    Patterns of chlamydia testing in different settings and implications for wider STI diagnosis and care: a probability sample survey of the British population

    Background: Following widespread rollout of chlamydia testing to non-specialist and community settings in the UK, many individuals receive a chlamydia test without being offered comprehensive STI and HIV testing. We assess sexual behaviour among testers in different settings with a view to understanding their need for other STI diagnostic services. Methods: A probability sample survey of the British population undertaken 2010–2012 (the third National Survey of Sexual Attitudes and Lifestyles). We analysed weighted data on chlamydia testing ( past year), including location of most recent test, and diagnoses (past 5 years) from individuals aged 16–44 years reporting at least one sexual partner in the past year (4992 women, 3406 men). Results: Of the 26.8% (95% CI 25.4% to 28.2%) of women and 16.7% (15.5% to 18.1%) of men reporting a chlamydia test in the past year, 28.4% of women and 41.2% of men had tested in genitourinary medicine (GUM), 41.1% and 20.7% of women and men respectively tested in general practice (GP) and the remainder tested in other non-GUM settings. Women tested outside GUM were more likely to be older, in a relationship and to live in rural areas. Individuals tested outside GUM reported fewer risk behaviours; nevertheless, 11.0% (8.6% to 14.1%) of women and 6.8% (3.9% to 11.6%) of men tested in GP and 13.2% (10.2% to 16.8%) and 9.6% (6.5% to 13.8%) of women and men tested in other non-GUM settings reported ‘unsafe sex’, defined as two or more partners and no condom use with any partner in the past year. Individuals treated for chlamydia outside GUM in the past 5 years were less likely to report an HIV test in that time frame (women: 54.5% (42.7% to 65.7%) vs 74.1 (65.9% to 80.9%) in GUM; men: 23.9% (12.7% to 40.5%) vs 65.8% (56.2% to 74.3%)). Conclusions: Most chlamydia testing occurred in non-GUM settings, among populations reporting fewer risk behaviours. However, there is a need to provide pathways to comprehensive STI care to the sizeable minority at higher risk

    Approche méthodologique de la modélisation du transport des HAP dans les sols et les eaux

    Afin de préciser le cadre d'utilisation de codes de transport de polluants dans les sols et les eaux, un programme d'intercomparaison de méthodes et de codes a été mis en oeuvre avec la collaboration de huit équipes appartenant à des instituts de recherche ou des bureaux d'études. Parmi les types de polluants étudiés, les hydrocarbures aromatiques polycycliques (HAP) ont fait l'objet de simulations par deux groupes de travail. Le premier groupe, composé de cinq équipes, s'est intéressé à leur comportement en phase dissoute à partir d'un cas réel de pollution. Disposant des mêmes données d'étude, chaque équipe avait pour mission de concevoir un modèle conceptuel (description géologique, définition de la source, paramètres d'écoulement et de transport) et de simuler la pollution de la nappe avec le code de son choix. Les résultats obtenus montrent des différences significatives qui trouvent leur explication dans les paramètres des modèles conceptuels adoptés plus que dans les simulateurs choisis. Cet exercice a confirmé l'importance des paramètres suivants qui sont souvent incertains mais justifieraient d'être mieux appréciés lors de diagnostics : paramètres hydrodynamiques (détermination du champ de vitesse), coefficient de partition (pour chaque horizon géologique), temps de demi-vie du ou des polluants, extension de la source de pollution. Le second groupe a étudié les écoulements en phase libre (phase liquide non aqueuse) sur un cas théorique inspiré d'un cas réel de déversement massif de naphtalène. La modélisation a été conduite par trois équipes avec trois codes polyphasiques différents (SIMUSCOPP, TOUGH/T2VOC, UTCHEM). Cet exercice comprend le suivi du déversement et de la migration du naphtalène sous formes liquide et dissoute sur une distance de 300 m et une durée de 10 ans. Le naphtalène révèle un comportement d'hydrocarbure " lourd " avec une phase huile (partiellement miscible) qui tend à descendre à travers la nappe. Ce comportement a été reproduit par chacune des équipes, de manière plus ou moins complète en fonction des possibilités de modélisation offertes par les codes. Les résultats obtenus permettent de vérifier la cohérence des différentes approches polyphasiques entre elles. Bien que plus complexe (paramètres plus nombreux, difficulté numérique accrue), l'approche polyphasique se justifie pour mieux comprendre et déterminer la répartition spatiale de HAP en profondeur dans une nappe

    MIKE-SHE integrated groundwater and surface water model used to simulate scenario hydrology for input to DRIFT-ARID: the Mokolo River case study

    A fully integrated, physically-based MIKE SHE/MIKE11 model was developed for the Mokolo River basin flow system to simulate key hydraulic and hydrologic indicator inputs to the Downstream Response to Imposed Flow Transformation for Arid Rivers (DRIFT-ARID) decision support system (DSS). The DRIFT-ARID tool is used in this study to define environmental water requirements (EWR) for non-perennial river flow systems in South Africa to facilitate ecosystembased management of water resources as required by the National Water Act (Act No. 36 of 1998). Fifty years of distributed daily climate data (1950 to 2000) were used to calibrate the model against decades of daily discharge data at various gauges, measurements of Mokolo Dam stage levels, and one-time groundwater level measurements at hundreds of wells throughout the basin. Though the calibrated model captures much of the seasonal and post-event stream discharge response characteristics, lack of sub-daily climate and stream discharge data limits the ability to calibrate the model to event-level system response (i.e. peak flows). In addition, lack of basic subsurface hydrogeologic characterisation and transient groundwater level data limits the ability to calibrate the groundwater flow model, and therefore baseflow response, to a high level. Despite these limitations, the calibrated model was used to simulate changes in hydrologic and hydraulic indicators at five study sites within the basin for five 50-year land-use change scenarios, including a present-day (with dam), natural conditions (no development/irrigation), and conversion of present-day irrigation to game farm, mine/city expansion, and a combination of the last two.     DRIFT-ARID: A method for assessing environmental water requirements (EWRs) for non-perennial rivers

    Environmental water requirement (EWR) assessment methods, for ascertaining how much water should be retained in rivers to sustain ecological functioning and desired levels of biodiversity, have mostly been developed for perennial rivers. Despite non-perennial rivers comprising about 30–50% of the world’s freshwater systems, data on their hydrology, biota and ecological functioning are sparse. Current EWR assessments require hydrological and other data that may not be available for such rivers and some adaptation in the methods used seems necessary. DRIFT is an EWR method for perennial (or near-perennial) rivers that has been developed in South Africa over the past two decades and is now widely applied nationally and internationally. When applied to the semi-permanent Mokolo River, challenges particular to, or accentuated by, non-perennial rivers included the reliable simulation of hydrological data, the extent of acceptable extrapolation of data, difficulties in predicting surface-water connectivity along the river, and the location and resilience of pools, as well as whether it was possible to identify a reference (natural) condition. DRIFT-ARID, reported on here, is an adaptation of the DRIFT approach to begin addressing these and other issues.     A Systematic Quality Scoring Analysis to Assess Automated Cardiovascular Magnetic Resonance Segmentation Algorithms

    BACKGROUND: The quantitative measures used to assess the performance of automated methods often do not reflect the clinical acceptability of contouring. A quality-based assessment of automated cardiac magnetic resonance (CMR) segmentation more relevant to clinical practice is therefore needed. OBJECTIVE: We propose a new method for assessing the quality of machine learning (ML) outputs. We evaluate the clinical utility of the proposed method as it is employed to systematically analyse the quality of an automated contouring algorithm. METHODS: A dataset of short-axis (SAX) cine CMR images from a clinically heterogeneous population (n = 217) were manually contoured by a team of experienced investigators. On the same images we derived automated contours using a ML algorithm. A contour quality scoring application randomly presented manual and automated contours to four blinded clinicians, who were asked to assign a quality score from a predefined rubric. Firstly, we analyzed the distribution of quality scores between the two contouring methods across all clinicians. Secondly, we analyzed the interobserver reliability between the raters. Finally, we examined whether there was a variation in scores based on the type of contour, SAX slice level, and underlying disease. RESULTS: The overall distribution of scores between the two methods was significantly different, with automated contours scoring better than the manual (OR (95% CI) = 1.17 (1.07–1.28), p = 0.001; n = 9401). There was substantial scoring agreement between raters for each contouring method independently, albeit it was significantly better for automated segmentation (automated: AC2 = 0.940, 95% CI, 0.937–0.943 vs manual: AC2 = 0.934, 95% CI, 0.931–0.937; p = 0.006). Next, the analysis of quality scores based on different factors was performed. Our approach helped identify trends patterns of lower segmentation quality as observed for left ventricle epicardial and basal contours with both methods. Similarly, significant differences in quality between the two methods were also found in dilated cardiomyopathy and hypertension. CONCLUSIONS: Our results confirm the ability of our systematic scoring analysis to determine the clinical acceptability of automated contours.     Environmental exposures in young adults with declining kidney function in a population at risk of Mesoamerican nephropathy.

    OBJECTIVES: There is an epidemic of Mesoamerican nephropathy (MeN) in Central America, where sugarcane production is prominent. Numerous causes are proposed, but to date limited evidence supports any one hypothesis. A nested case-control study using biosamples from a rural, community-based follow-up study of 350 young adults from Northwest Nicaragua at risk of MeN was conducted with the aim of characterising the associations between urinary concentrations of metals, pesticides and mycotoxins from samples collected in the first 6 months and decline in kidney function over 2 years. METHODS: Urine samples collected at baseline (pre-sugarcane harvest) and the first 6 month follow-up (post-sugarcane harvest) visit were tested. Twelve metals and metalloids (aluminium, total arsenic, cadmium, chromium, cobalt, copper, lead, manganese, mercury, selenium, silicon and strontium) were analysed by inductively coupled plasma-mass spectrometry. Twelve pesticides or their metabolites (2,4-dichlorophenoxyacetic acid, 3-phenoxybenzoic acid, 4-fluoro-3-phenoxybenzoic acid, chloro-3,3,3-trifluoro-1-propen-1-yl-2,2-dimethylcyclopropanecarboxylic acid, cis/trans 3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropane carboxylic acid, ethylenethiourea, glyphosate, 4-chloro-2-methylphenoxy acetic acid, 3-hydroxy-pyrimetanil, 5-hydroxytiabendazole, hydroxy-tebuconazole and 3,5,6-trichloro-2-pyridinol) and two mycotoxins (ochratoxin A (OTA) and citrinin (CIT)) were analysed by liquid chromatography coupled-mass spectrometry. Differences in the creatinine-corrected urinary concentrations of the measured exposures between outcome groups (participants with stable vs declining kidney function) were examined. RESULTS: Elevated levels of aluminium and total arsenic as well as metabolites of several pesticides were detected across the population. No differences were identified between the declining and stable groups in the levels of metals or pesticides tested. OTA and CIT were below the limit of detection. CONCLUSIONS: The tested metals, metalloids, pesticides and mycotoxins were not associated with loss of kidney function in participants at-risk of MeN

    Independence in complaints procedures: lessons from community care

    This article looks at internal complaints procedures and considers the role of independent elements in procedures which are designed to be simple, informal and low cost. Taking the example of local authority community care services as a case study, the article discusses research which looked at the views of complainants, potential complainants and those who run the procedure. Most people do not make formal complaints at all and very few people seek an independent review of their complaint. When they do seek such a review, they expect it to be transparently independent of the body complained about. The article concludes that the current system of local authority complaints review panels or committees does not provide the independent element that complainants seek

    The Benson Complex Figure Test detects deficits in visuoconstruction and visual memory in symptomatic familial frontotemporal dementia: A GENFI study

    Objective: Sensitive cognitive markers are still needed for frontotemporal dementia (FTD). The Benson Complex Figure Test (BCFT) is an interesting candidate test, as it assesses visuospatial, visual memory, and executive abilities, allowing the detection of multiple mechanisms of cognitive impairment. To investigate differences in BCFT Copy, Recall and Recognition in presymptomatic and symptomatic FTD mutation carriers, and to explore its cognitive and neuroimaging correlates.Method: We included cross-sectional data from 332 presymptomatic and 136 symptomatic mutation carriers (GRN, MAPT or C9orf72 mutations), and 290 controls in the GENFI consortium. We examined gene-specific differences between mutation carriers (stratified by CDR (R) NACC-FTLD score) and controls using Quade's / Pearson X2 tests. We investigated associations with neuropsychological test scores and grey matter volume using partial correlations and multiple regression models respectively.Results: No significant differences were found between groups at CDR (R) NACC-FTLD 0-0.5. Symptomatic GRN and C9orf72 mutation carriers had lower Copy scores at CDR (R) NACC-FTLD >= 2. All three groups had lower Recall scores at CDR (R) NACC-FTLD >= 2, with MAPT mutation carriers starting at CDR (R) NACC-FTLD >= 1. All three groups had lower Recognition scores at CDR (R) NACC FTLD >= 2. Performance correlated with tests for visuoconstruction, memory, and executive function. Copy scores correlated with frontal-subcortical grey matter atrophy, while Recall scores correlated with temporal lobe atrophy. Conclusions: In the symptomatic stage, the BCFT identifies differential mechanisms of cognitive impairment depending on the genetic mutation, corroborated by gene-specific cognitive and neuroimaging correlates. Our findings suggest that impaired performance on the BCFT occurs relatively late in the genetic FTD disease process. Therefore its potential as cognitive biomarker for upcoming clinical trials in presymptomatic to early-stage FTD is most likely limited

    The Revised Self-Monitoring Scale detects early impairment of social cognition in genetic frontotemporal dementia within the GENFI cohort.

    BACKGROUND: Although social cognitive dysfunction is a major feature of frontotemporal dementia (FTD), it has been poorly studied in familial forms. A key goal of studies is to detect early cognitive impairment using validated measures in large patient cohorts. METHODS: We used the Revised Self-Monitoring Scale (RSMS) as a measure of socioemotional sensitivity in 730 participants from the genetic FTD initiative (GENFI) observational study: 269 mutation-negative healthy controls, 193 C9orf72 expansion carriers, 193 GRN mutation carriers and 75 MAPT mutation carriers. All participants underwent the standardised GENFI clinical assessment including the 'CDR® plus NACC FTLD' scale and RSMS. The RSMS total score and its two subscores, socioemotional expressiveness (EX score) and modification of self-presentation (SP score) were measured. Volumetric T1-weighted magnetic resonance imaging was available from 377 mutation carriers for voxel-based morphometry (VBM) analysis. RESULTS: The RSMS was decreased in symptomatic mutation carriers in all genetic groups but at a prodromal stage only in the C9orf72 (for the total score and both subscores) and GRN (for the modification of self-presentation subscore) groups. RSMS score correlated with disease severity in all groups. The VBM analysis implicated an overlapping network of regions including the orbitofrontal cortex, insula, temporal pole, medial temporal lobe and striatum. CONCLUSIONS: The RSMS indexes socioemotional impairment at an early stage of genetic FTD and may be a suitable outcome measure in forthcoming trials
