In dit rapport is de opzet van het monitoringnetwerk beschreven, evenals de wijze waarop vanaf 2008 over de resultaten zal worden gerapporteerd. Het rapport geeft onder andere aan wanneer welke cijfers beschikbaar zijn, en welke rekenmethoden gebruikt zullen worden om onder andere de bemesting en gewasopbrengst te berekenen. De Europese Nitraatrichtlijn verplicht lidstaten het stikstofgebruik via dierlijke mest te beperken tot maximaal 170 kg per hectare. Een lidstaat kan de Europese Commissie vragen hiervan onder voorwaarden af te wijken. Nederland heeft in december 2005 toestemming gekregen om vanaf 2006 tot en met 2009 onder voorwaarden af te mogen wijken van de gestelde norm. Dit betekent dat landbouwbedrijven 250 kilo stikstof per hectare mogen toedienen via dierlijke mest afkomstig van graasdieren (vooral koeien). Een van die voorwaarden is dat minimaal 70 procent van het totale areaal grasland is. Daarnaast is de Nederlandse overheid verplicht een monitoringnetwerk in te richten en de Commissie over de resultaten daarvan te rapporteren. De driehonderd deelnemers die worden gevolgd, zijn een steekproef van de circa 27.000 Nederlandse landbouwbedrijven die zich hebben aangemeld voor derogatie. Het netwerk is een onderdeel van het Landelijk Meetnet effecten Mestbeleid (LMM).In 2006 RIVM and LEI set up a monitoring network that measures the consequences of the option for farmers to deviate from the European use-standard for animal manure (derogation). The monitoring network follows three hundred farms benefiting from individual derogations and records the consequences for agricultural practice and water quality. This report describes the design and set-up of the monitoring network, as well as the content and layout of the annual reports from 2008 onwards containing results of the monitoring network. The report indicates, for example, what data will be available at a certain point in time, and what methods will be used to calculate such factors as fertilisation and yield. The Nitrates Directive obliges Member States to limit the use of animal manure to a maximum of 170 kg of nitrogen per hectare. A Member State may request the European Commission to deviate from this obligation under certain conditions. In December 2005, the European Commission granted the Netherlands the right to derogate from the obligation from 2006 till 2009 inclusive, implying that farmers could use up to 250 kg of nitrogen via manure of grazing livestock (particularly cows). One of the conditions is that 70% or more of the acreage available for manure application is grass. The Netherlands authorities are also obliged to set up a monitoring network and to report the results of this network to the European Commission. The 300 farms participating in the monitoring represent a sample taken from the approximately 27,000 Netherlands farms benefiting from individual derogations.     Clinical Outcomes of Percutaneous Coronary Intervention for Bifurcation Lesions According to Medina Classification

    Background: Coronary bifurcation lesions (CBLs) are frequently encountered in clinical practice and are associated with worse outcomes after percutaneous coronary intervention. However, there are limited data around the prognostic impact of different CBL distributions. / Methods and Results: All CBL percutaneous coronary intervention procedures from the prospective e‐Ultimaster (Prospective, Single‐Arm, Multi Centre Observations Ultimaster Des Registry) multicenter international registry were analyzed according to CBL distribution as defined by the Medina classification. Cox proportional hazards models were used to compare the hazard ratio (HR) of the primary outcome, 1‐year target lesion failure (composite of cardiac death, target vessel–related myocardial infarction, and clinically driven target lesion revascularization), and its individual components between Medina subtypes using Medina 1.0.0 as the reference category. A total of 4003 CBL procedures were included. The most prevalent Medina subtypes were 1.1.1 (35.5%) and 1.1.0 (26.8%), whereas the least prevalent was 0.0.1 (3.5%). Overall, there were no significant differences in patient and procedural characteristics among Medina subtypes. Only Medina 1.1.1 and 0.0.1 subtypes were associated with increased target lesion failure (HR, 2.6 [95% CI, 1.3–5.5] and HR, 4.0 [95% CI, 1.6–9.0], respectively) at 1 year, compared with Medina 1.0.0, prompted by clinically driven target lesion revascularization (HR, 3.1 [95% CI, 1.1–8.6] and HR, 4.6 [95% CI, 1.3–16.0], respectively) as well as cardiac death in Medina 0.0.1 (HR, 4.7 [95% CI, 1.0–21.6]). No differences in secondary outcomes were observed between Medina subtypes. / Conclusions: In a large multicenter registry analysis of coronary bifurcation percutaneous coronary intervention procedures, we demonstrate prognostic differences in 1‐year outcomes between different CBL distributions, with Medina 1.1.1 and 0.0.1 subtypes associated with an increased risk of target lesion failure

    Lack of Detection of XMRV in Seminal Plasma from HIV-1 Infected Men in The Netherlands

    Background: Xenotropic murine leukaemia virus-related virus (XMRV) is a recently discovered human gammaretrovirus with yet unknown prevalence and transmission route(s). Its presence in prostate stromal fibroblasts and prostatic secretions suggests that XMRV might be sexually transmitted. We chose to study a compartment closely connected to the prostate, a location where XMRV was detected in independent studies. Seminal plasma samples from HIV-1 infected men were examined as they have an increased probability of acquiring sexually transmitted pathogens. Methodology/Principal Findings: We studied the prevalence of XMRV in 93 seminal plasma samples of 54 HIV-1 infected men living in The Netherlands with a nested PCR amplification specifically targeting the XMRV gag gene. As a control for the presence and integrity of retrovirus particles, HIV-1 was amplified from the same samples with a PCR amplification targeting the env gene of the virus, or HIV-1 was quantified with a real-time PCR amplifying part of the pol gene. Conclusions/Significance: Although HIV-1 was amplified from 25 % of the seminal plasma samples, no XMRV was detected, suggesting that either the prevalence of XMRV is very low in The Netherlands, or that XMRV is not naturally present in th

    External quality assessment of the molecular diagnostics and genotyping of meticillin-resistant Staphylococcus aureus

    Two multicentre external quality assessments (EQA) for the molecular detection and genotyping of meticillin-resistant Staphylococcus aureus (MRSA) were arranged. Firstly, 11 samples containing various amounts of inactivated MRSA strains, meticillin-susceptible S. aureus (MSSA), meticillin-resistant coagulase-negative staphylococci (MRCoNS) or Escherichia coli were distributed to 82 laboratories. Samples containing 102 or 103 MRSA cells were correctly scored in only 16 and 46% of the datasets returned, respectively. Two of the used MSSA strains contained an SCCmec cassette lacking the mecA gene. There was a marked difference in the percentage of correct results for these two MSSA strains (37 and 39%) compared to the MSSA strain lacking the SCCmec cassette (88%). Secondly, a panel for MRSA genotyping, consisting of ten samples (two identical, three genetically related and five unique strains) was distributed to 19 laboratories. Seventy-three percent of the datasets recorded all samples correctly. Most pulsed-field gel electrophoresis (PFGE) protocols proved to be suboptimal, resulting in inferior resolution in the higher or lower fragment regions. The performance of molecular diagnostics for MRSA shows no significant changes since our first EQA in 2006. The first molecular typing results are encouraging. Both assessments indicate that programme expansion is required and that major performance discrepancies continue to exist

    Genetic Covariance Structure of Reading, Intelligence and Memory in Children

    This study investigates the genetic relationship among reading performance, IQ, verbal and visuospatial working memory (WM) and short-term memory (STM) in a sample of 112, 9-year-old twin pairs and their older siblings. The relationship between reading performance and the other traits was explained by a common genetic factor for reading performance, IQ, WM and STM and a genetic factor that only influenced reading performance and verbal memory. Genetic variation explained 83% of the variation in reading performance; most of this genetic variance was explained by variation in IQ and memory performance. We hypothesize, based on these results, that children with reading problems possibly can be divided into three groups: (1) children low in IQ and with reading problems; (2) children with average IQ but a STM deficit and with reading problems; (3) children with low IQ and STM deficits; this group may experience more reading problems than the other two

    Three-dimensional reconstruction of myocardial contrast perfusion from biplane cineangiograms by means of linear programming techniques

    The assessment of coronary flow reserve from the instantaneous distribution of the contrast agent within the coronary vessels and myocardial muscle at the control state and at maximal flow has been limited by the superimposition of myocardial regions of interest in the two-dimensional images. To overcome these limitations, we are in the process of developing a three-dimensional (3D) reconstruction technique to compute the contrast distribution in cross sections of the myocardial muscle from two orthogonal cineangiograms. To limit the number of feasible solutions in the 3D-reconstruction space, the 3D-geometry of the endo- and epicardial boundaries of the myocardium must be determined. For the geometric reconstruction of the epicardium, the centerlines of the left coronary arterial tree are manually or automatically traced in the biplane views. Next, the bifurcations are detected automatically and matched in these two views, allowing a 3D-representation of the coronary tree. Finally, the circumference of the left ventricular myocardium in a selected cross section can be computed from the intersection points of this cross section with the 3D coronary tree using B-splines. For the geometric reconstruction of the left ventricular cavity, we envision to apply the elliptical approximation technique using the LV boundaries defined in the two orthogonal views, or by applying more complex 3D-reconstruction techniques including densitometry. The actual 3D-reconstruction of the contrast distribution in the myocardium is based on a linear programming technique (Transportation model) using cost coefficient matrices. Such a cost coefficient matrix must contain a maximum amount of a priori information, provided by a computer generated model and updated with actual data from the angiographic views.     The main promise of the digital humanities is the ability to perform scholar studies at a much broader scale, and in a much more reusable fashion. The key enabler for such studies is the availability of suciently well described data. For the eld of socio-economic history, data usually comes in a tabular form. Existing eorts to curate and publish datasets take a top-down approach and are focused on large collections. This paper presents QBer and the underlying structured data hub, which address the long tail of research data by catering for the needs of individual scholars. QBer allows researchers to publish their (small) datasets, link them to existing vocabularies and other datasets, and thereby contribute to a growing collection of interlinked datasets.We present QBer, and evaluate our rst results by showing how our system facilitates two use cases in socio-economic history

    Genetic analyses of maternal and teacher ratings on attention problems in 7-year-old Dutch twins

    The goal of the present study is to examine genetic and environmental influences on maternal and teacher ratings of Attention Problems (AP) in 7-year-old children. Teachers completed the Teacher Report Form (N=2259 pairs), and mothers the Child Behavior Checklist (N=2057 pairs). Higher correlations were found in twins rated by the same teacher than in twins rated by different teachers. This can be explained by rater bias or by a greater environmental sharing in twins, who are in the same classroom. We further found that 41% of the variation in maternal and teacher ratings is explained by a common factor. The heritability of this common factor is 78%. The heritabilities of the rater specific factors of mothers and teachers are 76% and 39%, respectively. Because Attention Problems that are persistent over situations may indicate more serious behavior problems than context dependent Attention Problems, we believe that gene finding strategies should focus on this common phenotype

    In this article we present the worldsheet integrand for one-loop amplitudes in maximally supersymmetric superstring theory involving any number n of massless open string states. The polarization dependence is organized into the same BRST invariant kinematic combinations which also govern the leading string correction to tree level amplitudes. The dimensions of the bases for both the kinematics and the associated worldsheet integrals is found to be the unsigned Stirling number S_3^{n-1} of first kind.     Rising incidence of breast cancer among female cancer survivors: implications for surveillance

    The number of female cancer survivors has been rising rapidly. We assessed the occurrence of breast cancer in these survivors over time. We computed incidence of primary breast cancer in two cohorts of female cancer survivors with a first diagnosis of cancer at ages 30+ in the periods 1975–1979 and 1990–1994. Cohorts were followed for 10 years through a population-based cancer registry. Over a period of 15 years, the incidence rate of breast cancer among female cancer survivors increased by 30% (age-standardised rate ratio (RR-adj): 1.30; 95% CI: 1.03–1.68). The increase was significant for non-breast cancer survivors (RR-adj: 1.41, 95% CI: 1.04–2.75). During the study period, the rate of second breast cancer stage II tripled (RR-adj: 3.10, 95% CI: 1.73–5.78). Non-breast cancer survivors had a significantly (P value=0.005) more unfavourable stage distribution (62% stage II and III) than breast cancer survivors (32% stage II and III). A marked rise in breast cancer incidence among female cancer survivors was observed. Research to optimise follow-up strategies for these women to detect breast cancer at an early stage is warranted
