    Spectrum from the warped compactifications with the de Sitter universe

    We discuss the spectrum of the tensor metric perturbations and the stability of warped compactifications with the de Sitter spacetime in the higher-dimensional gravity. The spacetime structure is given in terms of the warped product of the non-compact direction, the spherical internal dimensions and the four-dimensional de Sitter spacetime. To realize a finite bulk volume, we construct the brane world model, using the cut-copy-paste method. Then, we compactify the spherical directions on the brane. In any case, we show the existence of the massless zero mode and the mass gap of it with massive Kaluza-Klein modes. Although the brane involves the spherical dimensions, no light massive mode is excited. We also investigate the scalar perturbations, and show that the model is unstable due to the existence of a tachyonic bound state, which seems to have the universal negative mass square, irrespective of the number of spacetime dimensions.Comment: Journal version (JHEP

    The Effective Field Theory of Multifield Inflation

    We generalize the Effective Field Theory of Inflation to include additional light scalar degrees of freedom that are in their vacuum at the time the modes of interest are crossing the horizon. In order to make the scalars light in a natural way we consider the case where they are the Goldstone bosons of a global symmetry group or are partially protected by an approximate supersymmetry. We write the most general Lagrangian that couples the scalar mode associated to the breaking of time translation during inflation to the additional light scalar fields. This Lagrangian is constrained by diffeomorphism invariance and the additional symmetries that keep the new scalars light. This Lagrangian describes the fluctuations around the time of horizon crossing and it is supplemented with a general parameterization describing how the additional fluctuating fields can affect cosmological perturbations. We find that multifield inflation can reproduce the non-Gaussianities that can be generated in single field inflation but can also give rise to new kinds of non-Gaussianities. We find several new three-point function shapes. We show that in multifield inflation it is possible to naturally suppress the three-point function making the four-point function the leading source of detectable non-Gaussianities. We find that under certain circumstances, i.e. if specific shapes of non-Gaussianities are detected in the data, one could distinguish between single and multifield inflation and sometimes even among the various mechanisms that kept the additional fields light.Comment: 62 pages, 1 figure; v2: JHEP published version, minor corrections, comments and references adde

    Delayed gastric emptying and reduced postprandial small bowel water content of equicaloric whole meal bread versus rice meals in healthy subjects: novel MRI insights

    BACKGROUND/OBJECTIVES: Postprandial bloating is a common symptom in patients with functional gastrointestinal (GI) diseases. Whole meal bread (WMB) often aggravates such symptoms though the mechanisms are unclear. We used magnetic resonance imaging (MRI) to monitor the intragastric fate of a WMB meal (11% bran) compared to a rice pudding (RP) meal. SUBJECTS/METHODS: 12 healthy volunteers completed this randomised crossover study. They fasted overnight and after an initial MRI scan consumed a glass of orange juice with a 2267 kJ WMB or an equicaloric RP meal. Subjects underwent serial MRI scans every 45 min up to 270 min to assess gastric volumes and small bowel water content and completed a GI symptom questionnaire. RESULTS: The MRI intragastric appearance of the two meals was markedly different. The WMB meal formed a homogeneous dark bolus with brighter liquid signal surrounding it. The RP meal separated into an upper, liquid layer and a lower particulate layer allowing more rapid emptying of the liquid compared to solid phase (sieving). The WMB meal had longer gastric half emptying times (132±8 min) compared to the RP meal (104±7 min), P<0.008. The WMB meal was associated with markedly reduced MRI-visible small bowel free mobile water content compared to the RP meal, P<0.0001. CONCLUSIONS: WMB bread forms a homogeneous bolus in the stomach which inhibits gastric sieving and hence empties slower than the equicaloric rice meal. These properties may explain why wheat causes postprandial bloating and could be exploited to design foods which prolong satiation

    Degenerate higher order scalar-tensor theories beyond Horndeski up to cubic order

    We present all scalar-tensor Lagrangians that are cubic in second derivatives of a scalar field, and that are degenerate, hence avoiding Ostrogradsky instabilities. Thanks to the existence of constraints, they propagate no more than three degrees of freedom, despite having higher order equations of motion. We also determine the viable combinations of previously identified quadratic degenerate Lagrangians and the newly established cubic ones. Finally, we study whether the new theories are connected to known scalar-tensor theories such as Horndeski and beyond Horndeski, through conformal or disformal transformations

    Haptoglobin Phenotype, Preeclampsia Risk and the Efficacy of Vitamin C and E Supplementation to Prevent Preeclampsia in a Racially Diverse Population

    Haptoglobin's (Hp) antioxidant and pro-angiogenic properties differ between the 1-1, 2-1, and 2-2 phenotypes. Hp phenotype affects cardiovascular disease risk and treatment response to antioxidant vitamins in some non-pregnant populations. We previously demonstrated that preeclampsia risk was doubled in white Hp 2-1 women, compared to Hp 1-1 women. Our objectives were to determine whether we could reproduce this finding in a larger cohort, and to determine whether Hp phenotype influences lack of efficacy of antioxidant vitamins in preventing preeclampsia and serious complications of pregnancy-associated hypertension (PAH). This is a secondary analysis of a randomized controlled trial in which 10,154 low-risk women received daily vitamin C and E, or placebo, from 9-16 weeks gestation until delivery. Hp phenotype was determined in the study prediction cohort (n = 2,393) and a case-control cohort (703 cases, 1,406 controls). The primary outcome was severe PAH, or mild or severe PAH with elevated liver enzymes, elevated serum creatinine, thrombocytopenia, eclampsia, fetal growth restriction, medically indicated preterm birth or perinatal death. Preeclampsia was a secondary outcome. Odds ratios were estimated by logistic regression. Sampling weights were used to reduce bias from an overrepresentation of women with preeclampsia or the primary outcome. There was no relationship between Hp phenotype and the primary outcome or preeclampsia in Hispanic, white/other or black women. Vitamin supplementation did not reduce the risk of the primary outcome or preeclampsia in women of any phenotype. Supplementation increased preeclampsia risk (odds ratio 3.30; 95% confidence interval 1.61-6.82, p<0.01) in Hispanic Hp 2-2 women. Hp phenotype does not influence preeclampsia risk, or identify a subset of women who may benefit from vitamin C and E supplementation to prevent preeclampsia

    A case of sigmoid endometriosis difficult to differentiate from colon cancer

    BACKGROUND: Although endometriosis with sigmoid serosal involvement is not uncommon in women of childbearing age, the mucosal involvement is rare and differential diagnosis from colon cancer may be difficult due to the lack of pathognomonic symptoms and the poor diagnostic yield of colonoscopy and colonic biopsies. CASE PRESENTATION: We present a case of a young woman with sigmoid endometriosis, in which the initial diagnostic workup suggested colon cancer. Histologic evidence, obtained from a second colonoscopy, along with pelvic ultrasound findings led to the final diagnosis of intestinal endometriosis which was confirmed by laparoscopy. CONCLUSION: Colonic endometriosis is often a diagnostic challenge and should be considered in young women with symptoms from the lower gastrointestinal tract
