used 2012–2018 and 2020 National Health Interview Survey data among US pregnant women aged 18–49 years old (N = 1,999) to assess the prevalence of food insecurity status by race/ethnicity and to investigate household food security status in relation to ideal CVH, using a modified ideal CVH (mICVH) metric. We categorized food security status as “very low/low”, “marginal”, or “high”. To assess mICVH, a summary score of 7 clinical characteristics and health behaviors was dichotomized as yes [(7)] vs. no [<7]. Prevalence ratios (PRs) and 95% confidence intervals (CIs) of associations between food security status and mICVH were estimated using Poisson regression with robust variance. Models were adjusted for age, household income, educational attainment, geographic region, marital status, alcohol consumption, survey year, and race/ethnicity (in overall model).ResultsThe mean age ± standard error was 29.0 ± 0.2 years. Among pregnant women, 12.7% reported “very low/low”, 10.6% reported “marginal”, and 76.7% reported “high” food security. “Very low/low” food security prevalence was higher among NH-Black (16.2%) and Hispanic/Latina (15.2%) pregnant women compared to NH-White (10.3%) and NH-Asian (3.2%) pregnant women. The mICVH prevalence was 11.6% overall and 14.5% for NH-White, 4.1% for NH-Black, 5.0% for Hispanic/Latina, and 26.7% for NH-Asian pregnant women. Among all pregnant women, “very low/low” and “marginal” vs. “high” food security status was associated with a lower prevalence of mICVH {[PRvery low/low = 0.26 (95% CI: 0.08–0.75)]; [PRmarginal = 0.47 (95% CI: 0.23 −0.96)]}.ConclusionHousehold food insecurity was higher among pregnant women in minoritized racial/ethnic groups and was associated with lower mICVH prevalence. Given the higher burden of food insecurity among minoritized racial/ethnic groups, food security may be an important intervention target to help address disparities in poor CVH among pregnant women

    Environment and Co-occurring Native Mussel Species, but Not Host Genetics, Impact the Microbiome of a Freshwater Invasive Species (Corbicula fluminea)

    The Asian clam Corbicula fluminea (Family: Cyneridae) has aggressively invaded freshwater habitats worldwide, resulting in dramatic ecological changes and declines of native bivalves such as freshwater mussels (Family: Unionidae), one of the most imperiled faunal groups. Despite increases in our knowledge of invasive C. fluminea biology, little is known of how intrinsic and extrinsic factors, including co-occurring native species, influence its microbiome. We investigated the gut bacterial microbiome across genetically differentiated populations of C. fluminea in the Tennessee and Mobile River Basins in the Southeastern United States and compared them to those of six co-occurring species of native freshwater mussels. The gut microbiome of C. fluminea was diverse, differed with environmental conditions and varied spatially among rivers, but was unrelated to host genetic variation. Microbial source tracking suggested that the gut microbiome of C. fluminea may be influenced by the presence of co-occurring native mussels. Inferred functions from 16S rRNA gene data using PICRUST2 predicted a high prevalence and diversity of degradation functions in the C. fluminea microbiome, especially the degradation of carbohydrates and aromatic compounds. Such modularity and functional diversity of the microbiome of C. fluminea may be an asset, allowing to acclimate to an extensive range of nutritional sources in invaded habitats, which could play a vital role in its invasive success

    African-American mitochondrial DNAs often match mtDNAs found in multiple African ethnic groups

    BACKGROUND: Mitochondrial DNA (mtDNA) haplotypes have become popular tools for tracing maternal ancestry, and several companies offer this service to the general public. Numerous studies have demonstrated that human mtDNA haplotypes can be used with confidence to identify the continent where the haplotype originated. Ideally, mtDNA haplotypes could also be used to identify a particular country or ethnic group from which the maternal ancestor emanated. However, the geographic distribution of mtDNA haplotypes is greatly influenced by the movement of both individuals and population groups. Consequently, common mtDNA haplotypes are shared among multiple ethnic groups. We have studied the distribution of mtDNA haplotypes among West African ethnic groups to determine how often mtDNA haplotypes can be used to reconnect Americans of African descent to a country or ethnic group of a maternal African ancestor. The nucleotide sequence of the mtDNA hypervariable segment I (HVS-I) usually provides sufficient information to assign a particular mtDNA to the proper haplogroup, and it contains most of the variation that is available to distinguish a particular mtDNA haplotype from closely related haplotypes. In this study, samples of general African-American and specific Gullah/Geechee HVS-I haplotypes were compared with two databases of HVS-I haplotypes from sub-Saharan Africa, and the incidence of perfect matches recorded for each sample. RESULTS: When two independent African-American samples were analyzed, more than half of the sampled HVS-I mtDNA haplotypes exactly matched common haplotypes that were shared among multiple African ethnic groups. Another 40% did not match any sequence in the database, and fewer than 10% were an exact match to a sequence from a single African ethnic group. Differences in the regional distribution of haplotypes were observed in the African database, and the African-American haplotypes were more likely to match haplotypes found in ethnic groups from West or West Central Africa than those found in eastern or southern Africa. Fewer than 14% of the African-American mtDNA sequences matched sequences from only West Africa or only West Central Africa. CONCLUSION: Our database of sub-Saharan mtDNA sequences includes the most common haplotypes that are shared among ethnic groups from multiple regions of Africa. These common haplotypes have been found in half of all sub-Saharan Africans. More than 60% of the remaining haplotypes differ from the common haplotypes at a single nucleotide position in the HVS-I region, and they are likely to occur at varying frequencies within sub-Saharan Africa. However, the finding that 40% of the African-American mtDNAs analyzed had no match in the database indicates that only a small fraction of the total number of African haplotypes has been identified. In addition, the finding that fewer than 10% of African-American mtDNAs matched mtDNA sequences from a single African region suggests that few African Americans might be able to trace their mtDNA lineages to a particular region of Africa, and even fewer will be able to trace their mtDNA to a single ethnic group. However, no firm conclusions should be made until a much larger database is available. It is clear, however, that when identical mtDNA haplotypes are shared among many ethnic groups from different parts of Africa, it is impossible to determine which single ethnic group was the source of a particular maternal ancestor based on the mtDNA sequence

    Design, Synthesis, and Preliminary Evaluation of a Potential Synthetic Opioid Rescue Agent

    BACKGROUND: One of the most prominent opioid analgesics in the United States is the high potency agonist fentanyl. It is used in the treatment of acute and chronic pain and as an anesthetic adjuvant. When used inappropriately, however, ingestion of just a few milligrams of fentanyl or other synthetic opioid can cause opioid-induced respiratory depression (OIRD), often leading to death. Currently, the treatment of choice for OIRD is the opioid receptor antagonist naloxone. Recent reports, however, suggest that higher doses or repeated dosing of naloxone (due to recurrence of respiratory depression) may be required to reverse fully fentanyl-induced respiratory depression, rendering this treatment inadequate. To combat this synthetic opioid overdose crisis, this research aims at identifying a novel opioid reversal agent with enhanced efficacy towards fentanyl and other synthetic opioids. METHODS: A series of naltrexone analogues were characterized for their ability to antagonize the effects of fentanyl in vitro utilizing a modified forskolin-induced cAMP accumulation assay. Lead analogue 29 was chosen to undergo further PK studies, followed by in vivo pharmacological analysis to determine its ability to antagonize opioid-induced antinociception in the hot plate assay. RESULTS: A series of potent MOR antagonists were identified, including the highly potent analogue 29 (IC50 = 2.06 nM). Follow-up PK studies revealed 29 to possess near 100% bioavailability following IP administration. Brain concentrations of 29 surpassed plasma concentrations, with an apparent terminal half-life of ~ 80 min in mice. In the hot plate assay, 29 dose-dependently (0.01–0.1 mg/kg; IP) and fully antagonized the antinociception induced by oxycodone (5.6 mg/kg; IP). Furthermore, the dose of 29 that is fully effective in preventing oxycodone-induced antinociception (0.1 mg/kg) was ineffective against locomotor deficits caused by the KOR agonist U50,488. CONCLUSIONS: Methods have been developed that have utility to identify enhanced rescue agents for the treatment of OIRD. Analogue 29, possessing potent MOR antagonist activity in vitro and in vivo, provides a promising lead in our search for an enhanced synthetic opioid rescue agent
